BACKGROUND: Modern conflicts have brought about a paradigm shift in military medicine. In Prolonged Field Care, recommendations have so far focused on damage control and care aimed at ensuring survival. But, particularly in large scale combat operation, not all patients will survive, and despite the development of palliative care in the civilian sector, no extension of this approach has yet been envisaged for military medicine. This review aims to identify knowledge gaps in palliative and supportive care in combat situations.
METHODS: We conducted a literature review in PubMed, CAIRN, Scopus

BACKGROUND: Postoperative delirium is common after coronary artery bypass grafting (CABG) and is associated with prolonged hospitalization and delayed recovery. Non-pharmacological educational interventions may improve outcomes, but evidence on structured video-based education for delirium prevention remains limited.
OBJECTIVE: To evaluate the effect of a preoperative video-based educational program on postoperative delirium incidence and hospital length of stay among patients undergoing CABG surgery.
METHODS: A parallel-group randomized controlled trial was conducted at Queen Alia Heart Ins

BACKGROUND: Delirium is a common neuropsychiatric complication among critically ill patients, particularly those who are mechanically ventilated. Evidence suggests family involvement may improve delirium-related outcomes in adult intensive care units (ICUs). However, limited evidence exists on intensive care nurses' (ICNs) experiences and attitudes towards family involvement in delirium care in South Africa.
AIMS: The study aimed to explore intensive care nurses' experiences and attitudes towards family involvement in the care of mechanically ventilated patients with delirium in adult ICUs.

BACKGROUND: Sleep deprivation and poor sleep quality are critical problems among intensive care unit (ICU) patients, with significant implications for recovery, delirium onset and clinical outcomes. Despite growing scientific interest, no bibliometric study has systematically mapped the intellectual structure and thematic evolution of this field.
AIM: To analyse the bibliometric characteristics and thematic structure of publications on sleep deprivation, sleep promotion and sleep quality in ICU patients indexed in the Web of Science (WoS) database between 1983 and 2026.

BACKGROUND: Open aortic arch surgery exposes patients to hypothermic circulatory arrest, selective antegrade cerebral perfusion, rewarming, and arch-vessel manipulation. Postoperative delirium and early cognitive decline remain clinically important complications. Because single-modality cerebral oximetry cannot fully capture perioperative cerebral physiological burden, this study evaluated whether a multimodal neuromonitoring-guided cerebral-protection protocol was associated with lower cerebral physiological burden and improved early neurocognitive outcomes after aortic arch surgery.

INTRODUCTION: ICU-acquired weakness (ICU-AW) is a frequent complication of critical illness, and real-world evidence on nurse-led early mobilization (EM) in Asian ICUs remains limited.
METHODS: We conducted a single-center, retrospective before-after cohort study of invasively ventilated adults admitted during usual-care (n = 153) and nurse-led EM (n = 164) periods. ICU-AW, defined as a Medical Research Council (MRC) sum score <48, was assessed among evaluable ICU survivors (n = 266); ICU death made volitional strength testing non-evaluable and was addressed in a death-or-ICU-AW composite sen

Introduction Hospital admission in older adults can be associated with functional decline, loss of independence, delirium, disruption of routine, and psychological deterioration. Virtual Wards, also described as Hospital-at-Home models, provide acute clinical care in a patient's usual place of residence and may reduce some harms associated with inpatient admission. Although Virtual Wards are commonly evaluated in terms of bed capacity, admission avoidance, and clinical safety, their psychological impact remains less well explored. This audit compared depressive symptoms and self-rated mood in

INTRODUCTION: Early identification of hospitalised patients with or at risk for delirium is important to treat patients adequately. The aim of this study is to compare three approaches to delirium identification for identifying patients being at risk for or with a delirium during hospitalisation in a cohort of older patients presenting to the emergency department, including retrospectively extracted indicators of delirium based on prehospital care notes, by risk assessment (VMS), and screening instrument (4AT).
METHODS: A single-centre cohort study was conducted with the use of data from pati

BACKGROUND: Multifocal atherosclerosis (MFA) is one of the common diseases in elderly patients. Currently, there is no single approach to treating patients with MFA. The active implementation of endovascular surgery has significantly expanded the treatment options for comorbid elderly patients with MFA.
OBJECTIVE: Of the study: to evaluate the effectiveness and safety of various methods of surgical revascularization in geriatric patients with multifocal atherosclerosis.
MATERIALS AND METHODS: This single-center observational cohort study was conducted in the Department of Cardiovascular Surg

BACKGROUND: Monitored anesthesia care (MAC) is increasingly used as part of minimalist transcatheter aortic valve implantation (TAVI) pathways, but evidence directly comparing remimazolam- and dexmedetomidine-based MAC with respect to intraprocedural hemodynamic stability and early clinical outcomes remains limited.
METHODS: In this retrospective, single-center, propensity score-matched cohort study, 433 patients who underwent TAVI under MAC between January 2021 and May 2026 were identified. After 1:1 propensity score matching, 264 patients were included, with 132 patients in each group. The

OBJECTIVE: To evaluate the effect of early mobilization in patients with acute respiratory distress syndrome (ARDS).
METHODS: Computerized searches were conducted in PubMed, EMbase, The Cochrane Library, Web of Science, CNKI, Wanfang Data, CBM, and VIP databases for randomized controlled trials (RCTs) on early mobilization in ARDS patients. The retrieval period was from the establishment of each database to September 30, 2025. Two reviewers independently assessed the quality of included literature and extracted data in accordance with Cochrane systematic review methodology. Meta-analysis was

Perioperative cognitive vulnerability in older adults reflects interactions among inflammatory stress, altered brain state, sleep-circadian disruption, and recovery capacity. This narrative review examines astrocyte-centered temporal resilience as a hypothesis-generating framework for integrating these brain-immune-behavior processes. Astrocytes participate in sleep-pressure signaling, circadian regulation, neurometabolic support, inflammatory responses, extracellular homeostasis, and clearance-related physiology; however, these functions operate within broader neuronal, microglial, vascular,

BACKGROUND: Delirium is common in critically ill patients but is frequently under-recognised, inconsistently documented, and poorly explained to families. In our 48-bed intensive care unit (ICU), baseline chart review identified probable delirium in 65% of sampled admissions, yet the term "delirium" was documented in only 23% of medical records.
OBJECTIVES: The objective of this study was to increase the visibility of delirium as a hospital-acquired complication through reliable screening, explicit interdisciplinary documentation, and consumer co-designed family engagement.
METHODS: We condu

INTRODUCTION: The optimal ICU sedation strategy to improve patient-reported experience is unknown. In a planned secondary analysis of the A2B randomised trial, we evaluated the effect of sedation strategy (dexmedetomidine, clonidine or propofol) on recalled patient experience at 90 days post-randomisation. Using mediation analyses, we explored plausible mediators of the relationship between sedation strategy and patient experience.
METHODS: We analysed data for all patients from A2B who completed the Intensive Care Experience Questionnaire (ICE-Q) at 90 days. The primary outcome was patient-r

BACKGROUND: Postoperative delirium (POD) is a common complication in geriatric patients undergoing cardiac surgery. POD is associated with prolonged length of stay in the intensive care unit and hospital, increased mortality, institutionalization, distress, risk of cognitive impairment and dementia. The non-pharmacological, multimodal AKTIVER intervention from the multicenter PAWEL trial significantly reduced POD incidence and days with POD in older patients undergoing abdominal and orthopedic surgery. However, this positive effect on POD incidence has not yet been demonstrated in elderly pati
