Monosymptomatic psychoses which progress to schizophrenia.

Two cases of paranoid schizophrenia are described in which an olfactory delusion was the predominant symptom at some stage in the illnesses. A distinction is drawn between these cases, and cases of Olfactory Reference Syndrome, in which the olfactory delusion is the main feature and schizophrenic decompensation does not occur. An argument is advanced that the Olfactory Reference Syndrome (ORS) is one subtype of Monosymptomatic Hypochondriacal Psychosis (MHP) and reference is made to evidence suggesting that pimozide may be a specific and effective treatment in the various presentations of MHP.

Biochemistry and the schizophrenia. Old concepts and new hypothesis.

More than a decade of scientific inquiry into the biochemistry of schizophrenia has been organized by the dopamine hypothesis. The evidence that neuroleptics reduce brain dopamine activity is compelling and derives from both human and animal studies. In addition, agents which enhance brain dopamine activity, such as amphetamine or cocaine, can cause a syndrome that can be indistinguishable from acute paranoid schizophrenia. However, a major problem with the dopamine hypothesis is the lack of strong direct evidence of altered dopamine concentrations or metabolism when measured in large groups o

HLA antigens and subtypes of schizophrenia.

Studies on HLA antigens in schizophrenia have produced conflicting results, but there has been greater agreement when clinical subtypes of the disorder have been separated. In view of this, we reassessed 68 previously studied patients with hospital diagnoses of schizophrenia and, while blind to their HLA types, used operational criteria to define clinical subtypes. We compared and combined the results with those from all available similar studies. Those of our patients who fulfilled operational criteria for paranoid schizophrenia showed a nonsignificant increase in HLA A9 as compared with cont

[Treatment of therapeutically resistant paranoid schizophrenic patients with Moditen-depot combined with other psychotropic drugs and Leponex].

The results of treating 251 therapeutically resistant patients with paranoid schizophrenia by moditen-depo, its combination with other psychotic drugs or leponex was compared. The results obtained indicate that a comprehensive treatment (moditen-depo with other psychotropic drugs) and by leponex may be successfully applied to some of the paranoid schizophrenics resistant to moditen-depo and other neuroleptic drugs or their combinations. Comprehensive treatment appeared to be more effective in systematized delusions, the syndrome of psychic automatisms and in a crude emotional-volitional defect

[Crime as the 1st symptom of schizophrenia. Psychopathological analysis].

One case of paranoid schizophrenia which for a long period remained latent is reported. The first manifest symptoms of the disease resulted in crime. The importance of the observation of the premorbid personality and detailed psychopathological analysis of it especially in forensic psychiatry are stressed. The basic task of the expert is to disclose the diagnosis of the mental illness. To reveal the relationship between the disease and the crime committed-which is the bases of the experts conclusion- all the circumstances of the crime should be considered. In the case reported the crime itself

[Delusional psychopathology of the paranoid stage of paranoid schizophrenia].

The psychopathological traits of paranoial delusions were studied in 65 patients with paranoial schizophrenia. Of these patients, 16 had hypochondriacal delusions, 13 delusions of jealousy and 36 delusions of reference and persecution. All cases were studied from the standpoint of structural-dynamic aspects of the development of systematized interpretative delusions. Two variants of the development of such delusions were distinguished: paranoic and mixed. The paranoic variant by its nature appeared to be catathymic (4 cases), in a mixed variant separate elements of sensorial and imaginative de

[Dynamics of evoked potentials during the course of progressive schizophrenia].

A study of the functional activity of ascending projectional systems in 90 patients with medium progressive(paranoid) schizophrenia demonstrated a correlation between the parameters of evoked potentials and the clinical signs of the disease. During the early paranoidal stage of the pathological process, there was a certain activity of the nonspecific brain areas. As the disease progresses from paranoid states to signs of "secondary"catatonia, there ensues the blockade of the information conduction through nonspecific areas along with a progressive increase in the activity of specific afferent

[Constitutional-somatotopic factors in the clinical picture of alcoholic psychoses and obvious signs of paranoid schizophrenia].

The clinical and anthropometrical methods were used to study the constitutional traits of 240 patients with psychotic forms of alcoholism and 300 patients with paranoid schizophrenia. The somatotypes were assessed by the Rees-Eysenck method. The prepsychotic parameters of alcoholism and the structure of alcoholic delirium were studied in 78 patients, as well as the manifest symptoms of paranoid schizophrenia. The study demonstrated a syndromal modified expressivity in exo- and endogenous processes, depending upon the somatotypical constitution. Simple syndromes were more frequently found in pa

[Some data of quantitative E.E.G. in the study of brain mechanisms of human behavior (author's transl)].

Quantitative electroencephalographic methods (amplitude analysis) have been used to measure the relative degree of hemispheric activation (temporal lobes) in normal subjects, mild psychopathological states, and certain mental diseases. Special tasks were used to elicit either verbal (left hemispheric) or non-verbal (right hemispheric) processes. In normal subjects, the E.E.G. data obtained are in good agreement with mental performance. Under mild to severe psychopathology they reveal functional impairments of within- as well as of between-hemispheric activities. There are also indications of d

Circumcision and schizophrenia.

Two cases of paranoid schizophrenia associated with elective circumcision are presented. In both cases the request for circumcision came after a psychotic process was in progress. The author discusses these cases with regards to previous literature about the meaning of circumcision. Surgeons should be aware of the motivation in the request for these procedures.

Faking psychosis on the Rorschach: can expert judges detect malingering?

The susceptibility of the Rorschach to faking of psychological disturbance has been subject to few studies, all of which have significant methodological deficiences. Faking of psychosis was examined in this study by using Rorschach experts as judges to blindly evaluate both faked and actual psychotic protocols. The faked protocols came from role-informed and role-uninformed individuals who were asked to specifically malinger paranoid schizophrenia on the Rorschach. The results indicated that the expert judges were unable to detect the faked psychotic protocols regardless of whether the faker w

Delusional misidentification and platelet monoamine oxidase.

Recently, reduced platelet monoamine oxidase (MAO) activity was demonstrated in two cases of paranoid schizophrenia exhibiting Capgras' syndrome. The present authors report platelet MAO studies in an additional patient with the Delusion of Doubles. In this case, one of delusional hyperidentification (false recognition), decreased platelet MAO activity compared with normal controls was detected. The patient, while not schizophrenic, showed platelet MAO activity comparable with that of a chronic schizophrenic control group. The possible significance of the findings are discussed.

Effect of some beta-carbolines on phenylethylamine and apomorphine stereotypies in rats.

Tetrahydro-beta-carbolines (THBC:s) have recently been shown to occur in the human body and their role in mental diseases has been discussed. The effect of THBC:s and some other beta-carbolines (25 mg/kg IP) were studied on the stereotypies caused by apomorphine (APO, 2 mg/kg IP) and phenylethylamine (PEA, 50 mg/kg IP) in rats. These effects of dopaminergic drugs like apomorphine as well as of phenylethylamine have sometimes been used as animal models of paranoid schizophrenia. Dose effect relationships were studied from ther most potent substances. All beta-carbolines studied significantly in

ECT for schizophrenia.

Electroconvulsive therapy (ECT) in the treatment of schizophrenia was evaluated in a double-blind trial; the clinical change after ECT was compared with that after a treatment procedure identical to it but for two exceptions--no electricity was used and no convulsion was induced. All patients had paranoid schizophrenia according to Present State Examination criteria and all received standard doses of neuroleptics for at least 2 weeks before random assignment to the two groups. 20 patients completed the trial: 10 had ECT and 10 were in the control group. Treatment was given three times a week,

Factors related to increased risk of assaultive behavior in suicidal patients.

This study compared suicidal patients with assaultive behavior to suicidal patients without assaultive behavior. Of 667 suicidal male patients admitted to psychiatric hospitals during the study period, 90 (14%) were assaultive at the time of or just prior to admission, while of the 893 female suicidal patients admitted, 60 (7%) were assaultive. Male assaultive suicidal patients were more likely than non-assaultive males to be referred to hospitals by the law and mental health services, while female assaultive patients were more likely to be 34 years of age or younger and to have primary psychi

Relative potency of amphetamine isomers in causing the serotonin behavioral syndrome in rats.

High doses of amphetamine evoke a behavioral syndrome in man which is similar to paranoid schizophrenia. The similarities between the behavioral effects of amphetamine and the symptoms of an endogenous behavior disorder suggest a mechanistic relationship between the two states. This study deals with a specific set of behavioral signs in the rat which are caused by relatively high doses of amphetamine and are mediated by serotonin receptor activation. Experiments on the mechanism by which amphetamine evokes this behavioral syndrome reveal that the l-isomer, like the d-isomer, acts indirectly, i
