Use of ICD-10 diagnoses in Danish psychiatric hospital-based services in 2001-2007.

The Danish version of the ICD-10 chapter on mental and behavioural disorders has 380 different diagnoses when three digits are used. This study examines how many of the available diagnoses were used and to what extent in Danish psychiatric hospital-based services in the period from 2001 to 2007, through an analysis of the total number of diagnoses reported to the Danish Psychiatric Central Research Register (n=1,260,097). The 50th percentile (50.1%) was reached by using 16 diagnoses (4.2% of 380 available). The three most frequently registered diagnoses were paranoid schizophrenia, alcohol dep

Combined use of ECT with aripiprazole.

This case report is on the combined use of electroconvulsive treatment (ECT) with a partial dopamine agonist, aripiprazole, in the treatment of a 30-year-old man with chronic and refractory paranoid schizophrenia. The patient had been under high doses of typical and atypical antipsychotics with a lack of control of the symptoms, characterized by agitation, suspiciousness, and obsessive fears. ECT therapy was started while receiving antipsychotic treatment and aripiprazole was added to the pharmacological treatment after six sessions of ECT. The convulsive threshold and the seizure duration did

Increased motor activity in cycloid psychosis compared to schizophrenia.

Although cycloid psychoses share psychopathological features with schizophrenia, their course and outcome are comparable to those seen in bipolar affective disorder. The diagnostic category is of interest because it has been demonstrated that cycloid psychoses can be diagnosed reliably and distinguished from other psychoses based on treatment response and neurophysiology. Despite this, only few studies have investigated the neurobiological differences between cycloid psychoses and schizophrenia, and although hyper- and hypoactivity have been observed in patients with cycloid psychoses, no stud

Working effectively with patients with comorbid mental illness and substance abuse: a case study using a structured motivational behavioural approach.

This case describes the use of innovative person-centred motivational behaviour change tools to enhance chronic condition self-management with a person with chronic paranoid schizophrenia, significant drug abuse and multiple psychosocial issues. In standard care, the complexity of this patient's presentation, their cognitive impairment level and history of violence would likely exclude them from many therapies and treatment programs as unsuitable or in the "too hard" basket. In fact, using a motivational and highly person-centred approach proved to be extremely necessary and rewarding for the

A case report of neuroleptic malignant syndrome.

A 32-year-old male patient with a history of treatment resistant paranoid schizophrenia developed neuroleptic malignant syndrome (NMS) during changeover of his antipsychotic medication from zuclopenthixol depot to clozapine. This case highlights the difficulties of cross-tapering two antipsychotics-that is, converting from a typical depot medication to an oral atypical antipsychotic.

Evidence of epistasis between the catechol-O-methyltransferase and aldehyde dehydrogenase 3B1 genes in paranoid schizophrenia.

BACKGROUND: Schizophrenia is a common yet severe psychiatric condition characterized by complex genetic mechanism and diverse clinical presentations. Our previous study indicated that the combined effect of two intronic single nucleotide polymorphisms (SNPs), which are located in the catechol-O-methyltransferase (COMT) and aldehyde dehydrogenase 3B1 (ALDH3B1) genes, respectively, conferred genetic risk to paranoid schizophrenia. METHODS: To further explore the precise mechanism of the COMT and ALDH3B1 interaction involved in the pathophysiology of schizophrenia, we scanned all possible functi

Binocular depth inversion as a paradigm of reduced visual information processing in prodromal state, antipsychotic-naïve and treated schizophrenia.

The binocular depth inversion illusion test (BDII) represents a sensitive measure of impaired visual information processing that manifests in various experimental and naturally occurring psychotic states. This study explores impairment of visual processing in different major psychiatric diseases investigating 313 subjects, suffering of either an initial prodromal state of psychosis (IPS) or a first-episode, antipsychotic-naïve paranoid schizophrenia (SZ-N) as well as short-term antipsychotically treated schizophrenia (SZ-T), major depression (MDD), bipolar disorder (BD), dementia (D), and heal

Psychological causes of schizophrenia.

This article outlines not only the path of my recovery but also the conclusions of my therapeutic journal that developed over a period of several years. This journal evolved into the following structure: description of personal pain body, moving forward with transformations, and active living--implementation of intention and desire that continues. My journal has evolved from the written word to transformation, current lived experience, and the expectation of good things yet to come. Many transformations were integrated into my thinking and emotions over the years. As I developed solutions to m

Schizophrenia, narrative and change: Andalusian care homes as novel sociocultural context.

This study explores qualitative changes in the life narratives of persons diagnosed with paranoid schizophrenia who were receiving recovery treatment in special care homes in Andalusia, Spain. These narratives are related to patients' construction of social identity and the recovery process. Initially, 10 narrative interviews were performed. The first five patients (Group 1) had lived at the homes for only 3 months, while the other five (Group 2) had been there for 61.4 months. Two years after the initial interviews, three patients from Group 1 were interviewed once more. The qualitative analy

Olanzapine monotherapy in a long-term treatment for schizophrenia: case study.

A scientific progress, due to the advancements within the pharmacological industry nowdays, is offering an ever increasing number of atypical antipsychotics for schizophrenia treatment. The atypics are gradually taking over the leadership of the more conventional antipsychotics in treating schizophrenia. The advantages of using atypics are fewer instances of side-effects and a good tolerance of the drug, which promotes an adequate and a satisfactory collaboration of the patients during the treatment. The daily practice often shows a polypragmasia within the treatment itself and a less frequent

Influence of olanzapine on memory functions.

Even though memory dysfunction is not considered to be a diagnostic criterion for schizophrenia, results of a neuropsychological research indicate significant damage in mnestic functioning, especially in verbal memory (Saykin et al. 1991). This type of a disorder is recorded in patients with a first episode of schizophrenia, as well as in chronic patients (Saykin et al. 1994). These researches show that this deficit is not influenced by neuroleptics. Paulsen and associates suggest that the deficit of verbal learning in schizophrenia is related to difficulties in encoding retrieving information

Dysfunction of the social brain in schizophrenia is modulated by intention type: an fMRI study.

In this fMRI study, we investigated theory of mind (ToM) in patients with paranoid schizophrenia. We hypothesized that the network supporting the representation of intentions is dysfunctional in patients with schizophrenia dependent on the type of intention involved. We used a paradigm including a control condition (physical causation) and three intention conditions (private intention, prospective social intention and communicative intentions) differing in the degree of social interaction. In all four experimental conditions patients performed worse than controls regarding accuracy and reactio

[Liaison psychiatry in a general hospital: seven paradigmatic cases].

Through the use of case reports, this article reviews frequent causes that origin the need for psychiatric intervention in patients hospitalized in medical and surgical wards. Particular diagnosis aspects are focused, so is the necessity of integration of the biological, psychological and social dimensions of the patient. The integrated approach by the various members of the medical staff is also emphasised. The cases presented were observed in the Liaison Psychiatry Consult of the Psychiatry Service of Hospital Santa Maria, in Lisbon, between November 2007 and January 2008. Seven cases were s

[Development of schizophrenia-spectrum psychoses caused by psychotraumatic events].

A study included 213 patients diagnosed with schizoaffective disorder (56.3%) or paranoid schizophrenia (43.7%). In all patients, psychotic symptoms developed under the influence of psychogenic factors. The high percentage of pathological personality disorders (50.2%), neurotic disorders (52.1%) and psychogenic depressive disturbances (53.5%) in the premorbid were found that indirectly indicates a liability to psychogenically caused endogenous psychoses. Three stages of development of schizophrenia-spectrum psychosis under the influence of psychogenic factors were singled out: psychogenic, end

Very high-dose clozapine and electroconvulsive therapy combination treatment in a patient with schizophrenia.

We report here the case of a 19-year-old man with paranoid schizophrenia who responded only partially to treatment with daily doses of clozapine, 1800 mg; aripiprazole, 20 mg; and clonazepam, 6 mg. We reduced the clozapine to 1600 to 1700 mg, maintaining therapeutic serum levels, and added a course of electroconvulsive therapy that included 24 treatments. Therapeutic results were minimal. However, adverse effects were limited to mild cognitive disturbances. Our experience adds to other reports suggesting that clozapine and electroconvulsive therapy can be combined without causing excessive adv

[Shared psychotic disorder (Folie à deux) and Huntington's disease].

INTRODUCTION: Folie à deux describes a rare psychiatric disorder of mostly closely related persons. The main feature of this disease is the unconditional adoption of the delusions of the primarily diseased person by the second person. This disturbance most frequently originates from a paranoid schizophrenia. METHOD: Our case is the first published description of a married couple with shared psychotic disorders which were caused by a genetically verified Chorea Huntington of the husband. RESULTS: The symptoms of the wife quickly declined after spatial separation from her primarily diseased hu

Cognitive-behavioral therapy of delusions: mental imagery within a goal-directed framework.

Central to psychotic disorders, delusions are associated with disability and often respond inadequately to pharmacotherapy. Cognitive-behavioral treatments have been developed over the last 20 years that successfully address delusions. However, meta-analyses suggest only a modest improvement in psychotic symptoms. Because delusions share considerable overlap with anxiety, adapting principles and techniques that have demonstrated efficacy in the treatment of anxiety disorders might improve the impact of cognitive-behavioral treatment of delusions. We report a case illustrating a cognitive-behav

Unassisted smothering in a pillow.

We report the case of a 33-year-old man admitted to a psychiatric hospital because of paranoid schizophrenia. The man was found dead lying in his bed with the face pressed against a pillow and with elevated buttocks. The autopsy did not reveal a cause of death. The histological findings displayed the signs of the haemorrhagic-dysoric syndrome with acute emphysema; these findings are pathognomonic of obstructive asphyxia. The adverse effects of the neuroleptics demonstrated by the toxicological findings may have accelerated the loss of consciousness and facilitated the unusual position of the b

An initial report of a new biological marker for bipolar disorder: P85 evoked brain potential.

OBJECTIVES: Progress toward understanding the neurobiological and genetic underpinnings of bipolar disorder has been limited by the scarcity of potential biological markers that predict its occurrence. A measure of the integrity of brain inhibitory function, sensory gating, measured using the amplitude of the evoked potential at 50 ms to the first of two paired clicks divided by the response to the second, has been characterized as a biological marker for schizophrenia. Currently, no such biological marker exists for bipolar disorder. The goal of this research was to determine how gating of an
