Kappa Opioid Receptor Availability in Borderline Personality Disorder: An In Vivo Investigation With [11C]EKAP Positron Emission Tomography Imaging.

BACKGROUND: Borderline personality disorder (BPD) is a chronic mental health condition associated with marked elevations in disability. Limited pharmacological treatment options are currently available, and kappa opioid receptors (KORs) have been identified as a promising treatment target. However, the KOR system has not yet been directly examined in BPD. In this study, we examined KORs in individuals with BPD and investigated links between KORs and BPD's clinical features. METHODS: Twenty individuals with BPD and 20 demographically matched healthy control (HC) volunteer participants were sca

Dual benefit of esketamine-based opioid-free anesthesia on postoperative pain and mental health outcomes in patients undergoing bariatric surgery: a randomized controlled trial.

Patients with morbid obesity are at a high risk of comorbid depressive and anxiety symptoms. While bariatric surgery is an effective treatment, conventional opioid-based anesthesia carries significant perioperative risks. Opioid-free anesthesia (OFA) presents a safer alternative. Esketamine, known for its dual analgesic and rapid antidepressant effects, is a promising component of OFA. However, clinical evidence supporting its simultaneous benefits for both physical and psychological recovery in bariatric patients remains limited. In this double-blind, randomized clinical trial, patients sched

Hepatitis C Care Cascade in Opioid-Dependent Emergency Department Patients.

INTRODUCTION: Emergency departments (ED) have become important venues for hepatitis C (HCV) screening. Similarly, EDs have had increasing engagement in the management and treatment of opioid use disorder (OUD). Persons with OUD are at disproportionate risk for acquiring HCV. We sought to assess the intersection between OUD and HCV care through observing the uptake of HCV screening and subsequent steps in the HCV care cascade in an ED-OUD cohort. METHODS: This retrospective cohort study analyzed patients enrolled in an ED-OUD treatment program as a convenience sample subgroup of a larger insti

Dexmedetomidine-based opioid-sparing anesthesia in video-assisted thoracoscopic surgery: a randomized controlled trial.

BACKGROUND: Dexmedetomidine has increasingly been incorporated into opioid-sparing anesthetic strategies during video-assisted thoracoscopic surgery (VATS) because of its analgesic properties and minimal respiratory depression. However, dexmedetomidine remains associated with dose-related hemodynamic adverse effects, particularly bradycardia and hypotension. We evaluated whether a reduced-dose dexmedetomidine regimen decreases intraoperative opioid requirements while maintaining acceptable hemodynamic safety during VATS. METHODS: In this prospective randomized controlled trial, 68 patients un

Six-month follow-up of the 'Mi Mejor Plan' school-based prevention program: a pilot cluster randomized controlled trial among early adolescents in Chile.

BACKGROUND: School-based preventive interventions for adolescent substance use often show limited short-term behavioral effects, with evidence suggesting that sustained impacts may depend on continued exposure and reinforcement strategies such as booster sessions. However, few studies have examined the longitudinal effects of booster implementation on substance use and related psychosocial outcomes in culturally adapted interventions. METHODS: We conducted a cluster randomized controlled trial to evaluate the longitudinal impact of a culturally adapted school-based prevention program among si

The Sound of Hypnotherapy: Identification of Paralinguistic Speech Features Using Acoustic Analysis and Deep Learning.

Hypnosis is an altered state of consciousness characterized by focused attention and heightened suggestibility, with therapeutic applications in a range of mental and somatic conditions. While prior research has emphasized the linguistic content of hypnotic suggestions (ie, what is said), the paralinguistic and acoustic characteristics of the hypnotist's speech (ie, how it is said) remain underexplored. This study investigated condition-associated paralinguistic characteristics in recordings of 32 hypnotherapists across hypnosis and control conditions, during both reading and spontaneous tasks

Pharmacogenetic Impact of CYP2C19 Variants on Diazepam Treatment Outcomes in Alcohol Withdrawal Syndrome.

Diazepam (DZP) is widely accepted as an effective pharmaceutical intervention for treating Alcohol Withdrawal Syndrome (AWS), and its efficacy and metabolic profile are influenced by genetic variations in the CYP2C19 gene. The objective of the present study was to investigate the impact of the CYP2C19*2 and CYP2C19*17 phenotypes on DZP metabolism. Whole blood samples were collected from 114 patients with AWS following a 10-day course of oral DZP therapy. Genotyping was performed using PCR-RFLP. Plasma concentrations of DZP and nordiazepam (NDZP) were determined using HPLC-UV. The CYP2C19*17 TT

Nanocarriers in psychiatry: a comprehensive review of emerging frontiers in the management of psychiatric disorders.

Psychiatric disorders are characterized by clinically significant disturbances in cognition, emotional regulation, and behavior and represent a substantial global health burden. Pharmacological management relies primarily on antidepressants, anxiolytics, antipsychotics, mood stabilizers, hypnotics, and stimulants. However, conventional therapy may be limited by unfavorable physicochemical properties, variable pharmacokinetics, suboptimal brain exposure, frequent dosing, and systemic adverse effects, which may compromise therapeutic outcomes and long-term treatment adherence. Nanocarrier-based

Low-Dose Intravenous Dexmedetomidine for Postspinal Shivering During Cesarean Delivery: A Randomized Double-Blind Trial Showing Comparable Efficacy With Fewer Adverse Effects at the Lower Dose.

Shivering after spinal anesthesia for cesarean delivery is common and may increase metabolic demand, impair monitoring, and reduce maternal comfort. Dexmedetomidine is effective for control of shivering, but the optimal intravenous treatment dose remains uncertain. This prospective randomized double-blind trial compared dexmedetomidine 0.2 µg/kg and 0.5 µg/kg for treatment of established shivering after cord clamping. Parturients with grade 3 or 4 shivering received one of the two study doses intravenously over 10 minutes. The primary outcome was time to shivering attenuation to grade 1 or les

Cannabinoids and sleep bruxism: mechanistic rationale, polysomnographic evidence, and future research directions.

PURPOSE: Sleep bruxism (SB) is masticatory muscle activity during sleep, most often studied through its polysomnographic marker, rhythmic masticatory muscle activity (RMMA). Cannabinoids have attracted interest for their analgesic, anxiolytic and sleep-modulating properties, but whether they influence SB, and whether such modulation would benefit patients, remains unclear. This review examines cannabidiol (CBD), tetrahydrocannabinol (THC) and cannabinol (CBN) in relation to sleep, arousal and masticatory muscle activity, and separates what has been demonstrated from what is inferred. METHODS:

Differential Neurodevelopmental Toxicity of Propofol and Fospropofol: Mechanistic Insights and Potential Clinical Implications-A Zebrafish Model Study.

Propofol and fospropofol are used in pediatric surgical anesthesia, but their comparative developmental safety remains uncertain. In this study, zebrafish embryos were exposed to equianesthetic concentrations of propofol or fospropofol. Embryonic survival, hatching, malformation, and post-anesthesia motor behavior were assessed. RNA sequencing and molecular docking were performed to explore potential molecular mechanisms. Fospropofol exhibited lower developmental toxicity than propofol, as shown by the higher survival and hatching rates and a lower malformation rate. Comparative toxicogenomic

Quality of recovery after arthroscopic meniscectomy with remimazolam or propofol anesthesia: A randomized controlled trial.

BACKGROUND: Anesthetic technique may influence postoperative quality of recovery (QoR). Remimazolam may offer hemodynamic stability and favorable early recovery characteristics, but its effect on patient-reported QoR compared with propofol remains unclear. This is particularly relevant in minor orthopedic surgery, where QoR is a principal outcome. This randomized controlled trial compared postoperative QoR between propofol-based and remimazolam-based total intravenous anesthesia (TIVA) using the QoR-40 questionnaire. MATERIALS AND METHODS: Eighty adult patients undergoing elective arthroscopi

After the Trip: A Qualitative Study of Healthy Individuals' Motor Experiences Following Different Doses of Psilocybin.

INTRODUCTION: Psilocybin is a naturally occurring classic psychedelic associated with widespread changes in brain function and promising therapeutic outcomes in several neuropsychiatric disorders. Complementing these findings, qualitative studies following psilocybin administration have revealed diverse and highly individual effects across multiple domains of subjective experience. However, little is known about its impacts on individuals' subjective motor experiences. With the tendency of individuals to stay stationary during psilocybin's effects, investigating qualitative motor experiences f
