Course of illness in a sample of patients diagnosed with a schizotypal disorder and treated in a specialized early intervention setting. Findings from the 3.5year follow-up of the OPUS II study.

BACKGROUND: Previous studies report that 20% to 30% of those initially diagnosed with schizotypal disorder go on to develop a psychotic disorder (predominantly schizophrenia). Schizotypal disorder share some traits of those used to identify patients at ultra-high risk for psychosis. METHOD: As part of a randomized clinical trial testing the effect of prolonged specialized early intervention, we recruited 83 participants diagnosed with a schizotypal disorder. Participants were recruited 18 months into their two-year treatment program, and follow-up interviews were conducted three and a half ye

The incidence of schizophrenia and schizophrenia spectrum disorders in Denmark in the period 2000-2012. A register-based study.

INTRODUCTION: We aimed to examine changes over time in the incidence of broad and narrow schizophrenia spectrum disorders in Denmark from 2000 to 2012. METHODS: Patients were classified as incident schizophrenia if registered with a first time in- or outpatient contact with relevant diagnostic codes in the Danish Psychiatric Central Register between 2000 and 2012. Their history of contacts was traced back to 1969. Broad schizophrenia included schizophrenia, schizotypal disorder, persistent delusional disorder, acute and transient psychotic disorders, schizoaffective disorders, and other nonor

Orbitofrontal sulcogyral pattern and olfactory sulcus depth in the schizophrenia spectrum.

Morphological changes in the orbitofrontal cortex (OFC), such as an altered sulcogyral pattern of the 'H-shaped' orbital sulcus and a shallow olfactory sulcus, have been demonstrated in schizophrenia, possibly reflecting deviations in early neurodevelopment. However, it remains unclear whether patients with schizotypal features exhibit similar OFC changes. This magnetic resonance imaging study examined the OFC sulcogyral pattern (Types I, II, III, and IV) and olfactory sulcus morphology in 102 patients with schizophrenia, 47 patients with schizotypal disorder, and 84 healthy controls. The OFC

[Characteristics of neurological status in children with schizophrenia and schizotypal disorder].

AIM: To study characteristics of neurological status in children with schizophrenia and schizotypal disorder. MATERIAL AND METHODS: A study included 80 children, aged from 5 to 10 years: 38 children were diagnosed with schizophrenia (ICD-10 F20.8хх3) and 42 with schizotypal disorder (F21). RESULTS AND CONCLUSION: Children of both groups had a wide spectrum of neurological symptoms, diffuseness of which indicated the disintegration of the multilevel system of motor analyzer. No topical between-group differences were found whereas their intensity and frequency was not correlated to either the

[Schizophrenia or a group of endogenous diseases? The past and the present].

A critical analysis of the main concepts of schizophrenia (from the Kraepelin paradigm to the current studies attempted to «destruct» schizophrenia as nosologically independent disease) is presented. In author's opinion, the elimination of schizophrenia may lead to negative consequences as in aspect of clinical practice (the diagnosis of schizophrenia provides information about disease course, clinical and social outcomes) as well as in aspect of denial of autochthonous (endogenous) origin of mental disorders that fall under this definition. Based on the current level of knowledge, manifestati

A disembodied man: A case of somatopsychic depersonalization in schizotypal disorder.

In the general concept of self-disturbances in schizophrenia and schizophrenia spectrum disorders, somatopsychic depersonalization (SPD) occupies a special place as it constitutes a syndrome that comprises feelings of detachment from one's own body and mental processes. However, apart from clinical descriptions, to date the pathophysiology of SPD is not fully understood due to the rareness of the syndrome and a lack of experimental studies. In a case study of one patient with schizotypal disorder, we applied a multimodal approach to understanding the SPD phenomena. The patient's clinical profi

[Very low magnesium levels in red blood cells as a significant factor in the etiopathogenesis of borderline disorders].

AIM: Basic studies indicate magnesium deficiency as one of the important, but often neglected, risk factors aggravating the course of borderline disorders (BD). A clinical verification of this notion has been conducted. MATERIAL AND METHODS: Authors studied 62 patients with BD, aged 25-65 years, of inpatient and outpatient settings. Contents of magnesium and other blood electrolytes were determined. RESULTS: Authors found an extremely high prevalence of very low levels of magnesium (Mg) in erythrocytes (<0.3 mmol/l) in patients with BD compared to controls (patients without BD, Mg (er.) 1.62

Sequencing of five left-right cerebral asymmetry genes in a cohort of schizophrenia and schizotypal disorder patients from Russia.

OBJECTIVE: Schizophrenia is a severe psychiatric disorder, affecting ∼1% of the human population. The genetic contribution to schizophrenia is significant, but the genetics are complex and many aspects of brain functioning, from neural development to synapse structure, seem to be involved in the pathogenesis. A novel way to study the molecular causes of schizophrenia is to study the genetics of left-right (LR) brain asymmetry, the disease feature often observed in schizophrenic patients. METHODS: In this study, we analyzed by sequencing five candidate LR cerebral asymmetry genes in a cohort o

[The relation to the humor and laugh in patients with schizophrenia].

This study provides the data on gelotophobia (the fear of being laughed at), gelotophilia (the joy of being laughed at) and katagelasticism (the joy of laughing at others) in patients with schizophrenia in comparison with controls. Gelotophobia was significantly higher and katagelasticism was significantly lower in psychiatric patients than in controls. The negative correlation was found between gelotophobia and the duration of illness among patients with paranoid schizophrenia. Gelotophilia and katagelasticism were correlated positively both in patients and controls. In patients with schizoty

[Risk of manifestation of endogenous psychosis in patients with nonpsychotic mental disorders of juvenile age].

OBJECTIVE: To identify people at high risk of endogenous psychosis in patients of juvenile age with nonpsychotic mental disorders. MATERIAL AND METHODS: Authors examine 49 patients of juvenile age with symptoms of nonpsychotic mental disorders (ICD-10 diagnoses of mood disorder, personality disorder or schizotypal disorder). RESULTS: In all cases, symptoms related to signs of the premorbid stage of endogenous psychosis (schizophrenia and schizoaffective psychosis) were identified. After psychopharmacological treatment, an incomplete (<30%) reduction of psychopathological disorders recorded a

Body dysmorphic disorder and olfactory reference disorder: proposals for ICD-11.

The article reviews the historical background and symptoms of body dysmorphic disorder (BDD) and olfactory reference disorder, and describes the proposals of the WHO ICD-11 Working Group on the Classification of Obsessive-Compulsive and Related Disorders related to these categories. This paper examines the possible classification of BDD symptoms in ICD-10. Four different possible diagnoses are found (hypochondriacal disorder, schizotypal disorder, delusional disorder, or other persistent delusional disorder). This has led to significant confusion and lack of clear identification in ICD-10. Olf

Pituitary gland volume in patients with schizophrenia, subjects at ultra high-risk of developing psychosis and healthy controls: a systematic review and meta-analysis.

BACKGROUND: A larger pituitary size is thought to reflect a greater activation of the hypothalamic-pituitary-adrenal (HPA) axis, which may be related to an increase in the number and size of corticotroph cells. Some studies, but not all, indicate that pituitary volume increases before or at the onset of psychosis. There is a need for at critical appraisal of the literature on this topic accompanied by a meta-analytical evaluation of the data. METHODS: We included studies comparing the volume of the pituitary gland in healthy controls and patients with schizophrenia, first episode of psychosis

[Cognitive functions, social functioning and self-esteem in young male patients with adolescence onset schizotypal disorder].

The results of psychological examination of 33 male patients with a diagnosis of schizotypal disorder (SD), mean age 22 years, with age at onset in adolescence and 20 healthy young men are presented. Cognitive performance in patients with SD did not differ from healthy people on all measures, with the exception of social adaptation. There was a trend towards the change of normative responses and, respectively, estimation of the results of psychological techniques. The data suggest that personality deficits play a more significant role in the social maladaptation of patients with SD than cognit

[Neuropsychological syndromes of non-psychotic mental disorders of youthful age].

Seventy male patients with non-psychotic mental disorders of youthful age (mean age 19.2±3.7), were studied using A.R. Luria neuropsychological syndrome analysis. Patients were stratified into 3 groups by diagnosis: cyclothymia (20 patients), pubertal decompensation of schizoid personality disorder (30 patients) and schizotypal personality disorder (20 patients). It has been shown that the neuropsychological changes indicate the dysfunction of the amygdale/temporal region in patients of the first group and frontal/thalamic/parietal connections in the patients of two other groups. There were in

[Clinical characteristics of initial stages of schizotypal disorder].

The study of clinical-psychopathological characteristics of initial stages of schizotypal disorder was carried out on a sample of 54 male inpatients. Based on the results of clinical-psychopathological and follow-up studies, authors found that symptoms of initial stages of schizotypal disorder presented by non-psychotic depressive disorders. Dissociation of depressive symptoms, heterogeneity and unsteadiness of leading affect, marked polymorphism of depressive presentations, presence of concomitant neurotic-like and psychopathic-like disorders are characteristic of these depressive disorders.
