Psychotherapy for Delusional Disorder: Theoretical Models and Therapeutic Techniques.

Delusional disorder is a challenging mental health condition given its resistance to conventional treatment with antipsychotic medications. Although patients with delusional disorder often retain good functioning outside of their delusional theme, they often become functionally impaired as delusional ideas become more central in their lives. Psychotherapy represents an important treatment modality for these patients, in that it may be the most effective treatment option to date. Theorists from both the psychodynamic and cognitive-behavioral schools have offered frameworks for understanding and

Co-presentation of Cotard's Syndrome and Autohemophagia: A Report of a Rare Case.

Cotard's syndrome is a rare neuropsychiatric condition characterized by nihilistic delusions in which patients believe they are dead, do not exist, or have lost their internal organs or bodily functions. Autohemophagia, the act of consuming one's own blood, is an exceedingly uncommon behavior often associated with underlying psychiatric pathology, particularly within the spectrum of psychosis or severe personality disturbance. We present the case of a 28-year-old female who presented with profound nihilistic delusions consistent with Cotard's syndrome, alongside repeated acts of deliberate sel

Impact of Early Antipsychotic Prescription Choice on Weight Gain in the First 5 Years of Psychotic Illness: a Retrospective Cohort Study.

INTRODUCTION: We analysed the effects of antipsychotic drug prescribing in year 1 of treatment for psychosis on future weight gain over 5 years. METHODS: We studied how weight changed over 5 years after the first diagnosis of psychosis/schizophrenia/schizoaffective disorder/delusional disorder/affective psychosis in 17,570 individuals and investigated its association with antipsychotic drug treatments prescribed in year 1 following diagnosis, over 30 years. RESULTS: The majority (65%) were aged 20-59 years at the time of first antipsychotic prescription. Mean baseline body-mass-Index (BMI) w

Typology of Self-injurious Behaviors Admitted to a Tertiary Care Hospital.

BACKGROUND: If we can classify self-injurious behaviors without a priori assumptions about related variables, it could contribute to the nosology of self-injurious behaviors with etiological and management underpinnings. METHODS: A cross-sectional study was conducted among 90 adult subjects consecutively admitted to the medical and surgical wards of a tertiary care center. In addition to socio-demographic and relevant clinical variables, intent, lethality, impulsivity, social support, and stressful life events were measured. K-means cluster analysis was used to delineate groups. RESULTS: Hie

Prior Expectations of Volatility Following Psychotherapy for Delusions: A Randomized Clinical Trial.

IMPORTANCE: Persecutory delusions are common, distressing, and difficult to treat. Testing computational neuroscience models of delusions can identify new therapeutic targets. OBJECTIVE: To determine whether change in delusion severity is associated with a corresponding change in volatility priors and brain activation estimated during a belief updating task. DESIGN, SETTING, AND PARTICIPANTS: This randomized clinical trial was conducted from April 9, 2021, to December 5, 2023, within the Vanderbilt University Medical Center Psychiatric Hospital and at a community mental health center in Nash

Electroconvulsive therapy in erotomanic delusion: a path to belief updating.

OBJECTIVE: Erotomania is a rare delusional disorder marked by the persistent, unfounded belief that another person is in love with the affected individual. While antipsychotic medications may alleviate the intensity of delusions, the core belief often persists, leading to significant emotional distress and potential behavioral risks. The aim of this study was to illustrate the potential therapeutic role of electroconvulsive therapy (ECT) in the treatment of chronic, treatment-resistant erotomania. MATERIALS AND METHODS: We present a case report of a patient with a lifelong erotomanic delusion

When Somatic Delusions Meet Cyberchondria: An Unusual Case of Vitamin D Toxicity.

Patients with somatic delusional disorder are firmly convinced that something is wrong with their bodies and are often referred from one specialist to another without receiving a clear diagnosis. Some of these patients may turn to complementary or alternative medicine based on information found through online health searches. However, online health content is not regulated for quality, and much of it is inaccurate or potentially harmful, including fad diets and vitamin supplement advice from celebrity doctors. We present the case of a patient who believed he had Huntington's disease and develo

Acute onset Capgras syndrome after upper respiratory tract infection: a case report on the possible role of inflammation and drug exposure.

BACKGROUND: Capgras syndrome (CS) is a rare delusional disorder characterized by the delusional belief that a relative has been replaced by an identical person. This syndrome is frequently associated with psychotic disorders and neurological diseases. CASE PRESENTATION: In this case report, we describe a 42-year-old woman who developed Capgras syndrome following a severe upper respiratory tract infection. The patient believed that her husband had been replaced by another person and was found to exhibit delusional misidentification consistent with Capgras syndrome. Paliperidone treatment was i

Magical Thinking and Mystical Experience: An Exploration of Delusional Disorder in Schizophrenic Patients.

BACKGROUND: Delusions are common positive symptoms in schizophrenic patients. Individuals with delusional disorders have different experiences in memory and beliefs. Exploring patients' delusional beliefs is crucial to ascertaining appropriate psychotherapy programs. A few studies have explored the meanings of delusions for delusional patients. This study explores how schizophrenic patients lived experiences about their delusional beliefs. MATERIALS AND METHODS: The study was carried out at a psychiatric hospital in Indonesia in 2021. The participants included 30 adult schizophrenic patients

A Thirty-Year Forensic Case Report of Chronic Psychosis: Systemic Implications for Italian Forensic Psychiatry.

Background: Italy's forensic psychiatric system has undergone major reforms over recent decades, shifting from custodial institutions to a right-based, community-oriented model. While internationally praised for its commitment to dignity, recovery, and social reintegration, this framework presupposes a degree of clinical recoverability that is not always achievable. Individuals with chronic, treatment-resistant disorders-particularly those who remain socially dangerous-pose complex challenges that strain the system. Methods: We report a thirty-year longitudinal case of G.D.M.: a man initially

From collective memory to clinical cases: analyzing political delusions in patients with psychotic disorders.

BACKGROUND: Politically charged public life supplies powerful symbols that can be recruited into delusional meaning-making. This study examined how political contexts shape the content and experiential structure of delusions in patients with psychotic disorders in Turkey. METHODS: We conducted a retrospective, archive-based qualitative study of inpatient psychiatric records from a tertiary hospital spanning 1985–2024. Of 1,657 records screened by two clinicians, 122 cases with sufficiently rich, politically themed delusional content were included following consensus review. Analysis used refle

Transdiagnostic effectiveness and safety of clozapine in individuals with psychotic, affective, and personality disorders: nationwide and meta-analytic comparisons with other antipsychotics.

BACKGROUND: Pharmacotherapy with clozapine is listed as an optional treatment for several psychiatric disorders in guidelines across the globe. Nonetheless, its transdiagnostic effectiveness across most psychiatric disorders remains uncertain due to scant evidence. We therefore aimed to assess the effectiveness and safety of clozapine across multiple psychiatric disorders. METHODS: This cohort study used nationwide register-based data from Finland and Sweden in individuals aged 16 years or older diagnosed with schizophrenia, schizoaffective disorder, delusional disorder, bipolar disorder, psy

Systematized delusions in a patient with covert hepatic encephalopathy: A clinicopathological insight into prodromal dementia with Lewy bodies.

BACKGROUND: Late-onset psychosis is an early clinical manifestation of psychiatric-onset prodromal dementia with Lewy bodies (DLB); however, its underlying neuropathology remains poorly understood. Clinicopathological correlations are often limited by the gap between symptom onset and the autopsy. CASE PRESENTATION: A 66-year-old man with autopsy-confirmed DLB presented with persistent systematized delusions. After treatment for liver cirrhosis during hospitalization, the patient's physical symptoms improved; however, persecutory delusions developed. The patient was clinically diagnosed with

Morgellons-like Disease of the Scalp.

Morgellons disease remains a poorly understood condition, while the general medical consensus is that it is a form of delusional parasitosis in which individuals have sores they believe contain fibers. In fact, Morgellons disease may represent a culture-bound syndrome, i.e., a combination of psychiatric and somatic symptoms that are considered to be a recognizable disease within a specific society or culture. We present a case of Morgellons-like disease of the scalp and discuss its relationship to psychopathology and specific features of the scalp microenvironment. The propositus suffered from

Undiagnosed Delusional Infestation Leading to Severe Self-Inflicted Injuries: A Case Report.

BACKGROUND: Delusional infestation (DI), or delusional disorder, somatic type, is a rare psychiatric condition characterized by a fixed false belief of infestation despite lack of medical evidence. Patients frequently resist psychiatric care due to poor insight and somatic preoccupation, increasing the risk of delayed diagnosis and serious self-harm. CASE PRESENTATION: We report the case of a 60-year-old woman with no prior psychiatric history who presented with 11.5% total body surface area (TBSA) second- and third-degree self-inflicted burns resulting from attempts to eradicate perceived sk
