OBJECTIVE: This study aimed to identify the correlative factors of delirium in patients with dementia and to preliminarily develop a classification model for probable delirium superimposed on dementia (DSD).
METHODS: A total of 143 dementia inpatients from a tertiary hospital were enrolled. Participants were assessed using the Mini-Mental State Examination (MMSE) to evaluate baseline cognitive status, and the Confusion Assessment Method-Chinese Revision (CAM-CR) was applied to categorize participants into the probable delirium group (CAM-CR > 19) and the non-probable delirium group (CAM-CR ≤

BACKGROUND: Delirium and delirium superimposed on dementia (DSD) are common complications affecting patients suffering from ongoing neurodegenerative pathologies. Peripheral surgical trauma can trigger neuroinflammation and ensuing DSD via mechanisms that remain poorly understood. Given the multifactorial therapeutic effects of neuromodulation, including vagal nerve stimulation, we have tested a minimally invasive approach to combat DSD following orthopedic surgery. METHODS: We performed orthopedic surgery on 5xFAD and CVN-AD mice and tested the efficacy of minimally invasive percutaneous vagu

INTRODUCTION: Delirium and dementia represent significant cognitive impairment (CI) challenges in older adults globally, yet undergraduate nursing students receive limited education on these conditions, particularly delirium superimposed on dementia (DSD). Mixed reality (MR) simulation using HoloPatient technology offers innovative educational opportunities, but its effectiveness has not been rigorously evaluated using mixed methods approaches. This study examined the effect of MR simulation-based education on nursing students’ knowledge of CI and attitude towards older adults with CI, and exp

Microglial activation and neuroinflammation, important aspects of neurodegeneration and accumulation of amyloid-pathology, is often exacerbated by peripheral inflammation following surgical procedures. Subsequent postoperative delirium is a predictor for long-term cognitive decline and increased rick of Alzheimer’s Disease, and perioperative strategies to reduce inflammatory responses, may be a potential avenue to mitigate postoperative complications. In this issue of Bioelectronic Medicine, Song et al. utilize percutaneous vagus nerve stimulation (pVNS) as a potential novel avenue for the att

PURPOSE: Behavioral and psychological symptoms of dementia (BPSD) and delirium superimposed on dementia (DSD) can lead to severe complications if they are not accurately identified and managed. Effective dementia care therefore requires clear differentiation, systematic assessment, and appropriate nursing interventions. This study aimed to develop VRDementia: BPSD/DSD, a virtual reality simulation program, and to qualitatively examine its validity and usability as a development-based pilot study.
METHODS: Using the ADDIE model (Analysis, Design, Development, Implementation, Evaluation), the p

BACKGROUND: We aimed to compare agitation severity in dementia with versus without delirium (DSD vs. DwD); to identify clinical factors linked to agitation severity; and to examine psychotropic prescribing patterns, including clinical correlates.
METHODS: We retrospectively analysed 1709 consecutive patients with dementia admitted to acute geriatric units, comparing those with and without delirium. Delirium and dementia were identified from discharge diagnoses documents. Agitation severity was assessed with the Pittsburgh Agitation Scale (PAS) during the first 72 h. Functional status at admis

BACKGROUND AND OBJECTIVES: People living with dementia are at higher risk of developing delirium during healthcare admission. While there have been advancements in delirium interventions, challenges persist in translating evidence-based knowledge into practice. This review aims to explore the contextual factors and underlying mechanisms of knowledge translation for preventing and managing delirium in people with dementia, and to understand how these processes contribute to intended outcomes.
RESEARCH DESIGN AND METHODS: A realist synthesis was adopted to construct context-mechanism-outcome co

BACKGROUND: Delirium superimposed on dementia is associated with poor outcomes yet remains underdetected in home settings. Current detection relies on face-to-face clinical assessment (eg, the Confusion Assessment Method criteria), which is rarely applied outside hospitals.
OBJECTIVE: This proof-of-concept study developed a theory-driven framework for detecting delirium-consistent anomalous patterns in home-dwelling people with dementia, using passive smart home sensor data.
METHODS: The Technology Integrated Health Management dataset, an open access resource comprising a clinically derived

Delirium superimposed on dementia (DSD) is a serious condition causing poor outcomes. There is a significant gap in understanding the epidemiology in community settings. This systematic review aimed to estimate the prevalence and incidence of delirium in community-dwelling people with dementia. Medline, Embase, PsychINFO, Scopus, Web of Science, CINAHL, and Cochrane databases were searched from inception to January 6, 2025. Articles published in peer-reviewed journals that focused on the prevalence, incidence, or occurrence of DSD were included. Twenty-seven studies met the inclusion criteria.

Delirium, experienced by up to 50% of hospitalised older adults, continues to be underrecognised, undertreated, and often confused with depression or dementia. Distress is a frequent and impactful symptom experienced in delirium, encompassing the individual's holistic biological or emotional response. Delirium-related distress has been shown to extend beyond the experience of the patient to include the care partners and nurses who are witnesses to delirium. The essence of delirium-related distress can be best understood through qualitative methodologies; therefore, this review explored the qua

BACKGROUND: Nearly half of hospitalized adults with dementia develop delirium superimposed on dementia (DSD), which is associated with adverse outcomes. Family caregivers play an essential role in preventing, identifying, and responding to DSD, yet relatively little is known about how caregivers experience and respond to acute cognitive changes during illness and their preferences for involvement in DSD care.
METHODS: We performed a qualitative study of caregivers (N = 22) for patients with dementia admitted to two hospitals within a single health system. Because caregivers may be engaged in

BACKGROUND: Delirium is a frequent and serious neuropsychiatric syndrome in older adults. People living with dementia are at particularly high risk of developing delirium. In nursing homes, where dementia prevalence is high, delirium superimposed on dementia (DSD) represents a major clinical challenge and research gap. Aim of the study is to produce reliable data on prevalence and incidence of DSD in nursing homes as well as to investigate risk factors, clinical course, physical activity patterns, and care processes.
METHODS: This prospective multicenter cohort study will be conducted between

Delirium Superimposed on Dementia (DSD) is a common neuropsychiatric disorder in hospitalized elderly populations with poor prognosis, which can accelerate cognitive decline and increase mortality. Despite its clinical significance, there is a lack of effective therapeutic methods in clinical practice. Mitochondrial dysfunction, characterized by impaired energy metabolism, excessive reactive oxygen species (ROS) production and neuroinflammation activation, has been suggested as a potentially critical pathogenic link in DSD. As an emerging organelle-based therapy, mitochondrial transplantation

Background/Objectives: Delirium in patients with dementia is a multifactorial condition traditionally attributed to acute medical triggers. However, emerging evidence suggests that non-biological factors, including sensory deprivation and environmental instability, may contribute to delirium vulnerability, particularly in vulnerable populations. To explore the temporal associations between non-biological and environmental factors, particularly sensory deprivation and recurrent delirium in a patient with advanced Alzheimer's disease and progressive vision loss, a narrative review was conducted.

BACKGROUND: People with dementia admitted to hospital are at risk of developing delirium. Patients with delirium superimposed on dementia (DSD) have higher mortality rates, longer hospital stays and further cognitive loss. The role of family is often recommended as a resource to inform and support how patients with dementia's needs are understood. This review focuses on ward-based interventions that enable family carers and health care professionals to work together to improve patient experience and outcomes.
AIM: To review evidence on ward-based approaches involving family (or their proxies)
