Getting lucky: Clinical characterization of compulsive sexual behavior compared with gambling disorder and intermittent explosive disorder.

This study provides a comparative description of treatment-seeking adults with CSBD, Gambling Disorder (GD), and Intermittent Explosive Disorder (IED) across demographic, psychiatric, and personality correlates. Participants (N = 969; CSBD n = 233, GD n = 634, IED n = 102) were evaluated at a single specialty center. CSBD was ascertained using Goodman's criteria; GD and IED were diagnosed with DSM-IV SCID procedures. Psychiatric comorbidities were assessed with the Mini-International Neuropsychiatric Interview v5.0.0, and personality with the Temperament and Character Inventory. Group differen

Associations between eating disorder symptoms and latent bipolarity in patients with major depressive disorder.

OBJECTIVES: To determine the correlations between eating disorder symptoms and clinical and psychometric characteristics of major depressive disorder (MDD). METHODS: Patients aged 18 to 59 years diagnosed with MDD, regardless of the stage (exacerbation or remission), were recruited from both outpatient and inpatient settings across seven centres in Russia. The MDD diagnosis of each patient was confirmed using the Mini International Neuropsychiatric Interview. Depression severity was assessed using the clinician-administered Montgomery-Asberg Depression Rating Scale. Disordered eating behaviou

Digital Biomarkers for Passive Remote Monitoring of Bipolar Disorder: Systematic Review.

BACKGROUND: Bipolar disorder (BD) features episodic shifts among (hypo)mania, depression, mixed states, and euthymia. Timely detection of mood transitions is difficult due to infrequent clinical touchpoints. Digital health technologies, including wearables and smartphones, offer a unique opportunity to passively and continuously monitor behavior and physiology that could reflect underlying mood dynamics in real-world settings. OBJECTIVE: We aim to systematically review passively collected digital biomarkers for BD mood states, characterize devices/modalities and analytic approaches, appraise

The efficacy and safety of pharmacological treatment for major depressive episode with mixed features specifier: a systematic review and meta-analysis.

BACKGROUND: With the increased prevalence of major depressive episodes with mixed features specifier (MDE-MFS), the pharmacological treatment for MDE-MFS has attracted great clinical attention. This study aimed to investigate the efficacy and safety of medication use for MDE-MFS. METHODS: Commonly used databases were searched for the meta-analysis. Primary efficacy outcomes included response rate and the change in the Young Mania Rating Scale scores; the primary safety outcome was the rate of treatment-emergent hypomania/mania. Effects were expressed as relative risk (RR) or standardized mean

Childhood trauma and subclinical cin early adulthood: A genetically informative study.

BACKGROUND: There is preliminary evidence that childhood trauma (e.g., abuse) is associated with subclinical hypomania reported in adolescence. These findings need replicating in early adulthood, as clinical conditions emerge, and the mechanisms underlying this association need elucidating. This study aimed to examine the magnitude of shared genetic and environmental underpinnings of the association between childhood trauma with hypomanic symptoms and high-risk status for bipolar disorder (BD) using a twin design. Gene-environment correlations and interactions between childhood trauma and poly

Resting-state fMRI signals identify bipolar risk features in depressed individuals and their relation to cognitive function.

BACKGROUND: Emerging evidence suggests that individuals with major depressive disorder (MDD) at risk for bipolar disorder (BD) may represent a distinct subtype within the mood disorder spectrum. However, the neurocognitive and neurofunctional characteristics of this group remain underexplored. METHODS: Resting-state fMRI and cognitive assessments were obtained from currently depressed participants. BD risk factors were identified by Hypomania Checklist-32 (HCL-32) scores ≥14 and/or a first-degree family history of BD. 139 BD, 181 MDD+ (with BD risk), and 138 MDD- (without BD risk) patients we

(Es)ketamine as a treatment for depressive episodes with psychotic features: systematic review.

BACKGROUND: Psychotic symptoms in depression are linked to worse outcomes, and treatment options are limited. Ketamine and esketamine are effective antidepressants, yet most studies have excluded patients with a history of psychotic symptoms. AIMS: To evaluate by systematic review the efficacy and safety of ketamine and esketamine in treating patients with unipolar or bipolar depressive episodes with psychotic features. METHOD: A comprehensive search of the PubMed, Ovid and Web of Science databases was conducted up to 2 November 2023. We included any study that reported the use of ketamine o

Efficacy of aripiprazole long-acting injection monotherapy in pregnancy-related hypomania: A treatment choice through shared decision-making.

BACKGROUND: Managing bipolar disorder during pregnancy requires balancing maternal mental health and fetal safety. While guidelines often focus on severe mania, the management of hypomania, a common clinical challenge, remains poorly defined. Although shared decision-making (SDM) is recommended for treatment planning, its application in managing acute mood episodes during pregnancy remains underexplored, especially without robust evidence. CASE PRESENTATION: A woman in her thirties with DSM-5 Bipolar I Disorder developed a hypomanic episode at 32 weeks of gestation while on aripiprazole once-

Critical Overview of Screening Tools for Detecting Bipolar Disorders.

This overview aims to explore the key screening tools for detecting bipolar disorders (BDs): the Mood Disorder Questionnaire (MDQ), Bipolar Spectrum Diagnostic Scale (BSDS), Hypomania Checklist (HCL-32), and Rapid Mood Screener (RMS), while offering guidance to healthcare professionals in selecting the most appropriate tool for each clinical scenario. The MDQ is widely utilized due to its high specificity (0.90) for identifying Bipolar Disorder (BD) in psychiatric consultations, although it is more sensitive to bipolar I than bipolar II. The BSDS, designed to encompass a wider range of bipolar

Bipolar disorder in children, adolescents and young adults. Part 2. Therapeutic management. Recommendations under the patronage of the Executive Board of the Polish Psychiatric Association, National Consultants in the field of Psychiatry and National Consultants in Child and Adolescent Psychiatry.

Bipolar disorder is characterized by recurrent episodes of depression, mania, hypomania, or mixed states. Early diagnosis and comprehensive treatment, including pharmacotherapy and psychosocial interactions, are associated with a more favorable prognosis. The general principles for the use of drugs in bipolar disorder in children and adolescents remain similar to those in adults. The basic drugs in bipolar disorder are mood stabilizers of the first and second generation; however, their efficacy and safety profile in children and adolescents differ from those in adults. In addition, prophylacti

Differences in event-related potentials between unipolar depression and bipolar II disorder during depressive episodes: a retrospective case-control study.

BACKGROUND: Bipolar II disorder (BD II) is a chronic and severe mental illness frequently misdiagnosed as major depressive disorder (MDD) due to symptom overlap and the absence of objective diagnostic tools. Consequently, establishing pathophysiological markers to differentiate BD II from MDD is critical. METHOD: A total of 180 patients were enrolled in the study and allocated to three groups: patients with unipolar depression (UD group; MDD currently experiencing a major depressive episode, n = 60), patients with bipolar II disorder during depressive episodes (BD II group; n = 60), and age-

Uncovering predictors of bipolar II conversion to bipolar I: A machine learning analysis of national health records in Taiwan.

BACKGROUND: Bipolar II disorder (BD-II) can progress to bipolar I disorder (BD-I), carrying profound psychosocial consequences. However, limited research has investigated the transition from BD-II to BD-I. Our study uses machine learning to predict the conversion using real-world data. METHODS: We conducted a retrospective cohort study using the Taiwan National Health Insurance Research Database (2000-2013) to identify adults diagnosed with BD-II. A predictive model for BD-I conversion was developed using extreme gradient boosting (XGBoost), incorporating demographic factors, healthcare utili

Real-World Outcomes of Brexanolone to Treat Postpartum Depression.

BACKGROUND: While the efficacy of brexanolone, a U.S. Food and Drug Administration-approved infusion treatment for postpartum depression (PPD), has been demonstrated in controlled trials, few reports are available describing its real-world effectiveness. METHODS: Leveraging real-world clinical data, the current report describes the characteristics and treatment outcomes of a sample of women (N = 150) receiving brexanolone treatment for PPD in a residential-style outpatient treatment center with up to 12 months of follow-up. The sample had a mean age of 30.0 years (SD = 4.5), with almost one-t

Deep brain stimulation for obsessive-compulsive disorder: evolution of tractography-based targeting.

OBJECTIVE: Deep brain stimulation (DBS) in the anterior limb of the internal capsule (ALIC) for obsessive-compulsive disorder (OCD) can result in large improvements in symptoms and quality of life. However, without a consistent stimulation target within the anatomically variable ALIC region, the therapy has required long and unpredictable trial-and-error periods of parameter optimization to achieve these clinically meaningful benefits. To facilitate scalability and clinical implementation of ALIC DBS for OCD, this study aimed to demonstrate the evolution of prospective DBS targeting using pati

Psilocybin use in bipolar disorder: A comprehensive review.

INTRODUCTION: Bipolar disorder (BD) is a severe and persistent mental disorder characterized by recurrent mood episodes, with BD depression accounting for most of the illness burden. Although the mainstay treatment of BD consists of pharmacotherapy with mood stabilizers and atypical antipsychotics, a large proportion of patients with BD depression do not respond to adequate trials of medications. In addition, these medications can be associated with multiple, often significant adverse effects, highlighting the need for novel therapeutic agents that are acceptable, effective and safe for patien

Complete Remission With Rituximab in Relapsed Primary Focal Segmental Glomerulosclerosis Without Use of High-Dose Glucocorticoids.

Focal segmental glomerulosclerosis (FSGS) is a histopathological lesion characterised clinically by nephrotic range proteinuria and frequently progresses to end-stage kidney disease. Primary FSGS is a complex disease process hypothesised to result from circulating permeability factors resulting in podocyte injury. Glucocorticoids remain the cornerstone of immunosuppressive treatment in the disease; however there is emerging evidence that FSGS may be autoantibody-mediated, and patients may benefit from targeted B-cell depleting therapies, such as rituximab. There is minimal evidence evaluating
