IMPORTANCE: Cocaine and methamphetamine use disorders are major public health challenges with no approved pharmacological treatments. Reliance on complete abstinence as the sole end point in randomized controlled trials (RCTs) may hinder identification of effective therapies, as it is often overly stringent, whereas reduced stimulant use is a promising alternative given its association with improved health outcomes.
OBJECTIVES: To evaluate the treatment effect of pharmacological interventions tested in RCTs for stimulant use disorder, comparing reduced use to total abstinence.

AIM: To examine whether individuals with alcohol-cocaine use disorder exhibit selective impairments in self-initiated verbal retrieval compared to individuals with alcohol use disorder alone.
METHODS: Sixty outpatients meeting Diagnostic and Statistical Manual of Mental Disorders-Five Edition (DSM-5) criteria for Alcohol Use Disorder were classified into alcohol-only (n = 29) and alcohol-cocaine (n = 31) groups using the Structured Clinical Interview for DSM-5. Narrative delayed free recall was assessed in all participants (n = 60), while word-list learning and visual memory measures were ava

BACKGROUND: Over the past decade, cocaine use disorders (CoUD) have become increasingly prevalent in Europe's outpatient addiction care settings. This study examines clinically meaningful subgroups among individuals seeking outpatient care for CoUD in Berlin, a metropolitan area with particularly high prevalence rates to inform treatment strategies.
METHODS: Using 2024 person-level routine data of the Berlin Addiction Care Statistical Service, latent class analyses (LCA) were conducted to identify subgroups among clients entering outpatient care with primary CoUD (n = 1223) based on patterns

BACKGROUND: Psychological distress is common among individuals with stimulant use disorder (StUD) and may affect treatment outcomes, with potential differences in men and women. We examined the impact of comorbid psychological distress on StUD treatment outcomes across behavioral interventions, with a focus on differences by sex.
METHODS: Data were pooled from 4 randomized controlled trials of behavioral interventions (eg, motivational interviewing, 12-step, and smoking-cessation) for StUD (n = 1858) from the National Institute on Drug Abuse Data Share. Psychological distress was assessed usi

BACKGROUND: Despite increasing interest in the medical use of substances such as cannabis and psychedelics, little is known about their use to manage premenstrual symptoms. This study assessed self-reported use of substances to manage premenstrual mood and physical symptoms, including among those with premenstrual dysphoric disorder (PMDD) and in perimenopause.
METHODS: Inclusion criteria were age 18 and older with a menstrual cycle within the past year. Participants responded to an online survey querying the use of substances (alcohol, nicotine, cannabis, psychedelics, cocaine, stimulants, o

INTRODUCTION: Substance use disorders (SUDs) are associated with significant sleep disturbances, which contribute to increased relapse risk and poorer treatment outcomes. Repetitive transcranial magnetic stimulation (rTMS) has emerged as a promising intervention for SUDs and has been shown to improve sleep in other clinical populations. However, its influence on sleep in individuals with SUDs has not been systematically examined. Understanding this relationship is critical, as improving sleep could enhance treatment outcomes and reduce relapse risk.

The Brief Symptom Inventory (BSI) is a commonly used measure to understand the influence of psychological distress on substance use treatment outcomes. However, few studies have examined whether the BSI functions equivalently across groups to allow for interpretable comparisons among individuals who use substances. The present study evaluated measurement invariance of the full 53-item BSI and the BSI-18 across race, sex, and time among individuals with cocaine use disorder. Data from seven clinical trials evaluating behavioral and/or pharmacological treatments for cocaine use were combined, pr

BACKGROUND: In January 2023, British Columbia (BC) became the first Canadian province to implement a legally sanctioned drug decriminalization policy, removing criminal penalties for adults possessing 2.5 g or less of opioids, cocaine, methamphetamine, and MDMA. Introduced as a three-year pilot, it aimed to reframe substance use as a public health issue, reduce stigma, and improve health and social service engagement. Criminal penalties were reintroduced for drug possession in most public spaces in May 2024, and the pilot ended in January 2026. Its termination has been interpreted as policy fa

BACKGROUND AND AIMS: Cocaine use disorder (CUD) is an increasingly widespread concern worldwide. Various pharmacological treatments have been investigated for CUD, but their efficacy remains unclear, and none have been licensed for treatment. Therefore, we assessed the comparative effectiveness, safety and acceptability of pharmacological interventions for the treatment of CUD and prevention of relapse.
METHODS: Systematic review and network meta-analyses (PROSPERO: CRD42024596434) of double-blind randomized controlled trials (RCTs) of pharmacological interventions specifically tested for the

BACKGROUND: This case describes a substance-induced manic episode with psychotic features in which interaction with an AI (artificial intelligence) chatbot appeared to corroborate and reinforce the patient's delusional thought content and to contradict medical advice. Excerpts from the patient's interactions with the AI chatbot provide novel clinical insight into this phenomenon, which to date has primarily been reported in news media.
CASE PRESENTATION: A man in his 30s presented to the emergency department with a one-week history of escalating behavioural disturbance, severe insomnia, press

Drug addiction is characterized by orchestrated transcriptional changes in brain reward regions, including the nucleus accumbens (NAc). The transcription factor E2F3a has emerged as a novel regulator of cocaine's rewarding effects, yet its sex- and cell-specific mechanisms, as well as its genome-wide targets, remain undetermined. Here, we investigated the motivational and reinforcing roles of E2F3a in cocaine reward using conditioned place preference (CPP) and self-administration, combined with behavioral economics and viral-mediated gene manipulation. Selective overexpression of E2F3a in D1-t

INTRODUCTION: People with concurrent substance use and mental disorders (CD) experience a disproportionately higher risk of overdose compared to people with substance use disorder alone. However, predictors among individuals with severe concurrent disorders (SCD) in tertiary care settings remain poorly characterized. We aimed to identify factors associated with frequent non-fatal overdose using statistical learning methods.
METHODS: Data were obtained from the Reducing Overdose and Relapse: Concurrent Attention to Neuropsychiatric Ailments and Drug Addiction (ROAR CANADA) longitudinal cohort

BACKGROUND: [¹ ¹C]PHNO is a positron emission tomography (PET) radioligand for dopamine D2/D3 receptors (D2R/D3R), while [¹ ¹C]P943 selectively targets 5-HT1B receptors. Although prior studies have shown differences in dopaminergic and serotonergic receptor availability in people with and without cocaine use disorder (CUD), these systems have been examined in isolation across separate studies and samples.
METHODS: Ten non-treatment-seeking individuals with CUD (3 women; mean [SD] age, 40.6 [7.5] years) underwent PET imaging with [¹ ¹C]PHNO and [¹ ¹C]P943 following an average of 7 ± 3 days of

Substance use disorders are an increasing concern globally, causing a tremendous uptick in public health burden in recent years. Despite the growing attention these disorders receive, there remain no approved pharmacotherapies for stimulant use disorder, thereby contributing a major portion of this burden. This review examines the potential use of Brixadi, an extended-release injectable formulation of the widely used pharmacotherapy buprenorphine, indicated for opioid use disorder, in the treatment of stimulant use disorder. Psychostimulants, including cocaine, methamphetamine, and designer st

The United States continues to experience an opioid epidemic, which has been described by three waves: prescription opioid use, heroin use, and use of synthetic fentanyl analogs. Currently, a new "fourth wave" has emerged, which includes the use of stimulants (methamphetamine and cocaine) and synthetic opioids. Combining stimulants with high-potency synthetic opioids, such as fentanyl and fentanyl analogs, is associated with an increased mortality. Recent changes in methamphetamine production have led to a surge in supply, purity, and potency, thereby exacerbating the crisis. Nurses must under
