CONTEXT: A decolonial turn in psychology has inspired the practice of Western psychotherapy and clinical psychology. Recently, an increasing number of therapists have begun using the method of power animal journeying. This is a guided imagery-based technique originated from shamanic healing traditions. However, its use opens many questions regarding clinical applicability within the Western psychotherapeutic context.
OBJECTIVE: The study aimed to explore the therapeutic use, mechanism, suitability, risks, and future promises of power animal journeying for Western psychotherapy.
DESIGN: Fourt

Abrupt discontinuation of clozapine can result in serious and potentially life-threatening psychiatric and physical complications, including psychotic rebound, increased risk of suicide, catatonia, cholinergic rebound, and autonomic instability. Although exceedingly rare, the literature contains case reports describing seizures following abrupt clozapine withdrawal. We report and discuss an additional case of this phenomenon. A 45-year-old man with a diagnosis of paranoid schizophrenia, treated with clozapine 300 mg/day for at least 20 years, abruptly discontinued the medication. Two weeks lat

Stercoral colitis is an inflammatory complication of prolonged fecal impaction, with presentations ranging from mild colonic inflammation to life-threatening perforation. Although gastrointestinal manifestations are well recognized, secondary involvement of adjacent pelvic structures due to mass effect remains underreported and may contribute to atypical clinical presentations. An elderly woman with paranoid schizophrenia receiving long-term antipsychotic therapy presented with a three-day history of lower abdominal pain, distension, and urinary retention. Although there was no documented hist

A 29-year-old male sustained a penetrating abdominothoracic injury after falling onto a sharpened wooden branch. The object entered through the right hemiscrotum, traversed the abdomen, and extended through the diaphragm into the left pleural space. On admission, he had hemorrhagic shock, hypoxemia, tachypnea, and hypotension. Computed tomography showed hemoperitoneum, bowel and mesenteric injuries, diaphragmatic rupture, left hemopneumothorax, pulmonary contusion, subcutaneous emphysema, and rib fractures. Emergency laparotomy and thoracoscopy included small bowel resections with anastomoses,

Forensic psychiatry lacks objective biological markers and relies on clinical methodologies inherently influenced by the evaluator's cultural background and cognitive biases. This case study, spanning 30 years of clinical and forensic assessments, highlights the challenges associated with achieving diagnostic consistency and objectivity in expert opinions. A 63-year-old man, first detained at 19 for attempted robbery and homicide, underwent multiple forensic psychiatric evaluations from 1989 to 2025. Initial assessments consistently found him not guilty by reason of insanity due to a schizophr

Electroconvulsive therapy (ECT) remains among the most effective interventions in psychiatry for severe depression, catatonia, and treatment-resistant psychosis. Despite its well-established efficacy and safety profile, particularly in medically complex and older patients, its use continues to evoke controversy due to the required medical clearance. Ethical dilemmas arise when patients lack insight or capacity to consent, creating tension between respect for autonomy and the duty of beneficence. We present the case of a 62-year-old woman with a longstanding history of paranoid schizophrenia, g

INTRODUCTION: Hyperprolactinemia, an endocrine disorder, has witnessed a notable increase in incidence in recent years, exerting a substantial influence on patients' physiological states and quality of life. Hyperprolactinemia has been detected in 65% of women of childbearing age and 40.70% of men using antipsychotic medications. The extent to which antipsychotic drugs cause elevated serum prolactin levels primarily depends on their affinity for D2 receptors and their ability to cross the blood-brain barrier. The stronger their binding to D2 receptors and the poorer their ability to cross the

BACKGROUND: Antipsychotics are commonly used in the long-term treatment of schizophrenia and bipolar disorder, but rare blood-related adverse effects can complicate therapy.
CASES: We report three Indian patients who developed pancytopenia during prolonged Olanzapine use. Thorough clinical and laboratory evaluation ruled out nutritional deficiencies, infections, autoimmune disorders, and primary hematological diseases. A 43-year-old woman with schizophrenia developed severe anemia, hyponatremia, and recurrent infections that improved after stopping Olanzapine. A 40-year-old woman with paranoi

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

The classification of psychotic disorders has undergone a variety of changes. Since Karl Ludwig Kahlbaum's (Kahlbaum 1874) first descriptions of catatonic states and Emil Kraepelin's (Kraepelin 1883) nosological classification of psychotic syndromes in the second half of the nineteenth century, the diagnostic criteria for these disorders have been repeatedly modified, significantly impacting clinical practice. Eugen Bleuler (Bleuler 1911) coined the term "schizophrenia", emphasizing the disturbances in thinking, feeling and acting that he had observed. With the introduction of the 11th version

Catatonia is a severe but often underrecognized neuropsychiatric condition in adolescents. We present the case of a 16-year-old girl with early-onset schizophrenia who developed acute catatonic symptoms during a relapse following risperidone reintroduction. The clinical presentation raised a diagnostic dilemma with neuroleptic malignant syndrome (NMS). A lorazepam trial led to partial improvement, but symptom resolution occurred only after adding low-dose aripiprazole. This case underscores the importance of early identification of catatonia in adolescents, careful differentiation from NMS, an

BACKGROUND: Screening for breast cancer has been effective in decreasing mortality. Mammography is not readily available in resource-limited countries like India. Annual clinical breast examination has been demonstrated to be as effective as biennial mammography in reducing mortality with much less cost. In absence of an organized systematic screening program opportunity of women visiting the hospital can be used to educate and screen. The current study was designed to assess the acceptability, determinants of acceptability and effectiveness of opportunistic breast cancer screening.

Perception integrates external sensory signals with internal predictions that reflect prior knowledge about the world. Previous research suggests that this integration is governed by slow alternations between an external mode, driven by sensory signals, and an internal mode, shaped by prior knowledge. Using a double-blind, placebo-controlled, cross-over experiment in healthy human participants, we investigated the effects of the N-methyl-D-aspartate receptor (NMDAR) antagonist S-ketamine on the balance between external and internal modes. We found that S-ketamine causes a shift of perception t

BACKGROUND: Craniocerebral self-inflicted gunshot wounds (SIGSW) are fatal in 90% of individuals, usually before reaching a hospital setting. We report a unique SIGSW survivor's residual neurologic deficits, imaging, neuropsychiatric findings, and improvement in psychosis.
CASE PRESENTATION: The patient is a 32-year-old right-handed Caucasian male with a medical history of paranoid schizophrenia who attempted suicide by SIGSW. After transport to the Emergency Department (ED), an emergent craniotomy was required for wound debridement, hematoma evacuation, and craniofacial reconstruction. He tr
