Long-Term Outcomes in Adult Patients with Tick-Borne Encephalitis in Latvia.

BACKGROUND: Tick-borne encephalitis (TBE) is an endemic neuroinfectious disease prevalent in parts of Europe and is often associated with persistent neurological and cognitive sequelae. The aim of this study was to evaluate the long-term outcomes and predictors of post-encephalitic sequelae in adult patients with TBE in Latvia. METHODS: A retrospective cohort with prospective follow-up was used that included 105 adult patients hospitalized with laboratory-confirmed TBE between 2018 and 2024. The patients' clinical and demographic data were extracted from medical records, and reassessments wer

Complications and long-term sequelae of tick-borne encephalitis: association with age and other predictors - a multicenter observational study.

Tick-borne encephalitis (TBE) is the most common non-purulent neuroinfection in Central Europe and a major public health concern in the Czech Republic, where vaccination coverage remains low. Even after recovery from the acute disease, many patients experience long-term sequelae known as postencephalitic syndrome (PES). This prospective multicentre study examined predictors of acute disease severity and long-term outcomes, including motor deficits and other forms of PES. A total of 209 patients with laboratory-confirmed TBE were enrolled at four Czech centres in 2023 and 2024; 187 completed si

Postencephalitic syndrome with immune-mediated psychosis in an adult with meningitis due to Streptococcus pneumoniae: A case report.

INTRODUCTION: A first psychotic episode may be related to neurological diseases, especially encephalitis of infectious or autoimmune origin. It is remarkable that an immune-mediated encephalitis triggered by a confirmed subacute bacterial meningitis is documented, and this is the case we will present. CLINICAL CASE: A 22-year-old woman with no previous medical history, immunocompetent, with three months of behavioral, affective and cognitive symptoms with subsequent compromise of sensory perception and psychosis. Examination of cerebrospinal fluid showed inflammatory signs with positive FilmA

Long-term neurological and neurocognitive impairments after tick-borne encephalitis in Lithuania - a prospective study.

BACKGROUND: The aim of this study was to characterise long-term neurological and neurocognitive sequelae after tick-borne encephalitis (TBE) in adults. METHODS: 98 prospective consecutive TBE patients, classified by disease severity, were included. Immediate outcomes were evaluated with Glasgow Outcome Scale (GOS) and Rankin Scale (RS). After 6 and 18 months, long-term disability was evaluated using Modified Rankin Scale (MRS) and neurocognitive assessment was performed with Matrics Consensus Cognitive Battery (MCCB), measuring processing speed, attention/vigilance, working memory, verbal lea

Progressive Course of Chronic Tick-Borne Encephalitis Manifesting as Amyotrophic Lateral Sclerosis-like Syndrome 35 Years after the Acute Disease.

The chronic form of tick-borne encephalitis (TBE) is understudied and seems to be linked exclusively to Siberian and Far Eastern TBE virus (TBEV) subtypes. There are limited clinical descriptions demonstrating that chronic TBE can resemble an amyotrophic lateral sclerosis (ALS)-like disease. Here, we present a description of a clinical case of progressive chronic TBEV infection with a relapse 35 years after the initial acute infection following a tick bite. The disease manifested as an ALS-like syndrome with bulbar signs, progressive muscle weakness and atrophy, decreased reflexes, and eventua

Inflammatory Immune Responses in Patients with Tick-Borne Encephalitis: Dynamics and Association with the Outcome of the Disease.

Information on the association of inflammatory immune responses and disease outcome after tick-borne encephalitis (TBE) is limited. In the present study, we assessed the levels of 24 cytokines/chemokines associated with innate and adaptive immune responses in matched serum and cerebrospinal fluid (CSF) samples of 81 patients at first visit, and in serum at follow-up time points. Serum levels of several cytokines/chemokines obtained during the meningoencephalitic phase of TBE differed compared to the levels at a follow-up visit 2 months later; several significant differences were also found in

Gabrielle Lévy and the Roussy-Lévy syndrome.

In 1934, Gabrielle Lévy died at the age of 48. She became well known for an article she published on a hereditary polyneuropathy in cooperation with Gustav Roussy, resulting in the eponym Roussy-Lévy syndrome. Not much is known about this extraordinary neurologist/neuropathologist. Her family declared that she died from the disease she was studying. She was a pupil of Pierre Marie, with whom she worked at the Salpêtrière in Paris and wrote on war neurology. In cooperation with Marie, she published a number of articles on postencephalitic syndromes, which also became the subject of her 1922 the

The long-term outcome of tick-borne encephalitis in Central Europe.

BACKGROUND: Information on the long-term outcome of tick-borne encephalitis (TBE) is limited. OBJECTIVES: To assess the frequency and severity of post-encephalitic syndrome (PES) at different time points after TBE, and to determine the parameters associated with unfavourable outcome. METHODS: Adult patients diagnosed with TBE in Slovenia in the period 2007-2012 were followed-up for 12 months and also examined 2-7 years after TBE. Each patient was asked to refer a person of similar age without a history of TBE to serve as control. RESULTS: A total of 420 patients and 295 control persons part

Alternative causes of hypopituitarism: traumatic brain injury, cranial irradiation, and infections.

Hypopituitarism often remains unrecognized due to subtle clinical manifestations. Anterior pituitary hormone deficiencies may present as isolated or multiple and may be transient or permanent. Traumatic brain injury (TBI) is recognized as a risk factor for hypopituitarism, most frequently presenting with isolated growth hormone deficiency (GHD). Data analysis shows that about 15% of patients with TBI have some degree of hypopituitarism which if not recognized may be mistakenly ascribed to persistent neurologic injury and cognitive impairment. Identification of predictors for hypopituitarism af

Tick-borne encephalitis: a retrospective study of clinical cases in Bornholm, Denmark.

Tick-borne encephalitis (TBE) is caused by a flavivirus, tick-borne encephalitis virus (TBEV). For decades TBE has been known to cause disease in Bornholm, a Danish island. A retrospective study was performed to identify undiagnosed cases of TBE among patients hospitalized with signs and symptoms of meningitis or encephalitis in the 5 y period 1994-1999. This investigation revealed 5 new, and initially undiagnosed cases of TBE. In total, 14 cases of TBE were found in the 7 y period 1994-2000; 2 patients were tourists and 12 cases were inhabitants in Bornholm, giving an incidence of 3.81 per 10

[Late sequelae of early summer meningoencephalitis].

The incidence of tick-borne encephalitis varies widely in different geographic regions due to local difference in the rate of infected vectors (Ixodes ricinus) transmitting tick-borne encephalitis virus. In the Cantonal Hospital Winterthur a large number of cases are hospitalised due to endemic areas with infected ticks nearby. From 1976 until 1996 132 patients with tick-borne encephalitis were hospitalised (an average of 7 patients per year), compared with 535 cases (41 per year) in the whole of Switzerland during a similar period (1984-1992). While previously tick-borne encephalitis was cons

[Active vaccination against early summer meningoencephalitis].

TBE is a rather rare disease (incidence in Switzerland 0.46/1,000,000 population per year). Acquisition occurs only in well known endemic areas through bites of infected ticks. The course of the disease is not trivial: 0.5-2% of cases are fatal, 2.7% have persistent serious paralysis and some 33% a postencephalitic syndrome. There is no specific therapy. An active vaccine is commercially available which is reliably immunogenic (99% of vaccinated persons develop protective antibodies) and involve only a few mild side effects. Mass vaccination seems not to be cost-effective but would be epidemio

A 10-year follow-up study of tick-borne encephalitis in the Stockholm area and a review of the literature: need for a vaccination strategy.

143 people treated for tick-borne encephalitis (TBE) were included in a retrospective follow-up study. Sequelae and epidemiological characteristics in 114 individuals were analysed. The case fatality rate and the prevalence of residual paresis were low, 1.4 and 2.7%, respectively. However, 40 (35.7%) individuals were found to have a postencephalitic syndrome after a median follow-up time of 47 months, and a majority (77.5%) of these were classified as moderate to severe. Various mental disorders, balance and co-ordination disorders and headache were the most frequently reported symptoms. Incre

Chronic intestinal pseudoobstruction as a possible sequel to encephalitis.

A patient is reported who presented with a typical intestinal pseudoobstruction syndrome. Before this illness, the patient had suffered from measles encephalitis at the age of 15 months. A postencephalitic syndrome was present which included severe mental retardation, parkinsonism, and epilepsy. There were no relatives with a similar disease. Clinically, there was a frank recurrent pseudoobstruction syndrome with occasional diarrhea. Radiographic barium studies showed delayed transit, hypomotility of the intestines, and gross distension of the esophagus, stomach, small intestine, and colon. Hi

The human Klüver-Bucy syndrome.

Twelve patients with the Klüver-Bucy syndrome (KBS) are described. The syndrome occurred in head trauma, Alzheimer's disease, Pick's disease, and following herpes encephalitis. KBS was transient after head trauma but was a persistent feature of the postencephalitic syndrome. In all cases KBS was combined with aphasia, amnesia, or dementia. Human KBS resembles the monkey syndrome, but in humans there is a more elaborate complex of behavioral disturbances. The behavioral manifestations are produced by bilateral temporal lobe dysfunction. Partial expression of the syndrome may have localizing val
