This article provides an overview of the neglected tropical diseases caused by fungi, which include mycetoma, chromoblastomycosis and sporotrichosis. These are chronic infections of the skin and subcutaneous tissue that can be locally destructive, leading to disfigurement and disability. Management of these conditions is challenging, often requiring prolonged antimicrobial therapy in combination with surgical treatment.

BACKGROUND: Fever accompanied by disseminated lymphadenopathy presents a significant diagnostic challenge. Our prior clinical observations indicated a high detection rate of Nocardia farcinica (N. farcinica) deoxyribonucleic acid (DNA) in patients with this presentation. Kikuchi-Fujimoto disease (KFD) is a common cause in young adults with fever and lymphadenopathy, yet its etiological drivers remain largely unknown.
OBJECTIVE: This dual-center study aimed to investigate the prevalence of N. farcinica infection in a pathologically confirmed KFD cohort, to analyze its correlation with clinical

Nocardia seriolae is the predominant causative agent of piscine nocardiosis, exhibiting broad piscine host specificity along with prolonged latency periods and high mortality rates, thereby posing a severe threat to global fish health. However, reliably effective control measures remain limited. Subunit vaccines, derived from virulence factors or immunodominant proteins of the pathogen, are widely acknowledged as one of the most secure and potent strategies for mitigating disease outbreaks. Here we focused on five T7SS gene cluster proteins (MTAP, HP05, HP23, HP24, and VST) and the dominant N.

Pulmonary sequestration is a congenital lung malformation involving the systemic arterial blood. Infection is a common complication; however, Nocardia infection is rare. We report the case of a 55-year-old immunocompetent male with recurrent infection of a pulmonary cyst in the left lower lobe. Despite the use of empirical antibiotics, the inflammation recurred. Contrast-enhanced computed tomography confirmed pulmonary sequestration, and bronchoscopy revealed Nocardia cyriacigeorgica. Although trimethoprim-sulfamethoxazole was effective, it was discontinued because of severe drug-induced rash.

BACKGROUND: Nocardia spp. can cause a range of clinical syndromes in immunocompromised individuals, such as solid organ transplant recipients, haematopoietic stem cell transplant recipients, and people with HIV (PWH) with advanced HIV disease. Owing to its rarity as an opportunistic infection, its nonspecific clinical presentation, and the difficulties in establishing a definitive diagnosis, clinicians often face challenges in recognizing this infection. Furthermore, the limited literature and the lack of evidence-based guidelines leave clinicians without clear indications in the management of

A 79-year-old man in the United States with large B-cell lymphoma and chronic obstructive pulmonary disease had disseminated Nocardia ignorata infection involving the brain and spleen. Despite antimicrobial therapy, he died from complications. This rare manifestation highlights the need to consider Nocardia in immunocompromised patients with central nervous system and abdominal lesions.

This article reports the clinical, diagnostic, and therapeutic aspects of a rare case of cutaneous and subcutaneous scedosporiosis in a 16-year-old domestic cat. The animal presented with a painful, ulcerated nodule at the base of the tail. Histopathology and fungal culture confirmed a pyogranulomatous dermatitis with hyaline hyphae, and molecular analysis identified Scedosporium sphaerospermum, a soil-dwelling fungus newly described. Despite targeted antifungal treatment with itraconazole and topical ciclopirox olamine, the cat developed systemic complications and died. This case emphasizes t

INTRODUCTION: Tedizolid, a second-generation oxazolidinone, exhibits potent in vitro activity against Gram-positive bacteria, including Nocardia species, and has a more favourable safety profile than linezolid during prolonged use. However, data on itsCSF penetration and efficacy remain scarce. We describe two cases of Nocardia farcinica brain abscess treated with tedizolid and report measured serum and cerebrospinal fluid (CSF) exposures.
CASE REPORTS: Two patients with N. farcinica brain abscesses (MIC for tedizolid 0.75 mg/L) treated with tedizolid as part of combination therapy. Total and

Disseminated nocardiosis with endogenous endophthalmitis is a rare but life-threatening condition associated with significant morbidity and risk of permanent vision loss. We report a fatal case of bilateral chorioretinitis with unilateral endogenous endophthalmitis accompanied by soft tissue and central nervous system involvement caused by Nocardia farcinica. An 86-year-old woman presented with a five-day history of a progressively worsening blind spot in the left eye, and ophthalmic examination revealed bilateral chorioretinal lesions with marked anterior chamber and vitreous inflammation in

BACKGROUND: For decades, poor Sudanese communities in rural areas have silently battled mycetoma, a stigmatizing, disabling, neglected tropical disease. Its linkage to specific, ecology-dependent livelihoods such as farming, woodcutting and collecting and animal herding has obscured its cause and environmental and structural dimensions at both the local and global levels. This ethnographic study challenges the prevailing singular focus on the injuries that have dominated biomedical reasoning for years. METHODS: We deployed an ethnographic approach utilizing participant observation, in-depth in

BACKGROUND: Nocardia farcinica is a rare opportunistic pathogen predominantly affecting immunocompromised hosts. Infectious arthritis, cutaneous and deep soft tissue infections caused by this organism often present with nonspecific clinical manifestations. Additionally, due to its slow-growing and oligotrophic nature, both cultivation and identification pose considerable challenges, thereby complicating clinical diagnosis and management.
CASE PRESENTATION: This case report described an elderly female patient presenting with right shoulder redness, swelling, and pain. Her history included diab

BACKGROUND: Immunocompromised patients are at risk of disseminated nocardiosis, which can involve the central nervous system. The factors mediating dissemination to the central nervous system in this patient population have not been fully characterized. This study aimed to identify patient and pathogen-related variables associated with a higher risk of central nervous system infection among immunocompromised patients with nocardiosis.
METHODS: Retrospective single-center analysis of all immunocompromised patients diagnosed with nocardiosis between 2013 and 2024. Patients without brain imaging

Mycetoma is a neglected tropical disease characterised by chronic, granulomatous inflammation of the subcutaneous tissues, often leading to disfigurement, disability, and significant socioeconomic burdens. Caused by a diverse array of bacterial and fungal pathogens, eumycetoma is predominantly driven by Madurella mycetomatis, and current treatment strategies are limited and often ineffective. Conventional antifungal therapies, such as itraconazole, require prolonged administration, frequently combined with surgical interventions, yet cure rates remain suboptimal, and recurrence is common. Nocardia species are aerobic, filamentous, weakly acid-fast, Gram-positive bacteria. They cause opportunistic infections, particularly in patients receiving chemotherapy or corticosteroids. Otologic involvement is extremely rare, with only a few cases of Nocardia otomastoiditis reported worldwide. We report a 31-year-old woman with gastric and pancreatic diffuse large B-cell lymphoma who presented with fever and a one-week history of worsening purulent discharge from the left ear during chemotherapy-induced neutropenia. She had a 15-year history of intermittent otorrhea, previously treated wi
