From law enforcement to public safety: Police officer experiences of naloxone administration to reverse opioid overdose in New York City.

The overdose epidemic has reshaped law enforcement's relationship with public health, as police increasingly adopt overdose response measures, including naloxone training and use. This study analyzed 15 interviews with New York Police Department (NYPD) officers to examine their experiences administering naloxone in their duties. Naloxone was seen as facilitating a shift from traditional law enforcement to a broader public safety role, and officers noted its potential to improve public perceptions of police. However, tensions emerged as officers navigated dual roles in enforcement and health, w

Effectiveness and dose stability of opioids in patients with restless leg syndrome: 5-year findings from the National Restless Legs Syndrome Opioid Registry.

STUDY OBJECTIVES: Large long-term assessments of opioid effectiveness and dose stability for those with dopamine agonist augmentation of restless legs syndrome (RLS) are lacking. We present 5-year longitudinal findings from the National RLS Opioid Registry. METHODS: Five-hundred participants taking opioids for RLS were interviewed at baseline about RLS symptoms, opioid medications and doses, sleep, and mental health. Participants subsequently completed online surveys every 6 months. Changes from baseline to 5-year follow-up in morphine milligram equivalents (MMEs), excluding those taking bupr

Variability in medication treatment of opioid use disorder in primary care: Comparison of PROUD trial intervention clinics and other exemplar clinics.

INTRODUCTION: Primary care patients with opioid use disorder (OUD) may receive treatment in primary care clinics or co-located specialty addiction treatment practices. To help guide operational leaders in organizing OUD care delivery systems, we described rates of OUD medication treatment among primary care patients in PRimary care Opioid Use Disorders treatment (PROUD) trial intervention clinics and four primary care clinics not in the trial because they already had OUD treatment programs in place (exemplar clinics). METHODS: Primary care patients seen at six PROUD trial intervention clinics

Optimizing Control Definitions in Opioid Use Disorder Genetic Research Using Electronic Health Records.

Amidst the opioid crisis, understanding the genetic basis of opioid use disorder (OUD) is crucial for identifying biological mechanisms and intervention points. However, genome-wide association studies (GWASs) have been hampered by inadequate sample sizes and often the use of control populations not assessed for prior opioid exposure. Because opioid exposure is a prerequisite for the development of OUD, consideration of exposure history in controls is important. Electronic health record data (EHR) paired with genomic information allow a broader sampling of patients with OUD and exposed control

Risk Factors Associated With Incident Opioid Prescription Among Attention-Deficit/Hyperactivity Disorder Patients Prescribed Stimulants.

BACKGROUND: Pharmacotherapy with stimulant or nonstimulant medications is recommended as first-line treatment for attention-deficit/hyperactivity disorder (ADHD). Among patients prescribed stimulants, concomitant opioid use may increase the risk of drug dependence, exacerbate comorbid conditions, and compromise treatment adherence. However, limited evidence exists on the risk factors for initial opioid prescription in this population. OBJECTIVE: To identify factors associated with incident opioid prescriptions among patients with ADHD prescribed stimulant therapy. METHODS: We conducted a ret

Epigenetic mechanisms and therapeutic innovations in chronic pain-associated neuropsychiatric co-morbidities.

Chronic pain, marked by nociceptive sensitization and maladaptive neuroplasticity, affects 30% of the global population with escalating socioeconomic burdens. Epidemiological data show a 2-3-fold increase in neuropsychiatric co-morbidities among individuals with chronic pain, where epigenetic dysregulation serves as a key mechanism linking ongoing pain to emotional disorders. This review systematically explores epigenetic signatures in supraspinal integration hubs, notably the limbic-paralimbic networks and prefrontal regulatory circuits. The identified epigenetic signatures encompass dysregul

Unintentional benzodiazepine use and frequency of drug checking service utilization: a cross-sectional study.

BACKGROUND: There are increasing accounts of benzodiazepine adulteration and contamination in the unregulated drug supply in Canada and internationally. In some Canadian settings, drug checking services (DCS) are available for people to identify the constituents of their drugs. This study examines the relationship between suspected, unintentional benzodiazepine consumption and DCS utilization frequency, and whether gender modifies this relationship. We hypothesized that suspected unintentional benzodiazepine consumption would predict frequent DCS utilization, particularly for women, who may be

Prospects of GLP-1 Therapies for Addiction and Mental Health Comorbidities-Quo Vadis?: A Review.

IMPORTANCE: Glucagon-like peptide-1 (GLP-1) therapies have revolutionized the management of chronic conditions like obesity and diabetes. Consistent with the overlap between feeding and metabolic pathways and those mediating addictive behaviors, growing evidence suggests that GLP-1 therapies may also be beneficial for treating alcohol and other substance use disorders (ASUDs). This review discusses the current landscape of GLP-1 therapies in the context of ASUDs, mental health considerations, and gaps and opportunities in this field. OBSERVATIONS: Preclinical evidence across several experimen

Dyadic association between support persons' attitudes towards medication for opioid use disorder and patients' mental health outcomes.

INTRODUCTION: Nearly one in three adults with opioid use disorder (OUD) has co-occurring depression or anxiety. Pervasive stigma towards medication for OUD has been identified as a barrier to OUD treatment initiation and retention, but whether stigma also complicates patient mental health outcomes remains unclear. METHODS: We analyzed longitudinal, dyadic data (n = 356 dyads) from a cohort of patients receiving buprenorphine from community health centers in California, who were accompanied by support persons (e.g., partner, family member, friend). We assessed how changes in support persons' s

Differences in MOUD receipt by legal referral type in outpatient treatment.

INTRODUCTION: Opioid use and opioid use disorder (OUD) are prevalent among persons with legal system involvement. Medications for opioid use disorder (MOUD) are recommended for treatment, yet access is limited for legal-involved populations. In outpatient treatment settings, persons with legal-system involvement are less likely to receive MOUD. However, the influence of the type of legal system involvement on MOUD access is understudied. The purpose of this study was to examine if MOUD receipt differs by the type of legal system referral among adults referred to outpatient treatment by the leg

The implications of the COVID-19 pandemic for clinical mental health care.

A Position Paper published in The Lancet Psychiatry in 2020 suggested an agenda for research about the effects of the COVID-19 pandemic on mental health, following which an interdisciplinary Lancet Psychiatry standing commission was established in 2022 to examine the emerging evidence and refine recommendations for more research. In this first Series paper from the standing commission, we focus on changes in the delivery of clinical mental health care during the COVID-19 pandemic. The second paper in the Series focuses on public mental health and policy perspectives, and the third will address

Is the antidepressant efficacy of ketamine and esketamine mediated via opioid mechanisms?

BACKGROUND: Ketamine and esketamine produce rapid and sustained antidepressant effects in persons with treatment-resistant depression (TRD). Although it is posited that these effects are largely attributed to N-methyl-D-aspartate receptor antagonism, the potential involvement of the opioid system remains unclear. This systematic review investigates whether ketamine and esketamine antidepressant efficacy is mediated through the opioid system. METHODS: We conducted a systematic search of preclinical and clinical studies investigating the potential involvement of the opioid system in the antidep

Understanding opioid use within the substance use journeys of adults receiving treatment: a qualitative study protocol of the participant-led timeline mapping approach.

INTRODUCTION: The opioid crisis remains a significant public health emergency. Key contributors to this crisis include an increasingly toxic drug supply and inadequate health policies that rely on strategies to address opioid-related harms and underutilise primary prevention and early intervention approaches. To inform comprehensive prevention and early intervention strategies, research is needed to explore pathways involving hazardous opioid use (ie, daily opioid use that results in, and may arise from, disruptions to health, relationships, work or social functioning) and the individual, soci

Opioid peptides in autism spectrum disorder and gluten-free casein-free diet as a therapeutic approach.

According to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Autism Spectrum Disorder (ASD) is characterized by persistent difficulties in social communication and interaction, together with restricted and repetitive behaviors, interests, or activities. The diagnosis of ASD currently relies on comprehensive assessments of developmental history and behavioral patterns, as there are no validated laboratory tests for a definitive clinical diagnosis. While evidence-based interventions are largely restricted to educational and behavioral programs, many individuals with ASD
