The neuroleptic malignant syndrome (NMS) is a rare but potentially fatal reaction associated with neuroleptic drugs. The role of an acute reduction in brain dopamine activity in the development of NMS is commonly accepted as underlying pathogenesis. The diagnosis is maintained by the classic findings of extrapyramidal signs, hyperthermia and autonomic dysfunction. Treatment consists primarily of early recognition and discontinuation of triggering drugs. We report on a young patient with an acute paranoid schizophrenia who suffered a severe polytrauma due to a jump from 10 m height initiated by

HISTORY AND CLINICAL FINDINGS: A 22-year-old oligophrenic patient had on several occasions over several months been given various neuroleptics (haloperidol, benperidol, levomepromazine) for exacerbations of paranoid schizophrenia. For a few days before hospitalization he had become quiet and withdrawn, but on the day of admission 100 mg chlorprothixene was administered intramuscularly when he had become agitated. At admission he was somnolent, his general condition was disturbed. He had hyperhidrosis and hypersalivation, as well as tachycardia (112/min) with a normal body temperature of 37.8 d

Delusional misidentification syndromes (DMS) are beginning to be well described clinically but little is known about their epidemiology. To try and obtain an estimate of their prevalence, a survey was performed of all admissions to a locked psychiatric inpatient unit from April 1983 to June 1984. 26 patients satisfied clinical criteria for DMS during this time and overall 835 patients were admitted to the unit. Thus, a crude prevalence of 3.1% was found. The median age of the patients was 29 years. The overwhelming majority had a principal psychiatric diagnosis of paranoid schizophrenia and on

In the present study, quantitative EEG analysis was performed at rest and during 10 Hz photic stimulation in 14 drug-naive patients with first-episode paranoid schizophrenia and 20 sex- and age-matched control subjects. Compared with the normal controls, the patients had significantly lower alpha-2 band amplitude in the resting EEG over all recording regions. No significant group differences were found in other frequency bands. In addition, EEG analysis during photic stimulation demonstrated that the patients had a rather uniform topographic profile in EEG amplitude for the alpha band, with si

Since the beginning of the neuroleptics in 1952, French psychiatrists have proposed a classification of neuroleptics, taking into account the pharmacological and therapeutic differences between these drugs. They distinguished 3 different clinical effects of neuroleptics: sedative effects, effects on the positive symptoms of schizophrenia and effects on the negative symptoms. The effect of some neuroleptics on negative symptoms is recognized by the international community, which considers clozapine to be effective. In France, in most cases, the indication of clozapine is still refractory parano

Referring to some critical comments on operational diagnostics in view of the adoption of the 10th Revision of the International Classification of Diseases, the relevance of symptomatology for diagnosis using ICD-8 and ICD-9 was investigated in a retrospective study. Inpatients with neurotic depression, endogenous depression, schizoaffective psychosis, manic psychosis and paranoid schizophrenia were included in the study. Data came from routine documentation of every episode of treatment, symptoms being assessed according to the AMP system. Discriminant analyses revealed psychopathology at adm

The scientific discussion on the interrelationship of "alcoholism and psychosis" has been dominated by nosological problems for years. Alcohol hallucinosis, a comparatively rare psychosis in chronic alcoholism, can mimic the symptomatology of paranoid schizophrenia. The clinical features (vivid acoustic hallucinations and fear) are often inconclusive, as, unlike other organic psychoses, alcohol hallucinosis does not entail clouding of sensorium and disorientation. On the one hand follow-up studies suggest that alcohol hallucinosis in some cases can take a chronic schizophrenia-like course. Esp

We present a 43-year-old woman with a right frontal parasagittal meningioma of the brain who developed the delusion that her husband and children had been replaced by doubles (look-alikes). This type of delusion is typical for Capgras' syndrome. After removal of the tumor the delusion disappeared. The majority of such patients are diagnosed as paranoid schizophrenia. However, in the past decade there has been an increasing number of cases in which the etiology is suspected of being organic, involving mainly the right hemisphere. The case presented is unique because it is the first with meningi

About one third (29.5%) of the population living within the area of Frankfurt, for which the psychiatric university clinic provides psychiatric service, are foreigners. For the better understanding of our foreign psychiatric inpatients treated in our clinic in in 1992, the diagnostic spectrum and the nationalities of these patients were analysed. More than half of them (58%) were raised up in former Yugoslavia, Turkey, Italy, Poland and Marocco. The other foreign patients came from 36 different countries. The diagnostic spectrum comparing german and foreign patients showed marked differences.

1. Patients with undifferentiated and paranoid schizophrenia, and normal controls were compared using 2 versions of the covert orienting of attention procedure which evaluate exogenous (automatic) and endogenous (controlled) cuing mechanisms. 2. For both tasks, attentional performance varied with illness subtype, but in different ways. 3. On measures of automatic orienting undifferentiated patients showed evidence consistent with a mild right visual field deficit, while paranoid showed a reduction of inhibition-of-return, a mechanism which biases against returning to previously attended locati

Clinical and biological measures were examined for their relationship to clinical response to clozapine. Associations were found between therapeutic response and the following variables: male gender, paranoid schizophrenia subtype diagnosis, older age at onset of illness, shorter duration of illness, higher levels of pretreatment acute EPS, low pretreatment CSF HVA/5-HIAA, greater decrease in prolactin (PRL) and increase in growth hormone (GH) response to apomorphine stimulation pretreatment and greater inhibition by clozapine treatment of PRL and GH response to apomorphine, and plasma clozapi

BACKGROUND: An Indian man now in Britain explained his criminal behaviour as episodic ghost possession. Traditional exorcisms failed to help.
METHOD: A 'Western' diagnosis of dissociative state or paranoid schizophrenia was made. Treatment commenced using trifluoperazine and clopenthixol.
RESULTS: The patient underwent remission during neuroleptic treatment, despite previous evidence of genuine possession.
CONCLUSIONS: Many cultures give rise to apparently genuine cases of ghost possession. Neuroleptics may relieve symptoms of exorcism-resistant possession.

BACKGROUND: Late paraphrenia is recognised as a heterogeneous disorder. This is reflected by the division of such patients into schizophrenia and delusional disorder in ICD-10. Earlier imaging studies have suggested that major structural abnormalities may be associated with the onset of psychosis in later life.
METHOD: Fifty late paraphrenics and 35 age-matched healthy controls underwent structural magnetic resonance imaging of the whole brain in the coronal plane. Measurements were made of intracranial and brain volumes and the volumes of the intracerebral and extracerebral cerebrospinal flu

OBJECTIVE: This study addressed the unique clinical properties attributed to the atypical antipsychotic clozapine, including its efficacy in patients with treatment-refractory psychosis and against negative symptoms, its lack of acute extrapyramidal side effects, and the longer time course of its therapeutic effects.
METHOD: The clinical responses of 84 schizophrenic inpatients (66 with treatment-refractory illness and 18 who were intolerant of antipsychotic treatment) were examined. After all previous antipsychotic medications had been withdrawn, the patients were treated with clozapine acco

OBJECTIVE: The authors sought to clarify differences in outcome and familial psychopathology among the classical subtypes of schizophrenia.
METHOD: In the epidemiologically based Roscommon Family Study, personal interviews were conducted with 88% of traceable living probands (N = 415) an average of 16 years after illness onset and with 86% of traceable living first-degree relatives (N = 1,753). Probands meeting the DSM-III-R criteria for schizophrenia were subtyped by DSM-III-R and ICD-9.
RESULTS: By both diagnostic systems, age at onset differed significantly across subtypes, being earliest

This paper presents the results of a study carried on an urban population in Belgrade investigating the connections between relapse in schizophrenia and the expressed emotion (EE) status of families where at least one of the patient's parents was a member of the household. The overall rate of high EE was just under 50%, in the middle of the range of values reported in studies carried out elsewhere. Relapse was found to be 10 times more frequent in patients whose families were rated high in EE. The sample consisted of 30 patients with hebephrenic schizophrenia and 30 with paranoid schizophrenia
