[Structure of the paraphrenic syndrome in the clinical picture of paranoid schizophrenia].

Seventy-one patients with paranoid schizophrenia were examined to study the pattern and distinctive psychopathological features of the paraphrenic syndrome which developed following the paranoiac, paranoid and hallucinational-paranoid syndromes. Transformation of the paranoiac syndrome into the paraphrenic one was of a gradual nature, with delusional perception in a reduced variant comprising the phenomenological structure of delusion. The interpretative mechanism of delusional ideas remained constant. The transformation of the paranoid syndrome into the paraphrenic one was attended by a furth

[Clinical manifestations of schizophrenia in right and left-handed patients].

In a series covering 1177 right-handed and 93 left-handed schizophrenic individuals, the author elicited the age of the patients at the onset of disease and correlated different forms of the disease course. Among the right-handed patients, the disorder was more likely to become manifest at the age of 35 to 44 years and the incidence of the shift-like form was elevated. The left-handed subjects showed an earlier onset of disease (20-24 years) and the prevalence of progressive paranoid schizophrenia. In the author's opinion, the cerebral functional asymmetry related to the type of one's handedne

The patient exhibiting episodic violent behavior.

This case presentation describes a 21-year-old white male patient seen for episodic violent behavior. The case is seen as relevant for primary care physicians because of the frequency of violent acting out in the general population, the importance of making an accurate diagnosis, and the many medical, psychological, and social implications involved in diagnosing such disorders. In such cases it is important that the physician keep an open mind and include within the differential diagnosis a variety of psychiatric disturbances such as paranoid schizophrenia, antisocial personality disorder, dru

Brain disconnection, schizophrenia, and paranoia.

A lateral deficit explanation of schizophrenic cognition maintains that a left hemisphere deficit is characteristic of nonparanoid schizophrenia whereas right hemispheric deficits may be common in paranoid schizophrenia. An alternate explanation postulates an interhemispheric deficit in schizophrenic functioning. A major piece of evidence for this position is Beaumont and Dimond's oft-cited experiment matching pairs of stimuli presented to left, right, and both hemispheres (Beaumont, J. G., and Dimond, S. J. Brain disconnection and schizophrenia. Br. J. Psychiatry, 123: 661-662, 1973). We expa

[Psychoses and unidentified flying objects].

Some individuals claim to have come into contact and communicated with occupants of flying objects of extraterrestrial origin who often would have entrusted them with "missions" regarding the safeguard of humanity. The authors, who have observed six such subjects, conclude that five of them suffered from a paranoid delusional state often akin to paraphrenia; whereas other analogous cases previously reported have been diagnosed as suffering from paranoid schizophrenia. Their beliefs, inaccessible to criticism and to reasoning, presumably help them to remain outside psychiatric reach. The presen

Schizophrenia and multiple sclerosis.

A case of multiple sclerosis was diagnosed as paranoid schizophrenia 10 years earlier. The schizophrenic disorder, severe and almost nonremittent for 10 years, failed to respond to adequate drug therapy. The computerized axial tomography scan revealed moderate atrophy of the anterior cerebellar vermis, a finding of interest in view of studies which have implicated anterior vermis pathology in the pathogenesis of schizophrenia.

Sociocultural factors in the psychiatric assessment of black patients: a case study.

Accurately diagnosing psychiatric disorders in black patients has been a controversial subject. Criticism has been made that blacks are more likely to receive a diagnosis of psychosis, eg, paranoid schizophrenia. With proper use of the Diagnostic and Statistical Manual of Mental Disorders (DSM III), the author anticipates greater diagnostic accuracy with blacks. The multiaxial perspectives of DSM III, particularly axis IV, help resolve some critical aspects of diagnostic difficulties. This case illustrates the diagnostic errors frequently made when variables of culture, ethnicity, and psychoso

A search for association of HLA antigens with paranoid schizophrenia. A9 appears as a possible marker.

Sixty-two Dutch patients with the diagnosis paranoid schizophrenia (SCH) were HLA-A-, -B- and -C-typed. An increase in the frequencies of A9 (P = 0.02, relative risk 1.8) and B5 (P = 0.04, relative risk 1.9) was found. Although these correlations do not remain significant after correction for the number of antigens tested, both findings confirm other data from the literature, including the first published report from a population in Sweden. From all hitherto published literature data, the combined relative risks for A9 and B5 is significantly increased. These data strongly indicate that the di

A survey of the criminal careers of restriction order patients.

The offenders committed to local hospitals in 1962 and 1964 under a hospital order with restriction of discharge were followed up for 15 years with regard to subsequent court appearances, hospital admissions, and death. One hundred and forty-six were traced. Of these, 49 per cent had been diagnosed as mentally ill: most of this group were committed for violent offences. Thirty-five per cent were mentally subnormal: they were frequently committed for sexual offences, arson and property offences. Twelve per cent were diagnosed psychopathic: this group had a wide variety of committal offences. Fo

Medical genetics, Huntington's chorea, and legal questions pertaining to autopsy.

A family with Huntington's chorea was evaluated. The 65-year-old male proband had been diagnosed as a paranoid schizophrenic. Because of Huntington's chorea in the family, he was considered "affected" by many of his relatives. Following his death, autopsy was refused by the patient's son, who had been his legal guardian. Legal consultation eventually led to autopsy, which showed no evidence of Huntington's chorea. Genetic counselling was provided for his children and siblings. Pertinent legal and neuropsychiatric issues in Huntington's disease are reviewed.

Continuous amphetamine intoxication: an animal model of the acute psychotic episode.

When amphetamines are administered to humans every few hours for several days, either during the 'speed runs' of addicts or in controlled laboratory settings, the psychosis which reliably results is similar to paranoid schizophrenia in a number of important aspects. This unique regimen of drug intake, which involves the continuous presence of stimulants over a prolonged period of time, can be simulated in animals using subcutaneously implanted slow-release silicone pellets containing d-amphetamine base. Monkeys and rats implanted with these pellets develop stages of behavioural alterations whi

[Operationalized diagnosis of schizoaffective and cycloid psychoses].

Case histories of 128 patients suffering from functional psychoses were evaluated. The operational definitions for schizoaffective psychoses (SAP) of Kendell, Welner, Spitzer (RDC) and Feighner were applied. Of the 30 cases of SAP based on ICD 8 (295.7), Kendell's criteria were fulfilled in 97% Welner's in 77%, Spitzer's in 70% and Feighner's in 30%. The highly sensitive criteria of Kendell are best qualified for delineating the schizoaffective (sa) syndroms, but have the disadvantage of not requiring a time limitation when examining the longitudinal course of the illness. In this respect, the

The psychodynamics of the presidential assassin and an examination of the theme/graphic variables of his threatening correspondence.

Many document examiners recognize the importance of an individuals 'state of mind' and its effect on handwriting but historically have steered clear of working closely with psychiatrists, linguists, or psychologists in understanding the interaction of nervous impulses and motor responses on the final handwritten product. In an attempt to better understand and evaluate those fears and motivational influences which should exist within the writings of the mentally disturbed in his threatening correspondence, research was conducted to ascertain whether there are consistently reappearing delusional

A survey of the psycho-social aspects of murderers in Iran.

The psycho-social aspects of 72 murderers (one female, 71 males) were studied. The murderers came from the Khoozestan and the Lorestan provinces of Iran. One of them had been transferred from the north of Iran (probably on exile). The murderers were mostly from lower social classes and crowded homes. The majority of them had worked as unskilled labourers or farmers. A great number of them regretted their action but a few were proud of what they had done. They believed that they had saved the chastity of their family which is very important in Islam. None of them were epileptic nor had any hist

[Incidence of paranoid schizophrenia].

The incidence of paranoid schizophrenia (the number of new cases per year) was examined in a population. The figure was found to be 0.17 per 10,000 of the population, being greatly dependent on the age and sex structure of the population examined. The method of the examination is presented in detail. It is shown, in particular, that the results are considerably influenced by the disease duration in the patients. With a short duration of the disease the results appear to be greatly underestimated because of the difficulties in making the correct diagnosis in many cases. The exact number of such

[Anthropometric characteristics of paranoid schizophrenics as a conjectured marker of the age of manifestation of the process].

Clinico-anthropometric investigation of 300 patients suffering from paranoid schizophrenia has been carried out. The results of a correlation analysis of anthropometric characteristics and of an analysis of manifestation age of schizophrenia showed that the closest connection of the traits studied exists in the group of females with the attack-like paranoid schizophrenia. The data are interpreted as the confirmation of the X-chromosome effect in schizophrenia and of interaction between the locus system of schizophrenia and the morphotype system.

Neurological differences between paranoid and nonparanoid schizophrenia: part I. sensory-motor lateralization.

Sensory-motor lateralization was studied prospectively in a consecutive sample of male chronic schizophrenic patients. The sample was divided into paranoid (N = 27) and nonparanoid (N = 53) subjects according to the Tsuang-Winokur criteria. Significantly more left handedness was found in the paranoid group, suggesting the left hemispheric dysfunction may be important in the etiology of paranoid schizophrenia.
