Reasoning in deluded schizophrenic and paranoid patients. Biases in performance on a probabilistic inference task.

An experiment is described in which deluded subjects with a diagnosis of schizophrenia or of delusional disorder (paranoia) were compared with a nondeluded psychiatric control group and a normal control group on a probabilistic inference task. Factors relevant to belief formation and maintenance were investigated. Deluded subjects requested less information before reaching a decision and were more ready to change their estimates of the likelihood of an event when confronted with potentially disconfirmatory information. No differences were found between the two diagnostic groups of deluded subj

[Paranoia disorder as the first manifestation of AIDS].

Psychotic features resembling those seen in a functional psychosis--Delusional Disorder--were found in a forty two-year-old male eighteen months before a diagnosis of A.I.D.S. was made. The causal relation between the two entities is discussed making reference to five other similar cases described in the literature.

Subtle signs of prenatal maldevelopment of the hand ectoderm in schizophrenia: a preliminary monozygotic twin study.

Genes that predispose to psychosis may act by making individuals more vulnerable to the disruptive effects of various prenatal insults. Fetal organogenesis is mostly completed in the first prenatal trimester. The second trimester is a critical period of massive neuronal migration from the periventricular germinal matrix to the cortex. A peripheral appendage developing simultaneously with this neural migration to the cortex is the distal upper limb. The ectodermal cells of the fetal upper limb migrate to form the hand skin during the fourth and fifth months of gestation (first two-thirds of the

Delusional disorder: the predictive validity of the concept.

A sample of 72 first-admitted patients with delusional disorder was personally re-examined after a mean of 10 years, and 42 of them after an average of 27 years. The mean observation period for all patients from admission to last follow-up was 20 years (3-39). There was a trend that patients with delusions of jealousy did better on course variables, but similar outcomes were revealed in groups with delusions of jealousy, persecution and a mixed group with different main delusions. Duration of symptoms prior to admission was a significant predictor of outcome, indicating dividing lines of 1 and

Progressive systemic sclerosis with mental disorder.

We present a clinical case of a 43-year-old female with progressive systemic sclerosis, Basedow's disease, atrial fibrillation and diabetes mellitus. She twice developed exacerbations of a psychotic state. She showed a depressive state followed by a paranoic hallucinatory state and stupor. Electroencephalograms revealed slow abnormal records both in psychotic and remitting periods and 123I IMP SPECT of the brain showed a low blood flow in the psychotic stage. A case of progressive diffuse sclerosis with a paranoic hallucinatory state is rarely reported. As her Basedow's disease had been well c

A multitrait-multimethod analysis of the MCMI-II clinical syndrome scales.

Multitrait-multimethod procedures and factor analyses were utilized to assess convergent/discriminant validity of the revised version of the Millon Clinical Multiaxial Inventory (MCMI-II) clinical syndrome scales and effects of item overlap on scale performance. Except for the Delusional Disorder Scale (PP), convergent validity was supported, but poor discriminant validity was found for the Alcohol Dependence (B) scale, Drug Dependence (T) scale, and Scale PP. Item overlap did not influence performance for many scales. In fact, results indicate that some scales, such as Bipolar:Manic (N), may

Diagnosis and clinical course of erotomanic and other delusional patients.

Twenty-eight patients with erotomanic delusions were compared with 80 patients with other delusions to clarify questions about diagnosis and course of illness in erotomania. The erotomanic patients were a heterogeneous group with respect to both diagnosis and course. They had significantly more manic symptoms than the comparison group and more affective diagnoses than would be expected from the literature; 25% (N = 7) had schizoaffective disorder and 7% (N = 2) had bipolar disorder. A subgroup of monodelusional erotomanic patients was identified who met the DSM-III-R criteria for delusional di

Primary and secondary erotomania: clinical characteristics and follow-up.

A survey within a defined area of the north-east of England (population of approximately 1.25 million) generated a number of patients with current or past erotomanic phenomenology. A casenote study was carried out on the 11 patients identified. Cases were classified in accordance with DSM-111-R criteria and an assessment was made of the extent to which each case fulfilled specific criteria for primary and secondary erotomania, as defined by previous authors. The response to treatment of both psychiatric syndrome and erotomanic phenomenology is described for each case. The findings are discusse

[Capgras syndrome or delirium of doubles. Presentation of a new case].

Capgras syndrome is an unfrequent delusional disorder based on the patient's belief that a significant person for him or her, mainly the spouse, becomes a "double", undistinguishable from the real one, but threatening in some way. There are about two hundred reports of the syndrome in international psychiatric literature. This paper describes a new case in a woman aged 68, diagnosed of Major Affective Disorder, without signs of any other organic mental disease. The authors review some ethiological theories in relation with the disorder, which are contrasted with the characteristics of the new

A delusional disorder that occurred in one of a pair of monozygotic twins.

The authors reported a pair of male monozygotic twins in one of whom a delusional disorder occurred, and discussed the etiology of that disorder. The analysis of the twins indicates the following: 1. The disposition for the delusional disorder might be related to a psychological tendency to experience a delusion that might be based on a genetic factor. This might be strengthened by the development of a hypersensitive personality (Kretschmer, E.) promoted by environmental factors. 2. A hypersensitive personality might be a significant risk factor for the delusional disorder.

Clinical diagnostic considerations on cocaine abuse.

Following a review of the research literature on the psychophysiological effects of cocaine, a study is described of a group of 120 cocaine addicts. Of the 120 patients, 10 (8.33%) exhibited fleeting, unformed, organic delusions and hallucinations. Case reports of the 10 cases are presented. The quality of the adverse subjective effects of cocaine is emphasized, and the differential diagnosis between Cocaine Delusional Disorder and Paranoid Schizophrenia is discussed. Guidelines for a more accurate differential diagnosis are provided.

Female genital self-mutilation: case reports and literature review.

Three cases are reported that demonstrate the variety of clinical conditions in which female genital self-mutilation may be found. The most common variety appears to be the habitually self-harming individual with a severe personality disorder. One of the cases reported, however, is a woman with a delusional disorder whose body image distortions led her to ablate one of her labia majora. The three cases presented are discussed in the context of the eight cases previously reported of genital self-mutilation found in women.

Classification and outcome in nonschizophrenic, nonaffective delusional psychoses.

In a prospective study of nonschizophrenic, nonaffective delusional psychoses, 46 patients are classified according to ICD-8, DMS-III, Kendler's criteria for delusional disorder, Opjordsmoen's criteria for reactive delusional disorder, and Jørgensen and Jensen's criteria for reactive delusional psychosis. No single diagnostic group includes all patients. About one third of the patients are employed continuously, and less than half the patients have weekly social contacts. At a 2-year follow-up, big differences are revealed between the diagnostic groups as regards (1) signs and symptoms (no sig

Delusions of fatal contagion among refugee patients.

Delusions of fatal contagion were encountered in about 10% of refugee psychiatric patients from Southeast Asia belonging to one ethnic group, the Hmong. Psychotic depression was the most common diagnosis among the Hmong patients with delusions of contagion, whereas paranoid and schizophrenic diagnoses predominate in patients with similar delusions from other refugee groups. Some cases also occurred as a shared delusional disorder. Associated findings included isolation from the community, intrafamilial conflict, failure to acculturate, and sexual frustration or conflict. Most patients responde

Psychotic disorders in DSM-III-R.

The authors review the important changes in the three sections of DSM-III-R that include only psychotic disorders (schizophrenia, delusional disorder, and psychotic disorders not elsewhere classified), outline the rationale for these changes, and, where available, review their empirical basis. In addition, they review two proposed changes that were not incorporated into DSM-III-R. They conclude by calling for an increasingly rigorous and data-based process of nosologic revision.

Dysmorphophobia: body dysmorphic disorder or delusional disorder, somatic subtype?

The DSM-III-R proposes to divide dysmorphophobia into two separate disorders: delusional disorder, somatic subtype and dysmorphic disorder. We believe that this distinction of delusional and nondelusional is impossible from a phenomenological point of view, and that dysmorphophobic beliefs are better classified as overvalued ideas. The concept of dysmorphophobia and its relationship to monosymptomatic hypochondriacal psychoses and DSM-III-R categories is reviewed. A case report is described to support these arguments.

Delusional disorder: jealous and nonjealous types.

We studied the records of 101 patients with delusional disorder admitted to the Psychiatric Hospital between 1920 and 1980 and divided the sample into those with jealous (43) and those with nonjealous (58) delusions. Patients and relatives were located by telephone and follow-up information was collected. Compared with the nonjealous patients, the jealous ones were more likely to have had a single delusion, and to have experienced a more benign course as indicated by a lower rate of hospitalization and outpatient treatment. The original delusion tended to remain and the illness did not develop

Folie à deux.

A review of folie à deux or induced psychotic disorder (DSM-III-R) is provided. The author believes it to be a more frequent phenomenon than usually thought, especially when hospitalized patients are evaluated with their families. An argument is made for viewing it as a description of a relationship and possible influence between individuals who may have very different disease processes. This includes, in the secondary partner, a continuum from being very "impressionable" to having an autonomous and independent delusional disorder. A case report and suggestions for treatment are given.
