Aging is associated with an attrition of cell subcellular components of the endomembrane system due to long-term exposure to environmental stresses or physical/chemical insults. Tissue morphology physiology and function are strongly dependent on the linked dynamic activities of the endomembrane of organelles of the endomembrane system (ES). This is especially true for nervous tissue and the brain where ES components must interact over long (neurite) distances for proper synapse functions. As the endoplasmic reticulum (ER) is the primary organelle responsible for generating plasma membrane and

BACKGROUND: Huntington's disease (HD) is a progressive, hereditary neurodegenerative disorder currently without curative treatments, thus making quality of life (QoL) an important outcome for clinical care and therapeutic evaluation. Existing HD-specific patient-reported outcome measures (PROMs) do not adequately capture the experiences of individuals with manifest HD. This study presents the multinational development and validation of the Huntington's Disease Manifest Quality of Life measure (HD-mQoL), the first needs-based, disease-specific PROM for assessing QoL in individuals with manifest

Huntington's disease (HD) is a neurodegenerative disorder caused by the expansion of the CAG trinucleotide in the exon 1 of the huntingtin gmodellerene. This abnormal expansion produces a mutant huntingtin (mHTT) protein with extended polyglutamine (polyQ) tracts. Although the molecular mechanisms underlying HD onset and progression remain poorly understood, aberrant folding, aggregation, and membrane interactions of mHTT are considered central to disease pathogenesis. In this study, we used molecular dynamics (MD) simulations to investigate the structural properties, dimerization propensity,

BACKGROUND: Huntington's disease (HD) is an autosomal dominant neurodegenerative disorder characterized by progressive motor, cognitive, and psychiatric symptoms. While early striatal degeneration is a well-established hallmark, emerging evidence points to broader network-level dysfunction involving the cerebellum and profound alterations in mitochondrial energy metabolism. However, in vivo studies systematically examining region-specific bioenergetic changes across disease stages are scarce.
METHODS: Using 31Phosphorus magnetic resonance spectroscopic imaging (31P-MRSI), we quantified metabo

Huntington's disease (HD) is an inherited neurological disease caused by variations in the huntingtin (HTT) gene, which leads to neuronal degeneration. Conventionally, HD is affiliated with the gathering and misfolding of mutant HTT arising from an increased number of CAG triplets. Artificial Intelligence has emerged as an important tool in healthcare, supporting the monitoring, detection, and management of HD. Machine learning and deep learning methods are widely used for automated HD identification using neuroimaging, genetic, and clinical data. However, most DL models behave like a black bo

The canonical innate immune functions of astrocytes play a critical role in maintaining CNS homeostasis, including myelin remodeling - a physiological process that unfolds throughout life. Elimination of aged myelin and myelin debris is necessary to support remyelination. Myelin debris are cleared by professional CNS phagocytes microglial cells by classical phagocytosis mechanism and by non-professional phagocytes astrocytes through a mechanism which is not yet fully understood. Here, we demonstrate for the first time that, to process myelin, primary astrocytes from the mouse brain engage a sp

INTRODUCTION: Corticobulbar symptoms, including dysarthria and dysphagia, are frequent manifestations of Huntington's disease, yet evidence on speech and swallowing rehabilitation remains limited. This study explored the potential effects of a home-based corticobulbar rehabilitation program on selected speech and swallowing-related clinical outcomes in Huntington's disease.
METHODS: This prospective, non-randomized, observational controlled study included 40 adults with genetically confirmed Huntington's disease. Twenty-five patients entered a 6-month home-based corticobulbar rehabilitation p

BACKGROUND: Chorea is a hyperkinetic movement disorder with a broad differential diagnosis, ranging from acute symptomatic causes to slowly progressive neurogenetic diseases. While Huntington's disease (HD) remains the most prevalent hereditary form, numerous other genetic disorders may mimic its clinical presentation. A major diagnostic challenge arises in patients with a seemingly negative family history, which can obscure the suspicion of a genetic etiology. In patients with sporadic chorea, the potential contribution of genetic testing to the diagnostic process has not yet been systematica

Abnormal DNA methylation occurs in Huntington's disease (HD, but the underlying mechanisms remain unclear. Using a knock-in pig model, we identify significant alterations in 5mC and 5hmC levels linked to neural genes. TET1, which converts 5mC to 5hmC, decreases prior to symptom onset in pigs-a change not observed in HD mice. TBP binding sites are abundant in the porcine and human TET1 promoter, but scarce in mice. Mutant huntingtin (mHTT) binds more strongly to TBP in HD pig brains, blocking TBP's access to the TET1 promoter, thereby reducing TET1 transcription and altering 5mC/5hmC patterns.

Huntington's disease (HD) is a progressive neurodegenerative disorder characterized by motor, cognitive, and psychiatric symptoms. Tetrabenazine (TBZ), a vesicular monoamine transporter 2 inhibitor, is commonly used to manage chorea; however, it may exhibit fluctuating plasma levels and elicit frequent adverse events. In this study, we aimed to clarify the effects of TBZ deuteration on its safety profile. We compared the safety profiles of TBZ and deutetrabenazine (DTBZ) in real-world clinical practice using disproportionality analysis and the large-scale pharmacovigilance database, VigiBase-t

Huntington's disease (HD) is a genetic neurodegenerative disease characterized by striatum damage, which results in a number of uncontrollable muscle movements alongside intellectual and cognitive impairment. The progression of HD is accompanied by neuroinflammation, oxidative stress, and neuronal apoptosis. Resveratrol (RESV) is a naturally occurring compound known for its potent antioxidant and anti-inflammatory effects. RESV showed promising neuroprotective effects against Alzheimer's and Parkinson's disease. The current research aims to study the neuroprotective effects of RESV against 3-n

Huntington's disease (HD) is characterised by progressive motor impairment and neuropsychiatric dysfunction; however, early presentations may be dominated by psychiatric symptoms. We describe a man in his 50s with a documented diagnosis of HD established several years prior and a strong family history, whose early examinations showed minimal motor findings despite severe psychiatric manifestations including a prior suicide attempt and psychosis. Over subsequent years, serial examinations documented clear motor progression to abnormal gait, weakness and prominent choreiform movements, with inte

Although adult Huntington's disease (HD) studies have significantly advanced our understanding of the course of degeneration, they may underrepresent critical neurodevelopmental aspects of the disease. Significant gaps remain in understanding how mutant huntingtin affects early neurodevelopment, its long-term impact, as well as potential implications for treatment outcomes. The Childhood to Adult Neurodevelopment in Gene-Expanded Huntington's Disease (ChANGE-HD; NCT01951588) study aims to evaluate brain structure and function in premanifest, at-risk children and young adults, and explore HD's

Huntington's disease (HD) is a monogenic neurodegenerative disorder characterized by extensive brain pathology. While its underlying cause has been attributed to intracellular mutant huntingtin (mHTT), growing evidence reveals that mHTT is also present, and biologically active, outside cells. Detected in cerebrospinal fluid, plasma, and brain tissue, extracellular mHTT can induce toxicity and spread between cells. This commentary explores a challenging, yet the largely overlooked possibility that the extracellular matrix (ECM) - a dynamic and highly structured network surrounding neurons and g

Huntington's disease (HD) is a hereditary movement disorder caused by a CAG repeat expansion in the huntingtin gene. HD is characterized by deposition of mutant huntingtin (mHTT) aggregates, and by severe neurodegeneration of the basal ganglia and neocortex. No cure is currently available, and new treatment options are urgently needed. Here, we show that the oligomer modifying molecule anle138b (INN: emrusolmin) improves multiple disease phenotypes in cell culture and in two mouse models of HD. Application of anle138b reduced mHTT aggregate formation and ameliorated neurotoxicity in primary ne

Huntington's Disease (HD), a neurodegenerative disorder, is caused by the expansion of a polyglutamine (polyQ) tract near the N-terminus of the huntingtin protein (HTT), resulting in HTT aggregation. While associated with neurodegeneration, HTT is expressed ubiquitously throughout the body, leading to potential peripheral consequences of aggregation. However, the impact on peripheral tissues remains poorly understood in comparison to the central nervous system. Here, a Caenorhabditis elegans (C. elegans) HD model that expresses an N-terminal HTT fragment (nonpathogenic 15Q or pathogenic 128Q)

We applied transcriptome tomography to create a whole-brain model of early-stage Huntington's disease (HD) in R6/2 mice, which ubiquitously express truncated human mutant HTT containing approximately 150 CAG repeats. Medium spiny neuron (MSN)-related genes showed abnormal expression in the HD brain, in terms of expression similarity to wild-type Htt. Bdnf was the most probable upstream regulator of these genes. Smarca4, the Bdnf-regulator, was similarly expressed to wild-type Htt in the control brain; however, this was not observed in HD, implying a possible involvement of Smarca4 in glutamate

Huntington's disease (HD) causes progressive disability through motor, psychiatric, and cognitive symptoms. Machine learning speech analysis can detect motor and cognitive symptoms of HD, but not yet psychiatric symptoms. This study investigated whether speech analyses can detect the presence of psychiatric symptoms in HD. Audio recordings of six narrative tasks (cookie-theft picture description, red-riding hood storytelling, most recent 24 hours recalling, happy, sad, or angry storytelling) were prospectively collected from subsequent genetically confirmed HD participants from the BIOHD and R

BACKGROUND: Visuospatial working memory (VSWM) deficits are among the earliest cognitive impairments evident in Huntington's disease (HD). They often precede motor symptoms and likely reflect early disruption of cortico-striatal circuits. Both the cognitive components and brain correlates are, however, poorly delineated.
OBJECTIVES: This study aims to characterise the progression of VSWM deficits across HD stages and identify their structural brain correlates.
METHODS: In this prospective international study, 92 HD mutation carriers (HDmc) (21 premanifest and 71 manifest) and 39 healthy cont
