Mechanisms of mucosal immunity to oral Shigella infection in a physiological mouse model.

Shigella flexneri causes bacillary dysentery, a diarrheal disease responsible for significant global morbidity and mortality. Despite extensive efforts, there is no licensed Shigella vaccine, and due to the lack of tractable and physiological models, mechanisms of adaptive immunity to Shigella are poorly understood. Here, we establish a mouse model that permits mechanistic dissection of adaptive immunity to a physiological oral challenge with Shigella . We find primary Shigella infection confers robust cross-serotype protection against secondary challenge, in a manner strictly dependent on the

From plaques to pocks: carrying over bacteriophage assay techniques to the study of influenza and other animal viruses.

This article assesses the impact of the discovery of bacteriophages, which emerged from an investigation into a 1915 outbreak of bacillary dysentery in France, on influenza virus research. Specifically, it details the way in which the phages became a vehicle for importing certain assay techniques into the study of influenza and other viruses that cause infectious diseases in humans and other animals, thereby enabling the scaling up of vaccine production for these diseases. Very soon after his 1917 report of the discovery of bacteriophages, Felix d'Herelle developed an assay technique based on

Population Pharmacokinetic and Pharmacodynamic Prediction for Tebipenem Pivoxil Treatment of Pediatric Shigellosis.

Increasing antimicrobial resistance poses a serious challenge for the treatment of Shigella infections, and there is an urgent need for alternative antibacterial treatments. We conducted a clinical trial to investigate the efficacy of oral tebipenem pivoxil, compared to intravenous ceftriaxone, in Bangladeshi children with shigellosis. Using demographic data of 2249 Bangladeshi children with suspected shigellosis, population pharmacokinetic (PK) simulations were conducted to simulate tebipenem PK profiles in children with the proposed dosing regimen. Subsequently, the model predictions estimat

Shigella sonnei and Shigella flexneri: Epidemiology supporting development of cross-protective vaccines.

Shigellosis represents a major global health challenge, significantly contributing to mortality among children under five, particularly in sub-Saharan Africa and South Asia. Advances in molecular techniques have underscored the extensive global impact of Shigella, and the spread of multi- and extensively drug-resistant strains has sharply escalated the threat, emphasizing the urgent need for innovative interventions. Developing a broadly protective vaccine with a positive benefit-risk profile remains a critical and challenging objective due to the diversity of Shigella serotypes mediated by th

Shigella OspF blocks rapid p38-dependent priming of the NAIP-NLRC4 inflammasome.

The NAIP-NLRC4 inflammasome senses pathogenic bacteria by recognizing the cytosolic presence of bacterial proteins such as flagellin and type III secretion system (T3SS) subunits. In mice, the NAIP-NLRC4 inflammasome provides robust protection against bacterial pathogens that infect intestinal epithelial cells, including the gastrointestinal pathogen Shigella flexneri. By contrast, humans are highly susceptible to Shigella, despite the ability of human NAIP-NLRC4 to robustly detect Shigella T3SS proteins. Why the NAIP-NLRC4 inflammasome protects mice but not humans against Shigella infection r

The natural history of the emergence of sexually transmissible shigellosis.

Shigellosis is a gastrointestinal illness caused by bacteria belonging to one of four species of Shigella. Sexually transmissible (ST) shigellosis was first reported in 1974, but recently there has been a global increase in the transmission of extensively drug-resistant (XDR) strains. Here, we sought to characterise the natural history of ST shigellosis through literature review and genomic epidemiological analysis of early outbreaks. The literature review revealed a significant gap in reporting of ST shigellosis between the first report in 1974 and the early 2000s, after which reporting incre

Burden of Shigella and enterotoxigenic Escherichia coli infections among children under 5 years in Ethiopia, Kenya and Malawi: a systematic review and meta-analysis.

Diarrhoea remains a major problem among children in low- and middle-income countries, driven by multiple pathogens including rotavirus, Shigella and enterotoxigenic Escherichia coli (ETEC). Rotavirus vaccines have notably reduced diarrhoea deaths. However, the health consequences associated with Shigella and ETEC, along with rising antimicrobial resistance (AMR), have prompted the WHO to prioritise vaccine development against these two pathogens. Understanding their disease burden is crucial for guiding this effort and informing preparedness for vaccine adoption. We conducted a systematic revie

Emergence of Extensively Drug-Resistant Shigellosis - United States, 2011-2023.

Shigellosis is a nationally notifiable diarrheal illness caused by gram-negative bacteria. Shigella infection is spread through fecal-oral transmission and sexual contact. Although most infections are self-limited, antibiotics are indicated for severe illness or to reduce transmission in settings with high risk for spread. Since 2015, a growing proportion of cases has been caused by extensively drug-resistant (XDR) Shigella species, defined as being resistant to ampicillin, azithromycin, ceftriaxone, ciprofloxacin, and trimethoprim-sulfamethoxazole. No Food and Drug Administration-approved ora

Inhibitory Effect of Terminalia chebula Retz. on MDR Shigella flexneri Invasion of HeLa Cells.

INTRODUCTION: Shigella flexneri causes bacillary dysentery by invading the colon epithe-lium, leading to inflammation and tissue damage. The emergence of multidrug-resistant (MDR) Shi-gella strains and the lack of effective vaccines have highlighted the need for alternative treatments. Natural plant compounds have been explored as potential antimicrobial agents. This study evaluates the antibacterial activity of ethanolic (70%-96%) and methanolic (70%-99.9%) extracts of Termi-nalia chebula Retz. against MDR S. flexneri and their effect on bacterial invasion of HeLa cells in vitro. We assessed

A case of carbapenamase production in Shigella flexneri isolated in Ireland, 2025.

Shigella flexneri producing OXA-181 was detected from a patient in Ireland in October 2025. The isolate had a meropenem minimum inhibitory concentration (MIC) of 0.12 mg/L. Carbapenemase production was detected by lateral flow immunoassay. The sequence type was ST245 and closely related to sequences previously reported from Ireland and other countries but not previously linked to presence of the bla OXA-181 gene. Carbapenemase production in Shigella spp. has implications for laboratory detection and empiric treatment.

Efficacy of WRSs2, a live-attenuated Shigella sonnei vaccine, against shigellosis in a controlled human infection model in the USA: a phase 2, double-blind, randomised, placebo-controlled trial.

BACKGROUND: Despite long-standing research, no licensed vaccine exists for shigella, a leading cause of bacterial diarrhoea and dysentery. WRSs2 is a live-attenuated Shigella sonnei vaccine candidate which has previously shown safety and immunogenicity. In this trial, we evaluated its safety and efficacy in a controlled human infection model. METHODS: In this phase 2, double-blind, randomised, placebo-controlled trial at two sites in the USA, healthy adults aged 18-49 years were assigned using a site-stratified permuted-block schedule. The original three-arm design allocated participants 1:1:

Estimating the impact of Shigella vaccines on growth outcomes and implications for clinical trial design.

BACKGROUND: The value proposition for Shigella vaccines is strengthened by the potential for vaccines to prevent linear growth faltering. However, because expected effect sizes in Phase 3 vaccine trials are small due to limited Shigella incidence, a simple comparison of growth by randomized vaccine arm is likely underpowered and may yield null or even inverse results. METHODS: We consider a new approach that estimates vaccine effects in the subgroup that would be infected in absence of vaccination, termed the naturally infected. In simulations parameterized by multi-site studies of diarrhea,

CRISPR-Cas12a assay for rapid and specific detection of Shigella flexneri 2a in clinical samples.

Shigella flexneri 2a is the most common cause of shigellosis, a major public health concern in developing countries. Rapid and reliable diagnostic tools are critical for timely outbreak detection and management. Leveraging clustered regularly interspaced short palindromic repeats (CRISPR) technology, we developed a CRISPR-Cas12a-based assay for the rapid and specific detection of S. flexneri 2a and validated its performance using stool specimens from patients. Two guide RNAs targeting the gtrII and gtrX genes, unique markers of the S. flexneri 2a serotype, were designed to ensure specificity.

Qualification of a high-throughput luminescence-based Serum Bactericidal Activity assay to assess the functionality of human sera against five Shigella serotypes.

Shigellosis is one of the leading causes of diarrhoeal disease worldwide. It mostly affects children under 5 years and elderly persons over 70 years in low- and middle-income countries (LMICs). The high disease incidence in LMICs and the rise of antimicrobial resistance have prompted efforts to develop novel vaccines against shigellosis. Several multicomponent vaccines are currently under development; to date, however, no correlate of protection against shigellosis has been defined. The Serum Bactericidal Activity assay (SBA) is regularly applied to measure the complement-mediated functional a

Development of an 8-valent Salmonella and Shigella MAPS vaccine.

INTRODUCTION: Salmonella and Shigella infections are two of the leading causes of global diarrheal deaths in children younger than 5 years. Licensed vaccines against Salmonella Typhi have been shown to be effective, and other Salmonella and Shigella vaccine candidates are being evaluated in clinical trials. METHODS: We have previously described two 4-valent vaccines against Salmonella and Shigella strains separately. Here, we tested the combination of Salmonella and Shigella vaccines in animal models. ELISA was used to determine antibody titers against antigens, and bactericidal and opsonopha

Stephania cepharantha Hayata (Baiyaozi) attenuates cirrhosis-associated intestinal barrier injury by restoring mitochondrial function and suppressing PI3K/AKT signaling.

ETHNOPHARMACOLOGICAL RELEVANCE: Baiyaozi, the dried tuberous root of Stephania cepharantha Hayata, has long been used in traditional Chinese medicine for acute hepatitis, bacillary dysentery, acute gastroenteritis, abdominal pain, and bleeding disorders, indications that collectively correspond to hepatic inflammation, intestinal mucosal injury, and the bleeding tendency characteristic of cirrhosis-associated intestinal barrier injury. This ethnopharmacological correspondence provides a rationale for evaluating Baiyaozi (Stephania cepharantha Hayata) as a candidate therapy for the cirrhotic in
