A Hierarchical Multimodal Framework for Sedation Monitoring in ICU Patients.

In the intensive care unit (ICU), monitoring sedation levels is crucial. Clinicians often rely on intermittent behavioral scales like the Richmond Agitation-Sedation Scale (RASS), which can be subjective and delay timely interventions. While electroencephalogram (EEG) offers a continuous and non-invasive alternative, but the complexity of consciousness renders a unimodal signal insufficient for comprehensive representation. To address these challenges, we propose a novel multimodal deep learning framework, Hierarchical Multimodal Fusion with Dynamic Correction (HMDC), that synergistically inte

Identification and Verification of the Anti-Insomnia Compounds From Herbal Pair Albiziae Cortex and Polygoni Multiflori Caulis Using Immobilized 5-HT1A Receptor Chromatography.

This study employed an affinity chromatography approach to investigate the potential molecular mechanism of the Albiziae Cortex and Polygoni Multiflori Caulis herbal pair, which is commonly used for treating insomnia. Briefly, the haloalkane dehalogenase and its substrate were fused separately onto the serotonin 1A receptor and amino microspheres. Receptors were bound onto chromatographic stationary phases specifically through biological orthogonal interactions. Subsequently, catechin, methyl-O-digalloyl-hexose, 2,3,5,4'-tetrahydroxystilbene-2-O-β-D-glucoside (THSG), and julibroside J35 were i

Residual Effects of Acute and Subchronic Zolpidem Treatments on Attentional Processes in Aged Female Rats.

RATIONAL: Zolpidem, a hypnotic Z-drug commonly prescribed to promote sleep, is predominantly used over the age of 50. In this elderly population, adverse behavioral disturbances, impaired driving performance, and an increased risk of falls have been frequently reported. These concerns have raised questions about the residual adverse effects of zolpidem in older adults, particularly on cognitive processes such as executive and attentional functions. OBJECTIVES: This study aimed to investigate the residual effects of zolpidem on attentional performance following either acute or subchronic admin

Multi-Agent Learning for Precise and Collaborative Control of Anesthetics in TIVA.

Precise and collaborative control of multiple anesthetics in Total Intravenous Anesthesia (TIVA) is essential for ensuring patient safety and maintaining the target depth of anesthesia (DoA). However, existing automated anesthesia control methods often fail to effectively capture the complex synergistic interactions between anesthetics and lack adaptability to patient-specific physiological variability, thereby limiting their clinical applicability. To address these issues, we propose AnesMADRL, a novel Multi-Agent Deep Reinforcement Learning (MADRL)-based framework that leverages the Counterf

Immobilization of Free-Ranging Steller Sea Lions (Eumetopias jubatus) with Butorphanol-Midazolam-Medetomidine.

The western population segment of the Steller sea lion (Eumetopias jubatus) has experienced significant population declines. The Alaska Department of Fish and Game is studying the ecology and health of the population. We describe the use of butorphanol-midazolam-medetomidine (BMM) in the capture of free-ranging Steller sea lions. This combination is advantageous as the animals retain their breathing reflex and float at the surface if entering water after darting, and it is fully reversible. Sea lions were darted when resting on haulouts. The majority (45/55) of the adult female sea lions recei

Hydrophobic hydration of analgesics and diltiazem complexes explored by electrochemical impedance spectroscopy and diffusion-ordered spectroscopy.

In the context of drug-drug interactions, the combination of benzodiazepine (BDZ) or benzothiazepine (BTZ) antidepressant/hypnotics with commonly used analgesics, including over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs), has been reported to decrease the effectiveness of analgesics. This study aimed to analyze the diffusion behavior of analgesics and diltiazem (DTZ), which was used as a structural model of BDZ/BTZ, in solution using NMR diffusion-ordered spectroscopy (DOSY) and electrochemical impedance spectroscopy (EIS). Analgesics and DTZ were dispersed as free entities in d

Long-Term Effects of Pediatric Procedural Sedation: Neurotoxicity and Neurocognitive Outcomes.

There is growing concern regarding potential neurotoxicity and long-term neuro-cognitive outcomes in young children receiving general anesthetics (GAs) and sedatives. Most currently used anesthetics and sedatives have shown to be potentially neurotoxic in animal studies. However, three landmark human studies have provided compelling evidence that a single brief exposure to GAs at an early age does not affect neurocognitive or behavioral outcomes. It is unclear if multiple exposures to GAs and sedatives in early childhood lead to worse neurocognitive or behavioral outcomes. This review article

Safely Sedating Children and Adolescents Who Are Neurodivergent.

Children who are neurodivergent have higher rates of injury, number of health care visits, and perceived need for sedation compared with their peers who are not neurodivergent, resulting in unique medical care challenges for this patient population. Sedation of children who are neurodivergent is challenging due to increased risk of adverse events, medication interactions, and behavioral complexities. Safe and effective sedation requires individualized planning, careful agent selection, and incorporation of nonpharmacologic strategies. Significant variability exists in sedation practices for ch

Tools, Techniques, and Medications for Procedural Sedation in Children.

The practice of safe and effective procedural sedation in children requires a comprehensive sedation plan that accounts for procedural requirements, as well as patient-specific needs. When tailoring a sedation plan, a combination of nonpharmacologic and pharmacologic tools is often required, and within each group, many different options exist. This article presents a high-level overview of the most commonly employed tools, such as preparatory techniques, distraction modalities, nonpharmacologic analgesia, and comfort positions, as well as common sedatives, adjuncts, and combinations. Chiral sensitivity of medetomidine lateral flow immunoassay test strips.

Medetomidine has recently emerged in the illicit drug supply in the United States, and lateral flow immunoassay test strips are an inexpensive and easy-to-use field screening option for detecting this highly potent sedative. However, the drug is chiral, and the response of test strips to the different enantiomers has not been reported. This study evaluated the chiral sensitivity of 7 lots of medetomidine test strips produced by two manufacturers. Test strips were assessed using solutions of dexmedetomidine, levomedetomidine, and racemic medetomidine at varying concentrations, water types, and

Peripheral alpha-2 antagonist vatinoxan improves dexmedetomidine-induced perivascular cerebrospinal fluid flow without affecting electroencephalogram activity in female rats.

BACKGROUND: The glymphatic concept represents a brain-wide perivascular fluid network contributing to brain metabolite clearance. Dexmedetomidine, a sedative α2-adrenergic receptor agonist, enhances perivascular cerebrospinal fluid (CSF) flow by reducing central noradrenergic tone and inducing sleep-like electroencephalogram (EEG) slow-wave activity. Concurrently, α2-adrenergic agonists modulate peripheral physiological functions, possibly influencing the central glymphatic dynamics. Utilizing peripherally restricted α2-adrenergic antagonist vatinoxan, we evaluated the role of physiological pa

Association of dexmedetomidine use during combined spinal-epidural anesthesia with postoperative sleep quality and anxiety in cesarean section: a prospective cohort study.

BACKGROUND: Postoperative sleep disturbance and anxiety are common after cesarean delivery and may impair early maternal recovery and psychological well-being. Dexmedetomidine (DEX), a selective α₂-adrenergic agonist, provides sedative and anxiolytic effects that may confer perioperative benefits beyond hemodynamic stabilization. METHODS: This prospective observational cohort study enrolled 120 women (22-38 years, ASA physical status II) undergoing elective cesarean delivery under combined spinal-epidural anesthesia. Participants were categorized into DEX or control groups according to intrao

Effects of Dexmedetomidine on the Behavioral Outcomes in Streptozotocin-Induced Alzheimer's Disease Rats.

INTRODUCTION: Alzheimer's disease (AD) is a progressive and prevalent neurodegenerative disorder characterized by progressive cognitive decline and memory impairment. Intracerebroventricular (ICV) administration of streptozotocin (STZ) in rodents recapitulates key features of sporadic AD, including brain insulin resistance and oxidative stress. Dexmedetomidine (Dex), a highly selective α2-adrenergic receptor agonist, has demonstrated neuroprotective and anti-inflammatory properties, suggesting its potential utility as a therapeutic approach for AD. METHODS: Seventy adult male Wistar rats were

Agomelatine 3D-printed microneedles as a potential drug delivery system for the treatment of depression.

Microneedles (MNs) are small devices that help to overcome the skin barrier and, thus, increase the effectiveness of transdermal drug delivery. This approach could be beneficial, especially for drugs characterised by low oral bioavailability, such as the antidepressant agomelatine (AGM), which is now only available on the market as an oral tablet. The aim of this study was to obtain agomelatine-loaded microneedle systems for potential use in the treatment of depression, using the 3D-printing methods. 3D-printing is an emerging technology enabling the manufacture of drug dosage forms or devices
