Previous research has indicated that ability-oriented organicity tests are of limited value in the separation of brain-damaged from schizophrenic patients in psychiatric hospitals. While mania usually occurs as a phase of manic-depressive disease, it can occur in association with organic dysfunction--medical and pharmacological--in patients with no history of affective disorder. In reviewing the literature, we have found that mania occurs secondary to drugs, infection, neoplasm, epilepsy, and metabolic disturbances. These cases are best considered secondary manias. A case report is presented of a 62-year-old female admitted to a general hospital with a provisional diagnosis of mania and organic brain syndrome, whose unusual behavior and history prompted evaluation using computed tomography of her brain. Bilateral frontal lobe cystic lesions were discovered as a result of a bilateral transorbital leukotomy performed 21 years earlier. CT scan is presented, demonstrating the extent of these lesions.

Lithium was used to treat the manic symptoms of affective disorder in three patients with associated organic brain syndromes. In all three cases neuropsychological testing permitted evaluation of the degree of dysfunction during the acute and follow-up periods of treatment. Lithium was effective in controlling the manic symptoms of all three patients, and there was no evidence that it caused further organic deterioration.

A study of the heredity and exogenic factors in 340 patients with manic-depressive psychosis makes it possible to draw a conclusion that the disorder is polyetiological. In a bipolar development there is mainly a hereditary loading and organic noxious factors, in a monopolar depression--exhaustive factors. The assumption of an existence of 2 independent psychoses (bipolar and monopolar) which was made by several foreign authors was not confirmed.
