Long-term course and outcome in delusional disorder.

Out of 301 first-time admitted patients with delusional psychoses, 71 met Kendler's criteria of delusional disorder (DD) and 137 met the ICD-9 criteria of schizophrenia (S). DD was subdivided according to operational criteria into reactive delusional disorder (RDD) with precipitating factors (n = 31) and non-RDD (n = 40). At follow-up after 22-39 years, 20 RDD patients, 21 non-RDD patients and 85 S patients were personally interviewed. The delusions had faded in 61% of DD cases, were unchanged in 17%, and more prominent in another 17%. Recovery was recorded in 37%, mild defect in 32%, moderate

Clinical genetics as clues to the "real" genetics of schizophrenia (a decade of modest gains while playing for time).

Although a decade has passed since the genetics of schizophrenia was examined for the Schizophrenia Bulletin, the epigenetic puzzle of schizophrenia has not yielded its secrets to any scientific break-through. In this article we review a sample of the highlights relevant to enlightened genetic thinking, i.e., a broad diathesis-stressor framework with multifactorial causation assumed and with provision for the epigenetic interaction of psychosocial as well as neurobiological factors. The clinical genetic epidemiologist needs to know the lifetime morbid risks generated by different definitions o

Frégoli syndrome after cerebral infarction.

A case of a rare form of delusional misidentification, the Frégoli syndrome, is described. Although usually occurring in the setting of primary or secondary schizophrenic psychoses, delusional misidentification has been reported in affective, neurological, and toxic-metabolic disorders. In this instance a diagnosis of paranoia (delusional disorder) secondary to predominantly right hemisphere pathology, rather than schizophrenia, seemed more appropriate.

Toward an operationalization of reactive paranoid psychoses (reactive delusional disorder).

Reactive psychoses are relatively often diagnosed in Norway, although they are not operationalized in any classificatory system and the reliability may be questioned. The aim of this study was to define a group of reactive paranoid psychoses and compare its long-term outcome to the group of nonreactive paranoid psychoses and to paranoid schizophrenia. Approximately 200 out of 301 delusional subjects earlier studied by Retterstöl have recently been interviewed semistructurally, making a total follow-up period of 22-38 years. Diagnoses at discharge have been assessed retrospectively before the p

Classification of chronic psychoses including delusional disorders and schizophrenias.

A classification of chronic psychoses including nonparanoid schizophrenia, paranoid schizophrenia, paranoid state and paranoia (delusional disorder) is presented. This classification is dependent on a systematic increase in number of symptoms with each group. In particular, delusional disorder is examined with regard to family history. It is clear from the data which are presented that delusional disorder is more likely to be associated with a family history of such traits as suspiciousness, jealousy, secretiveness, and the presence of paranoid behavior or delusions. There is evidence that suc

Pemoline abuse.

Addictive use of the stimulant drug pemoline caused a delusional disorder in a middle-aged man. Discontinuance of use of the drug promptly reversed the psychosis, which subsequently recurred when pemoline was again used. Pemoline seems to be an addictive drug and has an additional property that heretofore has been attributed mainly to amphetamines--precipitation of a paranoid psychosis.

Two cases of "pure" or "primary" erotomania successfully treated with pimozide.

Erotomania is a mono-delusional disorder which, in its "pure" or "primary" form, should probably belong to the diagnostic category of Paranoia. Traditionally, the prognosis and response to treatment are very poor, but one of the present authors has previously proposed that the condition should respond to pimozide. Case histories of two patients successfully treated with this drug are presented.

Psychiatric illness in first-degree relatives of patients with paranoid psychosis, schizophrenia and medical illness.

This study examines the respective morbid risk for psychiatric illness determined by the family history method in the first-degree relatives of medical controls and patients with delusional disorder (paranoid psychosis) and schizophrenia. The morbid risk for schizophrenia and schizoid-schizotypal personality disorder was significantly greater in the relatives of the schizophrenic patients than in those of the delusional disorder or medical control patients, but no difference in the risk for affective illness or alcoholism was found in the three groups of relatives. Paranoid personality disorde

[Family picture studies in patients with paranoid psychoses].

Among the first-degree relatives of 77 patients with delusional functional psychoses the authors found 13 schizophrenics (3.10%), 8 manic-depressives (1.91%) and 14 cases with atypical psychoses (3.34%), following ICD-9 criteria. These figures were compared with the figures in the literature on the genetics of paranoid schizophrenics and nonschizophrenic paranoids, regarding the different composition of the samples. Subdivision of the original 77 patients, using the concept of the axial syndromes, was able to form two homogeneous subgroups of first-degree relatives: one with a schizophrenia pr

Demography of paranoid psychosis (delusional disorder): a review and comparison with schizophrenia and affective illness.

This article reviews the demographic characteristics of paranoid psychosis or delusional disorder (DD) and compares them with those found for schizophrenia and affective illness. Delusional disorder constitutes between 1% and 4% of all psychiatric admissions, with an incidence of first admissions between 1 and 3/100,000 population per year. Like affective illness, but unlike schizophrenia, DD is predominantly an illness of middle to late adult life, usually occurring in persons who have been married. Like schizophrenia, but unlike affective illness, DD occurs more frequently in low socioeconom

Paranoid psychosis (delusional disorder) and schizophrenia. A family history study.

Family studies have played a central role in the controversy over the nosologic status of paranoid psychosis or delusional disorder (DD). In this study, 12 cases of DD were blindly rediagnosed from a cohort of 146 schizophrenics. The prevalence of schizophrenia in the relatives of the DD probands was significantly less than that found in the relatives of the schizophrenics. Affective illness was equally uncommon in both groups of relatives. Inferiority feelings clustered significantly in the relatives of the DD probands. From a familial perspective, DD is not closely related to schizophrenia o

An independent analysis of the Copenhagen sample of the Danish adoption study of schizophrenia. III. The relationship between paranoid psychosis (delusional disorder) and the schizophrenia spectrum disorders.

To genetic relationship between paranoid psychosis (delusional disorder) and schizophrenia spectrum disorders (schizophrenia and schizotypal personality disorder) is examined by a blind independent diagnostic evaluation of the interviews of relatives from the greater Copenhagen sample of the Danish Adoption Study of Schizophrenia. While cases of schizophrenia spectrum disorders are strongly concentrated in the biologic relatives of the schizophrenic adoptees, this pattern is not found for delusional disorder. These results suggest that from a genetic perspective, delusional disorder is not par

The nosologic validity of paranoia (simple delusional disorder). A review.

The diagnostic status of paranoia (renamed simple delusional disorder [SDD]) has been debated since the time of Kraepelin. Three main views have emerged that consider paranoia (1) a subtype of schizophrenia, (2) a subtype of affective illness, or (3) a distinct nosologic entity. The literature pertinent to the diagnostic validity of a SDD is examined. Although the methodology of a number of the studies reviewed would not be considered rigorous by today's standards, certain tentative conclusions can nonetheless be reached. The available data do not suggest that SDD is a subtype of affective ill
