Exploratory Sex-Stratified Associations of Postoperative Visual and Auditory Hallucinations After Cardiac Surgery: Insights from the VAACS Study.

Background Postoperative hallucination experiences are distressing but under-recognized complications of cardiac surgery. This study investigated whether sex differences exist in their incidence and in the factors associated with postoperative visual and auditory hallucination experiences. Methods We conducted a prospective multicenter cohort study of 1,331 adults (1,000 males and 331 females) undergoing isolated coronary artery bypass grafting or isolated valve surgery at 10 cardiac surgery centers in the West Bank, Palestine (September 2022-May 2025). Postoperative visual (VH) and auditory h

Construction and validation of a nomogram prediction model for the risk of delirium after hip arthroplasty in elderly patients.

The aim of this study was to analyze the risk factors of postoperative delirium in elderly hip arthroplasty patients and to construct and validate a nomogram model. The clinical data of elderly patients with hip arthroplasty admitted to our hospital from January 2022 to December 2023 were retrospectively collected. Univariate and multivariate logistic regression analyses were used to determine the independent risk factors for delirium risk after hip arthroplasty in elderly patients, and the corresponding nomogram was established. Receiver operating characteristic curves and calibration curves

Artificial Intelligence-Driven Clinical Decision Support for Postoperative Delirium Prevention: A Pragmatic Randomized Controlled Trial.

BACKGROUND: Postoperative delirium affects up to 50% of older adults with cognitive impairment, yet preoperative identification of at-risk patients remains limited. Artificial intelligence methods, such as natural language processing, can extract cognitive impairment signals from electronic health records and enable targeted clinical decision support interventions to promote adherence to perioperative best practices and reduce postoperative delirium. We aimed to evaluate the effectiveness of an artificial intelligence-driven non-interruptive clinical decision support intervention in reducing p

Progress of nursing management for delirium in critically ill geriatric patients: a narrative review.

Delirium is the most common and modifiable acute cognitive complication in elderly critically ill patients and is associated with prolonged hospital stays, increased mortality, persistent long-term cognitive decline, and a significant healthcare burden. A history of mild cognitive impairment or dementia further increases delirium risk, exacerbates symptom severity, and worsens clinical outcomes. Nonpharmacological nursing interventions form the cornerstone of delirium prevention and management. However, nurse-centered integrated comprehensive care pathways remain insufficiently integrated. Extracting Symptoms Relevant to Geriatric Syndromes from Japanese Pharmaceutical Care Records: A Natural Language Processing Analysis.

Geriatric syndromes are multifactorial conditions associated with adverse outcomes in older adults, but how community pharmacists capture related symptoms remains unclear. Therefore, a natural language processing (NLP) model was used to analyze pharmaceutical care records. The records of 90651 patients (≥65 y of age) who had prescriptions filled at 58 community pharmacy branches in Japan between January and December 2022 were analyzed. Patients without the home-based patient dispensing fee (hereafter referred to as outpatients) and patients receiving home-based care were included in the study.

Managing the Sequelae of Delirium Associated With Critical Illness.

The Psychiatric Consultation Service at Massachusetts General Hospital sees medical and surgical inpatients with comorbid psychiatric symptoms and conditions. During their twice-weekly rounds, Dr Stern and other members of the Consultation Service discuss diagnosis and management of hospitalized patients with complex medical or surgical problems who also demonstrate psychiatric symptoms or conditions. These discussions have given rise to rounds reports that will prove useful for clinicians practicing at the interface of medicine and psychiatry. Prim Care Companion CNS Disord 2026;28(5):26f0424

[The right of older persons to consent to healthcare services: selected legal issues].

This review examines the legal validity of consent to healthcare services given by older patients, with particular emphasis on cognitive impairment, the patient's actual capacity to make healthcare decisions, and the protection of patient autonomy in the decision-making process. The study is based on a narrative literature review combined with a doctrinal legal analysis of Polish statutory provisions, case law, and literature in medical law, geriatrics, cognitive psychology, and bioethics. The analysis indicates that reaching 60 years of age does not create a separate legal regime for consent

Neurophysiological signatures of latent brain vulnerability in perioperative neurocognitive disorders: integrating EEG, cerebral oximetry, and processed EEG.

Perioperative neurocognitive disorders (PND) remain difficult to prevent because conventional cognitive assessments may fail to capture brain vulnerability in patients who appear cognitively normal before anesthesia and surgery. This narrative review examines the potential role of objective perioperative neuromonitoring markers as complements to cognitive scales, focusing on electroencephalography (EEG), near-infrared spectroscopy (NIRS)/cerebral oximetry, and processed EEG/bispectral index (BIS). Evidence from perioperative studies and related neurophysiological literature suggests that reduc

Association of mild cognitive impairment and comorbid depression with postoperative delirium in patients undergoing knee/hip arthroplasty: a prospective cohort study.

BACKGROUND: Postoperative delirium (POD) is common after surgery, involving acute disturbances in attention and cognition. While mild cognitive impairment (MCI) and depression each influence cognition, their combined impact on POD is unclear. We aimed to assess the effect of MCI with comorbid depression on POD and explore whether cerebrospinal fluid (CSF) biomarkers mediate this relationship. METHODS: This study included 1,464 knee/hip arthroplasty patients divided into four preoperative diagnostic groups: no disease, MCI only, depression only, and MCI with depression. CSF markers were compar

Individualized Treatment Effects of Cefepime vs Piperacillin-Tazobactam in Adults Hospitalized with Acute Infection.

RATIONALE: Among adults hospitalized with acute infection, a recent randomized trial observed no significant average treatment effect of cefepime versus piperacillin-tazobactam on acute kidney injury or death, but fewer days-alive-and-free-of delirium and coma with cefepime. OBJECTIVES: To determine whether patients' individual characteristics modify the effect of cefepime versus piperacillin-tazobactam on outcomes. METHODS: In this secondary analysis of the Antibiotic Choice On Renal Outcomes (ACORN) trial, an effect model was derived and validated to predict the effect of cefepime versus p

Effects of remimazolam versus propofol on cerebral oxygenation in elderly patients undergoing prone spine surgery: study protocol for a randomised controlled trial.

INTRODUCTION: Prone spine surgery may increase the risk of cerebral hypoperfusion and postoperative neurological adverse events, particularly in elderly patients. Remimazolam is a novel ultra-short-acting benzodiazepine anaesthetic that may provide greater haemodynamic stability than propofol. However, its effects on cerebral oxygenation during prone spine surgery remain unclear, indicating clinical equipoise between remimazolam and propofol. This study compares the effects of remimazolam and propofol on cerebral oxygenation in elderly patients undergoing thoracic or lumbar spine surgery in th

Olanzapine for the Prevention and Treatment of ICU Delirium: Protocol for a Systematic Review With Meta-Analysis.

BACKGROUND: Intensive care unit (ICU) delirium is a multifactorial acute brain dysfunction associated with prolonged hospitalization, increased morbidity and mortality, and higher healthcare costs. Despite these substantial negative impacts of delirium, the optimal prevention and treatment strategies are unclear. Olanzapine, an atypical antipsychotic drug, is commonly used for delirium management. We will perform an updated systematic review and meta-analysis to assess the benefits and harms of olanzapine for the prevention and treatment of ICU delirium. METHODS: This protocol is developed in

Relationships of Preoperative and 24-Hour Postoperative Plasma and Cerebrospinal Fluid Cytokines with Postoperative Delirium.

BACKGROUND: Postoperative delirium is a common syndrome of acute changes in attention, cognition, and consciousness that may result from inflammation and/or neuroinflammation, but few studies have distinguished the relationships of preoperative and 24-hour postoperative systemic inflammation (i.e. in blood) versus neuroinflammation (i.e. in cerebrospinal fluid, or CSF) in postoperative delirium. METHODS: We measured CSF and plasma cytokine levels before and 24-hours after non-cardiac/non-neurologic surgery in 199 patients age ≥ 60 years who were enrolled in two prospective cohort studies. Comorbidities and health-care use prior to early-onset Alzheimer's disease: A population-based case-control study.

INTRODUCTION: We examined whether comorbidities and health-care use differ between individuals later diagnosed with early-onset Alzheimer's disease (EOAD) and matched controls during the 15 years preceding diagnosis. METHODS: This population-based case-control study included 407 EOAD patients diagnosed at two Finnish university hospitals (2010-2021), and 4035 age- and sex-matched controls. Prior diagnoses and health-care visits were obtained from nationwide registers. RESULTS: EOAD patients had higher prevalence of vertigo (5.9% vs. 2.1%, p < 0.001), depression (5.7% vs. 2.3%, p < 0.001), he

Evaluating Generative Artificial Intelligence Chatbots in Answering Public Questions About Altered Consciousness: A Comparative Study of Safety, Accuracy, Reliability, and Readability.

INTRODUCTION: People increasingly use generative artificial intelligence chatbots to interpret health concerns before contacting clinicians. Although these systems are not formal autonomous emergency department triage tools, their responses may influence whether and when users seek professional care. Altered consciousness is especially high-risk because delayed or ambiguous escalation can postpone time-sensitive evaluation. This study evaluated the safety and quality of chatbot answers from an emergency nursing perspective. METHODS: We conducted a cross-sectional comparative evaluation of Cha

Daily sedation interruption versus no daily sedation interruption for critically ill adults and children requiring invasive mechanical ventilation.

RATIONALE: Daily sedation interruption (DSI) aims to limit drug accumulation and promote a more awake state, thereby reducing the duration of mechanical ventilation and its associated complications. This is an update of a Cochrane review first published in 2014. OBJECTIVES: To assess the effects of DSI versus sedation management without DSI on the duration of invasive mechanical ventilation, mortality, intensive care unit (ICU) length of stay, adverse events related to under- or oversedation (including accidental removal of endotracheal tube and tracheostomy), total doses of sedative and anal
