Characterization of altered brain functional activity in patients with hypomania.

Hypomania, a diagnostic phase of bipolar disorder (BD), has garnered considerable research attention regarding its underlying neurobiological mechanisms, which remain inadequately understood. This cross-sectional observational study aimed to investigate the neurobiological correlates of hypomania by examining local brain activity and functional connectivity (FC) through resting-state functional magnetic resonance imaging (rs-fMRI) analyses. The results revealed abnormal local brain activity in the anterior orbital gyrus (OFCant) and superior frontal gyrus (SFG) in patients with hypomania. Addi

A multicenter randomized clinical trial of portable transcranial alternating current stimulation for major depressive disorder.

High-definition transcranial alternating current stimulation (HD-tACS) is an emerging neuromodulatory intervention for major depressive disorder (MDD). Here, we evaluate the efficacy and safety of a portable, low-intensity (1.6 mA), 10 Hz HD-tACS protocol targeting the bilateral dorsolateral prefrontal cortex (DLPFC) in adults with MDD. This randomized clinical trial was conducted including 120 adult MDD participants. Participants underwent 20 sessions (5 sessions/week for 4 weeks) of either active 10 Hz HD-tACS (1.6 mA) or sham stimulation a ratio of 1:1 and were followed for an additional 4

Visual imagery and spectrum symptoms of depression and hypomania differentially modulate brain responses during emotional face anticipation and encoding.

BACKGROUND: Depressive (DD) and bipolar (BD) disorders are characterized by biases in anticipating and encoding emotional information. This study examined how lifetime dimensional symptoms of depression and hypomania interact with the vividness of visual imagery to modulate brain activation during the anticipation and encoding of happy and sad faces. METHODS: A total of 155 participants, including individuals with DD, BD, and healthy controls (HC), completed an fMRI task that involved processing cues predicting happy or sad faces and encoding the faces by judging them as pleasant or unpleasan

Emergence of hypomania and mania following initiation of a ketogenic diet: case series.

The ketogenic diet is a century-old, evidence-based treatment for drug-refractory epilepsy, and is now also utilized as an evidence-based treatment for obesity and type 2 diabetes. Emerging research suggests that it may play a role in the treatment of serious mental illness, including treatment-resistant depression, bipolar disorder, and schizophrenia. As with any powerful therapeutic intervention, there can sometimes be side effects. This case series of 9 participants describes 8 who experienced hypomania and 1 who experienced mania within 2 months of starting a ketogenic diet. Of note, 7 of

Rebuilding HiTOP from the ground up: Symptom-level analyses and a revised mapping to the DSM.

The evidence for the structure of the Hierarchical Taxonomy of Psychopathology (HiTOP) model relies heavily on analyses of correlations among Diagnostic and Statistical Manual of Mental Disorders diagnoses, which entrenches some of their limitations. In this study, we deconstructed the mental disorders covered in HiTOP into individual symptoms-with personality disorders represented via item-level assessment of the Alternative Model for Personality Disorders facets-and rebuilt the HiTOP model from the ground up. Participants self-selected into an online mental health study and were randomly spl

Associations of Anxiety and Depression and Hypomanic Traits with Self-Informant Discrepancies in Adults First Diagnosed with ADHD in Adulthood: A Cross-Sectional Study.

Background/Objectives: Adults first diagnosed with ADHD in adulthood are a clinically distinct subgroup, yet the factors associated with self-informant discrepancies in this group remain underexplored. This study examined the associations of anxiety, depression, and hypomanic traits with self-informant discrepancies on the DIVA-5. Methods: A total of 217 adults first diagnosed with ADHD in adulthood and their parents independently completed the DIVA-5. Patients also completed the Self-Rating Depression Scale (SDS), Self-Rating Anxiety Scale (SAS), and the 32-item Hypomania Checklist (HCL-32).

Case Report: Oral glutamatergic augmentation for trauma-related disorders with fluoxetine-/bupropion-potentiated dextromethorphan ± piracetam: a four-patient case series.

Traditional monoaminergic medications often offer limited relief for the physical and cognitive symptoms of post-traumatic stress disorder (PTSD) and complex PTSD. Growing data now point to fast-acting, glutamate-based treatments that boost synaptic plasticity and interrupt fear-conditioned neural circuits. We report four sequential cases of hard-to-treat trauma-spectrum disorders-somatic PTSD, acute bereavement-related PTSD, trauma-linked adolescent depression, and complex PTSD complicated by bipolar II disorder, ADHD, and borderline features-that showed clinically meaningful symptom improvem

Psychedelic-induced hypomania and mania: a systematic review and meta-analysis.

Serotonergic psychedelics are increasingly investigated as treatments for affective disorders. Concerns persist regarding their potential to induce hypomania or mania, particularly in individuals with bipolar spectrum vulnerability. Whether these substances precipitate transient mood switches or contribute to persistent bipolar illness or diagnostic transition remains unclear. We conducted a systematic review of human studies examining manic or hypomanic symptoms following exposure to serotonergic psychedelics (psilocybin, LSD, mescaline, DMT/ayahuasca) or MDMA (CRD420251160656). Detecting bipolarity using the Lebanese Arabic hypomania checklist (HCL-32): validation of shortened HCL versions.

INTRODUCTION: Due to the under diagnosis of bipolar disorder, screening instruments such as the hypomania checklist 32 items (HCL-32) is used to differentiate between Bipolar Disorder (BD) and Major Depressive Disorder (MDD). However due to its lengthy format, efforts were done to validate a shorter alternative without compromising its ability to differentiate between BD and MDD. We aimed to shorten the HCL-32 and assess the screening performance of the three Lebanese Arabic abbreviated HCL versions (HCL-20, -16, and -8) relative to the full HCL-32 in a sample of clinically diagnosed patients

Elucidating the Neural Basis of Bipolar Disorder - Secondary Publication.

Bipolar disorder is a mental disorder characterized by recurrent episodes of mania/hypomania and depression, with a strong genetic contribution and substantial functional burden. Recent genomic studies implicate multiple risk variants converging on intracellular calcium (Ca2+) signaling and synaptic function, while neurons derived from induced pluripotent stem cells of patients with bipolar disorder demonstrate altered neuronal excitability and lithium-responsive phenotypes. Building on early neuroimaging and postmortem observations, accumulating evidence supports the mitochondrial dysfunction

Cross-Sectional and Longitudinal Comparison of Commonly Used Screening Tools for Bipolar Disorders.

BACKGROUND: Misdiagnosis is common in bipolar disorder (BD). Currently used screening tools are brief and cost-effective, but there is a lack of understanding of their reliability over time, as well as whether responses are influenced by demographic or clinical factors. AIMS: To examine the cross-sectional and longitudinal reliability and validity of two commonly used screening tools, as well as their associations with current mood states and other participant characteristics. METHODS: A total of 331 adult patients with a diagnosis of BD completed the Mood Disorder Questionnaire (MDQ) and th

Basal ganglia neurophysiological markers of non-motor symptoms in Parkinson's disease: A systematic review.

BackgroundDeep brain stimulation (DBS) for Parkinson's disease (PD) primarily improves motor symptoms but leaves non-motor symptoms (NMS) largely unattended. Neurophysiological markers associated with specific symptoms could improve DBS programming. We systematically reviewed the evidence linking basal ganglia local field potentials (LFP) to NMS in PD.MethodsThe literature search (Medline, Embase, Scopus, and Web of Science) on August 20, 2024 yielded 1066 records. Studies were included if they focused on patients with idiopathic PD treated with DBS of the subthalamic nucleus (STN) or globus p

Can We Improve the Prediction of Early Onset Mania and Hypomania in the Community?

BACKGROUND: There is broad agreement that the onset of bipolar disorders (BD) can be predicted by using combined estimates of familial, genetic and clinical risk. However, there is a lack of consensus about the operationalisation of different risk attributes (e.g., symptoms vs. sub-threshold syndromes; disorder-specific polygenic risk scores [PRS] vs. multiple-disorder PRS dimensions) and their utility for predicting bipolar type 1 (BD-I) and 2 (BD-II). Likewise, analyses often fail to consider the optimal model for predicting outcomes where true cases will be in the minority. METHODOLOGY: Pr

Investigating behavioral inertia in passively sensed smartphone parameters to differentiate affective episodes in patients with bipolar disorder.

Bipolar disorders are severe, recurrent mental illnesses characterized by distinct mood states associated with different behavioral patterns. While (emotional) inertia, the tendency of a system to persist or change, has been linked to depressive episodes, research on its role in hypomanic or manic states remains sparse. Moreover, behavioral inertia, assessed via passive sensing using smartphone parameters is equally underexplored. This study investigated the temporal dynamics during (hypo-)manic and depressive episodes in passively sensed data in patients with bipolar disorder, complementing p
