Actinomycetal infections by Actinomyces, Nocardia, and Streptomyces appear to be increasing in incidence. Clinical and laboratory data from twelve patients believed to have subclinical actinomycete-streptomycete infections (ASI)* are presented. A variety of clinical forms of actinomycosis, difficulties of its early diagnosis and particularly of the differential diagnosis between this condition and tuberculosis, nocardiosis, aspergillosis necessitate the development of present-day methods for the early laboratory diagnosis of actinomycosis. Basing on the data on present-day methods for isolation of anaerobic and microaerobic diphtheroids, the authors characterize the bacterial associations occurring in maxillofacial actinomycosis and other odontogenic processes, discuss problems in isolation and identification of Actinomyces cultures.

We evaluated the histopathologic findings in 15 cases of panniculitis due to infection. Organisms were identified by special staining in 14 cases, and in 6 of these the etiologic agent was confirmed by culture studies. In one additional case, lesional culture was positive despite negative special staining. This study compares the in vitro ultrastructure of Streptomyces somaliensis with the grain formation seen in vivo. Cultured forms were cocco-bacillary with thick cell walls and septa. Individual cells stained with PAS.TCH and Alcian blue. Grains were largely composed of an electron-dense fibrillar matrix surrounding clear areas, some of which contained organisms. The ultrastructural appearances of the latter were not significantly different from those seen in vitro although the cell wall was less well defined and did not stain with Alcian blue, and there were more intracellular lipid droplets.

This paper describes the relationship between different forms of nocardia infection involving the skin. The best recognized of these conditions are systemic nocardiosis and actinomycetoma due to Nocardia spp. in which the organisms are present in either filamentous or grain form respectively. Pulmonary mycetomas (fungus balls) caused by mucormycosis and aspergillus developed in 2 patients after aspiration pneumonia and septic lung infarction.
