BACKGROUND: Although opioid use disorder (OUD) and hepatitis C virus (HCV) frequently co-occur, few studies have evaluated substance use and social determinants of health (SDOH) that predict HCV treatment initiation and sustained virologic response (SVR).
METHODS: We used administrative and clinical trial outcome data from HCV-infected methadone-treated participants ( n = 522 ) to evaluate changes in substance use and SDOH associated with HCV treatment initiation and SVR. We performed random forest analysis for investigating factors that predict HCV treatment initiation and SVR. Additionally

THEORETICAL FRAMEWORK: Cigarette smoking is the leading preventable cause of death worldwide, with nicotine dependence notably common among individuals with Substance Use Disorders (SUD). Smoking exacerbates both physical and mental health issues, further complicating the treatment of SUD. Current therapeutic approaches for SUD often prove inadequate, indicating a need for new strategies. Recent advancements in metabolomics and gut microbiome research have provided valuable insights into the biological mechanisms underlying addiction, warranting further investigation.
OBJECTIVES: This study a

This study assessed whether delivery of opioid use disorder (OUD) treatment through a hub and spoke (HS) model is associated with better adherence to psychotropic medication treatment, compared to usual treatment. Washington State’s HS model required each network to include at least one mental health program, so we hypothesized that it would improve psychotropic medication adherence for people with both a mental health disorder (MHD) and an OUD. The study sample comprises adult Medicaid beneficiaries who had a diagnosis of depression or bipolar disorder in Washington during 2017–2019, who also

BACKGROUND: Given sub-optimal harm reduction access, people in prison who inject drugs are at great risk of hepatitis C virus (HCV) infection, including reinfection following successful treatment. In New South Wales, Australia, prison-based opioid agonist treatment (OAT) is available, but there is no needle syringe programme. This study evaluated post-treatment HCV reinfection incidence among people in New South Wales prisons who inject drugs.
METHODS: SHARP-P was a cohort study of people with recent injecting drug use (previous six months) receiving direct-acting antiviral (DAA) treatment in

Anhedonia is a core feature of major depressive disorder (MDD), yet links between peripheral molecular signatures and cortical network architecture remain poorly defined. We enrolled 210 participants, including 56 unmedicated MDD patients with high-anhedonia (HA), 61 with low-anhedonia (LA), and 93 healthy controls (HC). Morphometric similarity networks (MSNs) from structural MRI were compared between HA and LA. MSNs index individual-level network organization by quantifying inter-regional morphometric similarity. Regional MSN patterns were linked to Allen Human Brain Atlas using Spearman corr

OBJECTIVE: Examine whether neonatal neurobehavioral profiles are related to need for pharmacological treatment among infants with prenatal opioid exposure.
STUDY DESIGN: Prospective cohort study of 217 infants with need for treatment determined using the Finnegan Neonatal Abstinence Tool (FNAST), Neonatal Withdrawal Inventory (NWI), or Eat Sleep Console (ESC). Neurobehavior was assessed with the NeoNatal Neurobehavioral Scale II (NNNS-II). Latent Profile Analysis (LPA) classified infants into neurobehavioral profiles, and logistic regression assessed the association between NNNS-II profiles a

BACKGROUND: Although alcohol outlet density (AOD) is associated with drinking behaviors in the general population, little evidence exists about this relationship among people who inject drugs (PWID). Establishing this connection is particularly important, as alcohol use exacerbates the risk of opioid overdose. This study investigated 1) the association between ZIP code-level AOD and binge drinking among PWID who use opioids, and 2) the potential moderating effect of race/ethnicity on the AOD-binge drinking relationship.
METHODS: This analysis linked 2018 National HIV Behavioral Surveillance (

INTRODUCTION: Maternal mental health conditions, comprising maternal suicide and drug overdose, are currently the leading cause of maternal mortality in the United States. However, the relationship between suicidality and drug use behavior in the perinatal period is not well understood. We examined the association between suicidality and drug use behavior among perinatal individuals. Given the racial disparities in both drug use and suicide rates in the U.S., we also examined any differences in suicidality and drug use behavior by race.

Mental health concerns (e.g., psychiatric disorders, traumatic experiences, suicidal ideation) are common among those with opioid use disorder (OUD) and are associated with an increased risk of overdose. Yet, few people with OUD receive integrated treatment for psychiatric disorders or other concerns, underscoring the need to reduce barriers to integrated care, such as a lack of knowledge about mental health conditions among frontline workers and providers. In this Community Case Study, we describe an effort at a single site (Massachusetts) of the HEALing Communities Study (HCS) to address bar

INTRODUCTION: Illicitly manufactured fentanyl has become increasingly relevant to pediatric populations. However, there are limited studies examining clinical course and treatment of fentanyl exposures in children to guide management. The objective of this study was to describe clinical characteristics, management, and outcomes in fentanyl exposures in children less than 10 years old.
METHODS: This was a retrospective chart review at a single children's hospital including patients less than 10 years old with a confirmed fentanyl exposure, as defined by urine fentanyl immunoassay or confirmato

OBJECTIVE: Despite high rates of opioid use disorder (OUD) among patients admitted for acute inpatient psychiatric treatment, medications for opioid use disorder (MOUD) are not routinely prescribed in this setting. Dually diagnosed individuals are more likely to present to the mental health system for treatment than to substance use disorder programs; therefore, initiating MOUD in psychiatric units would increase treatment access. Following the implementation of a protocol to offer MOUD to patients admitted to psychiatric units, clinical staff perspectives were evaluated.

INTRODUCTION: Opioid use disorder (OUD) represents a major challenge in addiction care, yet empirical insights into the sociodemographic, clinical, and care-related profiles of help-seekers remain limited. Understanding these profiles is essential for developing targeted care strategies. This study therefore aims to identify and describe latent classes of help-seekers with OUD and outline their specific needs.
METHODS: Latent Class Analysis was applied to routinely collected person-level data from the 2023 Berlin Addiction Care Statistical Service to identify latent classes of outpatient help

IMPORTANCE: Patient-directed discharge (PDD), when patients leave the hospital prior to completing recommended medical treatment, is associated with increased morbidity and mortality and occurs in 10% to 20% of hospitalizations for patients with opioid use disorder (OUD). Understanding risk factors associated with PDD is essential to improving outcomes for this population.
OBJECTIVE: To investigate whether hospitalized patients with OUD who received higher doses of methadone during the first 24, 48, and 72 hours after first contact with the emergency department had decreased odds of PDD.

OBJECTIVE: The rising prevalence of psychiatric disorders significantly impacts surgical outcomes. These conditions can adversely affect pain perception and recovery trajectories following lumbar spine procedures. The aim of this study was to investigate the association between specific psychiatric disorders and postoperative opioid use, complications, and 30-day readmission rates after transforaminal lumbar interbody fusion (TLIF).
METHODS: A retrospective matched cohort study using a national administrative database included patients with and those without psychiatric diagnoses who underwen

BACKGROUND: Chronic pain, opioid use, and mental health disorders frequently co-occur in older adults, forming a complex and mutually reinforcing triad. Neurobiological ageing processes-such as neuroinflammation, dopaminergic decline, and impaired top-down regulation-may increase vulnerability to maladaptive coping strategies, including opioid misuse. This review aims to integrate neurobiological, affective, and clinical evidence to propose a unified neuropsychiatric framework for understanding the intersection between chronic pain, emotional distress, and opioid vulnerability in later life, w

Long-Acting Buprenorphine Formulations (LAIB) have emerged as an alternative pharmacological approach for opioid use disorder, offering potential benefits extending beyond clinical stabilisation. Narrative medicine provides a unique approach to understand patients' perspectives and experiences with sublingual buprenorphine and LAIB dispensed to fourteen patients across different Italian Addiction Services, examining how they impact the emotional, social, and motivational dimensions of recovery. Narratives were analysed by thematic content across eight domains: dependence on daily treatment reg

OBJECTIVES: This quasi-experimental study investigates the effects of the first-ever home treatment (HT), equivalent to inpatient care, for individuals with dual diagnoses: severe alcohol and/or substance use disorder plus major psychiatric illness. Outcomes are compared to those of regular inpatient treatment (IT) of the same addiction service. The primary objective was to evaluate feasibility and safety of HT. Secondary outcomes measures included discontinuation of treatment and service utilization during the 12-month follow-up period as defined by the number of emergency department visits a

BACKGROUND: Rates of opioid use disorder (OUD) have increased among women over the past 2 decades. Medication treatment for opioid use disorder (MOUD) is effective but underused. Gender-specific treatments for women have been associated with improved substance use outcomes. However, these treatments have not specifically targeted women's engagement in MOUD, and the impact of existing gender-specific treatments is restricted by in-person delivery.
OBJECTIVE: The aim of this study was to develop a digital intervention to feasibly deliver gender-specific care that addresses the individualized ne

BACKGROUND AND AIM: Extended-release injectable naltrexone (XR-Naltrexone) is an effective treatment for opioid use disorder (OUD); however, initiation can be challenging as it requires an opioid-free period. This exploratory analysis examines patient characteristics associated with successful initiation of XR-Naltrexone in the National Drug Abuse Treatment Clinical Trials Network (CTN-0051) Extended-Release Naltrexone versus Buprenorphine for Opioid Treatment (X:BOT) trial.
METHODS: Patient demographics and clinical variables associated with successful XR-Naltrexone initiation were examined
