Pooled analysis assessing the efficacy and safety of amphetamine derivative adjunctive therapy for bipolar depression.

OBJECTIVE: The bipolar pharmacopoeia targeting depression is significantly underdeveloped. Despite high rates of attention deficit symptoms, there has been inadequate investigation of stimulants as a treatment intervention for adults with bipolar depression. METHODS: Data were pooled from two, 8-week, randomized, placebo-controlled trials of adjunctive mixed d-amphetamine/l-amphetamine salt (MAS) and lisdexamfetamine (LDX). The primary outcome measure was baseline-to-endpoint change in the Montgomery-Asberg Depression Rating Scale (MADRS). Secondary outcomes included treatment response and re

Development of the Reactive Aggression Assessment (RAGA-16) Part II: Clinical Validation.

OBJECTIVE: Reactive aggression (RA) is a transdiagnostic symptom associated with more severe illness and worse clinical course. Despite the clinical importance of RA, few validated screening measures or outcome assessments exist for use with children under 12. The 16-item Reactive Aggression Assessment (RAGA-16) is a novel parent-report instrument with promising evidence of psychometric reliability. The current work presents clinical validation evidence for RAGA-16. METHODS: A retrospective chart review examined the records of children seeking inpatient and outpatient services. Parents comple

Low inhibitory control mediates the negative relationship between hypomania and creativity in divergent thinking.

Hypomanic traits are often assumed to enhance creative thinking, but empirical evidence is mixed. The present study assessed creativity in divergent thinking across a broad range of hypomanic traits. Participants (n = 468, including both healthy adults and individuals with bipolar disorder) completed Alternate Uses Tasks (AUT) in which they generated novel uses for common objects. Independent raters scored each response for creative quality, considering both originality and usefulness, yielding two metrics per participant: mean creativity (average idea quality) and maximum creativity (average

Amygdala Functional Hyperconnectivity: A Multiple Symptom Subdomain Marker of the Bipolar Risk to Disorder Spectrum.

BACKGROUND: Identifying reproducible neural markers of bipolar disorder(BD) risk is critical for early detection and differentiation from unipolar/major depression. We previously demonstrated that inter-amygdala functional connectivity(FC) and bilateral-ventrolateral-right-dorsolateral-prefrontal-cortex(vlPFC-dlPFC)-FC were positively associated with both mania/hypomania and depression risk and mania/hypomania risk, respectively, as measured by the Mood Spectrum Self-Report(MOODS-SR) 'mood' subdomains, replicated in three independent samples. We tested whether these neural markers also general

Hypomanic symptoms and first manic episode during ketogenic diet treatment in a patient taking venlafaxine: a case report with longitudinal glucose-ketone index and clinical monitoring.

BACKGROUND: The ketogenic diet (KD) is gaining increasing interest as a metabolic intervention for neuropsychiatric disorders, including mood disorders. However, its potential role in clinically relevant affective changes, such as hypomania or mania, remains insufficiently characterized. METHODS: We present the case of a woman with a previous history of major depressive episode with psychotic symptoms and a posterior history of recurrent episodes of mild depression, who never achieved complete symptom remission. In an effort to ameliorate her chronic depressive symptoms, the patient started a

Neural markers of working memory and emotional regulation are predictors of future depression severity in at risk young adults: a replication study.

Objective neural markers that reflect the underlying pathophysiological mechanisms of affective disorders are needed to facilitate early identification of individuals most at risk of future affective disorders and ultimately provide neural targets to guide therapeutic interventions. Using an emotional n-back paradigm designed to examine working memory (WM) and emotional regulation (ER) capacity, we previously showed that WM-related elevated left dlPFC activity (a key node of the central executive network (CEN)) and elevated right precuneus activity (a key node of the default mode network (DMN)

Shared and disorder-related abnormalities in cortical steady-state responses in ADHD and hypomania at adolescence age.

OBJECTIVE: Bipolar disorder (BD) and ADHD share symptomatic overlap during adolescence, yet their distinct neurophysiological underpinnings remain unclear. This study compared cortical steady-state responses to visual (15 Hz SSVEP) and auditory (40 Hz ASSR) stimulation in adolescents with hypomania, ADHD, and healthy controls. METHODS: EEG was recorded from 105 adolescents (35 per group) during passive exposure to visual and auditory stimuli. SSVEP and ASSR amplitude and phase were analyzed across six cortical regions using repeated-measures MANOVA with FDR correction. RESULTS: The ADHD grou

Digital Markers for Passive Remote Monitoring of Bipolar Disorder: Systematic Review.

BACKGROUND: Bipolar disorder (BD) features episodic shifts among mania, hypomania, depression, mixed states, and euthymia. Timely detection of mood transitions is difficult due to infrequent clinical touchpoints. Digital health technologies, including wearables and smartphones, offer a unique opportunity to passively and continuously monitor behavior and physiology that could reflect underlying mood dynamics in real-world settings. OBJECTIVE: This study aimed to systematically review passively collected digital markers for BD mood states, characterize devices/modalities and analytic approache

Neural Markers of Reward Valuation and Impulsive Decision-Making in Mania Risk.

IMPORTANCE: Mania or hypomania, the pathognomonic feature of bipolar disorder (BD), is associated with reward hypersensitivity and impulsive decision-making, but the neural mechanisms underlying these distinct behavioral facets remain unclear. Reward expectancy (RE)-associated activation in the left ventrolateral prefrontal cortex (L-vlPFC) and pre-supplementary motor area (pre-SMA) is elevated in individuals at risk of mania or hypomania, but it is unknown whether elevated activation in these regions and associations with risk-related behavioral facets reflect convergent or dissociable pathwa

Bridging the gap between poles: Network structure and bridging mechanisms of depressive and hypomanic symptoms in a large-scale psychiatric outpatient sample.

BACKGROUND: The co-occurrence of depressive and hypomanic symptoms complicates mood disorder assessment and treatment. However, the symptom pathways linking these domains remain poorly understood. This study examined the network structure of depressive and hypomanic symptoms to identify key bridge symptoms. METHODS: Cross-sectional data from 6913 psychiatric outpatients were analyzed using the Beck Depression Inventory-II (BDI-II) and Hypomania Checklist-32 (HCL-32). A Gaussian Graphical Model estimated symptom associations, and bridge centrality indices identified symptoms linking depressive

Psychometric Properties of the Turkish Version of the Rapid Mood Screener for Bipolar Disorder (RMS-T): A Validity and Reliability Study.

OBJECTIVE: The Rapid Mood Screener (RMS) is a reliable and validated brief, point-of-care, patient-administered screening tool for bipolar disorders. Herein we aim to evaluate the reliability and validity of the RMS in Turkish adults with bipolar I disorder (BD). METHOD: Forward and backward translation was conducted for the English version of the RMS to the Turkish version of RMS (RMS-T). We conducted a ROC analysis for both the Mood Disorder Questionnaire (MDQ) and RMS-T in adults with major depressive disorder (MDD) or BD. The maximum sensitivity and specificity were also calculated. RESU

An Open-Label, Dose-Escalation Trial of Psilocybin-Assisted Therapy for Bipolar II Depression.

Objective: Individuals with bipolar II disorder (BD-II) and depression face limited treatment options and are often excluded from psilocybin therapy trials due to concerns about precipitating mania or psychosis. The objective of this study was to evaluate the safety, tolerability, and preliminary efficacy of psilocybin therapy in individuals with BD-II and depression. Methods: We conducted an open-label, single-arm pilot trial from 2022-2025 in which 14 participants with BD-II depression, diagnosed using adapted criteria requiring ≥2 consecutive days of hypomanic symptoms, received 10 mg of ps

Green space exposure predicts manic episodes in bipolar disorder: one year longitudinal analysis using smartphone-derived GPS data.

Urban living is linked to increased psychiatric morbidity, while green spaces may protect mental health via stress-buffering effects. Yet, the role of green space exposure in the onset of affective episodes in bipolar disorder (BD) remains unclear. Using 12 months of GPS data from 29 participants with BD-I/II, we quantified daily exposure to green spaces (green area & NDVI; covariates: population density & imperviousness). Affective status (euthymia, depressive or (hypo)manic episode) was assessed via SCID-based biweekly clinician interviews (26 assessments per participant). Multilevel logisti

Molecular and Neuroimaging Correlates of Bipolar Disorder: Linking Inflammation, Mitochondria, and Brain Circuitry.

Bipolar affective disorder (BD) is a severe mental illness characterized by recurrent episodes of mania, hypomania, and depression, accompanied by progressive neurobiological changes that go beyond the classical concepts of neurotransmitter dysregulation. Increasing evidence points to the key role of the interaction between inflammatory processes, mitochondrial dysfunction, and disturbances within neural networks in the pathogenesis, course, and treatment response of BD. Neuroinflammatory processes, including elevated levels of pro-inflammatory cytokines, chemokines, and microglial activation,

Mood switching in 1629 antidepressant-treated bipolar disorder patients.

Mood-switching from depression into [hypo]mania is prevalent in bipolar disorder (BD) patients, particularly during treatment with an antidepressant, but many characteristics associated with such responses require clarification. Using standard bivariate and multivariate measures we compared characteristics of 1629 prospectively evaluated BD patients who did or did not become [hypo]manic during antidepressant treatment. Mood-switching occurred in 27.6% of BD patients (type II [BD2]: 31.4%; type I [BD1]: 24.5%) observed prospectively for 4.98 years, most often (65.5%) with new hypomania. Factors
