Underexplored Connections Between Diabetes, Hypomanic States and Insecure Attachment.

Diabetes, dopamine, attachment style disorders, and hypomania share complex interrelations involving neuroinflammation, dysfunction in brain networks (DMN, CEN, SAL), and emotional regulation. Both Type 1 and Type 2 diabetes induce cognitive and structural changes in the brain through mechanisms such as hyperglycemia, insulin resistance, and chronic inflammation. These processes can affect the dopaminergic system, which plays a pivotal role in motivation, emotional regulation, and the manifestation of hypomania. Dopamine is directly linked to attachment styles, with disturbances in this system

Psychiatric Outcomes of Subthalamic Nucleus Deep Brain Stimulation: A Systematic Review of Short- and Long-Term Effects.

Background/Objectives: Deep Brain Stimulation (DBS) of the subthalamic nucleus (STN) is a widely used intervention for Parkinson's disease (PD) and obsessive-compulsive disorder (OCD). While motor and OCD symptom benefits are established, increasing evidence highlights psychiatric side effects. The underlying mechanisms involve stimulation parameters, electrode positioning, and medication adjustments. This systematic review aims to evaluate the short-term and long-term psychiatric effects of STN-DBS and identify influencing factors. Methods: A systematic literature search (PubMed, Scopus, Web

Deep brain stimulation for obsessive-compulsive-disorder: a systematic review and meta-analysis of individual participant outcome data from sham-controlled trials.

INTRODUCTION: Deep brain stimulation (DBS) is a neuromodulatory intervention for severe, treatment-refractory obsessive-compulsive disorder (OCD). We conducted the first meta-analysis using individual participant outcome data, systematically evaluating (1) efficacy of DBS compared to sham-stimulation in randomized controlled trials (RCTs), (2) adverse events and (3) methodological trial quality. METHODS: We conducted a systematic search across multiple databases, including all RCTs comparing DBS with sham in adults with OCD, regardless of stimulation target. We obtained Yale-Brown Obsessive-C

Systematic Review of Second-Generation Antidepressant Monotherapy for Acute Bipolar-II Depression.

PURPOSE: Individuals with Bipolar II disorder (BD-II) face high rates of depression and have limited FDA-approved treatments, leading to treatment delays and poor functioning. Although not commonly recommended, monoaminergic antidepressants are prescribed to 50% of patients. This review updates the evidence for second-generation antidepressant (SGAD) monotherapy in treating acute BD-II depression. METHODS: We searched Ovid MEDLINE, Embase, CENTRAL, PsycINFO, Scopus, and Web of Science from March 2021 to February 2025 and included five studies published before 2021. Randomized controlled trial

Genetic and environmental contributions to the link between synaesthesia and neurodevelopmental and psychiatric features: a twin study.

Synaesthesia is a sensory phenomenon where specific inputs such as written letters or tastes automatically trigger additional sensations (for instance colours). The phenomenon is more common in people on the autism spectrum compared to the general population and seems also to be associated with other neurodevelopmental and psychiatric conditions and features. We assessed the associations between self-reported synaesthesia and eight psychiatric / neurodevelopment features in 18-year-old twins and estimated the genetic and environmental contributions to these associations using classical twin mo

Towards Adaptive Deep Brain Stimulation for Non-Motor Symptoms in Parkinson's Disease?

Non-motor symptoms are prevalent, disabling, and difficult to treat at all stages of Parkinson's disease (PD). Whereas current deep brain stimulation (DBS) for PD targets motor symptoms, quality of life improvement, or lack thereof, is often largely determined by non-motor symptoms. So far, the effect of DBS on non-motor PD symptoms is insufficient, and neuromodulation targeted specifically at improving non-motor PD symptoms has been largely unaddressed. In this review, we provide a comprehensive overview of the effect of DBS on non-motor PD symptoms and discuss DBS-induced non-motor side effe

Exploring the alignment between clinician-reported assessment of social autonomy and patient-reported assessment of quality of life in mood disorders: a cross-sectional study.

INTRODUCTION: Patient-reported quality of life reflects subjective factors such as well-being and autonomy, while clinicians may focus on functional capabilities. Understanding the factors behind the alignment or discordance between these assessments can help comprehend patients' values and social contexts. METHODS: This study explored the agreement between clinician-reported assessment of social autonomy and patient-reported assessment of quality of life in 92 adult participants with a mood disorder. Validated scales were used to measure the severity of depression, hypomania, quality of life

Neurocognitive and neurobiological features of early-stage, unmedicated patients with bipolar disorder.

OBJECTIVE: This study aimed to identify young adults who met the criteria for bipolar disorder (BD) but had not yet been diagnosed or treated. It also aimed to evaluate their neurocognitive and neurobiological changes compared to healthy controls (HC). METHODS: Individuals at high risk for BD were identified by using the Hypomania Checklist 32 items (HCL-32R). Participants were then assessed with the Structured Clinical Interview for DSM-IV Axis I Disorders (SCID-I) to identify previously undiagnosed and drug-naive patients (DNP) (n=27). A matched HC group (n=27) was included for comparison.

Bipolar disorder.

The hallmark of bipolar disorder is hypomania or mania, and the predominant phase of illness is depression. Affecting approximately 40 million individuals worldwide, bipolar disorder is associated with a substantial psychosocial, medical, and financial burden and increased mortality from suicide and other causes. Diagnosis can be challenging due to symptom overlap with attention-deficit hyperactivity disorder, major depressive disorder, psychotic spectrum disorders, and personality disorders, which often leads to a delay in diagnosis. Recent advancements in understanding disease risk and patho

Can Sleep Parameters Predict Upcoming Mood Episodes in Bipolar Disorder?

BACKGROUND: Bipolar disorder (BD) is a recurrent disorder, characterised by episodes of (hypo)mania, depression and euthymia with variation in mood, cognition and sleep. Many patients identify changes in sleep before an episode; using daily sleep logs could help identify these changes. Such early warning signs can be a valuable tool for patients and clinicians alike in predicting and preparing for changes in mood. METHODS: In the BipoSense study, we followed patients with BD, who were in remission at the start of the study, daily for 1 year. Patients reported for each hour if they were awake

Neurocognitive Biotypes of Risk and Resilience for Mood Disorders in Adolescents: Insights From Behavioral and Graph-Theoretic Network Markers.

BACKGROUND: Adolescence is a developmental period of increased prevalence of mood disorders, but identifying adolescents who are at risk for or resilient to mood pathology remains a clinical challenge. In the current study, we addressed this challenge by evaluating multidimensional profiles of neurocognitive functioning (biotypes) that may confer vulnerability or protect against psychopathology. Biotypes were derived from neurocognitive data and identified as being resilient or high risk based on their association with future symptoms. METHODS: Adolescents (N = 146; 13-21 years, 66% first-deg

Subclinical hypomania, psychiatric and neurodevelopmental diagnoses: phenotypic and aetiological overlap.

BACKGROUND: Subclinical hypomanic symptoms are fairly common in the general population but are linked to psychiatric and neurodevelopmental conditions. However, the genetic and environmental origins of these associations are unclear. This twin study examined the phenotypic and aetiological associations between subclinical hypomania and psychiatric and neurodevelopmental diagnoses. METHODS: Participants were 4,932 twin pairs from the Child and Adolescent Twin Study in Sweden. Hypomanic symptoms were assessed using the parent-rated Mood Disorders Questionnaire when the twins were aged 18. Speci

Morphological and genetic signatures of hypomania: A multisite exploration of reward circuitry and transcriptomic profiles.

BACKGROUND: Hypomania, characterized by elevated mood, impulsivity, and heightened social vitality, is closely linked to alterations in the brain's reward circuitry. However, the relationship between reward circuitry and hypomanic traits, particularly in terms of structural and molecular mechanisms, remains unclear. METHODS: A total of 742 participants with varying proneness to hypomania were recruited across four sites. Hypomanic proneness was assessed using psychometric scales. Structural MRI data were analyzed with voxel-based morphometry and inter-subject representational similarity analy
