Effectiveness and Safety of Electroconvulsive Therapy in Adolescents with Schizophrenia and Schizoaffective Disorder: A Clinical Case Series.

Background: Electroconvulsive therapy (ECT) is a well-established intervention for severe and treatment-resistant psychiatric disorders, yet its use in adolescents remains limited, particularly in Europe. Data on its safety and effectiveness in this population are scarce. This study aimed to evaluate the clinical outcomes and tolerability of ECT in adolescents diagnosed with schizophrenia and schizoaffective disorder. Methods: We conducted a retrospective observational case series of 22 adolescents (mean age 16.7 ± 1.3 years) treated with ECT between 2017 and 2024 at a university psychiatric d

Acid sphingomyelinase activity suggests a new antipsychotic pharmaco-treatment strategy for schizophrenia.

Schizophrenia is a chronic and severe mental disorder. It is currently treated with antipsychotic drugs (APD). However, APD's work only in a limited number of patients and may have cognition impairing side effects. A growing body of evidence points out the potential involvement of abnormal sphingolipid metabolism in the pathophysiology of schizophrenia. Here, an analysis of human gene polymorphisms and brain gene expression in schizophrenia patients identified an association of SMPD1 and SMPD3 genes coding for acid- (ASM) and neutral sphingomyelinase-2 (NSM). In a rat model of psychosis using

Partial rescue of schizophrenia-related phenotypes in young adult Sp4 hypomorphic mice.

The Schizophrenia Exome Sequencing Meta-Analysis (SCHEMA) Consortium found that truncation of one copy of the SP4 gene has an odds ratio of 9.37 (3.38-29.7) for schizophrenia (SCZ). Human SP4 gene is also one of the 69 schizophrenia-risk genes prioritized by parallel genome-wide association studies (GWAS) and one of the two schizophrenia-risk genes shared between the SCHEMA and the GWAS, suggesting that more subtle common variants of the SP4 gene with a partial loss-of-function are likely involved in the pathogenesis of schizophrenia in the general population of patients. SP4 may also act as a

Blood-based biomarkers of neuroaxonal and astroglial injury identify ultra-treatment-resistant schizophrenia.

BACKGROUND: Ultra-treatment-resistant schizophrenia (UTRS) is the most severe subtype of treatment refractoriness, with persistent symptoms despite clozapine use. Although immunoinflammatory mechanisms have been implicated in schizophrenia, the biological basis of UTRS remains unclear. We hypothesized that blood-based biomarkers of astroglial and neuroaxonal injury could distinguish UTRS from other schizophrenia subtypes and healthy controls. METHODS: Plasma levels of glial fibrillary acidic protein (GFAP), neurofilament light chain (NfL), ubiquitin C-terminal hydrolase L1 (UCH-L1), and total

Cognitive behavioral therapy for insomnia in people with schizophrenia: a systematic review and meta-analysis.

BACKGROUND: Cognitive behavioral therapy for insomnia (CBT-I) is the first-line treatment for insomnia in general population, but only recently has it begun to be examined among people with schizophrenia. We aimed to summarize the currently available evidence by the systematic review and meta-analysis. DESIGN: Multiple databases, including PubMed, Cochrane Central Register of Controlled Trials and PsycINFO were searched up to 1st October 2024 to include all randomized controlled trials examining CBT-I among adults with insomnia and schizophrenia. We evaluated the certainty of evidence using G

Unveiling the prognosis of adult Catatonia: A systematic review.

INTRODUCTION: Catatonia is a neuropsychiatric syndrome observed in psychiatric and physical conditions. Linked to neuroinflammation, schizophrenia spectrum disorders remain its principal diagnostic context. Short-term outcomes are favourable with prompt treatment. Long-term prognosis remains poorly understood. This systematic review aimed to evaluate long-term outcomes and mortality rates in individuals with catatonia. METHODS: We conducted a PRISMA-compliant systematic review (PROSPERO: CRD420251027945) searching PubMed, Web of Science, EMBASE, SCOPUS, PsycINFO, and non-peer-reviewed sources

Acute Schizophrenia-like Psychotic Disorder Against the Background of COVID-19.

Background and Objectives: Research in this area focuses on acute schizophrenia-like psychotic disorder, as more than half of cases progress to a chronic course, manifesting as schizophrenia or schizoaffective disorder. Research has shown a link between viral infection and the onset of psychosis, and the influence of viruses on the clinical course of the disease is also being studied. Consequently, in cases where this type of psychosis co-occurs with a viral illness, there is a compelling rationale for identifying commonalities in both treatment and outcome. The ongoing global pandemic of COVI

Differentially expressed mRNAs of neural signaling pathway genes in peripheral blood leukocytes as biomarkers for schizophrenia.

Abnormal neurotransmitter regulation plays a role in the pathogenesis of Schizophrenia (SCZ), and the presence of brain tissue-like aberrant expression in the partial transcriptome of peripheral blood leukocytes from patients with SCZ suggests that these aberrantly expressed genes could be potential diagnostic markers. We designed a case-control study to analyze the association between the expression levels of mRNAs and SCZ in peripheral blood leukocytes and to explore their potential value as diagnostic biomarkers for SCZ. Differentially expressed mRNAs associated with neural signaling pathwa

Duration of untreated psychosis and diagnostic delay in homeless patients with schizophrenia- a Copenhagen based clinical study.

PURPOSE: Psychiatric disorder is a well-established risk factor for homelessness, and homelessness amplifies social, physical, and mental health risks. Yet, little is known about the extent and nature of diagnostic delays, including the duration of untreated psychosis (DUP), among homeless individuals with schizophrenia. This study aimed to address that gap by examining DUP, diagnostic delays, illness trajectories, psychopathology, and substance use in this population. METHODS: We conducted an exploratory cross-sectional study of 35 homeless individuals with schizophrenia, using in-depth psyc

Catatonia in ICD-11.

In the International Statistical Classification of Diseases and Related Health Problems Version 11 (ICD-11), the diagnostic criteria for catatonia have been extensively revised. We provide context for these changes beginning with a brief history of how catatonia has been conceptualized and a description of how the criteria for catatonia have changed across ICD versions. We also compare ICD-11 with the criteria in the latest Diagnostic and Statistical Manual of Mental Disorders (i.e., DSM-5-TR), consider clinical implications of the changes in ICD-11, and highlight conceptual areas in need of f

Hematological Correlations as Predictors of Disease Manifestations in Psychiatric Inpatients.

Background/Objective. Psychiatric disorders exhibit significant symptomatic and etiopathological heterogeneity, complicating diagnosis and treatment. Hematological parameters may serve as indicators of overall health and predictors of psychiatric symptom manifestation and remission, particularly in long-term hospitalized patients. This study evaluated hematological and biochemical markers, including vitamin B12, vitamin D, and glucose levels, to explore their potential role in psychiatric disorders and disease progression. Methods. This prospective observational study was conducted from 1 Janu

The influence of the Snoezelen method on postural stability and the risk of falls in people with paranoid schizophrenia.

Over a century of research has focused on improving our understanding of congenital cytomegalovirus (cCMV), yet it remains the most common congenital infection in the United States, affecting 3 to 6 per 1000 live born infants each year. Pregnancies affected by cCMV are at a heightened risk of spontaneous abortion and intrauterine fetal demise. Neonates born with cCMV are also at substantial risk for long-term neurodevelopmental sequelae and disability, including sensorineural hearing loss, even those born without clinically apparent disease. Considerable progress has been made in recent years

Case report: post COVID-19 encephalopathy and oral cenesthopathy.

Post-COVID-19 encephalopathy is a neurological complication characterized by cognitive impairment, memory loss, and other neuropsychiatric symptoms in COVID-19 survivors. Oral cenesthopathy, a rare somatic delusion characterized by unusual oral sensations without physical findings, is typically associated with affective disorders and schizophrenia. This case report describes a 73-year-old female who developed post-COVID-19 encephalopathy, presenting initially with cognitive decline, followed by major depression and oral cenesthopathy 16 months after the infection. Comprehensive investigations

The Nature of the Familial Risk for Psychosis in Bipolar Disorder.

BACKGROUND AND HYPOTHESIS: To clarify whether the familial liability to psychosis associated with bipolar disorder (BD) is nonspecific or has a greater effect on risk for psychosis in cases with prominent mood symptoms and/or a remitting course. STUDY DESIGN: We examined, in 984 809 offspring raised in intact families in Sweden, born 1980-1996 and followed-up through 2018, by multivariable Cox proportional hazards regression, risk in offspring of parents with BD for 7 psychotic disorders: Psychotic MD (PMD), psychotic BD (PBD), schizoaffective disorder (SAD), acute psychoses, psychosis NOS, d

Weight change following diagnosis with psychosis: a retrospective cohort study in Greater Manchester, UK.

INTRODUCTION: Weight gain in the months/years after diagnosis/treatment of severe enduring mental illness (SMI) is a major predictor of future diabetes, dysmetabolic profile and increased risk of cardiometabolic diseases. There is limited data on the longer-term profile of weight change in people with a history of SMI and how this may differ between individuals. We here report a retrospective study on weight change over the 5 years following an SMI diagnosis in Greater Manchester UK, an ethnically and culturally diverse community, with particular focus on comparing non-affective psychosis (NAP

[Online romance scams - report on an old victim with mental disorder].

Online romance scam is a form of cybercrime that has spread with the advent of social media and dating apps. In this specific form of crime, the perpetrators deceive the victim by imitating a love relationship. We can assume that patients suffering from mental disorders are more vulnerable to this form of cybercrime, but there have been hardly any studies or case reports in this direction so far. Presenting the case of an old female patient, we summarize the characteristics of online romantic scam as well as the possibilities of recognizing victimization and mitigating the consequences. The 70
