[The clinical peculiarities of psychogenically-induced exacerbations of paranoid schizophrenia].

One hundred and twenty-eight patients diagnosed with paranoid schizophrenia were studied. A main group comprised 84 patients with psychogenically-induced paranoid psychoses. A control group consisted of 44 patients with spontaneous exacerbations of the disease. Psychogenically-induced exacerbations of paranoid schizophrenia differed by the clinical polymorphism, heterogeneity and atypism. Three types of psychogenically-induced relapses of paranoid schizophrenia were singled out: endoreactive, delusional with affective symptoms and delusional. Higher frequencies of suicidal tendencies (32.1%),

[The use of nifedipine as a corrector of extrapyramidal side-effects of classical neuroleptics].

The aim of the article was to study efficacy and safety of nifedipine in the correction of extrapyramidal side-effects emerged during conventional neuroleptics therapy. Fifty-one patients diagnosed with paranoid schizophrenia received haloperidol in combination with nifedipine (25 patients) or haloperidol only (26 patients) during 26 weeks. Dynamics of psychopathological symptoms and extrapyramidal disorders were measured with PANSS, CGI, BARS, SARS and AIMS. Patients receiving haloperidol in combination with nifedipine had the lower AIMS scores indicating appearances of dyskinesia and less ma

[Conformational disturbances of serum albumin binding sites in schizophrenia].

The properties of serum albumin binding sites were studied using quenching of fluorescence of the molecular probe CAPIDAN (N-carboxyphenylimide of dimethylaminonaphthalic acid) with the nitrate anion. The samples of serum were obtained from 24 patients with paranoid schizophrenia and 24 healthy volunteers. In the absence of quencher the specific probe fluorescence was 1,4 times higher in patients than in volunteers. Fluorescence quenching constant for the probe bound to albumin was (M+/-m) 2,48+/-0,17 l/mol in patients versus 4,65+/-0,37 l/mol (p<0,01) in volunteers (p<0,01). The fluorescence

Two-stage designs to identify the effects of SNP combinations on complex diseases.

The genetic basis of complex diseases is expected to be highly heterogeneous, with many disease genes, where each gene by itself has only a small effect. Based on the nonlinear contributions of disease genes across the genome to complex diseases, we introduce the concept of single nucleotide polymorphism (SNP) synergistic blocks. A two-stage approach is applied to detect the genetic association of synergistic blocks with a disease. In the first stage, synergistic blocks associated with a complex disease are identified by clustering SNP patterns and choosing blocks within a cluster that minimiz

Neuropsychological aspects of delusional disorder.

Delusional disorders (DDs) are clinically rare syndromes characterized by false beliefs that are held with firm conviction despite counterevidence. The neuropsychology of DDs is poorly understood. Two partially opposing models--a cognitive bias model and a cognitive deficit model--have received mixed empiric support, partly because most research has been carried out in patients with paranoid schizophrenia, with which the nosologic association of DDs is unknown. Based on these models, we review empiric findings concerning the neuropsychology of DDs (narrowly defined). We conclude that DDs can b

[Schizophrenia and religiousness--a comparative study at the time of the two German states].

OBJECTIVE: Religious delusions are prevalent symptoms in schizophrenia. It is unclear, however, whether religious delusions reflect illness- or culture-related factors. METHODS: Cross-cultural retrospective comparison of the prevalence of religious delusions in 132 patients treated for paranoid schizophrenia in East Berlin (former GDR) from 1980 to 1985, and 151 patients from Regensburg (FRG) who were treated as in-patients from 1980 to 1983. RESULTS: Prior to the German re-unification, religious delusions were significantly less frequent in East Berlin compared to Regensburg (11.6% as oppos

Interactions between magnesium and psychotropic drugs.

Psychotropic drugs (antidepressants, antimanic drugs, antipsychotics, analgesic opioids, and others) are among the most frequently used medicines. Between these drugs and magnesium there are pharmacokinetic and pharmacodynamic interactions. Erythrocyte magnesium is decreased in patients with severe major depression (MD) vs normal subjects (44 +/- 2.7 mg/L in MD group vs 59.1 +/- 3.2 mg/L in control group, p < 0.01). Therapy with sertraline, 150 mg/day p.o. -21 days or with amitryptiline 3 x 25 mg/day p.o. 28 days increases significantly erythrocyte concentration of magnesium (56.9 +/- 5.22 mg/

Clinical and serotonergic predictors of non-affective acute remitting psychosis in patients with a first-episode psychosis.

OBJECTIVE: The study aimed to establish clinical predictors of non-affective acute remitting psychosis (NARP) and assess whether these patients showed a distinct serotonergic profile. METHOD: First-episode never treated psychotic patients diagnosed of paranoid schizophrenia (n=35; 21 men and 14 women) or NARP (n=28; 15 men and 13 women) were included. RESULTS: NARP patients showed significantly lower negative symptomatology, better premorbid adjustment, shorter duration of untreated psychosis, more depressive symptomatology and a lower number of 5-HT2A receptors than the paranoid schizophren

A mutation in the calreticulin gene promoter in a family case of schizoaffective disorder leads to its aberrant transcriptional activation.

We previously reported a G>C point mutation located at position -48 within the calreticulin gene core promoter in one individual suffering from paranoid schizophrenia that has also been diagnosed with affective disorder. However, a putative bona fide functional link between this mutation in the calreticulin gene and schizoaffective disorder has remained speculative. We have carried out calreticulin genotyping and psychiatric examination of all available members of the family of the proband and found that three more members carry the -48G>C mutation. Remarkably, two of them develop the disease

[Changes in the relationship between cognitive functioning and psychopathological symptoms in patients with paranoid schizophrenia in the periods of psychosis and development of remission].

The aim of our study was to investigate the relationship between cognitive functioning and psychopathological symptoms in patients with paranoid schizophrenia both in acute state and in the period of development of remission. Fourteen patients with paranoid schizophrenia (ICD-10, F20.0) were examined twice during 4,5-5 months, first--at the acute episode and second--at the period of development of remission during the treatment with olanzapine. EEG phase synchronization in patients with paranoid schizophrenia.

Recent findings suggest that specific deficits in neural synchrony and binding may underlie cognitive disturbances in schizophrenia and that key aspects of schizophrenia pathology involve discoordination and disconnection of distributed processes in multiple cortical areas associated with cognitive deficits. In the present study we aimed to investigate the underlying cortical mechanism of disturbed frontal-temporal-central-parietal connectivity in schizophrenia by examination of the synchronization patterns using wavelet phase synchronization index and coherence between all defined couples of

An unusual case of DRESS syndrome.

BACKGROUND: DRESS syndrome (drug reaction with eosinophilia and systemic symptoms) is a rare but severe drug reaction. OBSERVATIONS: A 27-year-old male with paranoid schizophrenia was hospitalized with all three diagnostic criteria of DRESS syndrome: cutaneous drug eruption, hematological abnormalities (presence of atypical lymphocytes on blood smear) and systemic involvement (generalized lymphadenopathy and hepatitis). On hospitalization the patient exhibited an unusual fever pattern of high temperatures in the morning hours and lower temperatures towards evening. In this case of a patient w

Classification of schizophrenic patients and healthy controls using [18F] fluorodopa PET imaging.

Striatal dopaminergic overactivity has been implicated in the pathophysiology of schizophrenia on the basis of in vivo neuroimaging studies. In particular, elevated striatal dopamine synthesis and storage has been repeatedly demonstrated in schizophrenia using the radiotracer 6-[18F] fluoro-l-DOPA ([18F] DOPA) and positron emission tomography (PET). Conventionally analysed [18F] DOPA PET imaging lacks the sensitivity or specificity to be used diagnostically. The aim of this study was to determine if the application of an Artificial Neural Network (ANN) would improve classification of images, a

Delusional disorder in old age and the risk of developing dementia: a nationwide register-based study.

OBJECTIVE: To examine whether very late first-contact delusional disorder carries a risk for later development of dementia. METHODS: By linkage of the psychiatric and the somatic nationwide registers of all out- and in-patients with hospital contact in Denmark, we included all 60+ patients with first ever from 1 January 1994 to 31 December 2001 with the index main diagnosis: delusional disorder. First contact osteoarthritis patients as well as the general population were used as controls. A total of 1,437 patients with persistent delusional disorder and 7,302 patients with osteoarthritis were

The influence of emotion clarity on emotional prosody identification in paranoid schizophrenia.

BACKGROUND: Identification of emotional facial expression and emotional prosody (i.e. speech melody) is often impaired in schizophrenia. For facial emotion identification, a recent study suggested that the relative deficit in schizophrenia is enhanced when the presented emotion is easier to recognize. It is unclear whether this effect is specific to face processing or part of a more general emotion recognition deficit. METHOD: We used clarity-graded emotional prosodic stimuli without semantic content, and tested 25 in-patients with paranoid schizophrenia, 25 healthy control participants and 2

Olanzapine-induced neuroleptic malignant syndrome in a patient with paranoid schizophrenia.

A case of a male patient with schizophrenic illness who developed neuroleptic malignant syndrome (NMS) following treatment with olanzapine is reported. Although typical neuroleptics are more frequently associated with NMS, atypical antipsychotics may also cause NMS. Case reports have been published concerning NMS and clozapine,(1) risperidone(2) and olanzapine.(3-6) This case report emphasizes the importance of being cautious when rapidly increasing doses of olanzapine are used in patients with psychiatric illnesses.

[Maintenance electroconvulsive therapy and treatment of refractory schizophrenia].

BACKGROUND: Electroconvulsive therapy, a standard treatment in mood disorders, is sometimes also indicated in psychotic disorders, especially in the treatment of refractory schizophrenia. In this instance, maintenance electroconvulsive therapy (M-ECT) can also become a long-term treatment. This paper presents the effects of M-ECT in the treatment of refractory schizophrenia using a retrospective analysis. Previous works showed that electroconvulsive therapy is effective on catatonia, anxiety with somatisation, lack of compliance, opposition, delusions especially with hallucinations and persecu

A new pathophysiological aspect of S100B in schizophrenia: potential regulation of S100B by its scavenger soluble RAGE.

BACKGROUND: Several studies have reported elevated S100B serum levels in schizophrenia. Our study focused on its scavenger, soluble receptor for advanced glycation end products (sRAGE). Given the benefits of sRAGE in metabolic and inflammatory diseases, we hypothesized a similar effect in schizophrenia. METHODS: S100B and sRAGE concentrations were explored during acute paranoid schizophrenia and during reconvalescence. Serum samples from 26 inpatients were investigated on hospital admission (T0) and 6 weeks posttreatment (T6) by S100B-immunoluminometry and sRAGE-ELISA. Thirty-two matched heal

The influence of temperament and character of psychotic individuals on the possibility of committing criminal offences.

Cloninger's theoretical biosocial model of personality represents the personality as a hierarchical organisational system consisting of 4 temperament dimensions and 3 character dimensions as 3 aspects of self-concept. It attempts to define behavioural and bio-genetic aspects of temperament, and the neuroanatomical and biochemical brain network responsible for activation, maintenance and inhibition of behaviour. The basic objective of this research is to establish whether temperament and character act as measures of personalities in psychotic persons on the possibility of committing criminal of
