BACKGROUND: This case study examines the application of Integrated Enhanced Cognitive Behavioural Therapy (I-CBTE) for a patient with severe, longstanding anorexia nervosa and multiple comorbidities, including organic hallucinosis, complex post-traumatic stress disorder (CPTSD), and severe self-harm. Such complex presentations often result in patients falling between services, which can lead to high chronicity and increased mortality risk. Commentaries from two additional patients who have recovered from severe and longstanding anorexia nervosa are included.
CASE STUDY: The patient developed

We report a case of a 63-year-old patient with psychiatric symptoms diagnosed with coexisting DiGeorge syndrome, Fahr syndrome and Turner syndrome. To our knowledge, this is the first reported case of coexistence of DiGeorge syndrome and mosaic Turner syndrome. Basal ganglia calcification, known as Fahr syndrome, may develop in patients with DiGeorge syndrome as a consequence of calcium-phosphate balance disturbances resulting from primary hypoparathyroidism. A deletion of chromosome 22q11.2 in DiGeorge syndrome, basal ganglia calcification and, according to some research, mosaic Turner syndro

BACKGROUND: Delusional infestation is a condition at the interface of tactile and visual hallucinations and delusions. Individuals with this condition hold the fixed and false belief that their body or their environment is infested with parasites, insects or other organisms.
AIMS: There are no guidelines or publications detailing the psychological assessment, formulation, intervention and evaluation of this presentation. This paper aims to address this gap.
METHOD: Single case experimental design methodology was employed to evaluate the use of cognitive behavioural therapy (CBT) for delusion

Charles Bonnet Syndrome (CBS) typically occurs in elderly people with peripheral vision impairment, or interruptions in the connections from eyes to visual cortex. The heterogeneity of presentations and causal mechanisms warrants clinical caution. In this report, we describe a case of CBS developing in the form of Lilliputian Hallucinations in an elderly gentleman, on the background of cataract and normal pressure hydrocephalus, the complexities arising out of such aetiogenesis, and its management. We emphasize upon the fact that CBS might develop through two concurrent mechanisms in a given i

A case of a 75-year-old woman suffering from delusional parasitosis is described. The patient was referred to the consultant psychiatrist by a dermatologist with the diagnosis of psychosis. In course of the psychiatric treatment the patient revealed the delusional parasitosis and received antipsychotic treatment resulting in full remission. Delusional parasitosis (Ekbom syndrome) is a condition where a person has the unshakeable mistaken belief of being infested with parasites. Patients give detailed descriptions of the activity and offending parasites: crawling, biting, burrowing. According t

A case series of 10 patients with a diagnosis of borderline personality disorder (BPD) presenting with auditory hallucinosis is examined. In this series, the hallucinations were persistent, longstanding, and a significant source of distress and disability. Extrapolating from this series to our sample of 171 patients with BPD suggests that a form of auditory hallucinosis may occur in almost 30% of this population. The failure to emphasize this phenomenon in current systems of classification risks misdiagnosis or inappropriate treatment. Use of terms such as pseudohallucination or quasi hallucin

Charles Bonnet syndrome (CBS) is characterized by the presence of visual hallucinations in elderly, mentally healthy people. We report a visually impaired 90-year-old woman suddenly complaining of visual hallucinations, suffering from hyperthyroidism and a relative digitalis overdose. The diagnosis of CBS could be made after the exclusion of an intoxication and other neurological and psychiatric syndromes. In this case, visual hallucinations ceased without specific psychopharmacological therapy. A brief review of this organic hallucinosis, differential diagnosis, especially hyperthyroidism-ind

A.L., a 42-yr-old man has been in treatment at our out- patient clinic since 1992 suffering from psychotic manifestations following a second severe closed-head injury. The patient had his first contact with a psychiatrist 3 yr after his first severe head injury in 1962 (when 10 yr old). During his first hospitalization (in 1994) he presented with signs of affective flattening and behavioural problems and indications of borderline intellectual functioning or mild mental retardation. The patient underwent several surgical interventions. The early inter-operative period was complicated by behavio

OBJECTIVE: To gain more understanding about the relationship between human immunodeficiency virus type 1 (HIV-1) infection and new-onset psychosis, we compared clinical and immunological findings, psychiatric symptoms, global cognitive performance and, when available, computerized tomography (CT) findings between HIV-1-seropositive patients with new-onset psychosis and well-matched nonpsychotic HIV-1-seropositives.
METHODS: Two groups of subjects: HIV-1-seropositives with new-onset psychosis (n = 12) and HIV-1-seropositives without psychosis (n = 15) were recruited through outpatient departme

Psychotic symptoms presenting in late life can offer a diagnostic challenge to the clinician. In this study, 140 geriatric outpatients were prospectively examined for psychotic symptoms and assessed on a number of demographic and clinical variables. Cognition was assessed using the Mini-Mental State Exam. Psychiatric diagnoses were made by DSM-III-R criteria. Twenty-seven per cent (N = 38) had psychotic symptoms, delusions being the most common type. Patients with psychosis were significantly more likely to have a previous history of psychosis, to have a lower MMSE and to live in a nursing hom

We derived a statistical model that discriminates between substance-induced psychosis (i.e., DSM-III-R organic delusional disorder or organic hallucinosis; ODD-OH) and DSM-III-R schizophrenia in patients who have both DSM-III-R psychoactive substance use disorders (PSUD) and prominent delusions or hallucinations. A sample of 211 PSUD inpatients was divided by year of admission into data sets A and B, each of which was divided between those with concurrent schizophrenia and those with concurrent ODD-OH. A six-predictor discriminant function correctly classified 76.2 percent of all set A patient

Drug-induced psychiatric states occur frequently in PD. In the prelevodopa era, depression and other psychiatric disorders were described in PD, but in untreated patients psychosis was rare. Since the development of levodopa and other pharmacological treatments for PD, however, psychotic symptoms have become much more common (10-50%). In some individuals these problems can be more disabling than the motor features of PD and, as a result, pose a serious threat to the patient's ability to maintain independence. The drug-induced psychoses consist of several distinct psychiatric syndromes that can

We present the case reports of 11 Parkinsonian patients who developed acute psychosis under continuous dopaminergic stimulation. Psychopathologically, two of the patients mainly suffered from organic hallucinosis, while nine patients showed the clinical symptoms of delirium. The clinical course and psychopathological findings in these patients did not differ from other acute organic psychoses. However, the symptoms of these dopaminergically induced psychoses varied significantly from the psychopathological findings of paranoid schizophrenic patients who were regularly treated and evaluated in

A previously healthy elderly patient with a recent onset of macular degeneration presented for evaluation of elaborate complex visual hallucinations. The patient's psychiatric evaluation and level of cognitive functioning were normal. A diagnosis of organic hallucinosis secondary to macular degeneration was made, and the hallucinations ceased with increased sensory stimulation in the hospital. Numerous diagnoses were considered and are discussed, including Charles Bonnet hallucinations and peduncular hallucinosis.

We report 4 patients who developed organic hallucinosis while receiving opiate analgesics for cancer pain. In all patients, the symptoms rapidly responded to haloperidol and change in the type of opiate. The hallucinations were exclusively visual. Cognitive status, determined by the Mini-mental State Questionnaire (MMSQ), was normal in all patients. Organic hallucinosis occasionally occurs in patients receiving opiates and can be a major source of distress for patients and their families.

A first comprehensive description of the clinical features of patients with the rare diagnosis of organic hallucinosis (OH) is presented, based on information from 11 OH patients among 14,889 patients who presented for evaluation over a 5-year period at our institution. This characterization is of particular current relevance to diagnosticians and clinicians because of the proposed major changes in the diagnostic system for OH in the upcoming DSM-IV and because of the virtual total lack of published information concerning this syndrome. This description includes a listing of the prevalence and

We present a clinical case of a 43-year-old female with progressive systemic sclerosis, Basedow's disease, atrial fibrillation and diabetes mellitus. She twice developed exacerbations of a psychotic state. She showed a depressive state followed by a paranoic hallucinatory state and stupor. Electroencephalograms revealed slow abnormal records both in psychotic and remitting periods and 123I IMP SPECT of the brain showed a low blood flow in the psychotic stage. A case of progressive diffuse sclerosis with a paranoic hallucinatory state is rarely reported. As her Basedow's disease had been well c
