Although toxicology screening is often used when treating trauma patients, its utility and significance remain controversial. Data from 623 toxicology screens performed in urban trauma center patients with mental status alterations are reported. The study patients were predominantly black and male, with a mean age of 32 (+/- 22) years. Overall, 86% of screens were positive. Substances of abuse, including ethanol, were noted in 525 (84%) of urine toxicology screens. Laboratory studies and anecdotal reports by users suggest that palpitations often follow the administration of marijuana or cocaine. Epidemiologic strategies are used to detect this association using interview data from 6702 household residents sampled and followed prospectively for a collaborative multi-site study of mental disorders in five U.S. communities. This paper examines the research evidence relating sustained use of marijuana to chronic cerebral impairment. After an epidemic increase in the use of marijuana from the mid 1960s to the late 1970s, there appears to be a recent decrease in the prevalence of use. Patterns of use of marijuana vary considerably depending upon the socio-cultural and psychobiologic characteristics of the user. There may also be a natural history of marijuana use in individuals that is influenced importantly by the age and perspectives of the user. Marijuana use has been shown to be an important cause of psychopathology in some people and an attempt at self-medication in others.

Stereoisomers have been used as tools to investigate the role of spatial (3-dimensional) features in neurochemical and behavioral effects of psychotropic drugs. Studies using enantiomers (optical isomers), geometrical isomers and semi-rigid molecules have shown that spatial features play a role in effects of antidepressant, antipsychotic, antiparkinsonian, and antianxiety drugs, as well as stimulants and psychotomimetics. An understanding of the role of spatial features in effects of psychotropic drugs might aid in reducing their side effects and in enhancing their therapeutic efficacy.

That cannabis use may provoke mental disturbances is well known to Scandinavian psychiatrists today. A review of the psychiatric aspects of cannabis use is given, and the clinical signs of 70 cases of cannabis psychoses collected in Sweden are described. Despite the fact that we do not yet completely understand the physiology of physical dependence, many treatment modalities are available. In this overview, an attempt is made to discuss available treatments and mechanisms to possibly attain a better understanding of the complex interactions that occur between systems. An attempt is also made to understand interrelations between drug misuse and mental disorders as one aspect of the treatment process--the ultimate goal being more efficacious and safer therapy.

What little is known for certain and what can be inferred from an enormous but incomplete and imperfect literature is that cannabis under certain conditions is harmful to health. Early and unfounded stories concerning marihuana led to many sanctions being placed on the drug. They have also brought about the requirement that the drug be proved safe beyond any reasonable doubt for any consideration to be given to removing restrictive sanctions. Some of the misinformation has been dispelled by controlled studies of its effects. Present available research does not support the thesis of permanent damage, either physical or mental. Yet there appears to be no ongoing research that will adequately demonstrate the safety or the dangers of the drug.
