Depression and apathy in frontotemporal dementia: a short assessment of facts and outlook.

Neuropsychiatric symptoms such as depression and apathy are frequently reported in all subgroups of frontotemporal dementia (FTD) or frontotemporal lobe degeneration (FTLD) and represent a substantial burden for both patients and caregivers. Both symptoms are most common in its behavioral variant (bvFTD) with a prevalence of up to 90%. Their severity is particularly high in bvFTD and the semantic variant of primary progressive aphasia (svPPA). Due to its clinical heterogeneity, bvFTD can mimic depression, especially in its initial stages, making their differentiation a major clinical challenge

The most vulnerable are prone to use AI therapists: The role of attachment, epistemic trust, and mental health symptoms in acceptance of digital mental health interventions.

OBJECTIVE: Digital mental health interventions (DMHIs) offer scalable solutions to traditional barriers in mental healthcare. Acceptability of DMHIs might differ among patients with differing level of symptoms and relational difficulties. This study investigated how a person's psychological profile, including their level of mental health symptoms, epistemic trust, and attachment security, predicts their acceptance of AI-based interventions (avatars, chatbots) versus human-delivered teletherapy. METHOD: Using cluster analysis on survey data from 1,612 (potential) patients and clinicians recrui

[From monitoring to improvement: a data-driven learning network in outpatient mental health care].

BACKGROUND: The quality of Dutch mental healthcare is under pressure due to staff shortages and an increasing demand for care. To improve the quality of care, four outpatient mental healthcare institutions formed a learning network in 2022 for anxiety, mood and trauma en stressor related disorders. Within this network, data on treatment processes and outcomes were shared and systematically compared. This facilitated the identification of best practices and provided insights to optimize workflows, ensuring treatments are delivered more efficiently and effectively. METHOD: An observational coho

[Diagnostic dilemmas in persistent nutritional complaints and the need for integrated care].

In patients with longstanding nutritional problems and gastrointestinal symptoms, underweight or with significant weight loss, mostly without an identifiable somatic cause, a diagnostic dilemma arises. There is considerable symptomatic overlap between anorexia nervosa (AN), avoidant/restrictive food intake disorder (ARFID), and somatic symptom disorder (SSD) in such cases, which can lead to divergent clinical interpretations, delayed interventions, and lack of therapeutical direction. <span class="CharOverride-2">We describe a 49-year-old patient who had been treated for several years by gastr

[Delirium in the elderly - common and dangerous].

Delirium is an acute, fluctuating neurocognitive disorder characterized by disturbances in consciousness and attention. Unlike dementia, delirium has somatic causes and is reversible if the triggers are identified and treated. Elderly people are particularly at risk, especially those in hospitals or long-term care facilities. Its development is multifactorial. The threshold concept explains the interaction of predisposing factors such as age, dementia, multimorbidity, malnutrition, substance use, and triggering factors such as surgery, infections, pain, and immobilization. Early detection and r

Impact of preoperative cannabis use on outcomes following surgical intervention for gastroparesis.

INTRODUCTION: Cannabis use has been associated with delayed gastric emptying and may worsen symptoms in patients with gastroparesis. However, its impact on postoperative outcomes after surgical treatment remains unclear. This study aimed to evaluate postoperative outcomes among patients with and without preoperative cannabis use undergoing surgical intervention for gastroparesis. METHODS: A retrospective cohort study was conducted using the TriNetX federated electronic medical record database. Adults with gastroparesis who underwent pyloric drainage surgery or gastric stimulator placement wer

Comprehensive analysis of risk factors for coronavirus disease 2019 infection and post-infection sequelae based on real-world data from a health insurance database in Japan.

OBJECTIVES: Using real-world data from Japan, this study aimed to examine disease risks in COVID-19 patients before and after infection. METHODS: We used the Japanese Health Insurance Database to identify hospitalized patients in Japan aged 60 years who were newly diagnosed with COVID-19 by December 31, 2020. To form the study population, these patients were matched 1:1 with individuals without COVID-19 on the basis of age, sex, and index date. Two analytical approaches were employed: a case-control study using conditional logistic least absolute shrinkage and selection operator (LASSO) regre

Amelioration of Symptomatic Alzheimer's Disease after Selective Impairment of p75NTR Function in Adult Forebrain Excitatory Neurons.

The p75 neurotrophin receptor (p75NTR) contributes to the development of Alzheimer's disease (AD) pathology by enhancing amyloid precursor protein (APP) cleavage and amyloid plaque formation. However, the cell type-specific and temporal roles of p75NTR in AD progression remain unclear. Here, we report that conditional knock-in of functionally impaired p75NTR variants lacking the death domain (ΔDD) or transmembrane Cys259 (C259A) specifically in forebrain excitatory neurons of male and female 5xFAD mice significantly attenuated multiple AD-associated pathologies, including amyloid plaque accumu

Engineering nanobodies for drug delivery systems in Alzheimer's disease.

Alzheimer's disease (AD) remains a major global health challenge, with current therapies offering only symptomatic relief. A significant constraint in the development of effective treatments is the blood-brain barrier (BBB), as it greatly limits the access of therapeutic drugs targeting amyloid-β (Aβ) aggregation, tau hyperphosphorylation and neuroinflammation. Nanobodies, single-domain antibody fragments derived from camelids, have emerged as versatile tools with unique properties such as small size, high stability and the ability to penetrate the BBB. Engineered formats allow for specific ta

An interdisciplinary fetal neonatal neurology collaborative promotes integrative life-course brain health.

A proposed interdisciplinary fetal neonatal neurology collaborative offers life-course brain health training across three time-sensitive teaching opportunities. The educational organization includes a broad representation of inter-related fields. Formal training will re-enforce career-long learning that fosters creative thinking. Acquiring a life-course perspective of brain health can contribute solutions to the global public health crisis involving neurological and mental health disorders across the lifespan. Teaching transdisciplinary interventions begins with parental childhood and reproduc

Long-term relapse: markers, mechanisms, and implications for disease management in alcohol use disorder.

OBJECTIVE: Much has been theorized and documented about factors involved in alcohol use disorder (AUD) relapse during the early months following a recovery attempt where biobehavioral classical conditioning ("cues/triggers") and neurophysiological explanatory theories predominate. Little has been documented, however, about long-term relapse (LTR) factors following sustained AUD remission where self-regulation and stress and coping theories may predominate because LTR precursors are centered less around neurophysiological dysregulation and cue reactivity and more around factors such as lowered

Using Smartphone-Tracked Behavioral Markers to Recognize Depression and Anxiety Symptoms: Cross-Sectional Digital Phenotyping Study.

BACKGROUND: Depression and anxiety are prevalent but commonly missed and misdiagnosed, an important concern because many patients do not experience spontaneous recovery, and the duration of untreated illness is associated with worse outcomes. OBJECTIVE: This study aims to explore the potential of using smartphone-tracked behavioral markers to support diagnostics and improve recognition of these disorders. METHODS: We used the dedicated Behapp digital phenotyping platform to passively track location and app usage in 217 individuals, comprising symptomatic (n=109; depression/anxiety diagnosis

Latent Profiles of Suicide Risk in University Students: a Multidimensional Model Integrating sleep, mood, interpersonal, and Behavioral Factors.

Suicidal ideation is prevalent among university students and is associated with a complex interplay of psychological, interpersonal, and behavioral factors. While prior research has examined individual predictors such as sleep disturbances, depressive symptoms, and impulsivity, less is known about how these factors co-occur in clinically distinct profiles. This study aimed to identify latent profiles of suicide risk using a multidimensional model. We conducted a secondary data analysis using the Assessing Nocturnal Sleep/Wake Effects on Risk of Suicide (ANSWERS) dataset, which includes self-rep

Experimental Models and Translational Strategies in Neuroprotective Drug Development with Emphasis on Alzheimer's Disease.

Neurodegenerative diseases (NDDs), including Alzheimer's disease (AD), Parkinson's disease (PD), amyotrophic lateral sclerosis (ALS) and frontotemporal dementia (FTD), are becoming more prevalent and still lack effective disease-modifying therapies (DMTs). However, translational efficiency remains critically low. For example, a ClinicalTrials.gov analysis of AD programs (2002-2012) estimated ~99.6% attrition, while PD programs (1999-2019) achieved an overall success rate of ~14.9%. In vitro platforms are assessed, ranging from immortalized neuronal lines and primary cultures to human-induced p
