Temporal dynamics of mixed symptom configurations in bipolar disorder: An intensive longitudinal study.

BACKGROUND: Mixed symptom presentations in bipolar disorder (BD) are clinically consequential, but their short-term temporal organization remains poorly understood. Using ecological momentary assessment (EMA), we mapped transitions across depressive, hypo/manic, co-symptomatic, and mixed symptom configurations and examined the symptom context surrounding mixedness. METHODS: Fifty adults with BD were recruited for the BADAS Study and completed the Bipolar Disorder Symptom Scale (BDSx) twice daily over 123 days on average (3 to 14 months). Symptom presentations were classified into 10 mutually

Cannabis use and subsequent onset of manic symptoms and bipolar disorder: A systematic review and meta-analysis.

BACKGROUND: Bipolar disorder is a disabling psychiatric condition affecting approximately 2% of the population, for which identifying modifiable risk factors prior to illness onset is a research priority. Cannabis use is highly prevalent, yet its role in the development of bipolar-spectrum outcomes remains unclear. Previous reviews have included few studies or combined distinct bipolar-spectrum outcomes, limiting outcome-specific inference. AIMS: To evaluate the association between cannabis exposure and subsequent incident BD and manic/hypomanic symptoms separately and examine potential moder

New-Onset Manic-Spectrum Symptoms Temporally Associated With Co-amoxiclav Exposure: A Case Report.

Antibiotics have been associated with a range of neuropsychiatric adverse effects, including manic-spectrum symptoms. However, such side effects are uncommon, and the mechanisms underlying these presentations remain uncertain.  A 62-year-old female with no previous psychiatric history developed manic-spectrum symptoms shortly after receiving co-amoxiclav for a suspected dental infection. Approximately two days after initiation of treatment, she developed marked anxiety, reduced sleep, irritability, restlessness, racing thoughts, increased speech, an elevated mood, and paranoid ideas. Symptoms

Bipolar Disorder and Perimenopause: An Update.

OBJECTIVES: Perimenopause has been associated with mood worsening in some individuals, including increased depressive symptoms. Yet, the link between bipolar disorder (BD) and perimenopause remains understudied. This review aimed to provide an update on recent original studies and contextualize findings within the broader literature to guide future research and clinical practice. METHODS: A systematic search was conducted on Embase (January 1, 2018 to August 5, 2026) to identify studies on BD (I, II, or NOS/BD spectrum) that directly or indirectly examined perimenopause and BD. Pooled samples

Addressing hypersensitivity in density functional theory for reliable machine learning.

Bipolar disorder is a chronic affective disorder that causes significant economic burden to patients, families and society. It has a lifetime prevalence of approximately 1.3%. Bipolar disorder is characterised by recurrent mania or hypomania and depressive episodes that cause impairments in functioning and health-related quality of life. Patients require acute and maintenance therapy delivered via inpatient and outpatient treatment. Patients with bipolar disorder often have contact with the social welfare and legal systems; bipolar disorder impairs occupational functioning and may lead to prem

The effect of early initiation of aripiprazole once-monthly 400 mg on health care resource utilization and health care costs in patients diagnosed with bipolar I disorder: Real-world evidence from US claims data.

BACKGROUND: Bipolar I disorder (BP-I) is a chronic and recurrent mental health disorder, broadly characterized by patients who alternate between the extremes of the mood spectrum: mania or hypomania; and depression. In 2015, the total estimated annual cost of BP-I in the United States reached more than $200 billion, approximately 2.5 times higher than the general population, largely driven by the increased use of acute health care services. Long-acting injectable antipsychotics such as aripiprazole were developed to significantly reduce patient burden for treatment adherence compared with oral

Mood Monitoring, Mood Tracking, and Ambulatory Assessment Interventions in Depression and Bipolar Disorder: Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Mood monitoring is widely used by people with depression and bipolar disorder (BD) to prevent relapse and improve insight into their condition, but it is unclear if these interventions have an impact on symptoms and for whom. As the capacity for passive mood monitoring increases, it is vital to improve our understanding of frequent mood assessment. OBJECTIVE: This systematic review and meta-analysis assessed the effect of mood monitoring interventions in people with depression and BD to decrease relapse risk and symptoms of depression and mania. METHODS: We conducted a systematic

Advancing Understanding of Mania/Hypomania Symptoms' Transdiagnostic, Multimethod Associations.

We assessed mania/hypomania and a range of other symptoms, substance use, and personality dimensions across interview and self-report methods toward advancing understanding of mania/hypomania's multimethod assessment. Our study, informed by transdiagnostic, dimensional models examined these issues in a clinically-oriented sample (N = 245; mean age = 32, SD =12; 59% currently accessing psychotherapy and/or medication), and as a secondary aim, we evaluated the structure of lifetime interview ratings of hypomania symptoms. We found evidence for a single-factor structure of interview-rated hypoman

Normative brain variability, clinical dimension, and molecular-transcriptomic correlates of bipolar risk in depressed patients.

AIM: Bipolar disorder (BD) is commonly misdiagnosed as major depressive disorder (MDD) due to depression often preceding manic symptoms. Yet, neurobiological mechanisms underlying emotional states transition in depressed patients remain largely unexplored. METHODS: Leveraging a lifespan normative model from a large healthy cohort (N = 1262), we quantified the structural or functional brain variability for 389 depressed patients (179 MDD, 138 BD, 72 transition to BD [tBD]). The MDD and tBD patients were followed up for 6~13 years. A dimensional approach was employed to dissect the neuroimaging

Effects of Bipolar Disorder on the Verbal Fluency Skills of Native Speakers.

BACKGROUND/OBJECTIVES: Bipolar disorder (BD) is a chronic psychiatric condition characterized by episodes of mania, hypomania, and depression. Due to the cognitive impairments associated with BD, patients frequently experience difficulties in attention, memory, and executive functions, which in turn adversely affect specific aspects of their language abilities, such as word retrieval, verbal fluency, and the organization of coherent speech. The present study aims to determine the extent to which the verbal fluency skills of native Turkish-speaking individuals with BD are impaired compared to h

Transient Mood Elevation Without a Manic Switch: Goldenseal-Augmented Oral Glutamatergic Regimen in Bipolar Depression With Comorbid Obsessive-Compulsive Disorder (OCD).

Treating bipolar depression that comes with obsessive, looping thoughts can be tricky because standard antidepressants often help only a little and can sometimes push the mood too high. In this report, we followed a 32-year-old man whose low mood and health-related ruminations lingered despite mood stabilisers (valproate, low-dose aripiprazole, pregabalin, and Deanxit). Hoping to copy some of ketamine's brain-circuit effects, he tried an over-the-counter mix of dextromethorphan, piracetam, and L-glutamine; the result was modest at best. Ten days after he added 600 mg of the herbal CYP2D6 inhib

5 Years of bipolar disorder conversations on Reddit: Methods, key topics and future directions.

Bipolar disorder (BD) is a prevalent mood disorder that can be associated with serious personal, societal, and economic costs. Growing attention is being paid to the importance of including the experiences of people living with BD in research and healthcare system advancement. However, there is still much to be learned about what people with BD are prioritizing, and where their needs are not yet being met in research and clinical settings. For the past five years, the Collaborative RESearch Team to study psychosocial issues in Bipolar Disorder (CREST.BD) has facilitated the world's largest onl

Is Longer Always Better? Clinical Validation of the Korean Altman Self-Rating Mania Scale With a Comparison of 5-Item Versus 11-Item Versions.

OBJECTIVE: The Altman Self-Rating Mania Scale (ASRM) has been recommended as a brief and psychometrically promising tool for the self-reporting of (hypo)manic symptoms. However, there is a shortage of validation in clinical samples, and none for the newly developed 11-item version. Thus, this study aimed to test the reliability and validity of the ASRM-11 in comparison with the ASRM-5. METHODS: Through a retrospective chart review, self-reported data on the Korean ASRM (K-ASRM) were collected from 122 patients diagnosed with either bipolar or depressive disorder via a (semi)structured diagnos

Elevated pre-supplementary motor area activity during reward expectancy: An impulsivity-related neural marker of vulnerability to bipolar and externalizing disorders.

Mania/hypomania is pathognomonic of bipolar disorder (BD), yet early identification remains challenging. Impulsivity is a key feature of mania/hypomania and of externalizing disorders that may predispose to BD, but neural markers of impulsivity-related risk remain unknown. This study aimed to identify reward expectancy (RE)-related neural correlates of impulsivity facets, test moderation by current affective/anxiety symptoms, and determine whether such markers differentiate BD and/or externalizing disorders from low impulsivity individuals. Two independent BD-risk samples aged 18-30 years, inc

Ketamine as an NMDA-modulating therapy in bipolar disorder: rationale and evidence.

BACKGROUND: Bipolar depression remains a leading cause of morbidity, functional impairment, and suicide risk in bipolar disorder. Conventional pharmacotherapies often provide delayed or incomplete relief and may increase the risk of treatment-emergent affective switching. Recent evidence highlights the therapeutic potential of NMDA receptor antagonists, particularly ketamine and its S-enantiomer esketamine, as rapid-acting antidepressants. METHODS: This narrative review synthesizes findings from randomized controlled trials, systematic reviews, and real-world studies investigating ketamine an

The American Society of Clinical Psychopharmacology (ASCP) task force on the deprescribing of psychotropic medications for mood disorders: Delphi expert consensus.

The American Society of Clinical Psychopharmacology convened a 45-member international expert task force to identify circumstances supporting the deprescribing of core psychotropic medications for major depressive disorder (MDD) and bipolar disorders. Three Delphi survey rounds plus a selective literature review identified points of consensus (predefined as ≥75% agreement) about when antidepressant, antipsychotic, mood stabiliser and sedative-hypnotic deprescribing is warranted. Twenty out of 32 statements (63%) achieved consensus across seven thematic areas. In MDD, panellists favoured discon

Predominant polarity as a potential moderator of group CBT outcomes in bipolar disorder: an exploratory post-hoc analysis.

INTRODUCTION: Bipolar disorder (BD) is a chronic and severe mood disorder, characterized by recurrent episodes of mania, hypomania, and depression. Adjacent to pharmacotherapy, positive evidence has been found for the benefit of adding cognitive behavioral therapy (CBT), family-focused therapy (FFT), interpersonal and social-rhythm therapy (IPSRT) and Group Psychoeducation for treating BD. OBJECTIVE: CBT is well established as a form of psychotherapy for bipolar disorders, although variables that may affect its results are still poorly studied. The present study aimed to investigate Predomina
