Neuropsychiatric sequelae in sporadic viral encephalitis.

BACKGROUND: Though sporadic cases of viral encephalitis are not uncommon in our country, there is a lack of prospective studies on the course and outcome of its neuropsychiatric complications. AIM: To study the neuropsychiatric sequelae in patients with sporadic viral encephalitis. MATERIALS AND METHODS: A prospective longitudinal study evaluated 34 consecutive cases of presumed viral encephalitis hospitalized during 1 year. A physician and a psychiatrist examined patients during recruitment and as and when necessary after that. Details of clinical presentation, investigation results, functi

[Late neurological consequences of a SARS-CoV-2 infection].

BACKGROUND: Persistent neurological late symptoms of SARS-CoV-2 infection are common and require regular follow-up treatment. In order to establish uniform therapy concepts, it is necessary to evaluate individual therapeutic approaches for long COVID and post-COVID-19 syndrome. ANAMNESE: A 62-year-old patient was admitted to our rehab clinic for follow-up treatment after severe SARS-CoV-2 infection with neurological symptoms. The initially extensive laboratory and imaging investigation did not reveal any organic cause for the sometimes apoplectiform, complex clinical picture, so that the pati

Impact of eclectic cognitive retraining in a case of high-altitude cerebral edema.

Cognitive impairment in cases of high-altitude cerebral edema is a less researched area of neuropsychiatry. Usually, it presents with depressive symptoms and can sway the treatment on the lines of organic depressive disorder and pseudodementia. We report one such case of which presented with depressive symptoms with cognitive dysfunction. The diagnosis was later revised to organic amnesic syndrome. His cognitive deficits improved with the use of cognitive retraining, especially with the use of mobile-based applications.

Memory dysfunctions.

Since the seminal work on the patient HM, who in his adulthood presented an acquired amnesic syndrome following the resection of the bilateral temporal lobe, other research has described several cases of isolated memory dysfunction in children. This chapter presents developmental and long-lasting memory disorders emerging from an organic or neurologic cause at birth or in infancy. More notably, we focus on developmental amnesic syndrome caused by neonatal bihippocampal damage and memory dysfunction caused by medial temporal developmental epilepsy. We describe these two pediatric populations an

Neurogenic speech sequelae following suicide attempt by hanging: a case report.

BACKGROUND: Attempting suicide by hanging has become one of the most preferred means among adolescents. Individuals who survive a suicide attempt by hanging have a range of deficits, including neuropsychological, neuropsychiatric, pulmonary and even speech and language deficits. Literature regarding speech and language deficits in cases of near hanging is especially limited. OBJECTIVE: This study aimed to demonstrate the sequelae of neurogenic speech deficits following a suicide attempt by hanging, the treatment strategies, and prognostic issues in one such case. METHODS: We report of Patien

[Pure Amnesia].

Pure amnesia (amnesic syndrome) is an organic brain syndrome characterized by impairment in episodic memory, with either an anterograde or sometimes retrograde loss of memories. Although episodic memory is impaired, semantic memory, immediate memory, and procedural memory are preserved. The Papez circuit is a network of nerve fibers and nerve centers that starts and ends in the hippocampus travelling by way of the fornix, mammillary bodies, anterior thalamic nuclei, cingulate gyrus, and parahippocampal gyrus. A lesion restricted to this circuit often produces pure amnesia. Regions concerned wi

Neuropsychiatric sequelae of attempted hanging and diagnostic dilemmas.

Suicide is an important cause of death worldwide, and India is not immune to this major health problem. Suicide by hanging is one of the lethal methods widely practiced. However, literature is very sparse as well as old in the context of neuropsychiatric consequences seen in those who survive such attempts. We present a case of a young boy who survived an attempted hanging and was left with neuropsychiatric sequelae in the form of retro/anterograde amnesia, aggression, lability of affect, and impaired memory and visuomotor deficits. The associated diagnostic dilemmas, namely whether to diagnos

Hydrophilic interaction liquid chromatography/mass spectrometry for determination of domoic acid in Adriatic shellfish.

This paper describes a new method for sensitive, specific and direct determination of domoic acid (DA), the causative toxin of amnesic shellfish poisoning (ASP) syndrome, in shellfish. It is based on combination of hydrophilic interaction liquid chromatography with mass spectrometry (HILIC/MS). The high percentage of organic modifier in the mobile phase and the omission of ion-pairing reagents, both favoured in HILIC, result in enhanced detection limits with MS detection. The new method was set up either on an ionspray ion trap MS instrument operating in MS and MS/MS scanning acquisition modes

Psychogenic amnesia.

Time- and content-based memory systems are briefly described so that their importance for a refined analysis of memory disturbances becomes evident. These memory systems are then related to their brain instantiation, emphasizing that there are limbic circuits for encoding different forms of memories, largely cortical networks for memory storage, and a combined temporofrontal network acting to trigger information retrieval. The terms functional amnesias and psychogenic amnesias are discussed and their symptomatology is compared to that of organic amnesias. The term "mnestic block syndrome" is i

Disorders of memory.

This paper reviews disorders of memory. After a brief survey of the clinical varieties of the amnesic syndrome, transient and persistent, selected theoretical issues will be considered by posing a series of questions. (i) What is impaired and what is spared in anterograde amnesia? (ii) Do temporal lobe, diencephalic and frontal lobe amnesias differ? (iii) How independently semantic is semantic memory? (iv) What determines the pattern and extent of retrograde memory loss? (v) Can retrograde amnesia ever be "isolated"? (vi) Does psychogenic amnesia involve the same mechanisms as organic amnesia?

["The 5 words": a simple and sensitive test for the diagnosis of Alzheimer's disease].

OBJECTIVE: Alzheimer's disease (AD) is under-diagnosed in France. Today only an estimated 50% of patients are identified. Diagnosis of AD is particularly difficult because the amnesic syndrome, characteristic of the disease, is often confused with memory dysfunction that is frequent during the process of aging. The improvement in the diagnostic conditions of AD relies on the availability of a simple and reliable tool for screening memory disorders of organic origin. METHOD: The 5-word test studies the recall of a short list, which the physician ensures the patient has registered. Its construc

[Differential diagnosis of post-concussive syndrome].

INTRODUCTION: Posconcussional syndrome is characterized by a heterogeneous group of somatic, cognitive and psychosocial symptoms, which occur in patients with head trauma, generally of mild severity. It is the neuropsychiatric postraumatic disorder more prevalent in the field of forensic medicine. DEVELOPMENT: Classical authors (Lishamn and Barraquer, for example) focused on controversial aspects of this syndrome, such as conceptual problems and etiology (organic versus functional). The objective of this report is to review the posconcussional literature in search of relevant aspects in foren

[A long-term organic brain syndrome and brain stem symptoms in an undiagnosed dialysis-associated encephalopathy].

HISTORY AND CLINICAL FINDINGS: A 73-year-old woman in renal failure for the past 22 years had been on haemodialysis for 16 years. Because of hyperphosphataemia and peptic ulcers she had been on aluminium-containing antacids with a total intake over time of about 8 kg "pure" aluminium. Over the past 11 years she had biphasic symptoms of death anxieties and depression. She also had amnesic aphasia and some extrapyramidal symptoms as well as generalized convulsive seizures and recurrent falls. INVESTIGATIONS: Cranial computed tomography merely revealed signs of a microangiopathy and an age-relat

Neuropsychological deficits in patients with an anterior communicating artery syndrome: a multiple case study.

The neuropsychological performance of five patients with an anterior communicating artery (ACoA) syndrome (amnesia, confabulation and personality changes) was studied. Neuroimaging techniques revealed a basal forebrain and frontal lobe pathology in all patients. The limbic system appeared intact. There was no evidence for an intellectual deterioration relative to the estimated premorbid IQ in four patients. Regarding attention, all patients showed significant deficits. Visuospatial disabilities could not be observed. On tests of executive functioning, all patients did exhibit severe problems.

Transient global amnesia and functional retrograde amnesia: contrasting examples of episodic memory loss.

We studied 11 patients with transient global amnesia (TGA) and ten patients with functional retrograde amnesia (FRA). Patients with TGA had a uniform clinical picture: a severe, relatively isolated amnesic syndrome that started suddenly, persisted for 4-12 h, and then gradually improved to essentially normal over the next 12-24 h. During the episode, the patients had severe anterograde amnesia for verbal and non-verbal material and retrograde amnesia that typically covered at least two decades. Thirty hours to 42 days after the episode, the patients had recovered completely and performed norma

[Preservation of procedural memory in 18 patients operated for aneurysm of the anterior communicating artery].

Organic amnesia is typically associated with lesions in either the diencephalic or medial temporal regions of the brain. However, amnesia can result from other kinds of lesions, in particular those resulting from an aneurysm of the anterior communicating artery (ACoA). In the present study, 7 patients who became amnesic following a ruptured and operated ACoA aneurysm were comparated neuropsychologically with 11 patients with ruptures but no cognitive complaints and 18 normal control subjects. They were submitted to explicit and implicit memory tests and to tests claimed to be sensitive to fron

Memory and amnesia.

Brain damage can cause several distinct disorders of explicit memory as well as several disorders of implicit memory. Organic amnesia is the best studied explicit memory disorder. It is a syndrome that can be caused by lesions in (a) the medial temporal lobes, (b) the midline diencephalon, or (c) the basal forebrain. It remains unresolved whether one or several functional deficits underlie the syndrome, how these deficits should be characterised, and what is the exact location of the causal lesions. There is good evidence that amnesics encode information normally so their deficit(s) must be of

[The concept of consciousness from the neuropsychiatric and interdisciplinary viewpoint].

In German neuropsychiatric literature there is an established tradition of viewing the disturbance of consciousness as a leading symptom of the acute, potentially reversible organic psychoses. Following the late H. H. Wieck acute organic psychoses which are characterized by a disturbance of wakefulness (syndromes of somnolence, sopor, coma), have to be distinguished from those with preserved wakefulness, which he called 'Durchgangssyndrom' (e.g. amnesic syndrome with affective lability). These two syndromes have found their way into textbooks and everyday language in medicine as 'disturbance o

Sensitive murine model and putative antidotes for behaviorial toxicosis from contaminated mussel extracts.

The recent outbreak of "amnesic" mussel poisoning syndrome, attributed to domoic acid contamination of edible mussels claimed several lives and left many victims impaired with a peculiar loss of memory. We administered the whole mussel extract (WMX) and the mussel hepatopancreas extract (MHX, hepatopancreas being the major site for sequestration of domoic acid in mussels) in Swiss-Webster mice. A characteristic syndrome featuring sluggishness, scratching stereotypy, convulsions and death was noticed. Infant mice were some 3- to 4-fold more sensitive to the WMX toxicity. Kynurenic acid (KYN), a
