Nocardia antimicrobial susceptibility testing by microdilution: evaluation of ready-to-use plate including tedizolid and meropenem.

PURPOSE: Nocardiosis is a rare but serious infection occurring predominantly in immunocompromised patients. The CLSI microdilution antimicrobial susceptibility testing is recommended for all nocardioses. Ready-to-use plates are available (Sensititre™ RAPMYCOI), but antimicrobials of interest (e.g. tedizolid, meropenem) are lacking. At the request of the French nocardiosis reference medical biology laboratory, a plate including meropenem and tedizolid was produced (Sensititre™ FRNOCAR1). METHODS: FRNOCAR1 was compared to RAPMYCOI using 133 clinical Nocardia isolates containing predominantly N.

Disseminated Trimethoprim/Sulfamethoxazole-Resistant Nocardiosis in Renal Transplant Recipient, India.

A renal transplant recipient in India receiving trimethoprim/sulfamethoxazole prophylaxis experienced fever, hemoptysis, and neurologic deficits. The diagnosis was pulmonary aspergillosis and disseminated nocardiosis caused by trimethoprim/sulfamethoxazole-resistant Nocardia. Antimicrobial drug therapy led to complete clinical and radiological resolution after 6 months. This case highlights a resistant breakthrough Nocardia pathogen in a transplant recipient.

Nocardiosis: death from the ground.

Nocardiosis is a rare but potentially life-threatening infection caused by aerobic, filamentous, gram-positive bacteria of the genus Nocardia found in soil and water. Although traditionally found in immunocompromised patients, up to one-third of cases occur in immunocompetent individuals, contributing to frequent diagnostic delays. Historically fatal before the antibiotic era, outcomes have substantially improved with modern antimicrobial therapy and diagnostic advances. However, nocardiosis remains underrecognized due to its nonspecific presentation and ability to mimic more common diseases.

Chronic infectious keratitis: A review of etiopathogenesis, clinical presentation and management challenges.

Chronic infectious keratitis represents a distinct clinical entity characterized by indolent progression, diagnostic difficulty, and prolonged therapeutic courses. Unlike acute bacterial keratitis, chronic infections often present with subtle inflammatory signs, recurrent episodes, or poor response to empirical antimicrobial therapy. Among the most important causes are herpetic keratitis, acanthamoeba keratitis, Microsporidial stromal keratitis, and atypical (non-tuberculous) mycobacterial keratitis. Herpes simplex virus keratitis may manifest as recurrent epithelial disease or immune-mediated

Case Report: Multiple brain abscess-like lesions with concurrent detection of Nocardia amamiensis and Leishmania infantum in cerebrospinal fluid.

Central nervous system nocardiosis is rare and potentially life-threatening, and human infection due to Nocardia amamiensis has seldom been reported; central nervous system involvement in visceral leishmaniasis is likewise uncommon. We report a 69-year-old man who developed progressive impairment of consciousness and coma after recurrent fever. Brain magnetic resonance imaging revealed multiple abscess-like lesions in both cerebral hemispheres, the cerebellum, and the brainstem, accompanied by acute or subacute cerebral infarctions. The cerebrospinal fluid opening pressure was 300 mmH2O. The w

When metagenomic next-generation sequencing detects Nocardia: from clinical interpretation to immune-based clinical management.

BACKGROUND: Metagenomic next-generation sequencing (mNGS) increasingly detects Nocardia, but interpreting positive results remains challenging. We aimed to identify host factors and read thresholds that distinguish nocardiosis (ND) from colonization/contamination, and to characterize clinical features linked to immune status. METHODS: A retrospective analysis from 74 patients at Qilu Hospital of Shandong University from April 2022 to October 2025 was conducted. We assessed mNGS for distinguishing Nocardia infection from colonization or contamination, identified risk factors for nocardiosis, a

Advanced Eumycetoma of the Right Foot Resulting in Below-Knee Amputation in Somalia: A Case Report.

INTRODUCTION: Mycetoma is a chronic, progressively destructive neglected tropical disease that presents with localised swelling, multiple sinus tracts and a discharge containing grains, and that may extend into deep tissue and bone. Somalia lies within the recognised mycetoma belt, but the national burden is poorly documented, and few patients reach specialist care early enough for the limb to be saved. CASE PRESENTATION: A 33-year-old man from Qoryooley District, Lower Shabelle, Somalia, presented with a seven-year history of progressive swelling of the right foot. Examination showed massive

Functional Annotation Identifies 3-Oxoacyl Synthase III as a Druggable Target in Nocardia asteroides FDAARGOS1485: A Multi-Tier Structure-Guided Computational Discovery of Marine-Derived Therapeutic Leads.

Nocardiosis is a global life-threatening disease that predominantly affects immunodeficient populations. This disease is caused by the nosocomial pathogen Nocardia asteroides. Existing therapeutics offer several limitations and challenges, highlighting the need for novel druggable targets. The pathogenic strain FDAARGOS1485 contains 6,521 proteins and has a higher completeness of 98%, as determined by CheckM analysis. Among these proteins, 863 (13.20%) hypothetical proteins (HPs) were designated as targets. The functional annotation pipeline identified 30 (4.40%) HPs, classified into 24 enzyme

Pulmonary nocardiosis in patients with underlying pulmonary alveolar proteinosis: a case series and review of the literature.

OBJECTIVES: Pulmonary alveolar proteinosis (PAP) and pulmonary nocardiosis are relatively rare, their co-occurrence constitutes a clinically important comorbidity. Nocardia infection exhibits variable clinical manifestations, and carries a substantial risk of mortality in the absence of timely, pathogen-directed antimicrobial therapy. This study aims to summarize the clinical features of Nocardia infection in PAP patients. CASE PRESENTATION: We present four confirmed cases of PAP complicated by Nocardia infection. These cases illustrate the distinct microbial species, disease courses and clin

Outbreaks of Granulomatous Disease Associated With Vibrio harveyi Infection in Grouper (Epinephelus fuscoguttatus × E. lanceolatus) Aquaculture in Taiwan.

Between 2015 and 2024, outbreaks of muscular and visceral granulomatous disease, resulting in moderate-to-severe mortality in farmed groupers, occurred in southern Taiwan. Initially, affected groupers were screened for granuloma-associated pathogens, including Mycobacterium spp., Nocardia spp., Photobacterium damselae and Edwardsiella spp., using PCR methods at local animal disease prevention and control centres. Twenty-five hybrid groupers (Epinephelus fuscoguttatus × E. lanceolatus) that tested negative for these pathogens were submitted to our laboratory for further examination. Infected fi

Laryngeal Nocardia: A Case Report.

Although malignancy often leads the differential for patients with obstructive laryngeal masses, infectious etiologies should also be considered. This case report describes a rare presentation of laryngeal Nocardiosis in an immunosuppressed patient with biopsy findings not suggestive of malignancy. Early consideration of infectious etiologies can reduce delays in treatment, prevent airway compromise, and minimize patient mortality.

A qPCR identification scheme to differentiate actinomycetoma from eumycetoma.

BACKGROUND: Mycetoma is a Neglected Tropical Disease of the skin. It is characterized by large tumorous lesions in the subcutaneous tissue which hinder the patients in their daily activities. The disease can be caused by either bacteria (actinomycetoma) or fungi (eumycetoma), and treatment depends on the entity. Therefore, in 2022, a Target Product Profile was published by the World Health Organisation in which it was stated that there is an urgent need to develop a diagnostic tool that can differentiate the two reliably within a day. OBJECTIVES: To develop a qPCR that can not only differenti

Comparative Evaluation and Optimization of a CRISPR/Cas-Based Assay for Rapid Detection of Tilapinevirus tilapiae (TiLV) in Indian Isolates, Validated Against Conventional Nucleic acid Detection Methods.

We report a 28-year-old-female who presented with primary amenorrhoea, absence of puberty, obesity and normal stature. The subject was clearly short as a child, with a height more than 2 SD below normal until the age of 15 years. The pubertal growth spurt failed to develop. She continued growing at a prepubertal rate until growth ceased at the age of 20 years, reaching her final adult height of 157 cm (SDS -0.86) without hormonal treatment. A combined pituitary hormone stimulation test of anterior pituitary function showed deficiencies of GH, LH and FSH, and low normal serum levels of TSH and

Fractal and Machine Learning Analyses of MALDI-TOF Mass Spectrometry Data in Glioblastoma.

Data preprocessing is a critical step in the analysis of matrix-assisted laser desorption/ionization-time-of-flight mass spectrometry (MALDI-TOF MS) spectra for machine learning applications, typically involving steps such as spectra trimming, baseline correction, smoothing, transformation, and peak picking or spectral binning. While traditional approaches focus on protein/peptide peaks as features, this study explores a novel method of feature extraction by treating MALDI-TOF spectra as one-dimensional signal array further processed as time-series data. This study investigates the use of comp

Ensemble machine learning-based scaling up of wetland carbon dynamics across the Southeastern U.S.

BACKGROUND: Although the Southeastern United States (SE US) has been identified as a major terrestrial carbon sink, the balance of CO2 and CH4 fluxes from its wetlands remains poorly constrained due to strong environmental heterogeneity and limited regional-scale assessments. In this study, we used observations from 24 wetland sites located in the SE US, including freshwater and salt marshes, to train machine learning models (Random Forest, XGBoost, Support Vector Machine, Artificial Neural Network, and Partial Least Squares Regression) to predict monthly mean gross primary productivity (GPP),

Brain Abscess Caused by Nocardia sp. in an HIV Patient: A Case Report Highlighting Challenges and Treatment Success.

BACKGROUND: Nocardia brain abscess is a rare opportunistic infection in people living with HIV at an advanced stage. Immunosuppression, especially with very low CD4 counts and non-adherence to treatment, increases the risk. These infections are diagnostically challenging due to their nonspecific presentation and the limited sensitivity of routine molecular panels in detecting Nocardia. CASE PRESENTATION: We present a 57-year-old HIV-positive female who developed a brain abscess after discontinuing antiretroviral therapy (ART) in 2019. She presented in 2023 with progressive headaches, fever, a
