Hyperactive Delirium Temporally Associated With Hydroxyzine in a Child With Succinic Semialdehyde Dehydrogenase Deficiency Presenting With Psychotic Symptoms: A Case Report.

BACKGROUND: Succinic semialdehyde dehydrogenase deficiency (SSADHD) is a rare autosomal recessive disorder of gamma-aminobutyric acid (GABA) metabolism caused by biallelic variants in ALDH5A1. The phenotype includes developmental delay, intellectual disability, seizures, sleep disturbance, anxiety, and other neurobehavioral symptoms. Psychotic symptoms are increasingly recognized in SSADHD but remain uncommon and incompletely characterized in children. In medically and neurodevelopmentally vulnerable patients, acute psychiatric decompensation may also be complicated by delirium, especially whe

Multicomponent non-pharmacological interventions for preventing delirium in critically ill patients: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: It is necessary to evaluate the efficacy of multicomponent non-pharmacological interventions in preventing and managing intensive care unit (ICU) delirium and identify the most effective intervention combinations. METHODS: A systematic literature search was conducted in PubMed, EMBASE, CINAHL, Cochrane Library, and ProQuest Dissertations and Theses A&I databases from inception to July 3, 2026, supplemented by a hand search of prior meta-analyses. Only randomized trials were included. Meta-analysis was performed using risk ratios (RR) or mean differences (MD) with 95% confidence in

Recent Evidence on Factors and Potential Interventions Affecting Patient Sleep in Inpatient and Hospital Settings.

PURPOSE OF REVIEW: Sleep is a critical but frequently disrupted part of hospital care, affecting patient recovery, length of stay, and physical and mental health. This review examines recent evidence (2022-2026) on why inpatient sleep is disrupted and how interventions can mitigate disturbance across different patient groups and hospital settings. RECENT FINDINGS: Findings span five domains: environmental factors such as noise and light, hospital infrastructure and room configuration, patient-related factors including pain and anxiety, care routines, and interventions. Recent high-quality tri

Domain-Specific Subjective Cognitive complaints and Cognitive Test Performance After Surgery in Older Adults: A Patient-Centered Prospective Cohort Study.

BACKGROUND: Subjective cognitive complaints are clinically important in the assessment of perioperative neurocognitive changes because current nomenclature incorporates cognitive concerns reported by patients or others. However, the relationship between patients' subjective cognitive complaints and contemporaneous changes in cognitive test performance during the immediate postoperative period remains poorly understood. OBJECTIVES: To identify if subjective cognitive complaints reported before hospital discharge were associated with cognitive changes measured with cognitive tests. DESIGN: Pro

Impact of Cardiopulmonary Bypass Duration on the Development of Postoperative Delirium in Cardiac Surgery: A Secondary Subgroup Analysis of an Observational Study.

OBJECTIVES: Postoperative delirium (POD) is an acute brain impairment linked to higher morbidity and mortality after surgery. This issue is especially prevalent in older cardiac surgery patients receiving cardiopulmonary bypass (CPB), who face unique pathophysiological risks and heightened POD susceptibility. Identifying risk factors is crucial for targeted prevention and therapy. DESIGN: This investigation represents a subgroup analysis derived from a prospective observational study. SETTING: In this subgroup analysis, 274 cardiac surgery patients from a prospective observational study were

Elevated blood urea nitrogen-to-albumin ratio is associated with incident sepsis-associated delirium: evidence from a weighted competing-risk analysis in a multicenter cohort study.

BACKGROUND: Sepsis-associated delirium (SAD) is a frequent manifestation of acute brain dysfunction in sepsis. Although the blood urea nitrogen-to-albumin ratio (BAR) has been associated with prognosis in critically ill patients, its relationship with incident SAD remains uncertain. We evaluated the association between early BAR and incident SAD. METHODS: This multicenter retrospective cohort study included patients with sepsis from MIMIC-IV as the primary cohort and eICU-CRD as an independent replication cohort. BAR was calculated from eligible blood urea nitrogen and serum albumin measureme

Characterization of Delirium in an Intensive Care Unit: Real-World Data Based on the Application of the Confusion Assessment Method.

Introduction Delirium is a neuropsychiatric syndrome of acute onset characterized by disturbances in attention, awareness, and cognition. It is common in intensive care units (ICUs), and the Confusion Assessment Method for the ICU (CAM-ICU) is the most widely used bedside instrument for its detection. Methods We conducted a retrospective, observational, single-centre study of all patients admitted to the ICU of a Portuguese community hospital, Unidade Local de Saúde do Alto Minho (ULSAM), between January 1, 2021, and December 31, 2022. Delirium was defined as at least one positive CAM-ICU asse

Acute trazodone treatment ameliorates behavioral abnormalities and improves subsequent short-term memory in a lipopolysaccharide-induced mouse model of acute brain dysfunction.

AIM: Systemic inflammation-associated acute brain dysfunction, typified by delirium, is linked to later cognitive impairment. Although trazodone is used clinically for delirium-related symptoms, its effects on subsequent cognition remain unclear. We examined whether acute-phase trazodone treatment influences behavioral abnormalities, later cognitive outcomes, and inflammatory/glial-related changes in an lipopolysaccharide (LPS)-induced mouse model. METHODS: Adult male C57BL/6J mice received saline or LPS (3.0 mg/kg, intraperitoneally). Control mice received saline, whereas LPS-treated mice su

Beyond the excited delirium controversy: two forensic cases and contemporary literature review.

Excited Delirium Syndrome remains one of the most controversial topics in contemporary forensic pathology. Although the term has been increasingly challenged because of the lack of universally accepted diagnostic criteria and formal recognition within major nosological systems, episodes of severe psychomotor agitation associated with sudden death continue to represent a significant clinical and medico-legal concern. We report two cases of sudden death occurring during physical restraint in individuals presenting with severe agitation. The first case involved a 60-year-old man with a history of

Pre-admission Nutritional Risk as an Independent Predictor of 28-Day Mortality After Hip Fracture: A Retrospective Cohort Study.

BACKGROUND: Hip fracture carries a substantial risk of early mortality in older adults. Malnutrition is common in this population and is closely correlated with age, frailty, and cognitive impairment, making its independent contribution to short-term mortality difficult to disentangle from these established prognostic factors. Whether nutritional risk on admission predicts 28-day mortality after accounting for age, sex, anesthetic risk grade, and cognitive status remains uncertain. METHODS: We conducted a retrospective cohort study of 709 consecutive patients admitted with hip fracture. Nutri

Comparison of Survival Prognostic Tools in Terminal-Stage Tumor Patients: A Multicenter Prospective Cohort Study.

AIM: Previous studies have discussed the effectiveness of the Karnofsky Performance Scale (KPS), Palliative Performance Scale (PPS), Palliative Prognostic Index (PPI), and Delirium-Palliative Prognostic Score (D-PaP) in predicting the survival of patients with terminal-stage tumors, but their effectiveness in the Chinese terminal-stage tumor population still needs to be explored. Therefore, we conducted this prospective cohort study to investigate the applicability of KPS, PPS, PPI, and D-PaP in predicting survival in Chinese patients with terminal-stage tumors. METHODS: This is a prospective

Profiles in safety: The Hospital From Home Program.

Although today's health care system has achieved unprecedented advances, geriatric adults remain vulnerable to iatrogenic complications associated with traditional hospitalization, including acute cognitive delirium, rapid functional decline, medical errors, and health care-associated infections. To improve patient safety, an organization initiated an acute hospital-at-home pilot program from June 2023 through February 2024. Building on the removal of prepandemic regulatory and reimbursement barriers, the pilot was formally expanded and rebranded as the Hospital From Home (HfH) Program. An eva

Delirium.

Delirium is a common, acute confusional state that is associated with adverse patient, caregiver, and health system outcomes. All patients are at risk for delirium, although those with advanced age or acute infection and those taking sedatives and anticholinergics are at greater risk. The pathophysiology of delirium is complex and includes neuronal injury, inflammation, metabolic imbalance, and blood-brain barrier disruption. It is important to note that delirium is preventable; recognizing people at risk and identifying and addressing predisposing and precipitating factors is paramount. No si

Intelligent thermoregulation strategy versus routine thermal management on myocardial injury after noncardiac surgery during gastrointestinal tumor resection in elderly patients (SAGE): a multicenter, parallel-group, superiority trial.

BACKGROUND: Inadvertent perioperative hypothermia is associated with adverse cardiovascular outcomes, but adequately powered trials using high-sensitivity troponin endpoints is lacking. The SAGE trial evaluates whether an intelligent, closed-loop thermoregulation strategy reduces myocardial injury after noncardiac surgery (MINS) in high-risk elderly patients. METHODS: This prospective, multicenter, parallel-group, randomized controlled superiority trial conducted at seven tertiary hospitals in China. Participants aged ≥65 years scheduled for elective gastrointestinal tumor resection under gen

Population-based incidence rate of in-hospital delirium in older adults in Norway-a HUNT study.

BACKGROUND: Most studies of delirium occurrence are based on selected hospitalised populations or administrative data. We aimed to estimate the incidence rate of in-hospital delirium in older adults and to assess use of the International Classification of Diseases, 10th Revision (ICD-10) delirium code (F05) in electronic health records. METHODS: This population-based cohort study included 9956 participants aged ≥70 years from HUNT4 70+. Participants were followed from the date of study participation (2017-19) until 31 December 2023. In-hospital delirium was identified by retrospective chart r

Beyond processed depth targets: frontal EEG patterns, brain vulnerability, and postoperative delirium in older adults.

Limited-channel frontal electroencephalography is widely used during anesthesia through processed EEG platforms, but clinical interpretation often remains centered on numerical depth indices. This narrative review examines whether frontal EEG signals, including raw waveforms, suppression behavior, spectral features, and processed index behavior, may provide information beyond hypnotic depth in older adults at risk for postoperative delirium. A targeted literature search was performed in PubMed/MEDLINE and Google Scholar for articles published from database inception through April 30, 2026. Pri
