[Alcohol-induced hallucinosis. Clinical aspects, pathophysiology and therapy].

In recent years, little research has been focused on alcohol hallucinosis. The psychopathology of alcohol hallucinosis (vivid acoustic hallucinations, paranoid symptoms and fear) resembles paranoid schizophrenia, but other organic mental disorders have to be excluded too. Prognosis is usually good, but in 10-20% of cases alcohol hallucinosis tends to become chronic. Possible pathophysiological mechanisms involved in the development of the syndrome are changes in dopaminergic transmission or other neurotransmitter systems and neuronal membranes, elevated levels of betacarbolines and an impaired

Patterns of psychiatric diagnosis in a county mental health system.

This article examines concordance in psychiatric diagnoses received by 280 clients of a county mental health system throughout their history with the system. Automated Information System records from all episodes of care since the first admission to the county system were analyzed, and diagnoses for each client for the first 5, first 10, last 5, and last 10 episodes were compared with generalized kappa. Sequential patterns of diagnosis were also examined. Overall concordance for diagnoses for the last 5 episodes was fair (kappa = .46), and concordance for major depression, bipolar disorder, an

Reversible thioridazine-induced magnetic resonance imaging-documented pituitary enlargement associated with hyperprolactinemia.

OBJECTIVE: To document a case of reversible thioridazine-induced pituitary enlargement associated with hyperprolactinemia in a patient with schizophrenia. METHODS: We describe a 19-year-old woman with paranoid schizophrenia who was taking thioridazine (a phenothiazine), in whom hyperprolactinemia, galactorrhea, oligomenorrhea, and a reversible anatomic pituitary abnormality developed. Serial magnetic resonance imaging (MRI) studies were used to assess the status of the pituitary gland during various pharmaceutical therapies. RESULTS: Laboratory evaluation revealed increased serum prolactin (

Treatment and diagnostic subtype in facial affect recognition in schizophrenia.

Patients with schizophrenia have been described as having deficits in the ability to recognize facial expressions of emotion. We report the results of a study on the effects of global psychopathology, positive and negative symptoms, diagnostic subtype, and antipsychotic medications, on the ability of subjects with schizophrenia to recognize facial affect. Eighteen SCID diagnosed patients with schizophrenia and ten matched controls were evaluated at a drug-free baseline for ability to identify facial expression expressed in a standardized series of photographs, with concurrent measures of globa

Maintenance medication for schizophrenia and schizoaffective patients.

This study assesses the different approaches to treating patients with schizoaffective and paranoid schizophrenia in remission. Individualized treatment of 220 outpatient schizophrenia patients was conducted for 4 years. The choice of treatment was based on the course of the disease and the frequency of relapses. The influence of changes in treatment on the patterns of relapses is presented. The results of this prospective followup open study were evaluated by comparing data received during our research with data from the two preceding 4-year periods and with data from the control group. Positron emission tomography and plasma biochemistry findings in schizophrenic patients before and after electroconvulsive therapy.

The clinical effects of electroconvulsive therapy (ECT) on the morbidity of paranoid schizophrenic patients were assessed by positron emission tomography (PET) and plasma biochemistry studies before and after ECT. The present study included five patients whose average age was 41.4 years. The average duration of illness was 23.0 years. To avoid any effect of changes in drugs on PET, no changes were made in the medication of any of the five patients during the study period. ECT improved the clinical symptoms in every patient. Regional cerebral blood flow (rCBF) on PET in both temporal lobes and

Perceived consequences to the predictor: a variable in the release of psychiatric patients.

The present study examined factors hypothesized to influence mental health professionals' perceptions of dangerousness, predictions of violence, and decisions on patients' release. 120 mental health professionals employed in state mental hospitals were each given one of 12 patient profiles. The independent variables, manipulated within vignettes, were (a) violence history, (b) paranoid schizophrenia versus nonparanoid schizophrenia, and (c) perceived consequences in terms of liability and publicity. Type of schizophrenia did not affect ratings, but violence history of the predictee and perceiv

Filgrastim treatment of three patients with clozapine-induced agranulocytosis.

BACKGROUND: Agranulocytosis is the most serious side effect of clozapine therapy, occurring in approximately 1% of all treated patients. Despite careful blood monitoring, a significant number of cases of agranulocytosis and resulting fatalities have occurred. Strategies are needed to manage clozapine-induced agranulocytosis more safely. METHOD: This report describes the management of three state hospital inpatients who developed clozapine-induced agranulocytosis. All patients were diagnosed as having chronic paranoid schizophrenia according to DSM-III-R criteria and had previously failed to r

State psychiatric hospital patients with past arrests for violent crimes.

OBJECTIVE: This study examined associations between four types of major psychopathology--schizophrenia, schizoaffective disorder, and bipolar and unipolar affective disorders--and history of violent crime. The effects of demographic variables, substance abuse, psychosis, and paranoia on history of violent crime were also determined. METHODS: Diagnostic assessments using the Schedule for Affective Disorders and Schizophrenia and Research Diagnostic Criteria identified 172 state hospital inpatients with the four diagnoses of interest, as well as those with co-existing substance use disorders. Initial assessment of hospital treatment by patients with paranoid schizophrenia: a predictor of outcome.

The value of schizophrenic inpatients' initial global assessments of treatment in the prediction of outcome was investigated. Within 3 days of admission, 31 patients with an acute paranoid schizophrenic psychosis according to ICD-10 rated on a visual analog scale to what extent they believed the treatment they were receiving was right for them. Outcome criteria were overall clinical changes measured on the Brief Psychiatric Rating Scale and the Intentionality Scale. The patients' initial global assessments of treatment were significantly correlated to both outcome criteria, indicating that pat

Court ordered obstetric intervention: a commentary.

A case is presented where the Courts have authorised an obstetric intervention deemed necessary for the well-being of both mother and child. Although the case is one of maternal psychosis, there are legal and ethical concerns whenever court-ordered intervention is deemed necessary. Approaches to this difficult medical decision making problem in the form of utilitarian "burdens v benefit" ratio analysis or the recognised traditional ethical principles of beneficence, nonmaleficence, justice and acting in the patient's best interest are considered. [A high serum creatine kinase picture due to too much or too little exercise].

In case I, a 31-year-old man who had undergone unusual high-performance training, painful swellings occurred in the left arm and creatine kinase activity was raised (28,644 U/l), as well as that of the transaminases. A rapid urine test for blood was positive, but the sediment was free of red blood cells. There were no other pathological findings. Fluid intake was increased to 31 daily and on the third day he was symptom-free and creatine kinase activity had fallen to 6475 U/l, reaching normal values on the 15th day. Case 2 concerned a 28-year-old woman with paranoid schizophrenia who had taken

Neurologic abnormalities in murderers.

Thirty-one individuals awaiting trial or sentencing for murder or undergoing an appeal process requested a neurologic examination through legal counsel. We attempted in each instance to obtain EEG, MRI or CT, and neuropsychological testing. Neurologic examination revealed evidence of "frontal" dysfunction in 20 (64.5%). There were symptoms or some other evidence of temporal lobe abnormality in nine (29%). We made a specific neurologic diagnosis in 20 individuals (64.5%), including borderline or full mental retardation (9) and cerebral palsy (2), among others. Neuropsychological testing reveale

[Symptomatologic characteristics and psychopathology of expression of an "attack of altered perception" in schizophrenic patients].

Eleven patients with schizophrenia who developed an "attack of altered perception" are reported and discussed from the viewpoints of the symptomatologic characteristics of their mental symptoms, physical symptoms, and drawing expression. It was found that the attack appeared in patients with non-paranoid schizophrenia as well as in non-schizophrenic patients on major tranquilizers. Some patients developed obvious visual and auditory hallucinations, etc. Most visual hallucinations were of, or related to, an eye. The hallucinations were largely non-schizophrenic organic hallucinations that J. Gl

[The characteristics of the cognitive defect in patients with different variants of late paranoia].

Evoked cortical activity in response to acoustic stimuli (oddball paradigm) was studied in patients with different paranoid disorders. The analysis covered the N150 negative wave and positive complex P300. In patients with paranoid schizophrenia and paranoid form of involutional psychosis cortical responses to the stimuli and cortex effects were not recorded. Such cognitive deficiency is believed to be typical for schizophrenia.
