COVID-19-Induced Narcolepsy Type I With Cataplexy: A Short Report.

Narcolepsy type I is a rare neurological sleep disorder characterised by chronic excessive daytime sleepiness, cataplexy, and low orexin (hypocretin) levels. Infection with SARS-CoV-2 has been linked to neurological and neuropsychiatric sequelae, but cases of post-COVID narcolepsy remain extremely uncommon. We report a previously healthy 42-year-old Caucasian female who developed excessive daytime sleepiness, cataplexy, and hypnagogic/hypnopompic hallucinations within 3 months of confirmed SARS-CoV-2 infection. Polysomnography (PSG) showed disturbed nocturnal sleep with a high arousal index an

Schizophrenia Pathophysiology: Neurotransmitter Dysfunctions and Biomarker Frontiers.

INTRODUCTION: Schizophrenia is a heterogeneous chronic brain disorder driven by multiple pathophysiological processes. While dopaminergic theories dominate current therapies, emerging evidence highlights glutamatergic dysregulation, particularly N-methyl-D-aspartate receptor (NMDAR) hypofunction, as a key mechanism alongside dopaminergic, serotonergic, and neurodevelopmental pathways. This article synthesizes mechanistic insights, focusing on neurotransmitter disruptions, oxidative stress, neuroinflammation, and Wnt signaling, to elucidate the clinical diversity of schizophrenia and identify b

Decoding neuronal vulnerability: Multidimensional analysis of D1R- and D2R- medium-sized spiny neurons in Huntington's disease.

Understanding the molecular mechanisms driving selective neuronal vulnerability to different neurodegenerative disorders remains a crucial, unsolved question. Here, we explored the case of Huntington's disease (HD), where the striatum and, specifically, dopamine receptor 1 (D1R) and dopamine receptor 2 (D2R) medium-sized spiny neurons (MSNs) exhibit differential susceptibility to the HTT CAG-repeat expansion mutation, with D2R-neurons being impacted earlier and more significantly. To unravel differences between D2R and D1R MSNs, we employed a multidimensional approach, integrating genomic, tra

Neonatal abstinence syndrome: a case series from Colombo-Sri Lanka.

INTRODUCTION: Neonatal Abstinence Syndrome (NAS) is an escalating global health challenge driven by rising illicit drug use during pregnancy, a condition that remains underreported due to the non-specificity of symptoms and limited access to toxicological testing. This case series explores the clinical complexities and systemic hurdles in managing NAS within a resource-limited setting. CASE PRESENTATION: We reviewed 23 neonates clinically diagnosed with NAS at two level III neonatal care centres in Sri Lanka over 18 months. A majority of mothers were aged below 25 years (70%), 86.9% disclosed

Brain Stimulation Therapeutic Interventions in Neuropsychiatry: A Cambrian Explosion.

Brain stimulation interventions alter neural activity by applying electrical, magnetic, sonic, or other forms of energy to the nervous system, using invasive or noninvasive methods. In recent years, the basic science and clinical science of neuromodulation have markedly advanced, and the evidence for clinical utility has resulted in new U.S. Food and Drug Administration (FDA) clearance for multiple forms of brain stimulation and multiple indications in neuropsychiatry. This review summarizes the status of the major brain stimulation interventions for neuropsychiatric disorders, emphasizing key

Sleep disorders in patients with HIV.

PURPOSE OF REVIEW: The improved survival of people with HIV (PWH) comes with increasing exposure to age-related, metabolic, neuropsychiatric, and cardiovascular risk factors that predispose them to sleep disorders and associated comorbidities. This narrative review examines the current burden, spectrum and causes of sleep disorders in PWH as well as current practice recommendations for their management. RECENT FINDINGS: Insomnia and sleep apnea and hypopnea syndrome (SAHS) remain the most common sleep disorders in PWH. Although there is paucity of data on the prevalence of circadian rhythm ab

Computer-aided Drug Design for Alzheimer's Disease: Recent Advancements and Future Perspectives.

Alzheimer's disease (AD) is a neurodegenerative disorder marked by a decline in cognitive function and memory loss, primarily resulting from cholinergic dysfunction, the accumulation of amyloid plaques, the formation of tau tangles, and the progressive degeneration of neurons. While existing treatments offer limited symptomatic relief, they do not effectively halt or reverse the underlying progression of the disease, presenting a major global challenge in Alzheimer's research. Developing therapeutic strategies for AD remains complex, largely due to the inability of current medications to signi

Current advances in platelet-rich plasma therapy for erectile dysfunction: a meta-analysis of randomized controlled trials.

INTRODUCTION: Erectile dysfunction (ED) is a prevalent condition with multifactorial etiology and significant impact on men's quality of life. Although standard therapies-such as phosphodiesterase type 5 inhibitors, vacuum devices, and penile prostheses-offer symptomatic relief, they do not address the underlying pathophysiology. Platelet-rich plasma (PRP), an autologous concentration of platelets and growth factors, has emerged as a potential regenerative treatment that may promote neurovascular repair and restore erectile function. AIM: To evaluate the efficacy and safety of PRP monotherapy

Cortistatin as a modulator of inflammatory and mitochondrial dysfunction in Huntington´s disease.

BACKGROUND: Huntington's disease (HD) is an inherited, fatal neurodegenerative disorder caused by expanded CAG repeats in the Huntingtin gene, leading to progressive motor, cognitive and psychiatric impairment. Despite its monogenic origin, HD pathogenesis is multifactorial, with convergent contributions from mitochondrial dysfunction, oxidative stress, synaptic failure, and chronic neuroinflammation, which drive neuronal vulnerability and degeneration, particularly within the striatum. Current clinical management remains exclusively symptomatic and fails to halt disease progression, highlight

Deciphering the Heterogeneity of Schizophrenia: A Multimodal and Multivariate Neuroimaging Framework for Unveiling Brain-Symptom Relationships and Underlying Subtypes.

BACKGROUND AND HYPOTHESIS: Schizophrenia manifests large heterogeneities in either symptoms or brain abnormalities. However, the neurobiological basis of symptomatic diversity remains poorly understood. We hypothesized that schizophrenia's diverse symptoms arise from the interplay of structural and functional alterations across multiple brain regions, rather than isolated abnormalities in a single area. STUDY DESIGN: A total of 495 schizophrenia patients and 507 healthy controls from 8 sites were recruited. Five symptomatic dimensions of schizophrenia patients were derived from the Positive a

Distinct proteomic CSF profiles in genetic frontotemporal lobar degeneration.

Fluid biomarkers to diagnose frontotemporal lobar degeneration (FTLD) are currently lacking. In this study, we aimed to identify proteomic changes in CSF associated with FTLD pathogenesis, focusing on signatures unique to different genetic groups. Additionally, we sought proteins distinguishing FTLD-spectrum disorders from controls. To this end, we measured a comprehensive library of over 2900 proteins in CSF using proximity extension assay technology in two well-characterized FTLD cohorts. The discovery cohort, selected from the GENFI cohort, included 47 symptomatic pathogenic variant carrier

Retired contact sports athletes with cognitive concerns: promoting lifelong brain health.

There is widespread concern among former athletes about the link between head injury and dementia. Neurologists are increasingly assessing ex-contact sports athletes with cognitive and behavioural issues following repetitive head impacts and traumatic brain injury. Their assessment and management can be challenging due to the broad differential diagnosis, including psychiatric issues, trauma-related impairment and, in some cases, neurodegeneration. There may be a range of pathologies present after trauma exposure, including Alzheimer's disease and chronic traumatic encephalopathy. Currently, w

Redefining PHOX2B-Related Neurodevelopmental Disorders as Circuitopathies: A New Paradigm in Autonomic Network Medicine.

The paired-like homeobox transcription factor 2B (PHOX2B) is a master regulator whose mutations cause a spectrum of severe neurological and neurodevelopmental disorders, including Congenital central hypoventilation syndrome (CCHS), Hirschsprung disease, and neuroblastoma. For decades, a central paradox has challenged the field: how can PHOX2B's seemingly restricted expression in the brainstem account for the broad, multi-system clinical phenotypes observed in patients? Here, we elucidate how this paradox has been resolved by recent methodological advances. We synthesize evidence from multiple
