[Search for neurophysiological markers of schizophrenia spectrum disorders: ocular microtremor and contrast sensitivity].

OBJECTIVE: To evaluate ocular microtremor (OMT) and contrast sensitivity (CS) as potential neurophysiological markers of schizophrenia spectrum disorders (SSD). MATERIAL AND METHODS: The study included 85 patients diagnosed with paranoid schizophrenia (F20.0), schizotypal (F21), and schizoaffective disorder (F25.X), as well as 29 healthy volunteers. The CS assessment was performed using an adaptive staircase procedure, and the OMT recording was conducted via video recording. Psychometric methods included the PANSS (Positive and Negative Syndrome Scale) and the Simpson-Angus Scale (SAS) for ex

Acid sphingomyelinase activity suggests a new antipsychotic pharmaco-treatment strategy for schizophrenia.

Schizophrenia is a chronic and severe mental disorder. It is currently treated with antipsychotic drugs (APD). However, APD's work only in a limited number of patients and may have cognition impairing side effects. A growing body of evidence points out the potential involvement of abnormal sphingolipid metabolism in the pathophysiology of schizophrenia. Here, an analysis of human gene polymorphisms and brain gene expression in schizophrenia patients identified an association of SMPD1 and SMPD3 genes coding for acid- (ASM) and neutral sphingomyelinase-2 (NSM). In a rat model of psychosis using

Partial rescue of schizophrenia-related phenotypes in young adult Sp4 hypomorphic mice.

The Schizophrenia Exome Sequencing Meta-Analysis (SCHEMA) Consortium found that truncation of one copy of the SP4 gene has an odds ratio of 9.37 (3.38-29.7) for schizophrenia (SCZ). Human SP4 gene is also one of the 69 schizophrenia-risk genes prioritized by parallel genome-wide association studies (GWAS) and one of the two schizophrenia-risk genes shared between the SCHEMA and the GWAS, suggesting that more subtle common variants of the SP4 gene with a partial loss-of-function are likely involved in the pathogenesis of schizophrenia in the general population of patients. SP4 may also act as a

Blood-based biomarkers of neuroaxonal and astroglial injury identify ultra-treatment-resistant schizophrenia.

BACKGROUND: Ultra-treatment-resistant schizophrenia (UTRS) is the most severe subtype of treatment refractoriness, with persistent symptoms despite clozapine use. Although immunoinflammatory mechanisms have been implicated in schizophrenia, the biological basis of UTRS remains unclear. We hypothesized that blood-based biomarkers of astroglial and neuroaxonal injury could distinguish UTRS from other schizophrenia subtypes and healthy controls. METHODS: Plasma levels of glial fibrillary acidic protein (GFAP), neurofilament light chain (NfL), ubiquitin C-terminal hydrolase L1 (UCH-L1), and total

Cognitive behavioral therapy for insomnia in people with schizophrenia: a systematic review and meta-analysis.

BACKGROUND: Cognitive behavioral therapy for insomnia (CBT-I) is the first-line treatment for insomnia in general population, but only recently has it begun to be examined among people with schizophrenia. We aimed to summarize the currently available evidence by the systematic review and meta-analysis. DESIGN: Multiple databases, including PubMed, Cochrane Central Register of Controlled Trials and PsycINFO were searched up to 1st October 2024 to include all randomized controlled trials examining CBT-I among adults with insomnia and schizophrenia. We evaluated the certainty of evidence using G

Unveiling the prognosis of adult Catatonia: A systematic review.

INTRODUCTION: Catatonia is a neuropsychiatric syndrome observed in psychiatric and physical conditions. Linked to neuroinflammation, schizophrenia spectrum disorders remain its principal diagnostic context. Short-term outcomes are favourable with prompt treatment. Long-term prognosis remains poorly understood. This systematic review aimed to evaluate long-term outcomes and mortality rates in individuals with catatonia. METHODS: We conducted a PRISMA-compliant systematic review (PROSPERO: CRD420251027945) searching PubMed, Web of Science, EMBASE, SCOPUS, PsycINFO, and non-peer-reviewed sources

Duration of untreated psychosis and diagnostic delay in homeless patients with schizophrenia- a Copenhagen based clinical study.

PURPOSE: Psychiatric disorder is a well-established risk factor for homelessness, and homelessness amplifies social, physical, and mental health risks. Yet, little is known about the extent and nature of diagnostic delays, including the duration of untreated psychosis (DUP), among homeless individuals with schizophrenia. This study aimed to address that gap by examining DUP, diagnostic delays, illness trajectories, psychopathology, and substance use in this population. METHODS: We conducted an exploratory cross-sectional study of 35 homeless individuals with schizophrenia, using in-depth psyc

Catatonia in ICD-11.

In the International Statistical Classification of Diseases and Related Health Problems Version 11 (ICD-11), the diagnostic criteria for catatonia have been extensively revised. We provide context for these changes beginning with a brief history of how catatonia has been conceptualized and a description of how the criteria for catatonia have changed across ICD versions. We also compare ICD-11 with the criteria in the latest Diagnostic and Statistical Manual of Mental Disorders (i.e., DSM-5-TR), consider clinical implications of the changes in ICD-11, and highlight conceptual areas in need of f

The influence of the Snoezelen method on postural stability and the risk of falls in people with paranoid schizophrenia.

Over a century of research has focused on improving our understanding of congenital cytomegalovirus (cCMV), yet it remains the most common congenital infection in the United States, affecting 3 to 6 per 1000 live born infants each year. Pregnancies affected by cCMV are at a heightened risk of spontaneous abortion and intrauterine fetal demise. Neonates born with cCMV are also at substantial risk for long-term neurodevelopmental sequelae and disability, including sensorineural hearing loss, even those born without clinically apparent disease. Considerable progress has been made in recent years

Oral Vitamin B12 Versus Intra-Nasal Vitamin B12 in Transient Neurosensory Disturbances Following Third Molar Surgery: A Prospective Cohort Study.

INTRODUCTION: Women with high Body Mass Index (BMI) are at increased risk of maternal and perinatal complications. These complications range from diabetes, hypertensive disorders, preterm births, increased rates of cesarean births, anesthesia related challenges, macrosomic fetuses, fetal anomalies, stillbirths, postpartum hemorrhage and surgical site infections. In this study, maternal and perinatal outcomes of women with BMI > 50 kg/m2 from a single center are presented. AIMS AND OBJECTIVES: The primary objective was to determine the risk of maternal and fetal complications in super obese pr

Searching for diagnoses and subgroups: a suggestion for criteria.

New subgroups of psychiatric disorders are often claimed. In contrast, classification systems have repeatedly had to abandon established subgroups such as paranoid vs. disorganised and catatonic schizophrenia due to lack of empirical evidence. Four criteria are proposed that should be met to claim valid subgroups: 1. distinct distribution of the defining characteristic between groups; 2. significant differences in variables other than those defining the subgroups cross-sectionally and longitudinally; 3. long-term stability; 4. significant differences between groups in aetiology, pathophysiolog

Palliative psychiatry for a patient with treatment-refractory schizophrenia and severe chronic malignant catatonia: case report.

BACKGROUND: Palliative psychiatry is an emerging field that suggests a role for palliative interventions in the management of severe and persistent mental illness (SPMI). Current literature describes using a palliative approach for patients with severe anorexia nervosa. To our knowledge, this is the first case report describing end-of-life care in a patient with treatment-refractory catatonic schizophrenia. CASE DESCRIPTION: We describe the case of a 49-year-old man with schizophrenia and severe chronic agitated/malignant catatonia who was hospitalized for ten months. Multiple treatment trial

Opioid-Induced Hallucinations: A Case Report.

This article presents the case of a 67-year-old African American male patient who was referred to a psychiatry clinic by his Internal Medicine Provider with a diagnosis of "schizophrenia, unspecified." The patient reported the onset of auditory and visual hallucinations (AVHs) two years ago, coinciding with his starting Norco (hydrocodone 5 mg/acetaminophen 325 mg) for chronic back pain. He noted that his AVH worsened when he increased his prescribed Norco dosage (within his prescribed recommended range) and observed that the hallucinations ceased when he discontinued the medication. This case

Case report: C1/2 rotational instability progressing to extreme subaxial hyperkyphosis in an adolescent with severe catatonia.

INTRODUCTION: Autism spectrum disorder (ASD) is characterized by deficits in social communication, repetitive behaviors, and can be accompanied by a spectrum of psychiatric symptoms, such as schizophrenia and catatonia. Rarely, these symptoms, if left untreated, can result in spinal deformities. RESEARCH QUESTION AND CASE DESCRIPTION: This case report details the treatment of a 16-year-old male ASD patient with catatonic schizophrenia and mutism, presenting with neck pain, left-rotated torticollis, and fever. MRI revealed atlantoaxial rotational instability and spinal cord compression from a

Transient Fever Response After ECT in a Patient with Catatonic Schizophrenia: A Case Report.

Electroconvulsive therapy (ECT) is an effective and safe treatment method for many psychiatric disorders. In general medical practice, ECT may cause side effects as most other treatment methods do. Headache, myalgia, nausea, vomiting, confusion, anterograde amnesia are common side effects of electroconvulsive therapy. Fever; in addition to general medical conditions such as infection, malignancy, connective tissue diseases, drug treatments, malignant hyperthermia, convulsions, it can also occur due to conditions such as neuroleptic malignant syndrome (NMS), serotonin syndrome, catatonia, malig

Recurrent high creatine kinase levels under clozapine treatment - a case report assessing a suspected adverse drug reaction.

Suspected adverse drug reactions (ADRs) during treatment with clozapine often prompt therapeutic drug monitoring (TDM) in clinical practice. Currently, there is no official recommendation for pharmacogenetic (PGx) testing in the context of clozapine therapy. In this case report, we demonstrate and discuss the challenges of interpreting PGx and TDM results highlighting the possibilities and limitations of both analytical methods. A 36-year-old male patient with catatonic schizophrenia was treated with clozapine. He experienced multiple hospitalizations due to elevated creatine kinase (CK) level

A multi-center study of the MicroNET-covered stent in consecutive patients with acute carotid-related stroke: SAFEGUARD-STROKE.

INTRODUCTION: Acute carotid-related stroke (CRS), with its large thrombo-embolic load and large volume of affected brain tissue, poses significant management challenges. First generation (single-layer) carotid stents fail to insulate the athero-thrombotic material; thus they are often non-optimized (increasing thrombosis risk), yet their use is associated with a significant (20-30%) risk of new cerebral embolism. AIM: To evaluate, in a multi-center multi-specialty investigator-initiated study, outcomes of the MicroNET-covered (cell area ≈ 0.02-0.03 mm2) carotid stent (CGuard, InspireMD) in co

Lymphocyte-related ratios in methamphetamine-induced psychotic disorder in Taiwan, comparing with patients with schizophrenia.

The lymphocyte-related ratios, neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR) and platelet-to-lymphocyte ratio (PLR) are new measures of inflammation within the body. Few studies have investigated the inflammatory response of patients with methamphetamine-induced psychotic disorder. Clinically, the psychotic symptoms and behavioural manifestation of methamphetamine-induced psychotic disorder are often indistinguishable from paranoid schizophrenia. We aimed to determine the differences in these inflammatory markers between patients with methamphetamine-induced psychoti
