Immunological Profile and Markers of Endothelial Dysfunction in Elderly Patients with Cognitive Impairments.

The process of aging is accompanied by a dynamic restructuring of the immune response, a phenomenon known as immunosenescence. Further, damage to the endothelium can be both a cause and a consequence of many diseases, especially in elderly people. The purpose of this study was to carry out immunological and biochemical profiling of elderly people with acute ischemic stroke (AIS), chronic cerebral circulation insufficiency (CCCI), prediabetes or newly diagnosed type II diabetes mellitus (DM), and subcortical ischemic vascular dementia (SIVD). Socio-demographic, lifestyle, and cognitive data wer

Psychiatric Symptoms in Wilson's Disease-Consequence of ATP7B Gene Mutations or Just Coincidence?-Possible Causal Cascades and Molecular Pathways.

Wilson's disease (WD) is an autosomal recessive disorder of copper metabolism. The genetic defect in WD affects the ATP7B gene, which encodes the ATP7B transmembrane protein, which is essential for maintaining normal copper homeostasis in the body. It is primarily expressed in the liver and acts by incorporating copper into ceruloplasmin (Cp), the major copper transport protein in the blood. In conditions of excess copper, ATP7B transports it to bile for excretion. Mutations in ATP7B lead to impaired ATP7B function, resulting in copper accumulation in hepatocytes leading to their damage. The t

The occurrence of comorbidities with affective and anxiety disorders among older people with intellectual disability compared with the general population: a register study.

BACKGROUND: Little is known regarding the burden of comorbidities among older people with intellectual disability (ID) who have affective and anxiety disorders. Therefore, we aimed to investigate the occurrence and risk of psychiatric and somatic comorbidities with affective and/or anxiety disorders in older people with ID compared to the general population. METHODS: This population study was based on three Swedish national registers over 11 years (2002-2012). The ID group was identified in the LSS register, which comprises of data on measures in accordance with the Act Concerning Support and

Understanding delirium trajectory and its importance in care provision for older people.

Delirium significantly increases morbidity and mortality in older people, especially those affected by other organic disorders, notably dementia (Siddiqi et al., 2006; Davis et al., 2012; Martins and Fernandes, 2012). Both delirium and dementia are characterized by cognitive decline through disintegration of brain functions, i.e. a "brain failure." Delirium has been described as an acute brain failure, in contrast to dementia being a chronic brain failure (Berrios, 1981). If we consider any other organ failure, for example that of kidneys, delirium superimposed on dementia resembles acute rena

[Very low magnesium levels in red blood cells as a significant factor in the etiopathogenesis of borderline disorders].

AIM: Basic studies indicate magnesium deficiency as one of the important, but often neglected, risk factors aggravating the course of borderline disorders (BD). A clinical verification of this notion has been conducted. MATERIAL AND METHODS: Authors studied 62 patients with BD, aged 25-65 years, of inpatient and outpatient settings. Contents of magnesium and other blood electrolytes were determined. RESULTS: Authors found an extremely high prevalence of very low levels of magnesium (Mg) in erythrocytes (<0.3 mmol/l) in patients with BD compared to controls (patients without BD, Mg (er.) 1.62

Differences in peripheral oxidative stress markers in Alzheimer's disease, vascular dementia and mixed dementia patients.

AIMS: We determined whether the possible roles of oxidative stress differ in the pathophysiology and cognitive decline of Alzheimer's disease (AD), vascular dementia (VaD) and mixed Alzheimer's/vascular dementia (MD). METHODS: We measured the levels of diacron reactive oxygen metabolite (dROM), reflecting the amount of organic hydroperoxides, and biological anti-oxidant potential (BAP), measuring the ferric reducing ability of blood plasma, in 72 patients with AD, 27 with VaD, 24 with MD and 53 non-demented outpatients (control group). In addition, endogenous plasma anti-oxidants, such as alb

[Brain concussion--a minor craniocerebral injury].

INTRODUCTION: Brain concussion is a brain dysfunction without any macroscopic structural damage, caused by mechanical force. This research paper presents the occurrence and basic characteristics of patients with brain concussion without skull fracture. The second aim of this paper is to answer questions, related to this problem, neurosurgeons are most often asked by doctors of other specialties. MATERIAL AND METHODS: Posttraumatic amnesia (patient unable to remember events before and/or after injury) was a condition to diagnose the brain concussion. In 1995 there were 240 patients with brain

Neuropsychological alterations in patients with computed tomography-detected basal ganglia calcification.

OBJECTIVE: To investigate the cognitive and mental status of patients with basal ganglia calcification on a computed tomographic scan. DESIGN: Eighteen ambulatory patients with basal ganglia calcification and without other radiological findings who were identified from the computed tomography records of a general hospital in a 2-year period and 16 control subjects who were matched for age, education, sex, and premorbid IQ estimation consented to participate. All subjects underwent a neurological evaluation, a comprehensive neuropsychological battery, and tests with psychiatric rating scales.

[Electroencephalographic findings in dementia diseases].

Even if generally the EEG cannot yield specific pointers that are typical for a special pattern of dementia, it is nevertheless meaningful and helpful to perform routine EEG in demented patients. EEG is the functional additional examination that does not place any stress on the patient and is easy to perform at a low cost (1). As a functional diagram the EEG reflects the electrical processes taking place in the cerebral cortex, yielding an excellent image of the cerebral functional state. Assessment of the EEG is done in accordance with internationally standardised criteria. Particular attenti

Performance on tests sensitive to frontal lobe lesions by patients with organic amnesia: Leng & Parkin revisited.

Groups of amnesics with aetiologies that included chronic alcoholism, encephalitis and ruptured anterior communicating artery aneurysm (ACoAA) were examined on the Cognitive Estimation Test (CET), FAS Word Fluency Test (FAS) and the full and Nelson (1976) versions of the Wisconsin Card Sorting Test (WCST). The alcoholic amnesics were impaired on all four tests, whereas the post-encephalitic amnesics were impaired on the FAS and CET but performed normally on both versions of the WCST. The ACoAA amnesics were impaired on both the FAS and the CET, and scored at a level in between the other amnesi

Performance of inattentive-overactive children on selected measures of prefrontal-type function.

Several theorists have suggested that childhood inattention-overactivity ("attention deficit disorder," "hyperactivity") may arise from a deficit in the inhibitory mechanisms of the prefrontal cortex. Accordingly, it was hypothesized that inattentive-overactive children would exhibit prefrontal-type deficits on several relevant neuropsychological measures. Subjects were 21 elementary school pupils who had been referred for disruptive behavior problems and who had been rated as high in inattention-overactivity. Controls were 26 age-matched normal children from the same school. It was found that

Pathological display of affect in patients with depression and right frontal brain damage. An alternative mechanism.

Two patients are reported with the acute onset of pathological crying following right inferior frontal brain damage. Both had severe endogenous depression and neither had pseudobulbar palsy. These and other cases argue that two organic brain diseases--one structural and the other "physiopharmacological"--may interact to produce pathological display of affect that cannot be accounted for by traditional neurological explanations. A pharmacological mechanism for the rapid amelioration of pathological affect by tricyclic medications and its possible relationship to the newly discovered descending

Neurophysiological and psychological disorders and occupational exposure to organic solvents.

A number of reports, particularly from Scandinavian countries, claim that painters and workers in other trades in which prolonged occupational exposure to organic solvents may occur develop a type of mental illness characterized principally by impairment of memory and co-ordination and some deterioration of personality. The condition, called 'organic solvent disease', is recognized as a cause of premature retirement and is classed as an occupational disease in certain countries. The conclusions of these reports have been contested and the existence of such a disease entity has been questioned.

The diagnosis of brain-damage by the MMPI: a comprehensive evaluation.

A number of research studies have proposed various methods for using the MMPI to identify brain dysfunction. This previous research has taken one of three major approaches. The present study compared the major approaches in a population consisting of 30 schizophrenic, 30 brain-damaged and 30 hospitalized normal patients. The results indicated that the most effective diagnosis device was the use of the Sc scale alone or in conjunction with the remaining clinical scales and the F scale. None of the organic scales or keys were able to match the performance of the Sc scale alone. The poor results

The clinical utility of reproduction drawing tests with low IQ patients.

Bedside mental status testing in low IQ, poorly educated patients is often difficult. These patients, who comprise a significant proportion of the population served by major inner city public hospitals, have a limited ability to perform adequately on many higher cortical function tests. Because of this, it is often difficult to accurately differentiate between an organic brain syndrome and the effect of depressed intelligence in these patients. Copying line drawings and other constructional tasks are a very important element in a complete mental status examination, as they are frequently faile
