Longitudinal Changes in Serum Interleukin-33 Levels During Acute Psychotic Relapse and Clinical Improvement in Schizophrenia: An Admission-Discharge Study.

Background/Objectives: Schizophrenia pathophysiology has been linked to immune dysregulation, but the role of interleukin-33 (IL-33) during acute psychotic relapse remains insufficiently investigated. This exploratory admission-discharge study aimed to evaluate serum IL-33 levels in inpatients with paranoid schizophrenia and in healthy controls. Methods: Serum IL-33 was measured at midday (12:00 h) and midnight (24:00 h) in 21 inpatients with paranoid schizophrenia and 21 age- and sex-matched healthy controls (HC). Patients were sampled at admission and discharge. HC underwent one sampling day

Extensive Craniofacial Trauma Caused by an Intraoral Pyrotechnic Explosion in a Patient with Paranoid Schizophrenia.

Background: Intraoral explosions of pyrotechnic devices are exceptionally rare and combine blast overpressure, penetrating fragments, thermal injury, and extensive wound contamination. The immediate threats include airway obstruction, hemorrhage, loss of facial buttresses, and destruction of the occlusal unit. This report describes the acute multidisciplinary management of a self-inflicted intraoral pyrotechnic explosion in a male patient during an exacerbation of paranoid schizophrenia. Case presentation: The explosion caused bilateral comminution of the midface, fractures of the mandibular b

Aortitis with eosinophilia as a rare complication of clozapine treatment ‒ a case of a patient with paranoid schizophrenia.

Clozapine is an antipsychotic medication used to control the symptoms of schizophrenia. Due to the frequent occurrence of adverse effects, it is reserved for patients meeting the criteria of treatment-resistant schizophrenia. Among the less typical side effects of clozapine are asymptomatic eosinophilia and eosinophilic inflammation of organs such as the myocardium, as well as vasculitis, which most commonly affects small vessels in the skin. We present the case of a 54-year-old woman with treatment-resistant paranoid schizophrenia, who developed aortitis accompanied by eosinophilia as an adve

Mapping the network of theory of mind in paranoid schizophrenia: Preliminary investigation of the role of attachment, emotional empathy and emotion recognition.

OBJECTIVES: This study aimed to examine the underlying dynamics between theory of mind (ToM), attachment, emotional empathy and facial emotion recognition (FER) in individuals with paranoid schizophrenia (PaSch) and healthy controls. METHODS: Using network analysis, we investigated patterns of associations and compared the structural organization of these constructs across two matched samples (N = 60). RESULTS: Results indicated that unresolved family traumatization emerged as the most central node in both networks, suggesting its key role in linking attachment-related vulnerabilities with s

Revealing the Roles of the SH3GLB1-Hydrogen Peroxide Axis in Glioblastoma Multiforme Cells.

OBJECTIVES: Glioblastoma (GBM) is a prevalent malignant brain tumor prone to drug resistance. We previously found a strong correlation between SH3 domain GRB2-like endophilin B1 (SH3GLB1) and superoxide dismutase 2 (SOD2), which converts O2 to hydrogen peroxide (H2O2). Prior studies show that H2O2 redox signaling is vital for physiological processes and can drive tumor progression. Therefore, we aim to define how H2O2 signaling regulates SH3GLB1 and AKT (protein kinase B) pathways in GBM and to assess whether modulating H2O2 reverses temozolomide (TMZ) resistance. METHODS: We used cultured ce

Clinical dilemma of mood stabilizer augmentation in treatment-resistant schizophrenia with residual phase: A case report of valproate-associated hypoglycemia and cytopenia, and subsequent lithium response.

BACKGROUND: Mood stabilizer augmentation is frequently used in treatment-resistant schizophrenia despite limited guideline recommendations. Evidence for its efficacy and safety in the residual phase, however, remains insufficient. CASE PRESENTATION: A 49-year-old Japanese woman with treatment-resistant residual schizophrenia exhibited persistent agitation and aggression despite antipsychotic therapy. Valproate augmentation (800 mg/day) improved irritability and psychomotor agitation but was complicated by hypoglycemia, suggestive of relative carnitine deficiency, and subsequent pancytopenia.

Sleep alterations in patients with Catatonia: A scoping review and a case illustration.

Catatonia is a transdiagnostic syndrome that remains insufficiently recognized despite its clinical impact. Although sleep disturbances are well documented in mood and psychotic disorders, their association with catatonia has only rarely been described. This study aimed to map the available evidence on objective sleep alterations in patients with catatonia, complemented by an illustrative clinical case. A scoping review was conducted in accordance with the PRISMA extension for Scoping Reviews (PRISMA-ScR). Electronic databases were searched in July 2026 for studies reporting objective overnigh

An unusual case of pancreatic metastasis from malignant melanoma. The diagnostic work-up to arrive at a final diagnosis.

OBJECTIVES: Working memory deficits might be one of the major cognitive impairments in schizophrenia. Some researchers argue, that cognitive control is especially disturbed among schizophrenia patients. It was found, that low working memory capacity in schizophrenia may be explained by the fact that irrelevant stimuli occupy patients' storage space that could otherwise be used to hold relevant information. AIM: We examined, whether increased distress as well as the tendency to focus on irrelevant information (worry) are related to cognitive control in schizophrenia. METHODS: The participants

Enhanced Episodic Memory Encoding Following Modulation of the Amygdala via Transcranial Low-intensity Focused Ultrasound.

The amygdala is considered crucial to the formation of emotional episodic memories, yet causal evidence in humans is limited due to challenges in noninvasive neuromodulation of deep brain structures. In a double-blind, sham-controlled, repeated-measures study, we tested whether transcranial low-intensity focused ultrasound (tFUS) targeting the left amygdala before encoding emotional and neutral pictures impacted memory for these pictures 24 hr later (n = 23). Using a putative inhibitory tFUS protocol previously shown to attenuate amygdala BOLD-dependent signal, we hypothesized that preencoding

Nuances of Gender Identity for a Transgender Patient Receiving Inpatient Treatment for Paranoid Schizophrenia: A Case Study.

INTRODUCTION: Nearly one-fourth of individuals diagnosed with schizophrenia may experience symptoms of gender dysphoria. Patients may identify with a gender other than the gender assigned at birth before or after psychotic disorder onset. In either case, the presence of both psychosis and gender dysphoria may complicate evaluation and treatment. CASE: We present the case of a 45-year-old patient with a past psychiatric history of paranoid schizophrenia who was assigned male at birth and began to identify as a woman in her youth around the age when her psychosis first developed. Successful Clozapine Rechallenge in a 40-Year-Old Male Patient With Paranoid Schizophrenia After Clozapine-Associated Myocarditis.

BACKGROUND: Clozapine-associated myocarditis (CAM) is the most common inflammatory adverse effect under clozapine treatment. In the absence of equally effective therapeutic alternatives for treating treatment-resistant schizophrenia (TRS), clinicians are often faced with the question of whether to rechallenge clozapine in patients who have experienced CAM. However, there is a lack of standardized protocols and published case reports of successful clozapine rechallenge following CAM. CASE PRESENTATION: A 40-year-old patient diagnosed with paranoid schizophrenia experienced CAM during his first

Schizophrenic Phenotype and Therapeutic Course Associated With Chromosome 22q11.2 Deletion in a Cohort Without Prior Diagnosis of Chromosome 22q11.2 Deletion Syndrome.

Chromosome 22q11.2 deletion is the greatest single genetic factor predisposing to schizophrenia. The prevalence of schizophrenia reported ranges from 2% to 30% in patients with chromosome 22q11.2 deletion syndrome with a still discussed phenotype. In the present study 1177 patients diagnosed with schizophrenia, schizoaffective disorder or affective disorder according to ICD-10 criteria, none of which had been diagnosed with chromosome 22q11.2 deletion syndrome before, were screened for the chromosome 22q11.2 deletion or duplication applying TaqMan real-time PCR. In six patients a deletion was

Juror decision making: Does juror stigma, mental health literacy, or the description of a defendant's mental health status, impact decision-making in a mock criminal trial?

There has been limited research considering how different types of mental health information can influence juror decisions of guilt. The present study adopted an experimental methodology in which the amount of mental health information presented to contextualise an alleged offence of Criminal Damage was varied. Participants (n = 243) were randomly assigned to one of three conditions ('control': a mental health explanation could be reasonably inferred but was not directly stated; 'symptoms only': clear mental health symptoms were described but no diagnosis was provided; 'symptoms + diagnosis':

Facial emotion recognition abilities of individuals with schizophrenia and the influence of parental bonding-An exploratory study in a forensic sample.

Individuals diagnosed with schizophrenia experience cognitive impairments, including a decline in social cognition, which encompasses facial emotion recognition. Facial emotion recognition is an important aspect of social interaction, guiding people's actions and influencing their social functioning. Early childhood experiences, such as parental attachment, are one of the most influential factors in the development of many psychiatric symptoms including impairment of social cognition. Our aim was to explore this poorly researched area. We investigate the hypothesis that dysfunctional parenting

Manual Dexterity Rehabilitation in Parkinson's Disease and Paranoid Schizophrenia: A Controlled Study.

BACKGROUND: Manual dexterity (MD) impairment is a frequent and disabling feature in patients with Parkinson's disease (PD) and paranoid schizophrenia (PS), significantly affecting functional independence and activities of daily living. However, rehabilitation strategies specifically targeting fine motor control remain insufficiently integrated into routine physiotherapy (PT). OBJECTIVE: This study investigated the effects of a structured, progressive PT program incorporating targeted MD training on upper limb function in patients with PD and PS. METHODS: A prospective, exploratory, intervent

Guardianship as a back-door insanity strategy: Parricide, antisocial personality disorder, and malingered psychosis in Brazil.

Intrafamilial homicide, particularly parricide, sits at a critical intersection of forensic psychiatry, criminal justice, and civil rights protection, and diagnostic labels generated in custodial settings may later shape high-stakes civil decisions. We report the case of a 24-year-old man imprisoned for parricide committed at age 19, who was referred to an independent forensic psychiatric evaluation within a Brazilian civil guardianship (curatela) action. The assessment integrated developmental and criminological reconstructions, collateral records (school and juvenile justice documentation, p

Improvement in psychiatric symptoms after subdural haematoma and decompressive craniotomy.

This case study reports unexpected improvement in psychiatric symptoms following traumatic brain injury (TBI) in a man in his mid-30s with a complex psychiatric history, including autism spectrum disorder, paranoid schizophrenia and a personality disorder. He sustained a left-sided subdural haematoma requiring decompressive craniotomy. Post-surgery, he demonstrated resolution of longstanding hallucinations and self-harming behaviours, enhanced affective regulation and successful tapering of antipsychotic medications, alongside improvement in social and functional abilities. Although such impro

Interferon-γ +874T/A polymorphism, Toxoplasma gondii serostatus, and risk of paranoid schizophrenia in males: a case-control study.

BACKGROUND AND HYPOTHESIS: Toxoplasma gondii (TG) exposure and host immunogenetic variation have been implicated in psychosis. Interferon-gamma (IFN-γ) is central to TG control; the IFN-γ +874T/A polymorphism has been linked to variability in IFN-γ production. We hypothesized that TG seropositivity and IFN-γ +874T/A jointly increase risk for paranoid schizophrenia in males. STUDY DESIGN: We conducted a case-control study of adult males (103 cases meeting DSM-IV criteria for paranoid schizophrenia; 102 healthy controls) from two mental health centers in Israel. TG status was assessed serologic
