Serum vitamin B12 levels and 90-day outcomes in hospitalized patients with dementia: A cohort study.

Elevated serum vitamin B12 levels have emerged as a paradoxical prognostic marker associated with increased mortality across various clinical settings, yet their significance in hospitalized dementia patients remains unexplored. Given the high vulnerability of this population, we aimed to investigate the association between elevated serum vitamin B12 levels and all-cause mortality. This retrospective cohort study evaluated 90-day outcomes in hospitalized adults with dementia who had serum vitamin B12 levels measured within 7 days of admission (2010-2024). Patients were categorized into exposur

Association between leukocyte telomere length and neurodegenerative diseases: a prospective cohort in the UK Biobank.

BACKGROUND: This study aims to investigate the association between leukocyte telomere length (LTL) and the risk of incident NDDs using a large-scale cohort from the UK Biobank. METHODS: Data from 459,902 subjects were analyzed using Cox proportional hazards models and machine learning (ML) algorithms to assess LTL's association with NDD risk. RESULTS: Shorter LTL was associated with an increased risk of NDDs, including Alzheimer's disease (HR: 0.52, 95% CI 0.40-0.67, P < 0.001), dementia in AD (HR: 0.53, 95% CI 0.39-0.73, P < 0.001), unspecified dementia (HR: 0.74, 95% CI 0.58-0.95, P < 0.05

Nonpharmacological Pain Management for People With Dementia: A Scoping Review Mapping Research Gaps From a Pragmatic Lens.

BACKGROUND: Nonpharmacological pain interventions are effective but underutilized in people living with dementia (PLWD). Leveraging an implementation lens (i.e., the Readiness Assessment for Pragmatic Trials [RAPT] model) to scope the literature may reveal research gaps contributing to this underutilization. The purpose of this scoping review is to summarize the literature, map the studies to the RAPT model, and identify research gaps. METHODS: Following Arksey and O'Malley's framework and Preferred Reporting Items for Systematic Review and Meta-Analyses Extension for Scoping Reviews (PRISMA-

The role of a specialized memory clinic supporting primary care providers in a safety net health system.

BACKGROUND: Although most dementia care occurs in primary care, consultation with dementia specialty care is sometimes indicated. Access to dementia specialists is limited, particularly in resource-limited environments such as the public health safety net, which may require triaging referrals to preserve access for patients with needs that can not be met in a primary care setting. METHODS: The eConsult system for primary care providers to refer patients to a subspecialty memory clinic is described for a large safety net health system. Demographic and clinical characteristics are presented for

Hematological Correlations as Predictors of Disease Manifestations in Psychiatric Inpatients.

Background/Objective. Psychiatric disorders exhibit significant symptomatic and etiopathological heterogeneity, complicating diagnosis and treatment. Hematological parameters may serve as indicators of overall health and predictors of psychiatric symptom manifestation and remission, particularly in long-term hospitalized patients. This study evaluated hematological and biochemical markers, including vitamin B12, vitamin D, and glucose levels, to explore their potential role in psychiatric disorders and disease progression. Methods. This prospective observational study was conducted from 1 Janu

Differential dementia detection from multimodal brain images in a real-world dataset.

INTRODUCTION: Artificial intelligence (AI) models have been applied to differential dementia detection tasks in brain images from curated, high-quality benchmark databases, but not real-world data in hospitals. METHODS: We describe a deep learning model specially trained for disease detection in heterogeneous clinical images from electronic health records without focusing on confounding factors. It encodes up to 14 multimodal images, alongside age and demographics, and outputs the likelihood of vascular dementia, Alzheimer's, Lewy body dementia, Pick's disease, mild cognitive impairment, and

Dementia Risk Due to Traumatic Brain Injury in Subtypes of Dementia in the Welsh Population.

BACKGROUND AND OBJECTIVES: Traumatic brain injury (TBI) can increase vulnerability to neurodegenerative disorders. The association between TBI and dementia has been previously reported, but studies have relied on self-reporting of TBI and often do not appropriately adjust for relevant risk factors or cover enough time to include both individuals at age of highest TBI risk and age of dementia onset. This study uses electronic health records, which include over 20 years of data and 1.7 million individuals with hospital or general practitioner diagnoses of dementia and TBI. Therefore, the aim of

Dementia Risk Among Patients with Major Depressive Disorder: Does Antidepressant Class Matter?

BACKGROUND: The association between the risk of dementia and different classes of antidepressants, as well as the potential influence of treatment resistance or response, remains unexplored in individuals with major depressive disorder (MDD). METHODS: Using the Taiwan National Health Insurance Research Database, 30,390 patients with MDD and 24,312 well-matched controls were enrolled between 2002 and 2004. We stratified the antidepressant-resistant depression (ART, n = 6,078) and antidepressant-responsive depression (ARP, n = 24,312) groups. Selective serotonin reuptake inhibitors (SSRIs) vers
