Attribution of psychiatric manifestations to systemic lupus erythematosus in Chinese patients: A retrospective study.

BACKGROUND: Not all neuropsychiatric (NP) manifestations in patients with systemic lupus erythematosus (SLE) are secondary to lupus. The clarification of the cause of NP symptoms influences therapeutic strategies for SLE. AIM: To understand the attribution of psychiatric manifestations in a cohort of Chinese patients with SLE. METHODS: This retrospective single-center study analyzed 160 inpatient medical records. Clinical diagnosis, which is considered the gold standard, was used to divide the subjects into a psychiatric SLE (PSLE) group (G1) and a secondary psychiatric symptoms group (G2).

The Potential of Using Screening Tools for Bipolar Disorder to Predict Lithium Response.

Background/Objectives: Lithium is the gold standard for treating Bipolar Disorder (BD), but its effectiveness varies widely. While clinical and environmental factors may influence response, it remains unclear if screening tools can reliably predict lithium response outcomes. This study explores this potential using two widely used screening instruments for BD. Methods: A total of 146 patients with BD were evaluated. Lithium response was assessed using the Alda Scale, while hypomanic and manic symptoms were characterized through the Hypomania Checklist-32 (HCL-32) and the Mood Disorder Question

Understanding Relapse in Bipolar Disorder at a Tertiary Mental Health Facility in Uganda.

BACKGROUND: Bipolar disorder (BD) is a chronic psychiatric condition characterized by recurrent episodes of mania, hypomania, and depression, affecting approximately 1 in 150 adults globally. BD contributes significantly to disability and years of life lost, with a notable prevalence in Africa. The high relapse rates in BD, exacerbated by limited mental health resources and underdiagnosis, highlight the need for targeted interventions. This study explores the factors contributing to relapse among BD patients at Butabika National Referral Mental Hospital, Uganda, a crucial mental health facilit

Real-world application of Short Term Psychodynamic Psychotherapy (STPP) as chronic migraine preventive therapy: Profiling responders and predictive factors.

INTRODUCTION: Among non-pharmacological approach for chronic migraine (CM), Short-Term Psychodynamic Psychotherapy (STPP) is suggested in CM and, particularly, in interrupting medication overuse (MO). In this study, we aim at identifying clinical predictive factors to identify CM patients who could more likely respond to STPP. METHODS: We designed a prospective real-world observational study on CM patients undergoing STPP. We collected clinical data related to migraine burden (baseline headache days, number of acute medications used, MIDAS, HIT-6), as well as psychiatric comorbidities (though

Using Natural Language Processing Methods to Build the Hypersexuality in Bipolar Reddit Corpus: Infodemiology Study of Reddit.

BACKGROUND: Bipolar is a severe mental health condition affecting at least 2% of the global population, with clinical observations suggesting that individuals experiencing elevated mood states, such as mania or hypomania, may have an increased propensity for engaging in risk-taking behaviors, including hypersexuality. Hypersexuality has historically been stigmatized in society and in health care provision, which makes it more difficult for service users to talk about their behaviors. There is a need for greater understanding of hypersexuality to develop better evidence-based treatment, support

Unipolar Mania: Prevalence and Patient Characteristics.

OBJECTIVES: Recurrent [hypo]mania without major depressive episodes ("unipolar mania" [UPM]) is an uncommon form of major affective disorder related to bipolar disorder (BD). We characterized UPM patients and estimated the prevalence of their characteristics based on prolonged times-at-risk. METHODS: Using standard bivariate and multivariate statistics, we compared the characteristics of 63 consecutive UPM patients to 1210 other BD patients over prolonged, close, prospective follow-up at expert mood disorder centers. RESULTS: UPM was uncommon (4.95% of 1273 BD cases during 18.2 years at risk

Global Trends in the Use of Pharmacotherapy for the Treatment of Bipolar Disorder.

Bipolar Disorder (BD) is a chronic mental health condition characterized by significant mood swings, including periods of mania or hypomania and depression. Affecting approximately 1-2% of the global population, BD is associated with impaired social functioning, decreased quality of life, and an increased risk of suicide. The disorder presents a substantial burden on healthcare systems and imposes significant economic costs due to lost productivity and the need for extensive treatment and support services. This comprehensive review synthesizes global trends in BD pharmacotherapy over the past

Positive predictive value of an ICD-10-based operationalization of bipolar II disorder for register-based research.

OBJECTIVE: Bipolar II disorder (BD-II) is a subtype of bipolar disorder characterized by recurrent episodes of depression and hypomania, without full manic episodes. Unfortunately, BD-II is not included as a diagnostic code in the ICD-10, which means that the Danish Psychiatric Central Research Register (DPCRR), where diagnoses are recorded according to the ICD-10, cannot be used to study BD-II without further ado. The aim of this study was to investigate whether BD-II can be operationalized retrospectively based on ICD-10 diagnoses with sufficient positive predictive value to allow for studie

"The captain wanted to stay with the sinking … (can't say boat)": How hypomanic personality traits interfere with inhibitory control?

Inhibitory control, the ability to suppress or countermand an action or thought, is notably impaired in patients with bipolar disorder. However, the specific characteristics of this impairment and whether deficits can manifest prior to a formal bipolar disorder diagnosis or among individuals with subsyndromal symptoms are relatively unknown. Here we test whether healthy individuals exhibiting a higher level of hypomanic personality traits respond more rapidly, albeit with weaker inhibitory control. We evaluated 83 native French speakers using the Hypomanic Personality Scale (HPS) and administe

Association of Human Endogenous Retrovirus HERV-K18 Variant with Bipolar Disorder Type I.

OBJECTIVE: Human endogenous retroviruses (HERVs) and associated sequences occupy ∼8% of the human genome and dysregulation of HERV transcripts may have significant impacts on human health including psychiatric disorders. HERV-K18 is still active in the human genome and its envelope gene encodes a superantigen (SAg) which may result in deregulation of the immune system. In the study, the possible associations of the two variants localized in the SAg-coding region of HERV-K18 with bipolar disorder type I (BD-I) were evaluated. METHODS: The subjects included 100 patients with BD-I and 100 age- a

Peripheral blood miR-16-5p as a potential biomarker for distinguishing unmedicated bipolar disorder type II from major depressive disorder.

OBJECTIVE: major depressive disorder (MDD) and bipolar disorder type II (BD-II) are difficult to distinguish clinically due to similar depressive symptoms and unrecognizable hypomania symptoms in the early stages. The study aims to identify these two disorders in the early stages through differential expression of microRNAs. METHODS: 93 subjects including 66 unmedicated patients (33 MDD, 33 BD-II), and 27 healthy controls (HC) were enrolled. At the time of enrollment, all subjects' demographic data, HAMD, HCL-32, and YMRS scales were assessed. 5 ml of peripheral blood for all subjects was col

Psilocybin-Assisted Psychotherapy for Treatment-Resistant Depression in Bipolar II Disorder.

BACKGROUND: Bipolar II disorder (BD-II) is often associated with chronic and treatment resistant major depressive episodes. Psilocybin has shown promise for its rapid-acting antidepressant effects, though its impact on bipolar depression remains unexplored. In the present subgroup analysis of an already published trial on treatment-resistant depression (TRD), we aimed to preliminarily evaluate the safety and efficacy of psilocybin in patients with BD-II. METHODS: Adults with TRD associated with BD-II, excluding those with psychosis were included. Participants underwent one or two psilocybin s

Genomics of Bipolar Disorder: What the Clinician Needs to Know.

Bipolar disorder (BD) affects approximately 2% of the global population, characterized by alternating episodes of mania or hypomania, and depression. It comprises two main types: bipolar I disorder, marked by severe manic episodes, and bipolar II disorder, defined by milder hypomanic episodes. Individuals often experience rapid cycling and significant comorbidities, leading to decreased productivity and increased mortality rates. Early diagnosis and intervention are crucial for better outcomes. Both genetic and environmental factors contribute to BD's etiology, with genetic research promising

Navigating Hypersexuality in Bipolar: Insights from a Corpus-Assisted Discourse Analysis of Reddit Posts.

Bipolar is a severe mental health condition affecting at least 2% of the global population. Clinical observations indicate that individuals in elevated mood states, such as mania or hypomania, are more likely to engage in risk-taking behaviours, including hypersexuality. Characterised by an excessive preoccupation with sex, hypersexuality can lead to serious consequences such as sexual assault, unplanned pregnancies, and relationship breakdowns. Existing evidence suggests that opportunities for intervention in cases of hypersexuality in bipolar may currently be overlooked. This study provides

Expressed Emotions in Families of Patients With Bipolar Disorder.

Introduction Bipolar affective disorder (BPAD) is a chronic psychiatric illness characterized by recurrent episodes of mania, hypomania, and depression, significantly impacting both patients and their caregivers. Expressed emotion (EE), which includes criticism, hostility, and emotional over-involvement, plays a crucial role in caregiver burden and patient outcomes. High EE environments have been linked to increased relapse rates and hospitalization, while low EE settings promote better adherence and stability. In India, where mental healthcare is predominantly family-driven, caregivers experi

Reward magnitude-specific delay discounting differentiates mania versus depression risk.

Mania/hypomania, the pathognomonic feature of bipolar disorder (BD), is characterized by elevated impulsivity, often assessed via delay discounting-the preference for smaller, immediate versus larger, delayed rewards. It remains unclear whether delay discounting differentiates BD from non-BD individuals or serves as an objective behavioral marker of mania/hypomania versus depression risk. Bipolar disorder (n = 40) and non-BD (n = 187) individuals were recruited, with the latter encompassing a range of mania/hypomania and depression risk and stratified into mania/hypomania and depression risk t

The influence of depressive and manic symptoms on suicidal ideation in mixed mood states.

BACKGROUND: While bipolar disorder is strongly linked to an increased risk of suicide, recent evidence has challenged the assumption that mixed symptoms play a distinct role in suicidal ideation beyond depressive severity. This study examines how depressive, hypo/manic, and mixed features influence suicidal ideation in individuals with bipolar disorder. Data from 903 participants in the Stanley Foundation Bipolar Network (1995-2002) were analyzed to assess associations between mood states, classified by the Inventory of Depressive Symptomatology-Clinician-Rated (IDS-C) and the Young Mania Rati
