Short-term PM2.5 air pollution and "deaths of despair" in Michigan, 2006-2016: a case-crossover study.

INTRODUCTION: Particulate matter is a known risk factor for cardiovascular and respiratory illness. Ecological analyses have indicated associations for mental health outcomes, but there has been only limited work on the impacts of individual exposure to air pollutants and mental health-related mortality. This research uses a novel case-crossover design to assess associations between short-term lag exposure to particulate matter 2.5 (PM2.5) and mortality from suicide, alcohol-related liver failure and opioid drug overdose. METHODS: Records for all deaths from suicide, alcohol-related liver fai

A qualitative study on cannabis use for harm reduction and pain among veterans enrolled in an SUD treatment program.

BACKGROUND: Two-thirds of adults in the United States (U.S.) find recreational and medical cannabis use acceptable. Evidence shows that cannabis use is an effective treatment for pain, and as a harm reducing therapeutic for Substance Use Disorder (SUD). In contrast to most SUD treatment models, some models allow illegal substance use as a harm reduction strategy, which is the case for the SUD treatment program examined in this study. Little is known about veterans with SUD and their perspectives on cannabis use to treat pain. This study characterizes veteran perspectives on cannabis use to man

Adolescent Cannabis Use and Risk of Psychotic, Bipolar, Depressive, and Anxiety Disorders.

IMPORTANCE: As cannabis becomes more accessible and socially accepted, concerns have grown about its potential implications for adolescent mental health. While prior research has linked adolescent cannabis use to psychiatric symptoms, few large, population-based, longitudinal studies have examined associations with clinically diagnosed psychiatric disorders. OBJECTIVE: To evaluate whether adolescent cannabis use is associated with an increased risk of incident psychotic, bipolar, depressive, and anxiety disorders during adolescence and young adulthood. DESIGN, SETTING, AND PARTICIPANTS: This

Tropical Cyclone Exposure and Psychoactive Drug-Related Death Rates.

IMPORTANCE: Tropical cyclone exposure exacerbates substance use and mental and behavioral health vulnerabilities in affected communities, potentially contributing to increases in psychoactive drug-related deaths (including drug overdoses, alcohol-related deaths, and other substance-related disorders). Prior research has not evaluated this association in a national multidecadal study by cyclone strength and demographic subgroup. OBJECTIVE: To estimate the association between county-level tropical cyclone exposure and psychoactive drug-related death rates in the US. DESIGN, SETTING, AND PARTIC

Effect of pericapsular nerve group block on postoperative cognitive function in older patients undergoing total hip arthroplasty.

PURPOSE: Postoperative cognitive dysfunction (POCD) is common in older patients undergoing orthopedic surgery and may hinder clinical recovery. This prospective study evaluated whether a preoperative pericapsular nerve group (PENG) block reduces POCD incidence in patients undergoing total hip arthroplasty (THA). METHODS: This prospective, randomized, double-blind study included older patients scheduled for elective THA under spinal anesthesia. Patients were randomized into 2 groups: PENG (group P) and control (group C). Group P underwent an ultrasound-guided PENG block containing 20 mL of 0.2

Patterns of psychotropic medication use, medication switching, augmentation, and non-persistence in adolescents and young adults with co-occurring psychiatric morbidities and opioid use disorder.

BACKGROUND: Adolescents and young adults (AYAs) with opioid use disorder (OUD) frequently experience psychiatric comorbidities, yet little is known about how these conditions are managed pharmacologically in real-world settings. We aimed to describe psychotropic prescribing patterns, polypharmacy, augmentation, therapeutic-class switching, and non-persistence among AYAs with OUD. METHODS: We conducted a retrospective cohort study using electronic medical records from the Substance Use Treatment and Recovery clinic at the Nationwide Children’s Hospital between 2009 and 2022. AYAs aged 10–26 yea

Impact of post-traumatic stress disorder on clinical outcomes following lumbar interbody spinal fusion.

BACKGROUND: The purpose of this study was to investigate the association between post-traumatic stress disorder (PTSD) and adverse clinical events following lumbar interbody fusion surgery. METHODS: A multi-institutional electronic medical record database was queried for patients undergoing lumbar interbody fusion surgery. The primary cohort of interest consisted of adult patients with a pre-operative diagnosis of PTSD. Propensity score matching was performed using demographics and comorbidities to generate matched controls. Patients with spine neoplasms, trauma, osteomyelitis, and inflammato

Estimated Prevalence of Current Posttraumatic Stress Disorder Among Pregnant Women With Opioid Use Disorder-MATernaL and Infant Clinical NetworK (MAT-LINK), 2014-2021.

OBJECTIVES: Posttraumatic stress disorder (PTSD) during pregnancy is associated with adverse consequences and has an estimated prevalence of 3% in community samples. The prevalence of current PTSD among pregnant women with opioid use disorder (OUD), a population at increased risk of adverse birth outcomes and other mental disorders, has been estimated at 16%-37% based on 3 small studies. We used the Centers for Disease Control and Prevention's MATernaL and Infant clinical NetworK (MAT-LINK) surveillance network to (1) further examine current PTSD prevalence among pregnant women with OUD and (2

Examining the Potential Role of Opioid Settlement Funds in the Face of Impending Federal Budget Reductions for Substance Use Disorders.

OBJECTIVE: To assess the potential of opioid settlement dollars disbursed to state and local governments to replace or supplement federal substance use disorder (SUD) funding. STUDY SETTING AND DESIGN: For each state, we calculated the percentage of Substance Use and Mental Health Services Administration (SAMHSA) SUD funding for each state that can be offset by settlement funds. DATA SOURCES AND ANALYTIC SAMPLE: We estimated annual opioid settlement distributions (2022-2038) from KFF Health News and SAMHSA-funded SUD awards (2024) from SAMHSA's website and the Tracking Accountability in Gove

Endocrinopathies and Their Recovery in a 20-Year Cohort Study of People With Heroin Dependence.

OBJECTIVE: Chronic opioid use suppresses hypothalamic-pituitary signalling, leading to hypogonadism and adrenal insufficiency. Whether these abnormalities persist following long-term opioid cessation remains unclear. In particular, data on the female gonadal axis are limited, while other endocrine outcomes are predominantly reported in male cohorts. This study examined endocrine function in a long-term prospective cohort of people with opioid use disorder, comparing those in sustained remission with those continuing opioid use, exploring associations with opioid exposure, sex and depression.

Higher suicidality among primary care patients who have opioid use disorder and co-occurring depression and/or PTSD.

BACKGROUND: Depression and/or posttraumatic stress disorder (PTSD) commonly co-occur with opioid use disorder (OUD), and such co-occurring disorders (COD) are especially common among patients in primary care settings. Therefore, addressing the mental health needs of people with COD in primary care is essential. METHODS: We compared the characteristics of three groups of patients who screened positive for OUD and depression and/or PTSD: (OUD + depression, OUD + PTSD, and OUD + depression + PTSD) and examined how membership in these groups was associated with suicidality and treatment. We analyz

An intervention mapping approach to closing the gap in maternal OUD and infant NOWS care: a study protocol.

BACKGROUND: The increasing prevalence of pregnant and postpartum individuals affected by opioid use disorder (OUD) in the US is highlighted by the growing number of babies referred to the NICU for neonatal opioid withdrawal syndrome after birth. Despite advances in integrating mental health and substance use treatment into adult healthcare settings, no treatment models exist to integrate maternal OUD treatment and referrals into the pediatric care setting. Integrating maternal OUD treatment into the NICU could help mothers access care and improve maternal functioning, while preserving the dyad

Buprenorphine prescribing is increasingly delivered by primary care nurse practitioners to Medicaid beneficiaries.

BACKGROUND: Medicaid beneficiaries have disparate access to buprenorphine, a lifesaving medication for opioid use disorder (OUD) that can be prescribed in primary care settings. Nurse practitioners (NPs) are a growing primary care workforce, but their buprenorphine prescribing habits compared with physicians and the impact of recent policy changes on their practice is unknown. PURPOSE: To characterize buprenorphine prescribing by primary care NPs and physicians to Medicaid beneficiaries with OUD and to examine the impact of the removal of the buprenorphine waiver on prescribing behavior. Emotional Burden and Selfcare Strategies Among Opioid Overdose Prevention and Response Personnel in Milwaukee and Waukesha Counties, Wisconsin, USA: A Qualitative Study.

INTRODUCTION: Opioid prevention personnel who interface with overdose events are at a risk of emotional challenges, though little research has explored their emotional experiences and self-care strategies. We explored the emotional experiences and self-care strategies to manage mental and emotional challenges among individuals who work in overdose prevention and response. METHODS: This was a qualitative study. Using a phenomenological approach, we conducted in-depth key informant interviews (KIIs) with forty-six purposively selected opioid overdose prevention and response personnel from Augus

Structural and Policy Determinants of Access to Medications for Opioid Use Disorder Among Pregnant People in U.S. Jails.

Pregnant people in U.S. jails experience high rates of opioid use disorder (OUD), yet access to medications for opioid use disorder (MOUD) remains inconsistent. This mixed-methods study examines how jail policies, treatment infrastructure, and political context shape MOUD provision for pregnant incarcerated individuals. We conducted a secondary analysis of a national survey of 2885 U.S. jails (analytic sample = 836). Logistic regression models assessed associations between MOUD provision and telemedicine capacity, community MOUD availability, state Medicaid expansion, and 2020 presidential vot
