[Defense mechanisms in paranoid schizophrenia].

A group of 90 hospitalized patients suffering from paranoid schizophrenia were examined and than compared with parallel group of 90 healthy volunteers. The following methods of examinations were used: Plutchick's LSI-Test, Spilberger's STAI-Test, Szustowa's Egocentric Association Test and an Inventory of Self-Description by Lukaszewski. The results indicate that schizophrenic patients in comparison to healthy persons use the specific defense mechanisms (rationalization and negation) more often and have higher level of egocentrism. On the other hand both groups can be characterised by a low lev

Replicated psychometric correlates of schizophrenia.

OBJECTIVE: The authors' goals are to use scales from the MMPI hypothesized in their previous research to be correlates of liability to schizophrenia to differentiate DSM-III schizophrenia from bipolar and unipolar affective illness and to cross-validate these correlates in an independently ascertained sample of patients with Research Diagnostic Criteria (RDC) schizophrenia or affective disorder. METHOD: The criterion sample consisted of 83 patients consecutively admitted to a state-operated community mental health center. Diagnosis of schizophrenia; bipolar disorder, manic; and major depressi

[Antibodies to neuroleptics and their role in the mechanisms of the development of drug resistance in psychopharmacotherapy of patients with paranoid schizophrenia].

A study was made of the content of drug antibodies (DAT) to chlorpromazine, triphthazine and haloperidol in the blood sera of 112 patients with paranoid schizophrenia (acute period, curable exacerbation associated with the chronic course, and a group of therapeutically resistant subjects). Antibodies to neuroleptics were demonstrable in all the patients' groups. However, the rate of positive reactions and the content of DAT in the therapeutically resistant patients differed significantly from the characteristics in "acute" patients and in the group of "nonresistant" ones. The maximum "antigeni

[Effect of delayed-action Mirenil (Polfa) on the hypothalamic function in patients with schizophrenia].

Hormonal tests--TRH, ITT and L-RH--were performed in 10 cases of paranoid schizophrenia before the paranoid deterioration and after--on average--ten months of treatment with fluphenazine depot injection. The drug did show a clear modulating effect on the mechanisms regulating secretion of gonadotrophins and prolactin --it did not effect the secretion of thyrotropin , thyroxine, triiodothyronine, cortisol, growth hormone or insulin .

Delusions of substitution and diabetes mellitus.

A male patient with chronic paranoid schizophrenia developed delusions of substitution that were of abrupt onset and short duration. This coincided with the detection of diabetes mellitus. Over the next three years, despite being on continuous moderate dose neuroleptic treatment the re-emergence and resolution of delusions of substitution varied with the control of his diabetic status. A selective review of reports on organic contributors to delusions of substitution is presented. It is suggested that the abruptly appearing, short lasting delusions of substitution may be more likely to have a

[Zotepine versus perazine in patients with paranoid schizophrenia: a double-blind controlled trial of its effectiveness].

The dibenzothiepine zotepine is a new potential "atypical" neuroleptic exhibiting powerful antiserotonergic and antidopaminergic properties. The efficacy of zotepine was evaluated in a double-blind controlled trial versus the tricyclic neuroleptic perazine in 41 patients suffering mainly from the paranoid-hallucinatory type of schizophrenia. The key outcome variable was the extent of mental disturbance as defined by the total score of the BPRS. Additional outcome variables were GAS and CGI. In addition, adverse reactions and extrapyramidal side effects were assessed according to the FSUCL scal

Cocaine-induced psychosis.

BACKGROUND: Chronic stimulant use can produce a paranoid psychosis that is similar to acute paranoid schizophrenia. While this phenomenon has been systematically explored in amphetamine abusers, it has been relatively unexplored in a systematic fashion in cocaine abusers. METHOD: The experience of cocaine-induced psychosis was evaluated in 55 individuals consecutively admitted for treatment of DSM-III-R cocaine dependence. Each subject was interviewed about their experiences of psychosis while intoxicated by means of a standardized, semistructured interview. RESULTS: Fifty-three percent (29/

Paranoid schizophrenia in adolescence.

BACKGROUND: Paranoid schizophrenia is considered to be a rare condition in adolescence. Since this is contrary to the authors' clinical experience, they hypothesized that a controlled study would show that a significant number of adolescents would be diagnosed with paranoid schizophrenia and that scores from the childhood version of the Schedule for Affective Disorders and Schizophrenia (K-SADS) would differentiate between the paranoid schizophrenic adolescents and adolescents with other types of schizophrenia or with affective disorder. METHOD: The authors conducted a prospective study of 12

EEG power variation in schizophrenic subgroups: effects of emotionally salient stimuli.

Eighteen unmedicated chronic schizophrenic patients and 13 normal controls were tested in a paradigm designed to examine functional changes in electroencephalographic (EEG) activity following presentation of emotionally salient auditory stimuli and control tones. Five standard bands of EEG spectral power were examined at bilateral frontal and temporal surface recording sites. The schizophrenic subjects were assigned to diagnostic subgroups on the basis of DSM-III-R criteria following independent clinical examination by two staff psychiatrists. Those subjects who met DSM-III-R criteria for para

Natural history of schizophrenia subtypes. I. Longitudinal study of paranoid, hebephrenic, and undifferentiated schizophrenia.

To explore the validity of different approaches for subtyping schizophrenia, the conditions of 187 schizophrenic patients from the Chestnut Lodge follow-up study were rediagnosed with the use of classic subtype criteria. Independently collected data allowed construction of a longitudinal profile of the natural history of illness for patients who met operational criteria for paranoid (n = 78), hebephrenic (n = 26), and undifferentiated (n = 83) schizophrenia. Paranoid schizophrenia had an older age at onset, often developed rapidly in individuals with good premorbid functioning, tended to be in

[T- and B-lymphocytes and their clinico-pathogenetic significance in patients with paranoid schizophrenia].

As many as 43 patients with first diagnosed paranoid schizophrenia running a shift-like course were examined for the blood counts of T and B lymphocytes with regard to the treatment conducted and disease standing. The patients received 3 types of the treatment: insulin, neuroleptics and combined treatment (insulin plus neuroleptics). During the treatment and after it, the content of B lymphocytes remained practically unchanged (p greater than 0.05) in all the patients. The content of T lymphocytes was reduced throughout the whole period of affliction with schizophrenia. However, during the com

Rape and homosexuality.

The relationship between rape and homosexuality can be best understood by studying male rape. Offender(s) may deny homosexuality and confuse an aggressive active behaviour with a masculine (even macho) behaviour. They may project their homosexual feelings onto the victim. In the aftermath of rape, the victim may be ashamed of having lost control and may question himself about his real sexual identity. An aspect of female or male rape by a group of males may be an attempt to annihilate latent homosexual tendencies existing between members of this group. Surprisingly, in the short term, post tra

Acquired immune deficiency syndrome as a novel content in delusion symptomatology.

The case histories of three female patients are presented (two of them diagnosed as paranoid schizophrenia, one as endogenous depression in the course of involution), who manifested the delusion of AIDS disease in the psychopathological image of their primary illness. This novel content of delusional symptomatology appears in the thinking of psychiatric patients as a distorted reflexion of a new objective reality of the world, the AIDS disease, becoming recently one of the global problems of the human population.

[Case report of a patient with psychomotor epilepsy and paranoid psychotic episodes].

This paper gives a brief review of psychomotor epilepsy, one of the most common forms of epilepsy manifestations. The psychopathology which is seen with psychomotor epilepsy includes the greatest part of known pathological psychic manifestations with epilepsy in general. Here, a patient is presented with psychomotor epilepsy since his youth. In the last eight years, psychotic manifestations appear with him from time to time, they are not related with epileptic seizures, and phenomenology-wise, the clinical manifestation is that of paranoid schizophrenia. In the end there is a stress on differe

Families, murder, and insanity: a psychiatric review of paternal neonaticide.

Neonaticide is the killing of a newborn within the first 24 h of life. Although relatively uncommon, numerous cases of maternal neonaticide have been reported. To date, only two cases of paternal neonaticide have appeared in the literature. The authors review neonaticide and present two new case reports of paternal neonaticide. A psychodynamic explanation of paternal neonaticide is formulated. A new definition for neonaticide, more consistent with biological and psychological determinants, is suggested.

Psychopathological characteristics in alcohol hallucinosis and paranoid schizophrenia.

In a retrospective study psychopathology, psychiatric family history and demographic data were studied in 53 patients with alcohol hallucinosis and an age- and sex-matched control group of 53 patients with paranoid schizophrenia who had been treated at the Psychiatric Hospital of the University of Munich between 1979 and 1986. The psychopathology was assessed by the AMDP system. The paranoid-hallucinatory symptoms in both conditions were very similar; verbal hallucinations and delusions of reference were most common in both groups. Disorders of ego were found in 87% of the schizophrenics, but

Neuropsychological deficits and violent behavior in incarcerated schizophrenics.

The authors studied 37 male schizophrenics in a county jail psychiatric unit. Subjects were classified as impaired or not according to Luria-Nebraska Neuropsychological Battery criteria. Twelve of the 37 patients were impaired. Violence ratings were made on inpatient behavior and adult criminal records. Impairment status was related to adult history of violence but not inpatient violence. The most violent patients (by criminal record) were from the impaired category.

Immunoglobulin profile in acute psychiatric disorders.

In a prospective study of 80 new female patients with acute psychiatric disorders (40 each with schizophrenia and major affective disorder), elevated (P less than 0.001) serum concentrations of IgG and IgA were found in comparison to healthy control groups matched for age and sex. In schizophrenic patients the mean levels of IgG (1682.76 mg/dl) and IgA (321.12 mg/dl) were found to be significantly more as compared to those in the control group (IgG-1213.18 mg/dl and IgA - 210.14 mg/dl). Patients with paranoid schizophrenia had significantly high mean serum levels of IgG (1896.86 mg/dl) and IgA
