Routine Screening Demonstrates Pediatric Delirium Hospital-Wide.

PURPOSE: To identify the rate of positive delirium screens in children within a pediatric hospital setting. METHODS: All Physical, Occupational, and Speech Therapists in our acute-care pediatric medical center were trained to use the Cornell Assessment of Pediatric Delirium (CAPD), an observational delirium screening tool validated for use in children of all ages and developmental stages. Delirium screening was then performed during all therapy sessions, hospital-wide. RESULTS: During the 15-week study period, 1259 delirium screens were completed on 791 unique patients. Delirium was diagnose

AI-Based Delirium Risk Prediction Compared With the Validated 4AT-Score Screening Tool in Patients With Trauma: Prospective Cohort Study.

BACKGROUND: Delirium is a complex neuropsychiatric syndrome whose severe and lasting consequences in hospitalized patients are largely considered preventable. Prevention primarily relies on the early identification of high-risk patients prior to syndrome onset. However, this remains challenging in clinical practice due to the absence of a diagnostic gold standard and the considerable administrative burden associated with established screening tools such as the 4AT score. OBJECTIVE: The aim of this study is to compare the 4AT score with an established AI-supported prognostic tool in applied cl

Inflammaging in older patients with cancer undergoing surgery: implications for anesthetic management.

Population aging has increased the number of patients aged 65 years or older with cancer undergoing surgery; however, chronological age alone does not adequately capture perioperative vulnerability. Inflammaging, a chronic low-grade inflammatory state that accompanies aging, may provide a biological framework for understanding the heterogeneity of perioperative risk. Senescent-cell accumulation and the senescence-associated secretory phenotype contribute to persistent inflammatory signaling, while age-related immune remodeling produces an "immune paradox" involving heightened inflammatory sign

[Strategies for perioperative pain management and the prevention and management of postoperative delirium in frail older patients].

Postoperative pain and postoperative delirium (POD) may share some pathophysiological mechanisms in frail older patients. Persistent nociceptive input, accumulation of analgesic drugs, hemodynamic fluctuations, and respiratory depression may increase the risk of acute brain dysfunction. Potential mechanisms include blood-brain barrier disruption and neuroinflammation, disturbances in cerebral perfusion and oxygen metabolism, and disruption of sleep and arousal networks. A systematic review suggests a dose-response relationship between postoperative pain intensity and POD, although the certaint

Virtual reality-based therapeutic intervention to prevent delirium in the intensive care unit: a systematic review of participant experiences, acceptability, and implementation implications.

OBJECTIVE: Delirium is common in intensive care units (ICUs) and is associated with adverse outcomes. Virtual reality (VR) is a non-pharmacological modality that may mitigate contributory factors such as sensory deprivation, anxiety, and immobility. This study aims to synthesise acceptability, feasibility, safety/tolerability, and implementation‑relevant outcomes of VR for ICU delirium prevention. METHODS: Systematic review of 11 databases (PubMed, Web of Science, EMBASE, PsycINFO, AMED, CINAHL, Cochrane Library, CNKI, Wanfang, Weipu, CBM) from January 2012 to June 2026. Eligible studies eval

VoiceLove for patient-family communication in the ICU: a usability study of a novel voice messaging app.

BACKGROUND: Family engagement has been associated with improved outcomes following critical illness, but communication barriers challenge family engagement. Traditional video or phone call strategies to navigate communication barriers are time-, resource-, and labor-intensive for hospital staff, often limiting their use. VoiceLove is a Health Insurance Portability and Accountability Act (HIPAA)-compliant mobile phone app that enables family members to send real-time voice messages to intensive care unit (ICU) patients without relying on the clinical team to coordinate calls. The Mask of Delirium: Uncovering a Presumed Cardioembolic Stroke Through Comprehensive Clinical Evaluation.

Delirium and aphasia exhibit overlapping clinical features, complicating the diagnosis of acute neurological disturbances in older adults. Both conditions impair communication and orientation, leading to incoherence; however, they arise from distinct pathophysiological mechanisms and require different management strategies. This case concerns an 83-year-old woman with a 48-hour history of acute-onset confusion and abnormal speech. Magnetic resonance imaging (MRI) revealed an acute infarct in the inferior division of the left middle cerebral artery (MCA) territory, with cardioembolism considere

Preoperative nutritional status defined by GLIM criteria and postoperative delirium in older patients receiving general anesthesia.

BACKGROUND: Preoperative malnutrition may reflect reduced physiological reserve and increased vulnerability to postoperative delirium (POD) in older surgical patients. However, evidence based on standardized diagnostic criteria remains limited. This study aimed to evaluate the association between Global Leadership Initiative on Malnutrition (GLIM)-defined malnutrition and POD among older patients undergoing surgery under general anesthesia. METHODS: This retrospective observational cohort study included consecutive patients aged ≥65 years who underwent elective or emergency surgery under gene

Global research trends and emerging themes of ketamine in perioperative neurocognitive disorders: a bibliometric analysis.

BACKGROUND: Within the contemporary perioperative neurocognitive disorder (PND) framework, postoperative neurocognitive outcomes include postoperative delirium, delayed neurocognitive recovery, and postoperative neurocognitive disorder. Interest in ketamine and its enantiomers has increased, but the clinical literature on perioperative neurocognitive outcomes remains heterogeneous. This study mapped the global research landscape, collaboration structure, thematic evolution, and recently intensified research themes at the intersection of ketamine and PNDs. Concurrent Management of Adolescent Cannabinoid Hyperemesis Syndrome and Delirium With Haloperidol.

Cannabinoid hyperemesis syndrome (CHS) manifests in adults as severe nausea accompanied by episodic vomiting and intense abdominal pain, typically associated with prolonged, high-dose cannabis use. The clinical course of CHS is divided into 3 distinct phases: prodromal, hyperemetic, and recovery. Traditional antiemetic and antinausea medications, such as ondansetron and metoclopramide, are often used in the acute clinical setting to address these isolated symptoms. Discussions and occasional use of haloperidol have demonstrated a level of symptom relief in patients with CHS. In several instanc

Comparison of the efficacy of pentazocine and tramadol for reducing emergence agitation in children after general anesthesia: a retrospective cohort study.

BACKGROUND: Emergence agitation (EA) is a common postoperative complication in pediatric anesthesia that can lead to serious safety events and increased healthcare burden. Both pentazocine and tramadol are frequently used for postoperative analgesia in children, but their comparative effectiveness in preventing EA remains unknown. This study aimed to compare the efficacy of a single intravenous dose of tramadol versus pentazocine for reducing EA in children undergoing tonsillectomy and adenoidectomy. METHODS: This single-center retrospective cohort study included 240 children aged 2-12 years

"They think the sea a true medow": calenture of the brain.

Calenture's once-famous symptom of hallucinated pastures at sea, perhaps first attested in Kenelm Digby's poorly datable manuscript Loose Fantasies, entered the medical literature in a 1648 work by James Cooke (1614-1694). The eventually canonized case being sailors drowning at night for puzzling reasons, descriptions remained few and contested, not least because febrile and afebrile variants were alleged. At least three seventeenth-century authors (Henry Stubbes, John Moyle, and William Oliver) explicitly construed calenture as a distemper of the animal spirits, formalizing its preexistent ve

Geriatric respiratory medicine 2026: from artificial intelligence to personalised management of the multimorbid patient.

The profound demographic shift toward an ageing population has decisively moved the burden of chronic and acute lung diseases towards older adults, necessitating a transition from organ-centred models to integrated, multidimensional care. This review outlines a pragmatic framework for managing the complex interplay of immunosenescence, frailty and multimorbidity. In acute care, "pneumonia-associated sarcopenia" is identified as a critical driver of recurrent infections and functional decline, requiring a shift towards aspiration prevention and the aid of diagnostic tools such as lung ultrasoun

Perioperative Complications of the Occipitocervical Fixation Surgery for Cervical Trauma in Patients Aged Over 80 Years.

INTRODUCTION: Although severe perioperative complications have been reported after occipitocervical fixation surgery for cervical trauma, no studies have specifically evaluated these complications in patients aged over 80 years. This study aimed to assess perioperative complications following occipitocervical fixation surgery for cervical trauma in patients older than 80 years. METHODS: Among the 1,512 patients with cervical injury treated between 2010 and 2020, 41 patients aged ≥65 years who underwent occipitocervical fixation surgery were enrolled at 77 institutions in Japan. Patients were

Terminal agitation in dementia: Challenges and opportunities.

Terminal agitation (TA), also referred to as terminal restlessness or terminal delirium, is a complex, multifactorial clinical syndrome that occurs at the end of life. While TA is evident in dementia (including Alzheimer's disease), it is not exclusive to the condition and also occurs in other terminal illnesses, e.g., cancer. Currently, no clinical tools exist or have been validated to assess TA in dementia. This may hinder the correct identification of TA and further contribute to the incorrect estimation of TA prevalence. Due to the complex diagnostic profile of TA, it may be frequently und

Rapid-Onset Opioid-Induced Neurotoxicity During Morphine Therapy.

PurposeOpioid therapy is central to cancer pain management and aims to provide effective analgesia while minimizing neurotoxic adverse effects; however, cognitive dysfunction, delirium, and hallucinations continue to be significant complications.SummaryOpioid-induced neurotoxicity (OIN) has been associated with higher opioid doses, poor functional status, and advanced disease burden, and is frequently an underrecognized contributor to delirium. We describe an unusual case of visual and auditory hallucinations shortly after morphine exposure, with prompt reversibility after opioid rotation and

[Dehydration in elderly: causes, prophylaxis and treatment].

Dehydration is common among older adults. A narrative literature review was conducted to determine the current state of knowledge regarding the symptoms, prevention, and early treatment of dehydration. Fifty-three full-text articles from the years 2015-2025 were selected for the final analysis. The most common cause of dehydration in older people is insufficient fluid intake. Dehydration can also be a symptom of diseases common in older adults, including urinary tract infections, pneumonia and other febrile illnesses, as well as diabetes. In addition to medical causes, dehydration can result f
