Irritable versus euphoric mania in psychiatric inpatients: a retrospective study of 251 inpatient episodes.

BACKGROUND: Contemporary diagnostic classifications recognise euphoric and irritable mood as core presentations of mania. Whether these predominant affective states reflect distinct clinical profiles or dimensional variations remains unclear, particularly in naturalistic inpatient settings. METHODS: We conducted a retrospective observational study of admissions for manic episodes between 2017 and 2025 at a tertiary psychiatric unit in Porto, Portugal. Eligible admissions had an ICD-10 discharge diagnosis within F30 (manic episode) or F31 (bipolar affective disorder). Electronic records were r

Presenting a new scoring system based on clinical and inflammatory factors to classify the severity of bipolar disorder.

BACKGROUND: Various clinical and laboratory factors are related to the severity of bipolar disorder, but there is no global agreement for the role of these factors. METHODS: Forty patients with the definitive diagnosis of bipolar disorder were included in this prospective study. The severity of the disease was measured by the Young Mania Rating Scale (YMRS). using the SPSS software. RESULTS: Using multivariable logistic regression modelling, the main correlates for moderate to severe bipolar disease included baseline interleukin-6 level > 5.5 pg/ml (OR = 2.820, p = 0.001), disease duration >

Cannabis use and subsequent onset of manic symptoms and bipolar disorder: A systematic review and meta-analysis.

BACKGROUND: Bipolar disorder is a disabling psychiatric condition affecting approximately 2% of the population, for which identifying modifiable risk factors prior to illness onset is a research priority. Cannabis use is highly prevalent, yet its role in the development of bipolar-spectrum outcomes remains unclear. Previous reviews have included few studies or combined distinct bipolar-spectrum outcomes, limiting outcome-specific inference. AIMS: To evaluate the association between cannabis exposure and subsequent incident BD and manic/hypomanic symptoms separately and examine potential moder

Nutraceuticals and phytoceuticals in the treatment of bipolar disorder: A systematic review and network meta-analysis.

Nutraceuticals and phytoceuticals are increasingly investigated for bipolar disorder (BD). We conducted a systematic review and network meta-analysis of randomized controlled trials (RCTs). PubMed/MEDLINE, EMBASE, Scopus, PsycINFO, and ClinicalTrials.gov were searched from inception to July 1, 2025. Co-primary outcomes were changes in depressive or manic symptoms (standardized mean difference [SMD]), treatment response, and acceptability (risk ratio [RR]); tolerability and remission (RR) were secondary outcomes. Heterogeneity, transitivity, inconsistency, small-study effects, risk of bias, and

Intoxicated with Madness: Exploring the Psychology of Intoxication in Uncovering the Experimental Roots of the Kraepelinian Dichotomy.

Scholarship surrounding the Kraepelinian dichotomy of dementia praecox and manic-depressive insanity has overwhelmingly approached Kraepelin's nosology as the product of Kraepelin's clinical method, use of diagnostic cards, and pre-existing disease concepts, rather than his work in experimental psychology. Revisiting the significance of Kraepelin's 1881-1895 concept of intoxication as model psychosis, this article establishes that Kraepelin's earlier psychological experiments with intoxicants were a potential source of latent, synthesizing concepts, informing the organization of clinical data

Using Electronic Health Records to Identify Medical Presentation for Self-harm as a Predictor of Psychotic or Bipolar Disorders: A Systems-level Alternative to the Clinical High-risk Approach.

BACKGROUND AND HYPOTHESIS: We examined associations between medical presentation for self-harm and subsequent diagnosis of a psychotic or bipolar disorder in a sample from the United States. Nordic registry studies have identified hospital (inpatient/emergency department) presentation of self-harm as a risk factor for developing psychotic or bipolar disorder, but it is unknown if this relationship exists within the United States and whether this relationship extends to outpatient settings. STUDY DESIGN: We extracted electronic health records of individuals aged 8-29 years from Boston Children

Parsing the Functional Heterogeneity of the Psychosis Spectrum: A Clinically Informed Application of Genomic Structural Equation Modeling.

Schizophrenia, bipolar disorder, and major depression vary in their age of onset, cognitive impacts, degree of impairment, and clinical course. However, since psychosis is a transdiagnostic feature that occurs across all three of these disorders, they can be represented as a spectrum. We leveraged genomic data to explore different functionally distinct components of the psychosis spectrum. We used Genomic Structural Equation Modelling (Genomic SEM) to define four factors indexing specific aspects of psychosis-related symptoms shared across depression, schizophrenia, and bipolar disorder: a gen

New-Onset Manic-Spectrum Symptoms Temporally Associated With Co-amoxiclav Exposure: A Case Report.

Antibiotics have been associated with a range of neuropsychiatric adverse effects, including manic-spectrum symptoms. However, such side effects are uncommon, and the mechanisms underlying these presentations remain uncertain.  A 62-year-old female with no previous psychiatric history developed manic-spectrum symptoms shortly after receiving co-amoxiclav for a suspected dental infection. Approximately two days after initiation of treatment, she developed marked anxiety, reduced sleep, irritability, restlessness, racing thoughts, increased speech, an elevated mood, and paranoid ideas. Symptoms

Beyond symptom resolution: Clinical burden and immunometabolic correlates of functional remission after mania.

BACKGROUND: Functional outcomes after manic episodes in bipolar disorder (BD) are heterogeneous and likely reflect the interaction between illness burden and biological processes, including systemic inflammation. However, the contribution of immunometabolic factors to functional remission remains unclear. OBJECTIVE: To identify clinical and immunometabolic predictors of functional remission in patients hospitalized for manic episodes. METHODS: We conducted a retrospective cohort study of 680 manic episodes. Functional outcomes at discharge were assessed using the Global Assessment of Functio

Bipolar Disorder and Perimenopause: An Update.

OBJECTIVES: Perimenopause has been associated with mood worsening in some individuals, including increased depressive symptoms. Yet, the link between bipolar disorder (BD) and perimenopause remains understudied. This review aimed to provide an update on recent original studies and contextualize findings within the broader literature to guide future research and clinical practice. METHODS: A systematic search was conducted on Embase (January 1, 2018 to August 5, 2026) to identify studies on BD (I, II, or NOS/BD spectrum) that directly or indirectly examined perimenopause and BD. Pooled samples

'It's a Balancing Act': A Qualitative Analysis of the Experiences of Caffeine Use in Individuals With Bipolar Disorder.

INTRODUCTION: Bipolar Disorder (BD) is a complex and often chronic mood disorder that can cause significant distress and disruption to everyday life. The condition often requires lifelong treatment and self-management. Caffeine is a lifestyle behaviour that could be useful in self-management alongside traditional treatments, but further research is needed to explore the relationship between this lifestyle behaviour and its potential significance on affective state. METHODS: Individuals with BD (N = 20) took part in qualitative semi-structured interviews, engaging in discussion about their per

The distinctive psychopathology of NMDAR-antibody encephalitis compared with primary psychoses: an international, multicentre, retrospective phenotypic analysis.

BACKGROUND: N-methyl-D-aspartate receptor (NMDAR)-antibody encephalitis is a life-threatening neuropsychiatric disorder requiring prompt immunotherapy. The earliest features are mental-state changes, often mistaken for primary psychosis. Improved clinical differentiation could assist rational diagnostic investigation and expedite immunotherapy. Inspired by patients' and relatives' lived experience, we aimed to explore the psychiatric phenotype of NMDAR-antibody encephalitis and the features common to and distinct from real-world episodes of psychosis. METHODS: In this international, multicent

Drug-induced mania and parkinsonism in substance use disorder patient: A case report.

This case report investigates mania and parkinsonism induced by olanzapine and disulfiram in a patient with substance use disorder. DSM-5 criteria were used for the diagnosis of drug-induced parkinsonism, and movement disorder severity was assessed with the Simpson-Angus Scale. After initiating olanzapine and disulfiram therapy, the patient developed extrapyramidal symptoms and manic features. Naranjo scale indicated a probable drug reaction (score = 7), while Hartwig Severity Assessment (Level 4) suggested that hospitalization was required and these reactions were nonpreventable.
