Impact of the 2017 secured-prescription regulation of zolpidem on hypnotic dispensing in France: a nine-year interrupted time-series analysis.

BACKGROUND: In April 2017, France required a secure prescription, normally reserved for narcotics, for all zolpidem prescribing, because of its misuse. Published evaluations covered the first years after the measure, before or at the start of the COVID-19 period. METHODS: Interrupted time series of national open reimbursement data (Médic'AM; all schemes, community pharmacies) from January 2015 to February 2026 (134 monthly points). Reimbursed boxes per day of zolpidem, zopiclone and all hypnotics combined were modelled by segmented regression with seasonality and Newey-West standard errors. T

Incidence of opioid use disorder among patients prescribed long-term opioid therapy: An observational study.

BACKGROUND: There are no estimates of incident opioid use disorder (OUD), using current diagnostic criteria, among patients prescribed long-term opioid therapy (LtOT). The U.S. Food and Drug Administration issued a postmarketing requirement to estimate the 12-month incidence of OUD among patients receiving LtOT. METHODS: Study 3033-1 was a prospective, observational cohort study conducted in 10 health systems with one year of follow up. Data were collected August 2017 to October 2021, analyses were completed December 2025. The primary outcome was pain-adjusted, moderate to severe DSM-5 OUD in

Evaluation of a novel body temperature sensor (CORE) in dogs under general anesthesia following premedication with dexmedetomidine or acepromazine: A pilot study.

Core body temperature monitoring in anesthetized dogs is a vital part of physiological evaluation. Depending on the procedure and patient positioning, consistent core temperature measurements can be difficult to obtain. The objective of this study was to evaluate the reliability and clinical validity of a wireless, non-invasive, superficial heat flux-based thermometric device (CORE, greenTEG AG, Rümlang, Switzerland). Three healthy beagle dogs were premedicated with either dexmedetomidine or acepromazine prior to induction with intravenous propofol and isoflurane maintenance. They were monitor

Understanding the Influence of Substance Use, Mental Health, and Stigma on Smoking Behavior Among People Living With HIV: Rapid Qualitative Analysis.

BACKGROUND: In Vietnam, an upper-middle-income country, the majority of people living with HIV smoke. Vietnam has established quitline services to support this population, but they remain underused. Text messaging is widely used in Vietnam, and studies in other countries have shown that text messaging interventions can promote smoking cessation. Our goal was to conduct a formative study to inform the development of a text messaging intervention to increase access to quitline services and promote smoking cessation among people living with HIV who smoke in Vietnam. OBJECTIVE: This study aimed t

Dexmedetomidine Inhibition of GABA Neurons in the Ventral Tegmental Area Attenuates the Depth of Sedation in Mice.

INTRODUCTION: Dexmedetomidine is a sedative anesthetic with a unique ability to produce an arousable state of sedation. However, the neural mechanisms underlying this arousable sedation remain incompletely understood. Previous studies have implicated dopaminergic neurons in the ventral tegmental area (VTA) in mediating this effect, but the upstream neural circuit mechanisms remain unclear. Furthermore, although VTA GABAergic neurons exert robust inhibitory effects on dopaminergic neurons, their specific role in dexmedetomidine-induced arousable sedation remains unclear. METHODS: Vgat-Cre tran

Predictors of hemodynamic instability during ERCP sedation in older adults: impact of sedation strategy in a retrospective cohort.

BACKGROUND: Older adults undergoing endoscopic retrograde cholangiopancreatography (ERCP) are particularly vulnerable to sedation-related hemodynamic instability. This study aimed to evaluate predictors of hemodynamic instability and the association between sedation strategy and hemodynamic outcomes in elderly patients. METHODS: This retrospective cohort study included patients aged ≥ 65 years who underwent ERCP under deep sedation between 2015 and 2025. Hemodynamic instability was defined as either a ≥ 30% reduction in mean arterial pressure or vasopressor requirement. Multivariable logistic

Ibogaine for Substance Use Disorders.

Substance use disorders (SUDs) are a major global health burden, and psychedelic-assisted therapies are now gaining scientific interest, with ibogaine being considered as a candidate for anti-addictive effects across multiple substance classes. Ibogaine is a multi-target indole alkaloid acting on opioid, glutamatergic, serotonergic, and sigma systems with potential to rapidly reduce withdrawal and cravings after a single administration. Ibogaine is metabolized into noribogaine, which has a longer half-life and contributes to sustained pharmacological effects. These compounds act on multiple ne

Psychedelic-assisted psychotherapy: Emerging paradigm.

OBJECTIVE: To review psychedelic-assisted psychotherapy (PAP), including its principles, mechanisms of action, safety considerations, and current state of evidence relevant to Canadian primary care. SOURCES OF INFORMATION: The evidence of PAP is evolving and is primarily derived from phase 2 and 3 clinical trials in specialized research settings. Systematic reviews and meta-analyses suggest potential benefit for selected conditions, though heterogeneity is high, and long-term comparative effectiveness relative to standard treatments remains uncertain. MAIN MESSAGE: PAP combines structured ps

Protocol for a cluster randomized controlled trial among young people and their peers to reduce substance use and inequitable gender beliefs in Southern Africa.

BACKGROUND AND AIMS: Substance use and the internalization of traditional gender norms, or inequitable gender beliefs, are co-occurring risk behaviors for poorer health and well-being in young people in Southern Africa. Addressing the social and structural factors that reinforce substance use and inequitable gender beliefs, including peer influence and socioeconomic adversity, such as lack of education or employment, is necessary to support sustainable change in these behaviors. However, most existing interventions focus only on changing individual behavior. This study will therefore aim to ad

An innovative hybrid cognitive behavioral therapy for insomnia approach to insomnia treatment: a retrospective chart review study.

BACKGROUND: Insomnia is a common and significant concern within family medicine; however, there is limited access to qualified sleep specialists trained in the first-line treatment: cognitive behavioral treatment for insomnia (CBT-I). Technology advancements allowed for "app delivery" of CBT-I, but platforms do not customize treatment. To our knowledge, DrLullaby was the first to offer a hybrid approach, to improve behavioral sleep medicine access, through "tech-enabled services," including telehealth sessions alongside our sleep data portal. We hypothesized that this approach would yield impr

Perineural versus intravenous dexmedetomidine as an adjuvant for ilioinguinal/iliohypogastric nerve block in open inguinal hernia repair: a randomised controlled trial.

PURPOSE: Dexmedetomidine is widely added to peripheral nerve blocks, but whether the perineural or intravenous route provides better analgesia-and at what haemodynamic cost-is unclear in inguinal hernia repair. We compared the two routes as adjuncts to an ilioinguinal/iliohypogastric (II/IH) nerve block. METHODS: This single-centre, triple-blind randomised controlled trial enrolled 90 adults (ASA I-III) undergoing elective open inguinal hernia repair. All received an ultrasound-guided II/IH block (0.5% bupivacaine with epinephrine, 20 mL) and standardised tumescent local anaesthesia, and were

"I couldn't even be in my own skin": Examining the role of withdrawal pain in the treatment journeys of people who use opioids.

INTRODUCTION: Withdrawal is a common and debilitating experience among people who use opioids (PWUO) yet the role it plays in treatment journeys is under-represented in the literature. The scant literature on withdrawal experiences focuses primarily on illicit opioid use with less attention paid to experiences across a variety of opioid usage, including withdrawal that occurs with medication for opioid use disorder (MOUD). This study examined PWUO's experiences of withdrawal across their treatment journeys to better understand the role of withdrawal in care engagement. METHODS: We analyzed tr

Sleep-related Brain Activity in an Arousal-reducing Agent-induced Mouse Model Evaluated with fMRI.

PURPOSE: Functional MRI (fMRI) has been established as a technique for capturing functional changes based on brain activity and is widely used to evaluate various diseases and physiological conditions. However, studies that systematically evaluate functional brain networks during arousal-reducing agent-induced sleep in mice by using resting-state fMRI (rsfMRI) are limited. This study aimed to compare and analyze whole-brain functional connectivity (FC) and reorganization patterns of large-scale brain functional networks in the mouse brain during arousal-reducing agent-induced sleep, focusing o
