Morbidity risks of schizophrenia and affective disorders among first degree relatives of patients with schizophrenia, mania, depression and surgical conditions.

One thousand five hundred and seventy eight first degree relatives of schizophrenics, manics, depressives and controls were personally interviewed using the Iowa Structured Psychiatric Interview Form without knowledge of the probands' diagnoses. Our data, based on blind diagnostic assessment of the relatives, support the distinction between schizophrenia and affective disorders, although the distinction between schizophrenia and mania was not clear-cut. Our data could not support familial subtyping of paranoid and non-paranoid schizophrenia, and unipolar and bipolar affective disorders. Future

An objective study of relationships and discontinuities between paranoid schizophrenia and Kretschmer's syndrome of sensitive delusions of reference.

The nosological status of Kretschmer's syndrome of sensitive delusions of reference (Sensitiver Beziehungswahn) has been a matter of discussion over many years. In an attempt to resolve the matter, a series of 147 consecutive patients who had been diagnosed as schizophrenic were reviewed. A group of 13 patients met specified criteria for Kretschmer's syndrome of sensitive delusions of reference, and these patients were compared with the remainder in a variety of ways. The patients with the syndrome of sensitive delusions of reference differed from the remainder in their antecedents (family his

Alcoholic hallucinosis and paranoid schizophrenia-a comparative (clinical and follow up) study.

In a Study Of 90 patients of Alcoholic Hallucinosis and 30 patients of Paranoid Schizophrenia, it was found that delusions, delusions of infidelity, third person and running commentary auditory hallucinations and insight were not different in the two groups.Delusions of grandeur, passivity, thought echo and thought broadcast were significantly more frequent in paranoid schizophrenic patients. Anxiety, visual iiafracinatians and hallucinations in more than one modality at the same time were commoner in alcoholic hallucionsis. Recovery from acute symptoms was much earlier in alcoholic hallucinos

[Influence of diagnosis, hospital and sex on the effects of bromperidol].

Based on the data analysis of the AMP-findings of 78 schizophrenic patients from 4 clinical trials, the following effects of Bromperidol could be demonstrated: a strong antipsychotic effect with an early onset (after 3 days), little sedation, good improvement of accompanying depressive symptoms, little autonomic but rather strong extrapyramidal side effects. No differences between the effects in men and women were noticed. Differences between the patient populations of two hospitals were small in comparison to the differences between the patients with catatonic and paranoid schizophrenia. In c

[Prevalence and chief variants of the course of schizophrenia with a predominance of delusional and hallucinatory-delusional disorders throughout the course of the disease (clinico-epidemiologic study)].

A nonselective group of 971 schizophrenic patients was studied. The development of the disease in these patients was characterized by the prevalence of hallucinative disorders during the whole duration of the disease. This group totalled 19.1% of the whole population of patients. The cases with continuous paranoid schizophrenia totalled 42.4% of the studied group (8.1% of the population). The rest observations formed the continual range between paranoid and attack-like schizophrenia.

Hypochondriasis: difficulties in diagnosis and management.

The hypochondriacal patient presents numerous frustrations for the physician. The typical hypochondriac is a middle-aged person dominated by a preoccupation with bodily symptoms. Bereavement, serious life failures, and severe blows to self-esteem put patients at high risk for somatization. Hypochondrisis has been cited as a symptom of masked depression, early paranoid schizophrenia, and dementia and/or delirium. A careful diagnostic work-up with attention to any correctable psychiatric illness sets the stage for regularly scheduled, long-term supportive therapy.

Hypochondriasis and paranoia: similar delusional systems in an institutionalized geriatric population.

Hypochondriasis and paranoia are common psychopathologies of aging. The former may be secondary to depression or organic brain syndrome, whereas the latter may be secondary to sensory impairment or organic brain syndrome. Paranoid schizophrenia, with delusions of grandeur, is rare in later life. The authors classified 273 institutionalized geriatric patients by means of staff ratings into categories such as lucid and alert, confused, hypochondriacal, paranoid, etc. Among confused patients there was a correlation of .45 (p less than .001) between hypochondriasis and paranoia. Even among nonconf

Pituitary and peripheral resistance to thyroid hormone.

A 32-year-old Caucasian male, clinically euthyroid, with paranoid schizophrenia and granulocytopenia, had elevated total and free serum T4 and T3; serum TSH was normal (2.7 +/- 0.7 micronU/ml). There was no goitre present, no evidence of Graves' disease, and no evidence of pituitary tumour. He had a normal response to methyl-TRH, with a TSH increment of 14.6 micronU/ml, T3 increment of 212 ng/dl, and T4 increment of 4.7 microgram/dl; baseline value and decreased the TSH increment in response to methyl-TRH. T3 therapy (100 microgram/day) decreased the thyroidal radioactive iodine uptake to less

Indications for electric convulsion therapy and its use by senior psychiatrists.

A survey by questionnaire of all senior psychiatrists in the Wessex Region showed that they considered depressive psychosis to be the major indication for electric convulsion therapy (ECT). A good clinical response was thought to be predicted by the presence of psychomotor retardation, depressive delusions, depressed mood, early morning wakening, diurnal variation, loss of appetite, and agitation. ECT was judged to be extremely useful for treating mania and acute undifferentiated, catatonic, and paranoid schizophrenia; of some use in hypochondriasis; but of little value or contraindicated when

Taijin Kyofusho: diagnostic and cultural issues in Japanese psychiatry.

Taijin Kyofusho or 'anthrophobia' is a Japanese culture-specific disgnostic label for the presenting problems of various fear reactions in interpersonal situations. The starting point of the present study was accumulating studies on Japanese cases of Taijin Kyofusho and the assertion in Japanese psychiatry that the symptom complex called Taijin kyofusho is Japanese culture-bound. In light of previous works on culture-bound disorders, the present study examines whether or not American mental health professionals could diagnose Japanese case descriptions of Taijin Kyofusho and what kinds of labe

Gentian violet keratoconjunctivitis.

A case of bilateral keratoconjunctivitis caused by self-instillation of a 1% aqueous solution of gentian violet was complicated by a secondary uveitis and gram-negative conjunctivitis. The patient was treated with cycloplegics and specific antibiotics and after resolution of the bacterial infection, a mild topical corticosteroid was used. After six months the patient had completely recovered from the keratitis, but bilateral corneal vascularization, had unilateral posterior synechiae, corneal scarring, and cataract formation were present. Cationic dyes such as gentian violet appear to be toxic

An amphetamine model of manic depressive illness.

Many features of manic-depressive illness can be mimicked in man by the use and withdrawal of amphetamines. In higher doses these drugs induce a syndrome virtually indistinguishable from paranoid schizophrenia. In this paper, some of the biochemical and physiological effects of the amphetamines are examined with the hope of clarifying the nature of the biological changes in these two major functional psychoses. The actions of the amphetamines are shown to reveal the operation of specific homeostatic or adaptive nervous mechanisms. These appear likely also to operate in adaptation to psychologi

[Clinical picture of continuous paranoid schizophrenia in the light of long-term catamnesis in old age].

A total of 100 patients with continuous paranoid schizophrenia who had lived to an old age were studied. It is revealed that a significant part of these patients (about 80%) display good social adaptation, do not require hospitalization and preserve this state in the long-term absence psychopharmacological treatment as well. It relates to the fact that in paranoid schizophrenia in old age signs of a decrease of progression of a morbid process and a tendency towards stabilization of clinical manifestations prevail. In other cases progression of a morbid process retains.
