This paper provides an overview of the major studies of bipolar affective disorder (BAD) and borderline personality disorder (BPD), and assesses whether the disorders might be better understood as variants of the same basic disorder. There is a shortage of research that delineates the features of both disorders within their representative samples. As a consequence the symptomatic overlap of the disorders, detected by categorical assessment instruments, is often misconstrued as an indication of the disorders' high rates of comorbidity (up to 81%). In paying particular attention to features of b

There is an expanding interest to study psychosis in the realm of daily life. The study of the person in the context of daily life may provide a powerful addition to more conventional and cross-sectional research strategies in the study of psychosis. This article first discusses the nature of experience sampling research in psychosis and demonstrates the feasibility and validity of studies using the experience sampling method (ESM) in this patient group. Second, the article presents a review of all ESM research in psychosis with a special focus on (a) the phenomenology, (b) the etiology, and (

Parkinson's disease is a progressive, neurodegenerative disorder affecting millions of people worldwide. Given the aging population, the prevalence of the disease is expected to increase substantially. The mainstay of treatment has been dopamine replacement therapy with carbidopa, levodopa, dopamine agonists, monoamine oxidase type B inhibitors, catechol-O-methyltransferase inhibitors and amantadine. Nonmotor features, such as cognitive impairment, mood disorders, autonomic dysfunction, gastrointestinal and genitourinary dysfunction, have a substantial impact on Parkinson's disease patients an

BACKGROUND: To determine whether a symptomatic presentation was associated with parathyroid hyperplasia, we retrospectively examined pre-operative symptom profiles of patients who underwent parathyroidectomy.
METHODS: From October 2007 to July 2008, 111 patients with primary hyperparathyroidism completed a preoperative symptom questionnaire prior to parathyroidectomy. The symptom profiles of patients with and without hyperplasia were compared.
RESULTS: Neurocognitive symptoms occurred in 51.4% of patients. Patients with 1 neurocognitive symptom had a 25% risk of parathyroid hyperplasia. Addi

Full symptomatic remission is the optimal outcome for patients with major depression. Unfortunately, antidepressant efficacy is limited to partial response for a significant minority of patients. Incomplete remission of depressive symptoms is associated with increased risk of relapse, decreased functioning in work and social settings, and increased risk of eventual suicide. Factors that increase the likelihood of incomplete remission include chronicity, severe symptomatology, and comorbid illnesses. Strategies to manage incomplete remission include "watchful waiting" (ie, continuing the origin

In this naturalistic cross-sectional study, the author tested the hypothesis that verbal working memory (WM) in major depressive disorder (MDD) would predict functional outcome. The subjects consisted of 54 clinic adult out-patients. The author found that, in the patients with current episode of MDD, functional outcome was significantly correlated with depressive scores, but not with Digit Sequencing Task scores. Meanwhile, in a sample of full remitted or partial remitted (mildly depressed) patients, functional outcome was significantly correlated with both Digit Sequencing Task scores and dep

OBJECTIVES: The aim of the present study was to evaluate, using serotonin transporter genotyping as an example, the preparedness of individuals from an urban general population identified with hypothetical genetic risk for a depressive disorder to moderate risk through cognitive or behavioural intervention. It also evaluated endorsement of genetic and environmental causal attributions of mental illness.
METHOD: A qualitative approach using focus group methodology was selected as most appropriate because these issues are relatively unexplored. Participants (n=36) aged > or =18 years from metro

BACKGROUND: Approximately 5% of the general population experiences at least one unprovoked epileptic seizure in the lifetime. This is in contrast to the cumulative incidence of epilepsy (approx. 3-4%) and incidence of acute symptomatic seizures (approx. 4%). Nearly 2% of the population experiences a febrile seizure before the 5 years of age. The aim of this article was evaluation of the distribution of acute symptomatic and unprovoked seizures in our patient cohort after the solitary epileptic seizures, as well as determination of particular etiological factors responsible for the occurrence o

OBJECTIVES: Many suicidal patients treated and released from emergency departments (ED) fail to follow through with subsequent outpatient psychiatric appointments, often presenting back for repeat ED services. Thus, the authors sought to determine whether a mobile crisis team (MCT) intervention would be more effective than standard referral to a hospital-based clinic as a means of establishing near-term clinical contact after ED discharge. This objective was based on the premise that increased attendance at the first outpatient mental health appointment would initiate an ongoing treatment cour

We report 4 children with late-onset (type III) multiple acyl-CoA dehydrogenase deficiency, also known as glutaric aciduria type II, which is an autosomal recessive disorder of fatty acid and amino acid metabolism. The underlying deficiency is in the electron transfer flavoprotein or electron flavoprotein dehydrogenase. Clinical presentations include fatal acute neonatal metabolic encephalopathies with/without organ system anomalies (types I and II) and late-onset acute metabolic crises, myopathy, or neurodevelopmental delays (type III). Two patients were identified in childhood following a me

OBJECTIVES: Tandem mass spectrometry is a major technological advance in the screening for inborn errors of metabolism. It has the advantage of sensitive and simultaneous multiple disease screening with minimal sample requirement. The diseases detected include aminoacidemias, fatty acid oxidation disorders, and organic acidemias.
DESIGN AND METHODS: Using automated electrospray tandem mass spectrometry we screened 3550, clinically selected, symptomatic children for inborn errors of metabolism by analyzing amino acids and acylcarnitines in dried blood filter-paper samples.

The clinical diagnosis of Alzheimer's disease (AD) is occasionally imprecise using consensus criteria for probable AD. Therefore, there is a great need for simple biomarkers that substantially aid early diagnosis and tract disease progression of AD and mild cognitive impairment. Of currently available biomarkers for AD, imaging markers are of particular importance based on their low invasiveness and reproducibility. In vivo detection of brain amyloid burden using positron emission tomography either by PIB or BF-227 would be quite attractive. In Japan, Alzheimer's disease neuroimaging initiativ

Nonclinical and clinical evidences have supported the view that accumulations of neurotoxic amyloid components initiate chain reactions of molecular and cellular pathologies, eventually leading to neuronal death and symptomatic onsets of neurodegenerative diseases. As this amyloid-triggered cascade is virtually composed of bidirectional causalities between upstream and downstream events, it is of critical significance to monitor all key processes, including amyloidosis, neuroinflammation, disrupted calcium homeostasis and impaired neurotransmissions, in living brains toward therapeutic regulat

This review provides an update on the current state of pharmacogenetic research in the treatment of Alzheimer's disease (AD) and Lewy body disease (LBD) as it pertains to the use of cholinesterase inhibitors (ChEI). AD and LBD are first reviewed from clinical and pathophysiological perspectives. This is followed by a discussion of ChEIs used in the symptomatic treatment of these conditions, focusing on their unique and overlapping pharmacokinetic and pharmacodynamic profiles, which can be used to identify candidate genes for pharmacogenetics studies. The literature published to date is then re

This work summarizes the efficacy of pharmacotherapy in the chronic course of schizophrenia and unipolar depresion. It is aimed to answer three questions: does it cure these diseases? Does it exert any significant effect on the symptomatic presentation of the disorders? Which is its action on the social dysfunction provoked by schizophrenia or depression? A conceptual analysis of available bibliography was performed. It could be concluded that antypsychotics improve the symptomatic course of schizophrenia, although their efficacy is limited, and that these drugs does not act on the social dysf

BACKGROUND: Headache is one of the most common symptoms in an emergency department (ED), while migraine is the most frequently observed headache in this setting. The aim of our study was to evaluate the influence of clinical and psychometric variables on the repeater phenomenon, ie, patients who make at least 3 visits to the ED at least 1 week apart during a 6-month period.
METHODS: According to the International Classification of Headache Disorders, 2nd edition (ICHD-II) criteria, we consecutively recruited Italian-speaking migraine subjects who came to the ED or outpatient service. All the

The aim of the study was to investigate automatic and effortful information processing with the Stroop paradigm in a long term perspective in patients with major depressive disorder (MDD). Patients were tested at two test occasions: at inclusion with a Hamilton Depression Rating Scale (HDRS) score >18, and after 6 months, when most patients had experienced symptom reduction. The Stroop paradigm is considered to measure aspects of attention and executive functioning and consists of three conditions/cards: naming the color of the patches (Color), reading of the color-words (Word) and naming the
